The suicide of Thomas Wentworth Wills.

Thomas Wentworth Wills was the most important Australian sportsman of his time. He captained the Victorian colony at cricket and was the first hero of Australian Rules football. Although his picture now adorns the conservative Melbourne Cricket Club, he died in 1880, an isolated, destitute alcoholic, after stabbing himself in the heart. Wills embodied a tradition, as prevalent today as it was over 100 years ago, that weds sport with alcohol in Australian culture.

Late traumatic flap displacement after laser in situ keratomileuisis.

Laser in situ keratomileusis (LASIK) has become the community standard in corneal refractive surgery and is being performed by surgeons in the Army, Navy, and Air Force. LASIK differs from photorefractive keratectomy (PRK) in that a partial-thickness corneal flap is created in the LASIK procedure before removing a microscopic amount of corneal tissue, whereas no flap creation is required in PRK. The benefits of LASIK include minimal discomfort after surgery, as well as a much faster return of visual function. PRK involves a surface ablation and therefore heals differently, involving more discomfort and a slower return of functional vision. LASIK flap integrity is a concern to anyone undergoing the procedure, as well as for those making recommendations on the best form of refractive surgery for military personnel. A case report and a review of the literature are presented on the identification and management of LASIK flap trauma.

Tibial sesamoid fracture in a softball player.

A single case of a tibial sesamoid fracture in a softball player is reported here. A review of the literature confirms that this is an unusual and difficult problem to treat in the athletic population given the significant loads placed on the sesamoids during athletic activity. In the case presented, conservative care was not effective, and the athlete underwent surgical excision of the fractured sesamoid. With use of a postoperative orthosis and cleat modification, surgical management was successful and allowed the athlete to return to her athletic endeavors without restrictions in 8 weeks.

MR imaging of the thrower's shoulder. Internal impingement, latissimus dorsi/subscapularis strains, and related injuries.

In conclusion, internal impingement apparently occurs in nearly all patients and is demonstrable on MR imaging. Pathologic changes associated with internal impingement seem to develop with repetitive placement of the arm into a position of extreme external rotation and abduction. Findings may include lesions of the posterior superior labrum, undersurface irritation, or tearing of the supraspinatus-infraspinatus junction near the attachment site and cystic changes of the posterior superior glenoid and posterior lateral greater tuberosity. There is no evidence for a particular sequence of pathologic changes. Instability may be associated with but does not appear to be a prerequisite for the development of the pathologic lesions of internal impingement.
