This paper reports a series of 43 patients with lung cancer treated by means of planned preoperative radiotherapy between 1961 and 1966. A five-year survival figure of 23% is reported for the series. The rationale of the treatment is discussed, together with some clinical situations in which it was felt that this regime had a definite advantage over other methods.

Whether the influx of calcium through voltage-operated channels is involved in the stimulatory effects of substance P and neurokinin A in airways smooth muscle is not yet firmly established. This question was addressed in the present study using guinea pig trachea and human bronchi suspended in normal or calcium-free Krebs solution and tested with inhibitors of calcium channels. In calcium-free Krebs solution, the myotropic effects of substance P (10(-7) M), neurokinin A (3.10(-8) M), acetylcholine (2.10(-5) M), and histamine (2.10(-5) M) were reduced by 27-57%, while those of potassium chloride and tetraethylammonium were practically abolished. Calcium antagonists such as verapamil or nicardipine, when applied at concentrations of 10(-8)-10(-6) M, inhibited the contractions produced by potassium chloride and tetraethylammonium, whereas higher concentrations (10(-5)-10(-4) M) of both inhibitors were needed to reduce the effects of substance P, neurokinin A, acetylcholine, and histamine. In neither preparation did the calcium agonist Bay K 8644 (10(-6) M) modify the effects of neurokinin A, substance P, acetylcholine, or histamine, but it potentiated potassium chloride's effect on human bronchi. We conclude that transmembrane calcium influx through voltage-operated channels plays a minor role in the stimulatory effects of neurokinins in airways smooth muscle.
The pharmacodynamic activity of noradrenaline was evaluated comparatively in vitro on isolated human bronchi and on guinea pig tracheal spirals. Noradrenaline exerted a contractile effect on both preparations under resting tone and in the presence of propranolol 10(-6) M; maximal noradrenaline-induced contraction was 15-20% of maximal acetylcholine (ACh)-induced contraction. Without propranolol, the contractile effect of noradrenaline was negligible when the preparations were under resting tone and absent when they were precontracted with ACh. In contrast, noradrenaline exerted a strongly relaxant effect on both human bronchi (-log ED50 5.24 +/- 0.17; N = 5) and guinea pig tracheae (-log ED50 6.15 +/- 0.29; N = 8). With maximal contraction induced by ACh 3.10(-3) M the -log ED50 of both preparations were shifted to the right by functional antagonism and became 4.72 +/- 0.17 and 5.31 +/- 0.11, respectively. The pKD values of noradrenaline, calculated according to Furchgott and Bursztyn (1967), were 4.79 +/- 0.04 in human bronchi (N = 5) and 4.77 +/- 0.16 in guinea-pig tracheae (N = 8). In the presence of cocaine plus phenoxybenzamine these values were not significantly modified in human bronchi and only slightly modified in guinea pig tracheae. It is concluded that noradrenaline induces a strong beta-adrenergic response and a negligible alpha-adrenergic response from both human bronchi and guinea pig tracheae in vitro.
1 The reactivity was investigated of cat isolated tracheal chains, bronchial spirals and lung parenchymal strips to carbachol, 5-hydroxytryptamine (5-HT) and histamine. 2 Carbachol and 5-HT produced concentration-dependent contractions of all three tissues, responses to the former drug being selectively blocked by atropine and to the latter by methysergide. 3 Histamine failed to elicit a contractile response from the tracheal or bronchial muscle. However, it induced dose-related concentrations of the lung strips, that were selectively blocked by mepyramine. 4 It is concluded that lung parenchymal strips of cats possess mepyramine-sensitive histamine H1-receptors. Receptors for 5-HT, susceptible to methysergide, occur in the tracheobronchial smooth muscles as well as in lung strips of cats.
Small calibre airway reactivity to different contractile and relaxant agents was tested in vitro using small segments (about 1 mm long and 0.2 mm in internal diameter) of guinea-pig isolated intralobular bronchi. EC50 values of, and maximal contractile responses to contractile agents were as follows (mean +/- s.e.mean, n = 6): acetylcholine 13.6 +/- 2.6 microM and 1140 +/- 80 mg; histamine 5.2 +/- 0.7 microM and 1094 +/- 95 mg; 5-hydroxytryptamine (5-HT) 0.7 +/- 0.1 microM and 595 +/- 61 mg; prostaglandin F2 alpha (PGF2 alpha) 8.8 +/- 1.2 microM and 1100 +/- 88 mg; tetraethylammonium 2.9 +/- 0.3 mM and 1055 +/- 94 mg; KC1 14.6 +/- 0.5 mM and 965 +/- 81 mg. EC50 values of, and maximal relaxant responses to beta-adrenoceptor stimulants on preparations precontracted with acetylcholine (1.4 X 10(-4)M) were: isoprenaline 0.40 +/- 0.5 microM and 782 +/- 65 mg, n = 18; salbutamol 0.19 +/- 0.02 microM and 494 +/- 55 mg, n = 5; terbutaline 0.87 +/- 0.18 microM and 263 +/- 40 mg n = 5; fenoterol 0.06 +/- 0.02 microM and 722 +/- 47 mg, n = 5; adrenaline 0.71 +/- 0.13 microM and 653 +/- 62 mg, n = 5; noradrenaline 10.8 +/- 0.9 microM and 566 +/- 97 mg, n = 5. Differences in the maximal relaxant effects between the beta-adrenoceptor stimulants showed that the preparation utilized is a relevant model for assessment of the intrinsic activity of these drugs. 5 The high ratio (about 180) of the ECm for noradrenaline (beta-adrenoceptor agonist) to that for fenoterol (beta 2-adrenoceptor agonist), and the lack of effect of prenalterol (beta 1-adrenoceptor agonist) suggested that beta 2-adrenoceptors are preferentially involved in the relaxant activity of beta-adrenoceptor stimulants in this preparation.
