One question: credit or debit? As health savings accounts gain in popularity, insurers and the financial services industry want to bank the cash.

The Blue Cross and Blue Shield Association has a new sideline-banking. By chartering its own bank, the Blues is joining other insurers that have moved into financial services to adapt to the changes being wrought by health savings accounts. And other insurers have been faster to make the move. The Blues "is certainly late to the game" of banking, says analyst Katy Henrickson, left.

[The popular use of therapeutic herbs in body care].

UNLABELLED: It is about a quantitative and exploratory study aimed at describing the clients' knowledge and use of phytotherapy. The theoretical referral derived from anthropology. The adopted instruments were semi-structured forms applied to 300 users in a Health Polyclinic in Niterói, Rio de Janeiro, Brazil. RESULTS: 60,4% of the subjects use herbs and 63,4% have acquired the knowledge on medicinal plants with the family. The reasons for using herbal therapy were: sensation of improvement (32,2%) and the natural character of the herbs (33,5%). The adoption of herbs meets the subjectivity of the subject in his/her social and cultural context and remains as a complement or supplement to the official therapeutics.

Aristolochic acid-related nephropathy associated with the popular Chinese herb Xi Xin.

Chinese herbs nephropathy is known as a subacute interstitial nephritis attributed to aristolochic acid. This work describes the case of a 49-year-old male who displayed subacute renal failure induced by ingestion of herbal powder containing Xi Xin, which includes aristolochic acid. Since Xi Xin is a common ingredient in traditional formulae, care needs to be taken in the future to identify the aristolochic acid concentration of different components of Xi Xin. Xi Xin containing aristolochic acid should be forbidden for use in remedies in order to prevent the harmful effects of aristolochic acid.

Electromyographic analysis of four popular abdominal exercises.

This study was designed to evaluate the effects of four specific sit-up exercises on muscular activity of the rectus abdominis. Pairs of surface electrodes were placed unilaterally on four quadrants of the rectus abdominis, delimited by tendinous inscriptions, in four male subjects. Electromyographic (EMG) recordings were taken while the subjects performed four different abdominal exercises. Each abdominal exercise was hypothesized to have a specific effect on one of the four quadrants of the rectus abdominis. The four exercises analyzed were: 1) long lying crunch, 2) bent knee crunch, 3) leg raise, and 4) vertical leg crunch. Analysis of the standardized EMG recordings demonstrated no significant differences in the mean muscle activity between the four different quadrants, in the mean muscle activity between the four different exercises, and in interactions between the exercises and the quadrants of the rectus abdominis. We conclude that none of the four abdominal exercises studied are specific for strengthening individual muscle quadrants of the rectus abdominis.

The effect of cooking method upon the titratable acidity of a popular vegetarian dish--scope for reducing its erosive potential?

The purpose of this in vitro study was to determine the effect of cooking method on the erosive potential of ratatouille. Two cooking methods, stewing and oven roasting, were applied to standardised ingredients taken from the same fruits and vegetables. The resultant dishes were liquidised and diluted with 100 mls of distilled water. Five 25 ml samples of each group were titrated to pH 7.0 against 0.1 Molar Sodium Hydroxide. In order to ascertain the relative contribution of each ingredient each was singly prepared and cooked by stewing or oven roasting following the same quantities and dilutions as for the dish as a whole. 25 ml samples of these were titrated to pH 7.0 against 0.1 Molar Sodium Hydroxide. For ratatouille as a dish significantly (P < 0.0001) more alkali had to be added to the oven roast group (Mean = 8.60 mls, S.D. = 0.31) than to the stewed group (Mean = 3.92 mls, S.D. = 0.50) in order to bring about neutrality. Oven roasting of ingredients significantly (P < 0.001) lowered the initial pH with the exceptions of tomatoes and red peppers. Stewing reduced the volume of alkali required to neutralise aubergine (P < 0.001), green peppers (P < 0.001) and courgettes (P < 0.05). Significantly (P < 0.001) more alkali however was required to neutralise stewed red pepper. It was concluded that the method of cooking identical ingredients affects the erosive potential of ratatouille. Although oven roasting results in a higher erosive potential of ratatouille compared to stewing the method of cooking, at an individual ingredient level, does not have a universal effect upon erosive potential as determined by titratable acidity. This should be borne in mind when advising patients.

[Cancer initiating cell theory: popularity and controversies].

The cancer stem cell model proposes that most tumors are derived from a single cell that is transformed into a cancer-initiating cell (cancer stem cell). Cancer stem cells have the capacity to proliferate, differentiate, and form tumors in vivo. However, the origin of cancer stem cells remains controversial. Normal stem cells are regarded as an ideal candidate for the origin of cancer stem cells when take similar characters and signaling pathways between them into consideration. In addition, cell fusion is an important physiologic process during development and tissue repair, and is closely related to several fundamental features of tumors, and thus could be involved in the development of cancer stem cells.

[Popular nosology of infantile diseases in West Côte-d'Ivoire and consequences for malaria care management].

In a rural area of intense and permanent malaria transmission in Southwest Côte-d'lvoire, traditional midwifes of the Yacouba ethnic group, with also an important function for the children health and care, were interviewed in 2002 about their knowledge of the infantile pathologies. Their nosology is greatly based on symptoms and etiologic explanations of the disorder of secretions. The accumulation of a viscous liquid in different parts of the body, especially in the chest, the throat or the head, explains numerous febrile or afebrile diseases, including malnutrition. Some pathologies, particularly convulsions, are described by analogy with animals behaviour Relations between these entities and the biomedical ones are difficult to establish. The traditional care and treatments result from these concepts. A vomiting child or a child suffering from diarrhoea is subject to devices to evacuate his excess of liquid. Yellow brews are used against jaundice. Furthermore, an important mistrust remains towards medical treatments particularly for all parenteral therapies. Health facilities are only used as a the last resort. Their bad reputation is confirmed by the high rate of mortality of patients coming often too late. To improve malaria care management, health-care workers have to take into consideration these concepts and also prove their abilities to ensure good medical practices.
