[Value of the BUN/creatinine ratio in localizing digestive system hemorrhage in children].

We have evaluated the usefulness of the BUN/Cr ratio in 62 children with gastrointestinal bleeding. For upper gastrointestinal hemorrhage, the calculated BUN/Cr ratios ranged from 10 to 66.6, with a mean value of 30.85. For lower gastrointestinal bleeders, the BUN/Cr ratios ranged from 12.5 to 28, with a mean value of 20.12. 18 patients with upper gastrointestinal hemorrhage had a BUN/Cr ratio greater than 30 and all patients with lower gastrointestinal hemorrhage had a ratio of less than 30. The patients with gastroduodenal peptic ulcer had the highest ratios. We concluded that the BUN/Cr ratio is useful to discriminate upper and lower bleeding sources, especially in patients with gastroduodenal ulcer.

[Upper digestive system hemorrhages in Cameroon (apropos of 172 cases examined via endoscopy)].

172 cases of upper gastrointestinal bleeding were explored endoscopically from 01 March 1987 to 15 May 1989 in 3 centers in Cameroon. The patients comprised 124 men and 48 women with an average age of 39.64 years. Haematemesis with resultant melena constituted the most frequent presentation (42.36%) while haematemesis and melena was the unique presentation in 37.22% and 17.91% of cases respectively. The aetiologic factors encountered were: duodenal ulcers (33.64%), acute gastric bleeding (22.08%) gastric ulcer (13.44%), ruptured gastroesophageal varices (13.92%). Predisposing factors included the ingestion of gastric irritant medication, spices and alcohol. The authors review the literature and in its light, discuss the epidemiology and aetiology of gastrointestinal bleeding in Cameroon.

[Role of the gravitation factor in the development of changes in the digestive system].

The functional state of the gastro-intestinal system of rats aboard of "Cosmos-782", "Cosmos-936" and "Cosmos-1129", was studied. The long-term exposure to zero-gravity resulted in activation of proteolytic enzymes, decrease of carbohydrases and redistribution of lipolytic activities in gastro-intestinal organs. Use of artificial gravity during the flights produced a favourable effect on the proteolytic enzymes.

[Inflammatory diseases of the digestive system and pregnancy: clinical case and review of the literature].

This paper reports the fertility with Inflammatory Bowel Disease (IBD), effects of IBD on pregnancy and conversely, effects of treatment on the mother and the child. Fertility is minimally, if at all, compromised. The foetus is likely to survive in spite of active disease. There is no good evidence for an effect of pregnancy on Ulcerative Colitis and Crohn disease. Sulfasalazine and steroids should be used in IBD during pregnancy much as they are in the non pregnant azathioprine and metronidazole are contraindicated. IBD is not a reason for a young woman to avoid pregnancy and does not constitute an indication for abortion.

[Predictability of the anamnesis in pathology of the upper tract of the digestive system. Computerized analysis concerning 1000 subjects submitted to esophagogastroduodenoscopy].

The ability of history taking to predict endoscopically verified pathology of the upper gastrointestinal tract was evaluated in a group of 1000 patients submitted to esophagogastroduodenoscopy (EGDS). The presence of one or more of the following symptoms at the time of EGDS or 4 weeks previously, was considered: epigastric pain, dysphagia, dyspepsia, gastrointestinal bleeding, pyrosis, anorexia and/or weight loss, nausea and/or vomiting. The results of this elaboration showed that the presence of recent symptomatology does not allow differentiation of patients with endoscopically verified pathology from those without it. The presence of an "at risk history" consisting of one or more of the following factors was also evaluated: smoking 10 cigarettes per day, drinking 100 g alcohol per day, previous diagnosis of upper gastrointestinal tract pathology, gastrointestinal-irritating therapy. Data analysis showed notable importance of the presence of an "at risk history"; in fact, when compared with subjects without this type of history, "at risk" patients were twice as likely to have a pathological condition diagnosed. Thus, when protocols for endoscopic examination are established the history of the patient and his lifestyle must be taken into consideration.
