[Incremental effect and mechanism of cyclosporine on blood concentration of statins and statin package insert information in Japan].

Cyclosporine is an indispensable immunosuppressant used in organ transplant patients, who frequently manifest hyperlipidemia. Statins, which are cholesterol-lowering agents, are often combined with cyclosporine in the treatment of hyperlipidemia of organ transplant patients. Since cyclosporine is a substrate and inhibitor of CYP3A4, researchers suspect that the immunosuppressant inhibits CYP3A4-mediated metabolism of statins, leading to an increase in statin plasma concentration and infrequently resulting in rhabdomyolysis. However, a number of clinical trials have shown cyclosporine to increase the plasma concentration of all developed statins, including those not metabolized by CYP3A4. Furthemore, recent mechanistic studies have shown organic anion transporting peptides (OATP) C to mediate the uptake of some statins and cyclosporine has been shown to inhibit the uptake via OATP-C in cultured cells. Therefore, the inhibition of hepatic uptake of statins is considered to be one of the mechanisms by which cyclosporine incrementally increases statin blood concentration. However, most current Japanese package inserts of statins give no information on change in pharmacokinetic parameters such as AUC and Cmax in the combined medication with cyclosporine. Furthermore, in the Japanese package inserts, it is either stated that cyclosporine inhibits CYP3A4-mediated metabolism or no comment is made on the mechanism. The package insert should properly provide available quantitative information on the change of pharmacokinetic parameters and the probable mechanism of action.

[Analysis of phthalates in plastic food-packaging bags by thin layer chromatography].

The method for simultaneous determination of four phthalates, namely dimethyl phthalate (DMP), diethyl phthalate (DEP), di-n-butyl phthalate (DBP) and di (2-ethylhexyl) phthalate (DEHP) in plastic food-packaging bags by thin layer chromatography (TLC) was developed. The plastic food-packaging bags were extracted with ethanol by ultrasonication, then the mixture was filtrated through membrane (0.45 microm). The mixture of ethyl acetate-anhydrous ether-isooctane (1 : 4 : 15, v/v) was used as developing agent on the TLC silica gel plate for development. The filtered liquid was spotted on the TLC plate dealt by acetone, and detected with scanning wavelength of 275 nm and reference wavelength of 340 nm. The qualitative analysis of the phthalates was performed using the R(f) values of the chromatogram. The quantitative analysis was performed with external standard method. Good linearities were obtained for DMP, DEP, DBP and DEHP. The detection limits were 2.1 ng for DMP, 2.4 ng for DEP, 3.4 ng for DBP and 4.0 ng for DEHP. The relative standard deviations (RSDs) of the four phthalates were 2.8% - 3.5%. The recoveries of the four phthalate standards in real sample were 78.58% - 111.04%. The method presented has the advantages of high precision, high sensitivity, small sample size, and simple pretreatment . The method was used to detect the four phthalates in the food-packaging bags. The contents in real samples were close to the results by gas chromatography.

Isolation and characterization of the smallest bacteriophage P4 derivatives packaged into P4-size head in bacteriophage P2-P4 system.

Bacteriophage P4, a satellite phage of coliphage P2, is a very useful experimental tool for the study of viral capsid assembly and cos-cleavage. For an in vitro cos-cleavage reaction study of the P2-P4 system, new shortened and selectable markers containing P4 derivative plasmids were designed as a substrate molecules. They were constructed by swapping the non-essential segment of P4 DNA for either the kanamycin resistance (kmr) gene or the ampicillin resistance (apr) gene. The size of the genomes of the resulting markers were 82% (P4 ash8 delRI:: kmr) and 79% (P4 ash8 delRI:: apr) of the wild type P4 genome. To determine the lower limit of genome size that could be packaged into the small P4-size head, these shortened P4 plasmids were converted to phage particles with infection of the helper phage P2. The conversion of plasmid P4 derivatives to bacteriophage particles was verified by the heat stability test and the burst size determination experiment. CsCl buoyant equilibrium density gradient experiments confirmed not only the genome size of the viable phage form of shortened P4 derivatives, but also their packaging into the small P4-size head. P4 ash8 delRI:: apr turned out to be the smallest P4 genome that can be packaged into P4-sized head.

