[Perforating stomach ulcer in newborns].

Acute peptic ulceration of the stomach and duodenum is rare in children. The surgical treatment in the event of complications should be kept as simple as possible and there is usually no ulcer diathesis present. The significance of the pneumoperitoneum without visceral rupture is stressed. The gastrographin meal should be considered if ulcer perforation is suspected.

[Differential indications in perforation of gastroduodenal ulcer].

The differential indication of perforated peptic gastroduodenal ulcer is presented giving emergency surgical intervention preference. Simple closure after radical excision with a mortality rate of 12.1% as demonstrated in our clinical material is generally performed and in some selected cases the so-called primary definitive therapy excision, SPV or distal gastric resection. In cases of doubt simple closure is performed. An absolute acute indication for simple closure is also valid for the atypical perforation.

[Surgical treatment of acute peptic hemorrhage associated with perforation of gastroduodenal ulcer].

In 1982-1996 yrs of 8822 patients with ulcer bleeding in 122 (1.4%) it was concurrent with gastroduodenal ulcer perforation. In 34 (27.9%) simultaneous perforation and bleeding were observed, in 88 (72.1%) perforation occurred in 1-7 days after bleeding, in 48 (54.5%) it was concurrent with recurrent bleeding. Postoperative mortality lowered from 25 to 7.1% due to application of the principles elaborated.
