Experience with duodenal necrosis. A rare complication of acute necrotizing pancreatitis.

Duodenal necrosis is a rare, but very serious complication of acute necrotizing pancreatitis that most likely is the result of vascular compromise and ischemia of the peri-Vaterian aspect of the duodenal wall. In this article, we present three patients with duodenal necrosis complicating acute necrotizing pancreatitis. The diagnosis was made at the time of necrosectomy. Management options of this challenging complication of necrotizing pancreatitis are discussed.

The therapeutic potential of somatostatin receptor ligands in the treatment of obesity and diabetes.

Since the development of synthetic somatostatin analogues, several therapeutic applications for somatostatin receptor agonist molecules have been defined. Established applications for somatostatin analogue treatment include pituitary tumours (growth hormone and thyrotropin-secreting adenomas), neuroendocrine tumours of the pancreas and gastrointestinal tract (so-called carcinoid tumours, vasoactive intestinal tumours) and gastroenterological conditions (pancreatitis, gastrointestinal bleedings, refractory diarrhoeas, pancreatic and intestinal fistulas, diarrhoea in AIDS). Further areas for development of somatostatin analogue therapy include obesity, polycystic ovary syndrome and diabetes mellitus, dysmetabolic conditions that are often interrelated. The challenge for the future will be to transform results from clinical trials conducted in heterogeneous clinical situations into novel options of somatostatin analogue use. Since obesity and diabetes mellitus both are disorders of marked heterogeneity, the subgroup of patients that will benefit most from somatostatin analogue treatment has yet to be defined. In addition, the development of more universal ligands covering all of the known somatostatin receptor molecules as well as receptor subtype specific agents is currently underway. The establishment of slow-release depot formulations of octreotide and lanreotide has already provided a more acceptable and consistent delivery mechanism. Use of biodegradable polymer microsphere formulations provides the basis for the development of novel applications, which include hyperinsulinaemia, obesity and polycystic ovary syndrome as components of the dysmetabolic syndrome. The most developed thus far is the use of octreotide in hyperinsulinaemic forms of obesity and in distinct stages of diabetic retinopathy.

Surgical treatment for Crohn's disease, with special reference to operative procedures and their relationship to recurrence.

Of 14 patients with Crohn's disease, 12 to whom operation was performed at our department were studied with regard to operative procedures and their relationship to recurrence. The following results were obtained: 1) Resection of the intestinal lesions was performed in all patients. The recurrence rate was as high as 42% (5 of 12 cases). 2) If the length of the normal intestine resected was more than 10 cm, there was no correlation between the length of the resection and recurrence of the disease. 3) The postoperative recurrence rate tended to decrease by means of regional lymph nodes dissection. From these results, it is concluded that the recommendable operative procedures for Crohn's disease are intestinal resection including the normal intestine more than 10 cm in length and regional lymph nodes dissection.

Gallstone ileus. The importance of contrast examinations in the roentgenographic diagnosis.

Our experience based on the analysis of 12 proved cases of gallstone ileus is presented emphasizing the fact that the plain abdominal roentgenogram may be inconclusive or deceiving. It allowed us to suggest the correct diagnosis in only 4 cases. A high index of suspicion, particularly in elderly female patients, followed by barium examination of the upper gastrointestinal tract leads to a correct and rapid diagnosis. The demonstration of a diverticulum-like structure or a fistulous tract adjacent to the first portion of the duodenum associated with jejunal dilatation and barium dilution is highly reliable.

Air in gallbladder: a frequent finding in gallstone ileus.

A retrospective assessment of the presence of air in the biliary tract is presented, based on plain film findings in 16 proven cases of gallstone ileus. In addition to five cases that demonstrated air in the biliary radicles, four cases exhibited air exclusively in the gallbladder and duodenal bulb. A clearly identifiable radiographic appearance composed of two adjacent small air-fluid levels in the right upper quadrant is described. This radiographic feature represents the air- and fluid-filled duodenal cap adjacent to the visualized shrunken gallbladder. Recognition of this combination of findings helps in establishing a prompt preoperative diagnosis in cases of gallstone ileus.

[Experiences in management of acute necrotic pancreatitis].

To establish a treatment program for patients with acute necrotic pancreatits (ANP), we treated 211 patients with ANP from 1973 to 1995. 122 patients in group A were treated mainly by early operation before 1992 and 75 in group B, by early nonoperativemethod. The results showed that the occurrance of complications (ARDS, renal failure, heart failure, pancreatic abscess, intestinal fistula) was lower than that in the group B (P < 0.01). Also the mortality in the group B (10.67%) was decreased as compared with that (22.95%) in the group A. We suggested that the nonoperative treatment with rigid criterion for ANP might be recommended first. An intensive nonsurgical management of ANP, surgical indications during nonoperative period and complete program of management for ANP were discussed.