Distribution of care information packages for the support of patients with cancer and their families during disaster situations and their evaluation via the internet.

With the aim of preparing for disasters, care information packages for the support of patients with cancer and their families during disaster situations were developed in our previous study, and were provided via the internet with a questionnaire asking the face validity and the usefulness of such information. Thirty replies were received. Most subjects thought the care packages were appropriate and useful in the actual settings, however 43.3% of them thought the amount of information was excessive. Many comments and suggestions on the care information packages also could be obtained via the internet.

The effect of the 1989 health benefits package on prescribing and consultation patterns in general practice.

OBJECTS: this study aims to assess the effect of the 1989 health benefits package on the prescribing and consultation rates of the contributing general practitioners. METHODS: following the introduction of the health benefits package in February 1989 information on the consultation rates and prescribing records of twenty-eight computerised general practitioners was obtained for the two three month periods of February, March and April 1988 and 1989. Analysis was undertaken both on the pooled data and by comparing consultations for adults, beneficiaries aged 60 or older, beneficiaries less than 60 years and children. RESULTS: the number of consultations increased from 57,209 in 1988 to 63,736 in 1989. The number of items prescribed decreased from 66,984 in 1988 to 62,284 in 1989 indicating a mean decrease in the number of prescription items per consultation from 1.02 in 1988 to 0.83 in 1989. Based on the 1988 figures provided by the study the actual percentage decrease in pharmaceutical use by the contributing doctors during the study time frame was 9.3% whereas the expected increase was 11.4%. CONCLUSIONS: there was an increase in consultation rates and a decrease in prescribing among the contributing general practitioners during the 1989 study period compared with the 1988 study period. These changes may be attributed to the introduction of the 1989 health benefits package.

A software package for event related potential experiments.

All too frequently, researchers find that experimental designs are dictated by procedures implemented by the available equipment rather than by the questions to be answered. Event related potential experiments are limited by equipment which restricts the type and number of stimuli which can be presented and the length, number, and type of averages which can be computed. A software package was developed for event related potential experiments which provides a high degree of flexibility in designing the stimulus presentation protocol and the averages which can be computed. The package includes a set of user friendly programs for constructing complicated stimulus presentation sequences. In order not to restrict the researcher to a set of predetermined averages computed at the time the experiment is conducted, the EEG data is digitized and stored continuously without regard to times of stimulus presentation. Averages are computed from the continuous EEG data by a separate averaging program which takes as input a table of starting times of the segments to be averaged. The software package is currently in use at Dr. Rohrbaugh's laboratory for a series of complicated event related potential experiments.

Agreement between analytical values and label declarations of sodium content of processed packaged foods.

As part of the Food Labeling and Product Surveillance Program of the Food and Drug Administration, samples from 1982, 1984, and 1986 surveys of processed packaged foods (canned, frozen, and dry packaged) were analyzed to determine how closely the foods' actual sodium content matched their label claims for sodium. Samples were classified into three groups: "sodium-free" products (less than 5 mg sodium per serving), and "very-low-sodium" products (5 to 35 mg per serving), and a larger group of products that were labeled as containing more than 35 mg sodium per serving. In the third group, the distribution of actual sodium content fell around 100% of label claims for sodium. For the three survey years, analyzed sodium averaged 101% of label claim (standard deviation [SD] = 25%) for 1982 samples (20 products), 99% of label claim (SD = 28%) for 1984 samples (108 products), and 94% of label claim (SD = 21%) for 1986 samples (265 products). Products labeled "very low sodium" were treated separately because sodium analyses varied widely from the label declarations. However, most "very-low-sodium" products were well under 35 mg sodium per serving. Results indicated that consumers receive reasonably accurate information about the sodium content of processed packaged foods from label claims.

