Vesicular transport: implications for cell polarity.

In polarized cells intracellular sorting of plasma membrane proteins occurs to a large extent at the trans-Golgi network, giving rise to vesicles destined for distinct plasma membrane domains. This review discusses the several pathways, both direct and indirect, which lead to protein incorporation into the correct cell surface, as well as the mechanisms involved. Proteins contain signals which direct their incorporation into the distinct vesicles destined for plasma membrane microdomains. Specific coat proteins are involved in vesicle assembly and are likely to play a role in the generation of discrete vesicle populations. Molecules involved in vesicle docking and fusion may also add specificity to the targeting process.

[Study of medical-legal consultations outside the health system in primary care].

OBJECTIVE: To evaluate how consultations in order to obtain a medical/legal document, whose destination is other than the health service. DESIGN: A descriptive, crossover study, using a questionnaire filled in by the doctor at the time of the medical-legal consultation. PATIENTS AND OTHER PARTICIPANTS: All those users who attended in one year the clinics of the doctors taking part in the study, in order to seek a medical-legal document (MLD) whose destination was other than the health service. MEASUREMENTS AND MAIN RESULTS: Between 1-9-93 and 31-8-94 there were 287 consultations for a MLD. Of the variables studied, most attendances were by the age group between 15 and 44, masculine, with schooling up to EGB level (basic) or lower and unemployed or pensioner. Average length of the consultation was 6.6 minutes. 42.5% had a clinical interview. 4.6% were referred to a specialist. In 77.7% of cases, a document belonging to the health service was issued. The reason for the procedure was: health certificate (40%), and a sick note (39%). CONCLUSIONS: The PC doctor normally receives a considerable number of visits to obtain this kind of document, which are required by official bodies to determine the health status of users or concede social benefits to them. There are gaps in the rules on what kind of document.

Guidelines for the prevention of malaria in travellers from the United Kingdom. PHLS Malaria Reference Laboratory, London School of Hygiene and Tropical Medicine.

These guidelines on malaria prevention are an aid to health care workers who advise travellers, particularly those who will be overseas for less than a year. They represent a virtual consensus of the views of 44 doctors, nurses, and pharmacists with special expertise in malariology or travel medicine who met to develop them in 1996 (see list on R152). The guidelines are in three parts. The first part is a summary that emphasises modifications to the advice given in the last set of guidelines, published in 1995. The second part discusses the issues addressed in formulating the guidelines. Doctors, practice nurses, and pharmacists are asked to read this section to avoid doing harm by giving chemoprophylaxis without due attention to the traveller's history or destination and by using oversimplified lists of recommendations by country. The second part also addresses the health care worker's consultation with prospective travellers. The third part gives specific recommendations for travellers to specific destinations and some details of individual drugs. Fuller information on some points was given in earlier versions of the guidelines, which should not be discarded. Meetings of the sort described above have been held since 1980 and the group's membership has included people with varied views and experience. The views expressed in these guidelines reflect experienced professional opinion, since data are inadequate for unequivocal views to be given on several issues. There is often a range of acceptable options, but to meet the requests of general practitioners the guidelines try to give one recommended option and state alternatives, suggesting when and how different regimens can be used to good effect. Decisions on the terms under which different drugs are licensed for use are the responsibility of the Licensing Authority, advised by the Committee on Safety of Medicines (not of these guidelines). The guidelines should be read as a supplement to and not as a substitute for the relevant data sheets. Chemoprophylaxis lies somewhere between vaccination (for which people expect governments to lay down schedules) and treatment of ill people (for which each physician does what seems most appropriate) in concept and practice. The risks of malaria need to be balanced against the risks of the preventive measures, on the basis of the data available. Travellers may ask for an explanation of these risks and doctors and practice nurses need to be well informed and able to present their knowledge to travellers. The second part of these guidelines may be of use to prospective travellers who wish to read about the options themselves. All readers are recommended to read part two in its entirety to get a balanced picture.

Case costing means, measuring and managing now! The journey traveled by a community hospital.

Funding for health care in Ontario is moving from global funding to equity funding. In the future, hospitals will be reimbursed for how efficiently they care for their various patient populations. The Ontario Case Costing Project (OCCP) was a joint venture by the Ontario Hospital Association and the Ministry of Health. Incentive for participation in this project was based on the need to assess efficiencies in caring for patient populations in surgical suites and to obtain Canadian data. Case Costing has the potential to forecast budgets, identify variances and highlight areas for cost savings. Case Costing can also determine cost per surgeon, cost per service, cost per procedure. The nurses at Markham Stouffville Hospital are empowered to enhance the focus of their practice to include managing human resources, processes and materials. This enhanced focus in the Operating Room maximizes efficiency and effectiveness of processes, and allows the organization to provide better service. This article documents the journey and growth of perioperative nurses toward the destination of case costing. Key to this journey is not only the destination, but the growth and change that occurred and enabled perioperative nurses to effectively champion initiatives such as case costing. Opportunities and Threats, a One Page Plan and our recommended learnings will be shared.

