Structural Chemistry of Poly(ethylene glycol) Complexes of Lead(II) Nitrate and Lead(II) Bromide.

The reaction of 1:1 stoichiometries (1:1.5 for the nitrate/tetraethylene glycol (EO4) and pentaethylene glycol (EO5) complexes) of PbX(2) (X = NO(3), Br) with five- to eight-donor poly(ethylene glycols) (PEGs) in 3:1 CH(3)CN/CH(3)OH (CH(3)CN only for the nitrate/EO5 complex) followed by solvent evaporation resulted in six crystalline materials upon which X-ray structural analyses were carried out: [Pb(NO(3))(2)(EO4)](n)(), [Pb(NO(3))(2)(EO5)], [Pb(NO(3))(2)(EO6)], [PbBr(EO5)(&mgr;-Br)PbBr(2)].H(2)O, [PbBr(NCMe)(EO6)](2)[PbBr(2)(EO6)][PbBr(3)](2), and [PbBr(EO7)][PbBr(3)]. The nitrates crystallize as tight ion pairs with the PEG ligands coordinating in an equatorial plane around the Pb(2+) ions. Because EO4 has only five oxygen donors, this complex exhibits steric unsaturation which is overcome by a monodentate interaction with a third nitrate anion that is also coordinated to a neighboring Pb(2+) ion. The six donors of EO5 coordinate in an equatorial plane resulting in a 10-coordinate complex with trans, twisted, bidentate nitrate anions. The seven-donor hexaethylene glycol (EO6) only uses six of its oxygen donors to coordinate Pb(2+). [Pb(NO(3))(2)(EO4)](n)() is monoclinic, P2(1)/c, with a = 7.902(3) Å, b = 22.136(6) Å, c = 8.910(2) Å, beta = 90.96(3) degrees, and Z = 4. [Pb(NO(3))(2)(EO5)] is triclinic P&onemacr;, with a = 9.332(3) Å, b = 10.025(3) Å, c = 11.688(4) Å, alpha = 68.41(3) degrees, beta = 68.39(3) degrees, gamma = 68.58(3) degrees, and Z = 2. [Pb(NO(3))(2)(EO6)] is monoclinic P2(1)/c, with a = 16.289(4) Å, b = 10.773(4) Å, c = 12.329(4) Å, beta = 106.77(2) degrees, and Z = 4. Lead(II) bromide complexes with PEGs tend to crystallize as PEG complexed cations with polymeric lead(II) bromide anions. In the EO5 complex, bromide anions in the polymer also coordinate to the PEG-wrapped Pb(2+) cations. The hexa- and heptaethylene glycol (EO6 and EO7, respectively) complexes contain discreet ions. In these halide complexes, EO7 is the only PEG to expand the Pb(2+) coordination number from eight to nine. [PbBr(EO5)(&mgr;-Br)PbBr(2)].H(2)O is triclinic P&onemacr;, with a = 7.922(6) Å,b = 15.802(9) Å, c = 19.001(9) Å, alpha = 73.19(8) degrees, beta = 88.91(9) degrees, gamma = 87.22(9) degrees, and Z = 4. [PbBr(NCMe)(EO6)](2)[PbBr(2)(EO6)][PbBr(3)](2) is monoclinic P2(1)/c, with a = 14.389(4) Å, b = 31.931(9) Å, c = 8.029(2) Å, beta = 97.76(3) degrees, and Z = 2. [PbBr(EO7)][PbBr(3)] is monoclinic Cc, with a = 13.165(3) Å, b = 24.732(5) Å, c = 8.007(1) Å, beta = 94.58(2) degrees, and Z = 4.

Lead extraction via the femoral artery of a left ventricular pacing lead inserted in the subclavian artery.

We describe the extraction of a pacing lead via the femoral artery 5 months after it was inadvertently introduced through the right subclavian artery and positioned in the left ventricle. The lead was dislocated from the myocardium by traction from the subpectoral area and subsequently removed with a retriever inserted through the femoral artery in order to minimize the risk of cerebral embolization and bleeding at the entry site. Bleeding was controlled at the subclavian artery entry site by temporary inflation of a balloon catheter inserted through the brachial artery, and at the insertion site of the retriever in the femoral artery with a closure device.

Globin chain synthesis in single erythroid bursts from cord blood: studies on gamma leads to beta and G gamma leads to A gamma switches.