Construction and use of a safe and efficient amphotropic packaging cell line.

An amphotropic retrovirus packaging cell line was constructed in which the gag, pol, and env genes of the helper virus are separated on two different plasmids and in which the packaging signals and 3' long terminal repeats are removed. To do this, a plasmid containing the Moloney murine leukemia virus gag and pol gene was transfected into NIH 3T3 cells, and a plasmid containing the 4070A amphotropic env gene was transfected into one of the resulting clones which produced a high level of reverse transcriptase. A clone producing a high level of amphotropic env protein (GP + envAm12) was then isolated. When transfected into GP + envAm12 cells, titers of the retroviral vector N2, containing a neomycin resistance gene, ranged from 10(2) to greater than 10(6) CFU/ml on 3T3 cells, from 1.3 x 10(4) to 2.7 x 10(5) CFU/ml on HeLa cells, and from 1.0 x 10(2) to 6.0 x 10(3) CFU/ml on K562 cells when assayed by G418 resistance. These titers were comparable to titers obtained using the PA317 cell line. Tests for the safety of the GP + envAm12 packaging line showed no evidence for the generation of wild-type virus. Thus, the efficiency and safety of the GP + envAm12 cell line in gene transfer into human cells may provide an optimal system for experiments whose goal is human gene therapy.

Effects of genome size on bacteriophage phi X174 DNA packaging in vitro.

Effects of the size of template DNA on the DNA packaging reaction of bacteriophage phi X174 were studied using plasmids of various sizes which contain the phi X174 origin of DNA replication and the in vitro phage synthesizing system (Aoyama, A., Hamatake, R. K., and Hayashi, M. (1983) Proc. Natl. Acad. Sci. U.S.A. 80, 4195-4199). DNA between 78.5% and 101% of the length of phi X174 DNA produced infectious particles efficiently. Packaging of DNA smaller or larger than this range produced uninfectious defective particles. Although these particles contained circular single-stranded DNA, they suffered structural changes which altered the sedimentation properties or the ability to adsorb to the cells. Mutant phage were found from the packaging reaction of DNA larger than 101% of phi X174 DNA. These mutants deleted the termination region of DNA, suggesting that they were produced by early termination of the phage synthesizing reaction.

Learning about diabetes: the production and evaluation of a teaching package for insulin dependent diabetic children.

Diabetes is one of the commonest long-term diseases to affect man; approximately 2 per cent of the population is affected and this figure is rising. This paper describes a project involving the production of a teaching package for insulin-dependent diabetic children. The package took the form of an illustrated booklet and its success was evaluated at a diabetic clinic in a Glasgow hospital. As a result of this evaluation it was concluded that the package was a useful means of teaching diabetic children.

Implementing a stand-alone packaged pharmacy computer system in a 580-bed hospital.

The problems experienced by a hospital pharmacy department in implementing a stand-alone packaged computer system are discussed, and recommendations for avoiding and managing these problems are presented. In 1984, a stand-alone packaged computer system was implemented in a 580-bed, tertiary-care institution that provides services from a central pharmacy and five satellite pharmacies. The department developed a request for proposal and contracted with a vender for a system that would support unit dose drug distribution and i.v. admixture services. During the implementation process, the following problems were experienced: The hardware was insufficient for the department's workload, the software design was limited, and personnel were frustrated with learning to use the system. These problems were intensified by the heavy workload and the large number of users. In the 18 months since implementation, the department has purchased more hardware, improved the software applications, and resolved many of the problems associated with employee frustration. Pharmacy departments at other large institutions might avoid some of these problems by training personnel adequately before implementation and by researching and estimating hardware and software needs in advance. In this large hospital, the efficiency of a stand-alone packaged pharmacy computer system has improved 18 months after implementation.

[Packaging in the process of radiation sterilization. II. Physicochemical studies].