[The elderly traveler].

An elderly traveler in good health is the same as any other traveler. However before departure elderly subjects should make sure that the proposed schedule of activities is compatible with their physical abilities. Medical counseling should be sought to determine that there are no conflicting health problems or physical impediments. Destination, itinerary, and transportation should selected accordingly. Immunization records should be checked and the main vaccination requirements for elderly subjects should be updated. To avoid running short of any prescribed mediation, an adequate supply for the whole trip must be packed. If air travel is planned, advice should be given on avoiding dehydration and thromboembolism. During his stay at the destination and especially in tropical areas, the subject should get adequate rest and guard against dehydration by drinking sufficiently, protecting against heat exposure, and controlling diarrhea promptly. Using these precautions, elderly subjects can travel as safely as possible.

[Which vaccination schedule, which vaccines? The constraints of time and age].

Several factors must be taken into account in planning vaccination schedules for overseas travelers. The first factor is to determine requirements mandated by applicable laws in the destination country and in France governing professional travel such as by military personnel. The other factors involve risk assessment including local health and epidemiological conditions, living conditions during the stay, and personal profile of the traveler (e.g. age and previous vaccination). Tropical areas are not the only destinations where infectious risks requiring vaccinations are found. Vaccination against diseases such as diphtheria and tick-borne encephalitis is necessary for several countries in Europe. Pre-travel planning provides a timely opportunity for updating basic vaccination requirements (e.g. tetanus and polio). For the growing number of elderly travelers, accurate evaluation of immune status may be difficult either because these subjects may never been vaccinated but only exposed to the wild germ during childhood or because their vaccinations may have been performed long ago. In both cases one cannot be sure of the quality of the anamnestic response to booster injections. A frequent limitation on vaccination planning for travelers is time available before departure.

[Neuroblastic migration: embryological aspects and mechanisms].

INTRODUCTION: The migration of immature neurons of the cerebrum is genetically programmed from the primitive neuroepithelium before the end of the final mitotic cycle. The orientation of the mitotic spindle determines when a neuroepithelial cell is ready to start migration and the proportion of major genetic material it is destined to receive. DEVELOPMENT: The gene LIS1, defective in lissencephaly type 1 of Miller and Dieker, is expressed in the neuroepithelial cells, in the ependyma and the Cajal-Retzius neurons. These transitory fetal cells are the first neurons of the cerebral cortex. Most of the neurons of the cortical plate arrive by means of glial radial cells which guide them towards their destination. Cell adhesion molecules from the neuroblasts themselves, the glial radial cell, the extracellular matrix and perhaps the ependymal cells are important in adhering the neuroblasts to the glial radial cells. Genetic deficiency of these molecules results in defective migration. The mechanism of cellular movement is still not fully understood. Disorders of migration may also be induced by non-genetic factors, such as infarcts or other lesions which damage or destroy the glial radial fibres during the fetal period.

Holidaying abroad: practical advice for a young family.

Try to tailor your advice to the individual family. Keep it simple and practical. Find out who is going, where and whether the family has any particular health needs. Encourage the family to plan ahead, finding out as much as they can about facilities for young children at their destination. Likely discussion points are vaccinations, infant and toddler feeding, managing the journey, avoiding sunburn and diarrhoea, and contraception. Depending on destination it may be advisable for the family to take some items with them from the UK, e.g. infant formula and feeding equipment, contraceptives, sun protection and insect repellents.

Basic fibroblast growth factor does not prevent heparan sulphate proteoglycan catabolism in intact cells, but it alters the distribution of the glycosaminoglycan degradation products.

Heparan sulphate proteoglycans on cell surfaces have been shown to mediate the degradation or recycling of several ligands. Since the interaction with ligand may affect proteoglycan catabolism once the complex is internalized, this could alter the cellular pool of heparan sulphate chains, with possible consequences for heparan sulphate-mediated cellular processes. We have recently demonstrated that the specific binding of basic fibroblast growth factor (bFGF) to heparan sulphate chains prevents the glycosaminoglycan from being degraded by partially purified heparanases from Chinese hamster ovary (CHO) cells [Tumova and Bame (1997) J. Biol. Chem. 272, 9078-9085]. The present study examines the effect of bFGF on heparan sulphate catabolism in intact cells. The distribution and size of the heparan sulphate degradation products in CHO cells was analysed in the presence and absence of bFGF using pulse-chase protocols. Although heparan sulphate molecules and bFGF are internalized through the same pathway, even relatively high concentrations of the growth factor do not have any inhibitory effects on glycosaminoglycan degradation. However, the interaction with the growth factor alters the distribution of heparan sulphate-degradation products, presumably by preventing secretion of the short heparanase-derived species. Our findings show that most of the free and bFGF-bound heparan sulphate chains are destined for lysosomes, which would be consistent with a recent hypothesis that the primary role of proteoglycan-mediated internalization of the growth factor is to remove bFGF from its site of action at the cell surface. However, in the presence of bFGF, a fraction of intracellular, heparanase-degraded heparan sulphate chains is delivered to the nucleus, suggesting that the glycosaminoglycan accompanies the growth factor to the organelle. It may be important for bFGF activity that the growth factor is protected from proteolytic degradation by its interaction with heparan sulphate. This work demonstrates that the internalization of a ligand along with the proteoglycan can affect the sorting of heparan sulphate-degradation products in endosomes, and the ultimate destination of the short glycosaminoglycan. It also provides evidence that formation of heparan sulphate-ligand complexes may regulate the recycling and degradation of both ligands and heparan sulphate chains and, consequently, affect their biological activities.