Erythroid bursts from cord or adult blood were grown in methylcellulose cultures (3 international units of erythropoietin per plate). On day 13, single bursts were picked up and reincubated for 16-24 hr with [3H]leucine. Radioactive globin chains [alpha, beta,G gamma, and A gamma (Ala-136)] were analyzed by either isoelectric focusing on polyacrylamide gels and fluorography or carboxymethylcellulose chromatography. In all cases, alpha to non-alpha globin radioactivity ratios were close to 1. In single cord blood bursts, the values of both gamma-to-beta and G gamma-to-A gamma ratios were spread over a large spectrum and further characterized by a continuous rather than a bimodal distribution. Morever, the G gamma-to-A gamma ratios demonstrated in single bursts appeared to be directly correlated with the respective gamma-to-beta ratios. These data suggest that both the gamma leads to beta and the G gamma leads to A gamma switches are mediated via mechanisms modulating the relative activities of the different genes in the non-alpha globin gene cluster rather than via selection of clones committed to the preferential synthesis of beta and A gamma globins. In contrast with the results obtained with cord blood, individual adult blood bursts synthesize a lower and hence relatively more uniform amount of gamma globin chains.

Information loss in limb-lead electrocardiograms compared with twelve-lead tracings in a population survey among the elderly.

A total of 235 telve-lead electrocardiogram tracings representing all those traces with positive Minnesota codings from a population sample of persons aged 65 and over were re-coded on the basis of the six limb-leads alone. The over-all loss of information amounted to about 22.3 per cent of major abnormalities, as defined, but was higher than 50 per cent for some individual abnormalities such as abnormal Q and QS waves. Possible implications of these findings are discussed.

An improved direct intermicroscopic (LM leads to SEM leads to TEM) correlative procedure for the examination of mammalian skeletal muscle.

The procedure for direct intermicroscopic (LM leads to SEM leads to TEM) correlation of muscle was modified. The major modification was the use of cryostat sections which showed far better ultrastructural preservation than did paraffin sections. The use of an osmium-thiocarbohydrazide ligand binding procedure (OTOTO) did not noticeably improve the scanning electron microscopic (SEM) image, but did result in a subsequent transmission electron microscopic (TEM) image which showed increased contrast of the intracellular membranous components. Besides improving the preservation at the TEM level of examination, the techniques used to locate the same structure or area previously examined under the complementary modes of microscopy (i.e. LM and SEM), were also refined.

Upgrading patients with chronic defibrillator leads to a biventricular system and reducing patient risk: contralateral LV lead placement.

Increasing numbers of patients with indwelling single- or dual- chamber internal cardioverter defibrillators (ICDs) will require upgrading of an existing system to a biventricular ICD providing cardiac resynchronization with back-up defibrillation. Upgrading, usually by the addition of a new left ventricular (LV) lead, can be technically challenging with central venous occlusion or stenosis often being the main obstacle to a successful procedure. We report a new technique of implanting a LV lead from the contralateral side to the existing ICD system to minimize the peri- and postoperative risk to the patient.

To lead or not to lead? Prospective controlled study of emergency nurses' provision of advanced life support team leadership.

BACKGROUND AND OBJECTIVES: In many emergency departments advanced life support (ALS) trained nurses do not assume a lead role in advanced resuscitation. This study investigated whether emergency nurses with previous ALS training provided good team leadership in a simulated cardiac arrest situation. METHODS: A prospective study was conducted at five emergency departments and one nurses' association meeting. All participants went through the same scenario. Details recorded included baseline blood pressure and pulse rate, time in post, time of ALS training, and subjective stress score (1 = hardly stressed; 10 = extremely stressed). Scoring took into account scenario understanding, rhythm recognition, time to defibrillation, appropriateness of interventions, and theoretical knowledge. RESULTS: Of 57 participants, 20 were ALS trained nurses, 19 were ALS trained emergency senior house officers (SHOs), and 18 were emergency SHOs without formal ALS training. The overall mean score for doctors without ALS training was 69.5%, compared with 72.3% for ALS trained doctors and 73.7% for ALS trained nurses. Nurses found the experience less stressful (subjective stress score 5.78/10) compared with doctors without ALS training (6.5/10). The mean time taken to defibrillate from the appearance of a shockable rhythm on the monitor by the nurses and those SHOs without ALS training was 42 and 40.8 seconds, respectively. CONCLUSION: ALS trained nurses performed as well as ALS trained and non ALS trained emergency SHOs in a simulated cardiac arrest situation and had greater awareness of the potentially reversible causes of cardiac arrest. Thus if a senior or middle grade doctor is not available to lead the resuscitation team, it may be appropriate for experienced nursing staff with ALS training to act as ALS team leaders rather than SHOs.

Cytogenetic and clinical studies in gonadal dysgenesis with 46,X,Xt(qter leads to p221::p223 leads to qter) karyotype: review and phenotype/karyotype correlations.

Chromosome analysis by Q, R, and C banding was performed in a case diagnosed clinically as gonadal dysgenesis and the karyotype was shown to be 46,X,Xt(qter leads to p221::p223 leads to qter). Localisation of the breakpoints in the fused X chromosomes and replication studies have led to a hypothesis on the origin of the translocation. A comparison of clinical and cytogenetical findings in this and other published cases has also been made in an attempt to detect some phenotype/karyotype correlations.