The penetrability of the ionizing radiation through the matter makes possible the sterilization of the medical devices in the packed form by radiation method. The effect of the radiation should not bring any destructive changes in the material used for the package. In this paper have been discussed the results of the investigations of the one--and multilayer packaging materials from the point of view their utility for the radiation sterilization purposes. The changes of the useful parameters of the investigated materials have been determined in the dependence on the absorbed dose immediately after irradiation and the period of the durable keeping after the sterilization. The results of the mechanical investigations in the correlation to the results of the microbiological effects of the sterilized materials enabled to draw practical conclusions concerned the usability of the particular packages.

A software package for administering and monitoring the Environmental Symptoms Questionnaire (ESQ-III).

The latest version of the Environmental Symptoms Questionnaire (ESQ-III) contains 67 symptoms designed to allow researchers to evaluate a broad range of environmental stresses. We have developed an interactive computer software package that administers and monitors the ESQ-III. This package, written in a format maximizing clarity, provides consistency of administration from one test or day to another, checks for response inconsistencies, maintains subject motivation, provides feedback to the subject and allows an investigator to quickly inspect raw and computed results. Further, because there is no interaction between investigators and subjects, no experimenter bias can be introduced. This package can be adapted to almost any computer system having a CRT and at least one disc drive.

Use of a treatment package in the management of a profoundly mentally retarded girl's pica and self-stimulation.

A treatment package was employed to manage the pica and self-stimulation of a 4-year-old profoundly mentally retarded girl. Each instance of pica was followed by a verbal reprimand, the removal of the particular objects/fingers from the subject's mouth, and physical restraint. Each instance of self-stimulation was followed by response interruption and positive practice overcorrection. In both cases, praise was contingent upon the absence of target behaviors. A multiple-baseline design across settings was employed. The treatment package resulted in rapid and dramatic decreases in the levels of pica and self-stimulation in each setting. These results significantly contributed to the reduction of the cost of treatment. Results suggest that efficacy and efficiency of treatment interventions should be emphasized when dealing with multiple problems of profoundly retarded children and special emphasis should be placed on the treatment package strategy to enhance the efficiency of a given therapeutic technique.

Educational package on hypertension for primary care physicians.

The usefulness of an educational package on hypertension that provides clinically important, up-to-date medical information and office "aids" to primary care physicians was tested in a randomized controlled trial. Fifty-six physicians completed a pretest multiple-choice questionnaire and were allocated at random either to a group that received the educational package (the "study group") or to a control group. There was a highly significant correlation between the pretest scores and the number of years since graduation (r = -0.55, p less than 0.0001), which indicated that younger physicians are more likely than older physicians to have an up-to-date knowledge of the management of hypertension. The increase in knowledge in the study group (17.5%) was significantly greater than that in the control group (2.7%). Furthermore, although the post-test scores in the control group were still significantly correlated with the number of years since graduation, those in the study group were not. It was concluded that although the older physicians knew less than their younger colleagues about hypertension, the use of the educational package significantly increased knowledge, and the increase was not limited by the physician's age.

Headspace sampling and gas-solid chromatographic determination and confirmation of greater than or equal to 1 ppb vinyl chloride residues in polyvinyl chloride food packaging.

The determination and confirmation of residual vinyl chloride (VC) in polyvinyl chloride (PVC) food packaging materials are described. PVC packaging materials are dissolved in dimethylacetamide (DMAC). VC is sparged from solution with helium gas and collected in sealed vials of ethanol. VC is detected and quantitated by using a headspace sampling technique and standard gas-solid chromatography (GSC) with flame ionization detection. GSC peak heights of about 5% full scale deflection are obtained for samples of PVC containing 1 ppb VC. Polymer samples taken from tubing, blood bags, food packaging films, bottles, and unprocessed resin were analyzed. Residual VC levels ranged from 0.3 to 913 ppb. VC was confirmed by GSC-mass spectrometry, using selected ion recording of m/z 62 and 64 in conjunction with full mass scans to identify components eluting near VC.