"An empire lost but a province gained": a cohort analysis of British international retirement in the Algarve.

"The growth of international retirement in the Algarve has coincided with a number of changes in the international framework for population mobility as well as in the nature of the Algarve [Portugal] as a destination area. Tourism development, which is intimately linked to subsequent retirement migration, is particularly important in this. This paper considers the nature of the link between cycles of migration and of development in recipient areas, in the context of the remarkable and relatively late development of the Algarve as an area of tourism and retirement.... Cohort analysis...provides the basis for an examination of changes over time in the socio-demographic profiles of the migrants, their motivations, their residential patterns and their integration."

Migration stock and the issue of competing and complementary flows in United States interstate migration.

"The purpose of this paper is to study the determinants of interstate migration in the United States from 1965-1970 when a new change in direction of migration has started, and to examine the flow creation or flow diversion that results from migration to some appealing regions. Several related variables have been selected and tested for gross interstate migration flows. The results show that overall both push and pull factors have not been important.... Distance was not found to act as a significant deterrence to migration, whereas population density of origin and destination was significant. Previous migration was found to have a very strong effect on migration. The results of the study also suggested that there has been a major change in the location of growth areas in the United States during 1955-1970."

Portuguese emigration 1958-1985: some empirical evidence.

"Supplying work in the home country or abroad must be seen as the result of the same decisional process. If that is true, the same set of variables should be used to explain the participation in the labor market and the emigration rates. Based upon empirical results [for Portuguese emigrants], we discuss some of the traditional conclusions of the economic literature. Our empirical results, for example, show that: (1) there is...strong support for considering home wages and the wages in the country of destination asymmetrically, (2) imperfections in the capital markets seem to play an important role when workers must pay for their moving expenses and (3) there is a differential in coefficients between the period before 1974 and after that date, as 1974 is the year most of the Central European countries changed their immigration policies."

Determinants of black interstate migration, 1965-70 and 1975-80.

"This paper presents and tests a regression-based model of black interstate migration. Explanatory variables include characteristics of origins and destinations, distance, and two migrant stock measures. The model is tested using black interstate migration flows published by the U.S. Bureau of the Census for 1965-70 and 1975-80." The emphasis is on the determinants of black migration, particularly for the period 1975-1980.

A second-phase diaspora: South African migration to Australia.

"South Africa has experienced a significant continuous outflow of professional and technical skills in recent decades. The international marketability of such skills has encouraged a 'brain drain' phenomenon of the type which is common from other developing countries.... The macrodynamics of international migration as a socio-cultural diffusion process. Part B: applications.

"This study formulates a model of the macrodynamics of international migration using a differential equation to capture the push-pull forces that propel it. The model's architecture rests on the functioning of information feedback between settled friends and family at the destination and potential emigrants at the origin.... Two specific paradigms of diverse nature serve to demonstrate the model's tenets and pertinence, one being Greek emigration to the United States since 1820, and the other total out-migration from Cyprus since statehood (1946)."

The effect of agricultural price-support policies on interregional and rural-to-urban migration in Korea: 1976-1980.

The impact of rice price-support policies, designed to increase farmers' income and reduce rural-urban migration in the Republic of Korea, is examined for the period 1976-1980 using a polytomous logistic model. "Our findings revealed that the elasticity of migration with respect to rice yield per origin farm household is positive and is significantly different from zero. The elasticities of migration with respect to rate of urbanization, particularly urban concentration-agglomeration, and population size of the destination are also positive and are significantly different from zero.... Our findings questioned the wisdom of employing rice price price-support programs as a viable policy for reducing interregional and rural-to-urban migration in Korea."

Does migration arbitrage regional labor market differentials?

"This paper examines the determinants of regional migration [in the United States] in the 1980s using a place-to-place migration model in which migration flows depend upon the relative economic opportunities in the origin and destination regions. Using the results of the model, we then examine the potential role for migration in diffusing the economic pressures associated with disparate regional economic growth. Although the results reinforce the notion that divergent regional growth was an impetus to migration in the 1980s, estimates of the employment status of movers suggest that the true effects of migration on unemployment differentials may be quite small."

The impact of overseas labour migration on rural Thailand: regional, community and individual dimensions.

"This paper provides a case study of the impact at origin of recent labour migration from rural Thailand to East Asian destinations. It does so through survey information on 63 villages and detailed biographic interviews with recently returned workers. It is concluded that work abroad is regarded by migrants as a strategy of life support; it is sometimes life-enhancing, but only rarely life-changing."