In human isolated bronchial preparations, the endothelinB (ETB) receptor-selective agonist, sarafotoxin S6c (Stx6c; 1 nM) increased nerve-mediated contraction in response to electrical field stimulation (EFS) at 0.5-1 Hz from 19 +/- 4% to 42 +/- 7% (n = 9). This effect was blocked in the presence of the ETB receptor-selective antagonist, BQ-788 (10 microM). These data are consistent with findings in some animal species that ET-1 and related peptides have marked neuromodulatory influences on the cholinergic system. Furthermore, they provide additional support for the concept that ET-1 may have a mediator role in bronchial obstruction in asthma.
We present a case of granular cell tumor (GCT) occurring in the esophagus 7 years after operation for bronchial GCT. A 59-year-old Japanese man complained of epigastralgia, and endoscopic examination of the upper digestive tract disclosed a submucosal tumor in the lower esophagus. Histological examination of the endoscopic mucosal resection of the esophageal tumor showed a proliferation of neoplastic cells with an eosinophilic and granular cytoplasm. The cytoplasm of the neoplastic cells was histochemically positive for PAS stain and immunohistochemically positive for S-100. This tumor did not fulfill any of the diagnostic criteria for malignancy at either the macroscopic or microscopic level. I believe that this is the first case of GCT occurring metachronously in the respiratory and digestive tracts. Clinicians and pathologists should bear in mind that GCT may arise metachronously in the respiratory and digestive tracts.

To investigate the role of BALT in chronic respiratory infection, we established a rat model of chronic bronchiolitis caused by Pseudomonas aeruginosa, and the light microscopic findings of the BALT were studied. Experimental pneumonia was produced in SD rats by intratracheal inoculation of 10(6) colony forming units (cfu) of mucoid P. aeruginosa enmeshed in agar beads. The mean cfu recoverable from the lung increased up to 10(8) cfu at day 1. The number of bacteria remained fairly constant at 10(4) cfu until day 28. Histologically, infiltration of neutrophils could be seen around the agar beads containing P. aeruginosa in the lung from day 1 to day 7. From day 14 to day 28. the histological changes in the lungs were characterized by accumulation of foamy cells accompanied with lymphocyte infiltrations and granulation tissues around the respiratory bronchioles, but without involved alveoli. These chronic inflammatory histologic changes were similar to those of diffuse panbronchiolitis (DPB). At the same time, there were massive accumulation of lymphocytes in lymphatics and in high endothelial venules of the BALT at day 7. The development of germinal centers in the follicular area was found from day 7 to day 28. The airway was narrowed as a result of BALT hyperplasia protruding to the bronchial lumen. These results suggest that histological changes represent the model of chronic bronchiolitis and BALT may have a specific role in developing the local immune responses against chronic respiratory infection due to P. aeruginosa.
A GaN based ultraviolet (UV) laser diode (LD) was used to study the autofluorescence (AF) spectrum of the normal and tumor human bronchial tissues under ex vivo conditions. The UV LD generates a coherent short wavelength (around 400 nm) light beam with an intensity of about a few watts. AF spectrum data can be obtained without interference by excitation light. A clear blue peak located at around 483 nm was observed along with a green peak at around 560 nm in the normal tissue. The peak intensities observed were very weak for the tumor tissues. The AF imaging and spectrum analysis were performed along with a histopathological study. The spatial distribution of the elastin in the bronchial tissue affected the intensity of the AF whereas the spectrum shape was not affected. Strong AF was observed from regions that include a high density of the elastin. Biopsy measurements were performed for ex vivo samples, and depth profiling of the elastin was studied along with variations of the AF spectrum. AF spectra excited by the UV LD for fluorescence materials including FAD, NADH, and elastin were measured. The spectrum shape of the elastin as well as of NADH was similar to that of normal bronchial tissues.