Are there differences among patients with inferior acute myocardial infarction with ST depression in leads V2 and V3 and positive versus negative T waves in these leads on admission?

We assessed whether there are differences among patients with inferior acute myocardial infarction and ST segment depression in leads V2-V3 between those with positive versus negative T waves on admission. We found no differences in the prevalence of concomitant significant left anterior descending coronary artery stenosis and anterior or posterior wall motion abnormalities on echocardiography between the two groups. Precordial ST segment depression with negative T wave may be the early electrocardiographic pattern of posterior infarction, reflecting ST segment elevation with positive T waves that would have been detected by leads facing the posterior wall.

Hb I alpha16 Lys leads to Glu and Hb Broussais alpha90 Lys leads to Asn in Australian families.

This paper describes the finding of Hb 1 alpha16 Lys leads to Glu and Hb Broussais alpha90 Lys leads to Asn in Australian families. Neither of these variants has been previously described in the Australian population. The variants were detected in an electrophoretic screening of 2500 blood samples. Both variants were clinically silent. The haematological parameters were within normal limits and the peripheral blood morphology was normal.

Two new hemoglobins. Hemoglobin Alabama (beta39(C5)Gln leads to Lys) and hemoglobin Montgomery (alpha 48(CD 6) Leu leads to Arg).

The amino acid substitutions in two new hemoglobins found by electrophoretic screening during a survey in Alabama have been determined by column chromatography and amino acid analyses of their tryptic peptides. They are hemoglobin Alabama (beta 39(C 5)Gln leads to Lys) and hemoglobin Montgomery (alpha 48(CD 6) Leu leads to Arg). No harmful symptoms have been attributed to the presence of either hemoglobin.

Chromosome 3 duplication q21 leads to qter deletion p25 leads to pter syndrome in children of carriers of a pericentric inversion inv(3) (p25q21).

Close phenotypic similarity between two cases carrying a rec(3) dup q,inv(3) (p25q21), 12 additional infants from the same inv (3)(p25q21) kindred who lived less than 1 year, and eight cases studied in other medical centers has led us to postulate the existence of a distinct chromosome 3 duplication-deletion syndrome. In the presence of trisomy for (3)q21 leads to qter and monosomy for (3)p25 leads to pter, the facial dysmorphy is unique: a distorted head shape due to irregular cranial sutures, thick low eyebrows, long eyelashes, persistent lanugo, distended veins on the scalp, hypertelorism, oblique palpebral fissures, a very short nose with a broad depressed bridge and anteverted nares, protruding maxilla, thin upper lip, micrognathia, low-set ears, and a short webbed neck. Port-wine stains, congenital glaucoma, cloudy corneas, cleft palate and harelip also occur frequently. Each infant has difficulty sucking and swallowing. Congenital anomalies of the cardiovascular system, of midgut rotation, and of the urogenital system are noted for the infants who died neonatally. Most frequent is a ventricular septal defect, followed by atrial septal defect, patent ductus arteriosus, patent foramen ovale, and coarctation of the aorta. Omphalocele, umbilical hernia, hyperplastic kidneys, polycystic kidneys, double ureter, hydro-ureter, hydronephrosis, and undescended testes often occur. The extremities are short in proportion to the length of the trunk. Clinodactyly, coxa valga, talipes, and spina bifida are frequently observed.

[Lateralization of saccadic latency during monocular presentation of stimuli to a leading and non-leading eye].

Saccadic latencies were studied in ten healthy subjects. Peripheral targets were presented monocularly to a leading and nonleading eyes in the right and left hemifields. SS (single step) and OVERLAP (200 ms) schemes of visual stimulation were used. Under OVERLAP conditions, the saccadic latency was longer by 30-39 ms and the number of long-latency saccades was higher than under SS conditions, especially in subjects with mixed asymmetry profiles. In the majority of subjects with right asymmetry profile, the latencies of saccades during stimulation of the leading eye were by 12 ms shorter than during stimulation of the nonleading eye, and the latencies of right saccades were by 24 ms shorter than that of the left saccades independently of the stimulated eye. The obtained results explain some characteristic features of hemyspheric asymmetry in organization of saccadic movements.

Partial duplication 5q syndrome: phenotypic similarity in two sisters with identical karyotype (partial duplication 5q33 leads to 5qter and partial deficiency 8p23 leads to pter).

Two sisters with statomotor developmental retardation microcephaly, hydrocephalus internus and externus without signs of pressure, heart defect (ventricular septal defect), early pulmonary resistance and characteristic facial changes were found to have the same unbalanced karyotype with partial trisomy 5q3300 leads to 5qter and partial monosomy 8p2300 leads to 8pter, derived from a balanced reciprocal paternal translocation: 46,XY,t(5;8)(q3300;p2300). The older girl was tested for the erythrocyte enzyme glutathion reductase. She had normal values.

