Recanalization of occluded superficial femoral arteries using the rotational transluminal angioplasty catheter system (ROTACS).

A total of 85 occluded superficial femoral arteries were treated using the rotational transluminal angioplasty catheter system (ROTACS). The mean length of the occlusions was 7cm; 76% were uncalcified or only slightly calcified whereas 24% were calcified or highly calcified. The mean preoperative ankle:brachial index was 0.51. Primary success was achieved in 62 of 85 cases (73%). The mean length of reperfused occlusions was 6.2 cm: 26% of these lesions were calcified. The mean ankle:brachial index was 0.91. There were 23 primary failures (27%): reperfusion was impossible in 11 cases (including one complicated by perforation) and there were eight dissections, three cases where residual stenosis exceeded 50%, and one other unspecified failure. The mean length of these occlusions was 10.5 cm; 17% were calcified. Two patients developed a distal embolus and one died 10 days after reperfusion. The probability of primary patency of a reperfused artery was 44% at 1 year. Forty-two of the 62 patients who achieved primary success remained symptom free; the mean length of the original occlusion was 4.5 cm. Fifteen patients developed a new area of stenosis whereas five others exhibited new occlusion after a mean interval of 6 months. The mean length of these reperfused arteries was 9 cm. The probability of secondary patency at 1 year was 58%. Arterial calcification did not appear to influence the feasibility of reperfusion using the catheter. The main factor determining successful reperfusion was the length of the occlusal defect (P < 0.05). Reperfusion using the ROTACS did not improve the feasibility of reperfusion by conventional transluminal angioplasty.(ABSTRACT TRUNCATED AT 250 WORDS)

[Auto-recanalization of anastomosis in the surgery of esophagus and large intestine ].

The authors developed a techniques of anastomosis formation after gastrectomy, proximal resection of the stomach, and resection of the descending colon and rectum in which the anastomosis begin functioning a few days after the operation. They called them autorecanalizing anastomoses. From the moment of the operation and to autorecanalization the lumina of the anastomosed organs are drained by original methods. The purpose of the developed techniques for the formation of autorecanalizing anastomoses and the methods for decompression and draining of the operated on organs is to reduce the risk of suture incompetence and the hazardous effect of this complication on the patient's life.

[Spontaneous recanalization after vasectomy].

Two cases of spontaneous recanalisation could be reoperated. The recanalisation was proved histologically. The reasons were analysed. The own technique is presented: resection of at least two centimeters of the vas, ligature, electrocoagulation, proximally turning-up fixation of the end, distally immersing the end in a tobacco-pouch.

[Emergency and elective percutaneous recanalization in ischemic heart disease].

The success and complication rates of programmed angioplasty performed after diagnostic coronarography and of ad hoc angioplasty performed at the time of the diagnostic catheterization are analysed in the study. We have attempted percutaneous recanalisation of 1071 lesions on 440 patients and dilated an average 2.4 stenoses or occlusions by patient. Immediate intervention was performed on 59.5% of patients with overall 91.7% success rate. The need for emergency surgery was 0.7%, periprocedural myocardial infarction was observed in 1.5%, and the mortality rate was 1.5%. The success and complication rates were similar for both studied groups. In the ad hoc angioplasty group there were relatively more easily solvable cases concerning the techniques, however the incidence of unstable angina and acute, emergency settings were more frequent. Ad hoc angioplasty does not diminish the success rate and resulted a faster, and more spectacular recovery in the above severe cases. Extensive experience with ad hoc angioplasty has proven it to be reasonable approach, efficient and safe variation not only in rescue, but also in complex angiological settings, if the considerable logistics are possible in an individual cardiovascular center. The provision of ischaemic heart disease patients is subject to the planning and creating such anti-ischaemic interventional cardiological centers in Hungary.

Microsurgical recanalization of fallopian tubes after tubosterilization and its related factors. Report of 278 cases.

Microsurgical tubocanalization was done in 412 patients. 278 women had the operation before the end of December 1987 and were followed up. The overall intrauterine pregnancy rate was 81.3% (92.86% for isthmus-isthmus anastomosis and 87.5% for ampulla-ampulla anastomosis). Two patients had ectopic pregnancy which was followed by term pregnancy subsequently. The factors affecting operative results were analyzed.

[Arterial recanalization with the Kensey catheter. Preliminary results].

Fifteen patients with chronic obstruction of leg arteries (mean length: 6 cm) were treated with the Kensey catheter and then with PTA. In 11 cases the procedure was successful (74%) and in 2 it was interrupted--in one case for dissection and in the other for vessel perforation. Two maneuvers, after dissection, were ended with PTA and stent placement (overall success rate: 87%). Emergency surgery was never needed. In the cases solved with the Kensey catheter, the cumulative patency rate was 51% at 6, 12 and 24 months. The Kensey catheter did not improve the results of conventional PTA, which means its role is complementary to that of PTA and an alternative to more expensive laser systems, if PTA fails.

THC:YAG nasolacrimal duct recanalization.

In an early exploration of a new technique for creating a patent nasolacrimal duct system, a chromium-sensitized and thulium- and holmium-doped YAG laser was used to canalize the nasolacrimal duct of a fresh-frozen bisected human cadaver head. The laser--long-pulsed (300 milliseconds), compact, self-contained, and solid-state--operates in the near infrared range (2.1 microns). The technique involved passing a 550-micrometer quartz fiberoptic through the dilated superior punctum and canalicular system, down through the nasolacrimal duct. The quartz fiberoptic was then withdrawn into the area of the lacrimal sac. Pulse energies of 500 mJ were used at a repetition rate of five pulses per second to ablate the duct lining. The nasolacrimal duct was then unroofed and the epithelial lining of the duct histopathologically evaluated.

Dacryocystorhinostomy↗

[Coronary recanalization in the acute phase of myocardial infarction].

Early reopening of the infarct-related artery limits infarct size, preserves left ventricular function and reduces short and long term mortality rate. The earlier the reopening, the higher the benefit, the best results being obtained within the first 3 hours, but remaining significant up to 12 hours after onset of symptoms. In addition, complete reperfusion of the infarct-related artery without delay of distal filling enhances the results. Early reopening can be obtained with use of intravenous thrombolysis. The indication of rescue angioplasty in case of failed thrombolysis or of immediate, deferred or elective adjunctive angioplasty after successful thrombolysis remain debated. Contraindications to thrombolysis exist in about 13% of patients, in whom the only solution is direct angioplasty without thrombolysis.

[Treatments of acute myocardial infarction other than recanalization ].

During the past 15 years, the in-hospital mortality of patients with acute myocardial infarction has considerably declined. The 2 main reasons for this decrease in mortality have been the introduction of intensive coronary care units, and reperfusion of myocardial infarction, either by coronary thrombolysis or percutaneous transluminal coronary angioplasty. However benefits of other therapies should not be overlooked. General care measures should include: close supervision, bed rest, liquid diet, analgesics, reduction of anxiety, oxygen therapy, intravenous heparin and aspirin, monitoring of blood pressure and heart rate. beta-blockers and angiotensin-converting enzyme inhibitors, combined or not with thrombolytic therapy, are also available to reduce the rate of mortality and morbidity. The benefits to be expected from these 2 medications combined have not yet been demonstrated, so that it appears reasonable in common practice to choose either drug according to the patient's functional state.
