[Genital herpes: practical impact of the 2001 consensus conference?].

Genital herpes, usually a benign infection, has nevertheless an important psychological impact by its recurrent character. The clinical management needs first a well done patient counseling, and second, the biological documentation of the virus excretion. When this is achieved, different treatment strategies may be proposed aiming mainly acute crisis. There is, however, a strong trend towards the prevention of recurrences using suppressive antiviral treatment or modulating the immune system (vaccines, immunomodulators). Indeed, there is a hope that these new approaches may lead to the reduction of disease transmission by diminishing the viral shedding.

Outcomes from the Patient Perspective Workshop at OMERACT 6.

The objective of the Patient Perspective Workshop at OMERACT 6 was to address the question of assessing the outcomes of intervention in rheumatoid arthritis (RA) from the perspective of those who experience the disease themselves. This was done by reviewing the current state of research in the area, identifying the requirements for the development of valid instruments, delineating a research agenda that can attain these requirements, and motivating participants to undertake the appropriate research. Through a series of meetings and discussion sessions a research agenda emerged that includes: exploring subjective experiences of RA identified by patients as important but not encompassed within the current "core set" of outcome measures (such as a sense of well being, fatigue, and disturbed sleep); clarifying terminology; and empowering patients to be more effective partners in outcomes research. These were supported by the OMERACT plenary session. Specific actions were required by both patient participants and organizers to ensure the nature of the conference, its focus and method of working were understood, and that the patient participants were sufficiently confident to make their contribution.

Heart failure: an overview of consensus guidelines and nursing implications.

Heart failure affects more than 350,000 Canadians and costs over $1 billion annually for inpatient care alone. Consensus guidelines have been developed to guide care and improve quality of life based on current evidence or best practice. This article will provide a brief overview of medications and lifestyle modifications described in guidelines developed by the American College of Cardiology/American Heart Association, the Canadian Cardiovascular Society, the Heart Failure Society of America, and the European Society of Cardiology. Medications for treating heart failure can be divided into two groups: those with a mortality benefit (angiotensin converting enzyme inhibitor, beta-blockers, and selective aldosterone receptor antagonists), and those that improve symptoms (diuretics and cardiac glycosides). Nursing implications include careful assessment of volume status, vital signs, monitoring electrolyte and renal function, as well as spacing of medications. Nurses play a key role in assisting patients to identify their lifestyle habits that require modifications, ultimately improving their quality of life and decreasing hospital readmissions. Education focusing on self-care activities, diet, rest, and exercise enables patients to retain a sense of control in their lives.

Cardiac rehabilitation and secondary prevention services in Ontario: recommendations from a consensus panel.

The Cardiac Care Network of Ontario Consensus Panel on Cardiac Rehabilitation and Secondary Prevention drew on the literature and its own expertise, and surveyed existing cardiac rehabilitation and secondary prevention (CR) services in Ontario to make recommendations for the delivery of CR services in Ontario. This report, which is not an official position paper for the Canadian Cardiovascular Society, presents these recommendations. The key recommendations were a regional coordination model for the delivery of CR services that would provide CR close to home and promote access to CR in groups traditionally underrepresented in CR; high quality central data collection; the creation of a provincial CR registry to allow future planning, coordination, monitoring and evaluation of CR services in Ontario; and the establishment of specific CR program funding from the Ontario Ministry of Health and Long Term Care.

[Conclusions of consensus development conferences on the prevention and treatment of venous thromboembolism].

An education program for hospital physicians and family doctors including up to date knowledge on diagnosis, prevention and treatment of venous thromboembolism has been elaborated, chiefly within the framework of hospital consensus conferences. Postgraduate consensus meetings led by representatives of specialities interested in prophylaxis of venous thrombosis have been held in 64 hospitals all over the country during the years 1996 to 2002, with a total number of more than 10,000 doctors working in hospital or general practice. In contrast to descriptive reports offered so far on drugs, the aim was a comprehensive clinical review of the problem, according to the principle of "disease management". By this way not only new diagnostic, therapeutic and prophylactic methods were presented but we managed to shape a new attitude based on new pathophysiological findings. As a result of the conferences the incidence and mortality of venous thromboembolism has decreased both on hospital and national levels. The consensus meetings acting as local interdisciplinary postgraduate forums have been supported by the Hungarian Thrombosis and Haemostasis Society and the haemostasis workshops of the country (Budapest, Debrecen, Pécs, Szombathely). Shaping the principles and methods for prophylaxis and therapy of venous thromboembolism can be regarded as a model which can subsequently be adapted to the problems of arterial thrombosis as well.

Proposal for establishing structure and process in the development of implicit chiropractic standards of care and practice guidelines.

With the advent of health care's "era of accountability", the chiropractic profession is now faced with generating implicit standards and guidelines for care or having it done for us by outside agencies. Already we see chiropractic groups in individual states and provinces being pressured into naive efforts of guideline development. Current knowledge and experience are available through recent health care literature that clearly defines the structure and process of guideline development and offers suggestions on how to measure outcomes of those processes. In addition, the United States Congress has directed a new federal agency to oversee this activity and monitor outcomes of quality improvement programs. The time has come for the chiropractic profession to define its exact role in health care delivery and develop implicit standards of care and practice guidelines. This sentinel effort should be managed by a commissioned body of empaneled experts that generally represent the academic and clinical chiropractic profession. A protocol for selection of these panelists and the panel chairperson needs to be developed and memorialized. Appropriate methodology (with definitions) needs to be developed for the process of standards/guideline development. Adherence to the accepted structure and process of guideline development will ensure the continuity of this dynamic process in the coming generations. This proposal offers a preliminary definition of the structure and process, including a "seed" policy statement and decision flow chart, specific to guideline development. Once the structure and process of guideline development for chiropractic are defined, the profession can then present this product to federal and state agencies, private sector health care purchasers, patient advocacy groups and other stakeholders of chiropractic care.(ABSTRACT TRUNCATED AT 250 WORDS)