Immunochemistry of Salmonella O-antigens: characterization of the antibody response to Tyvelose 1 leads to 3 alpha mannose 1 leads to bovine serum albumin representative of Salmonella 0-antigen 9.

Immunization of rabbits with the synthetic disaccharide-protein conjugate, tyvelose 1 leads to alpha 3 mannose 1 leads to bovine serum albumin (TM-BSA) in Freund's complete adjuvant, gave rise to antibodies directed against both the disaccharide hapten and the carrier protein. The hapten antibodies were specific for O-antigen 9 of Salmonella and were superior in terms of specificity to conventional O factor 9 antibodies, as judged by passive haemagglutination and enzyme-linked immunosorbent assay (ELISA). A complement dependent bactericidal assay demonstrated that the TM-BSA antibodies had bacteriolytic activity also directed against the O-antigen 9 Salmonella. The high specificity was evident in both the IgG and IgM antibody classes. In the bactericidal assay the IgM antibodies were more active than antibodies of the IgG class. The results suggested that IgG antibodies in hyperimmune serum blocked the complement dependent bactericidal activity of IgM antibodies.

Purification to homogeneity of a beta-galactoside alpha2 leads to 3 sialyltransferase and partial purification of an alpha-N-acetylgalactosaminide alpha2 leads to 6 sialyltransferase from porcine submaxillary glands.

Two different sialyltransferases (EC 2.4.99.1) have been resolved from Triton X-100 extracts of porcine submaxillary glands by affinity chromatography on CDP-hexanolamine agarose. The predominant sialyltransferase of this tissue, a CMP-N-acetylneuraminate: alpha-D-N-acetylgalactosaminide alpha2 leads to 6 sialyltransferase, has been obtained in a partially purified and stable form. A less abundant but highly active enzyme, a CMP-N-acetylneuraminate: beta-D-galactoside alpha2 leads to 3 sialyltransferase, was purified over 90,000-fold to homogeneity. Chromatography of the latter enzyme on Sephadex G-200 separated two noninterconverting forms, designated A and B, with Stokes radii of 51 A and 31 A, respectively. Both forms have equal specific activity toward lactose and contain a single polypeptide with a molecular weight of about 50,000 as estimated by gel electrophoresis. Form A appears to bind 1.18 g of Triton X-100 per g of protein, or nearly an entire detergent micelle per polypeptide, while Form B binds little or no detergent. The enzymatic properties of both forms are similar (Rearick, J.I., Sadler, J.E., Paulson, J.C., and Hill, R.L. (1979) J. Biol. Chem. 254, 4444-4451) supporting the conclusion that Form A may represent the native sialyltransferase with an intact membrane-binding site, and Form B may be a large proteolytic fragment of Form A.

Erythroglycan, a high molecular weight glycopeptide with the repeating structure [galactosyl-(1 leads to 4)-2-deoxy-2-acetamido-glucosyl(1 leads to 3)] comprising more than one-third of the protein-bound carbohydrate of human erythrocyte stroma.

Glycopeptides of molecualr weight range 7,000 to 11,000, unusual in size and structure, have been partially purified from pronase digests of lipid-free human erythrocyte ghosts; we term this fraction "erythroglycan." These substances comprise about one-third of the galactose and glucosamine of the ghost. Methylation analysis of erythroglycan yields mainly 4-linked glucosamine, 3-linked galactose, and 3,6-linked galactose, along with mannose and fucose derivatives. Hydrazinolysis and nitrous acid deamination degrade erythroglycan to galactosyl-2,5-anhydromannose, indicating a repeating structure of galactosyl, (1 leads to 4)-2deoxy-2-acetamidoglucosyl (1 leads to 3). Digestion with the endo-beta-galactosidase from Escherichia freundii gives only partial cleavage of the erythroglycan, probably because of the arborized structure indicated by the branched galactose. Since sphingosine is not detectable after methanolysis by chemical ionization mass spectrometry, and since amino acids are present, we conclude that these substances are probably glycopeptidic in origin and are not "macroglycolipids." Erythroglycan may have the same type of keratan-like core structure as the long chain blood group glycolipids from human erythrocytes and could be a protein-bound carrier of the ABO determinants.

Translocation of 9q/13q resulting in duplication (trisomy 9pter leads to 9q22) and deficiency (monosomy 13pter leads to 13q12).

A profoundly retarded, 12-year-old female is described. Her phenotype is compatible with the clinical features of the trisomy 9p syndrome. Cytogenetic analyses showed her to be trisomic for 9pter leads to 9q22 and monosomic for 13pter leads to 13q12, as the result of adjacent-2 segregation during meiosis in her mother. The family pedigree shows this (9;13) translocation to be present in at least three generations.
