13-cis-retinoic acid has been used as a single agent in the treatment of seventy-six patients with previously unresponsive cystonodular acne. The study was carried out in a double-blind fashion using three doses of the drug. Ninety per cent of the patients responded with a 70% improvement in the acne severity. Sixty-six per cent of the patients experienced no further problems with their acne during follow-up. Side-effects were frequent. The 0.5 mg/kg b.w. dose is recommended for the initial course of treatment.

The effects of combined treatment with the antiandrogen, cyproterone acetate, and ethinyl oestradiol on four women with long-standing hidradenitis suppurativa have been investigated. The condition was controlled successfully in all patients with 100 mg/day cyproterone acetate using the reversed sequential regimen; lowering the antiandrogen to 50 mg/day caused deterioration. Before treatment, plasma testosterone levels were within the normal range, but plasma androstenedione values were raised and sex hormone binding globulin levels were low. On treatment, the androstenedione concentration fell and sex hormone-binding globulin values were raised. However, since these levels were unaltered by reducing the antiandrogen dosage, the main action of the therapy is probably that of the antiandrogen within the target cells.

The principal functions of the skin include protection, excretion, secretion, absorption, thermoregulation, pigmentogenesis, accumulation, sensory perception and regulation of immunological processes. These functions are all affected by the structural changes in the skin with ageing and, after middle age, most functions are reduced, some by as much as 50-60%. The physiological changes associated with these reductions include impairment of the barrier function, decreased turnover of epidermal cells, reduced numbers of keratinocytes and fibroblasts, and a reduced vascular network particularly around hair bulbs and glands. These changes result in fibrosis and atrophy, and decreases in hair and nail growth, vitamin D synthesis and the density of Langerhans cells. Production of epidermal thymocyte-activating factor, which enhances the T-cell response, is reduced leading to a decrease in the immune response; there is also a decreased functioning of Meissner's and Pacinian corpuscles. An increased generation of free radicals is observed. Certain environmental factors, particularly exposure to sun, accelerate the ageing of skin and are important in cutaneous carcinogenesis.

BACKGROUND: In the past 30 years, many tests for assessing the irritancy of soaps have been introduced, but only very few tests for evaluating their cleaning properties. The urgent need for such a method is obvious. METHODS: The method is based on the principles developed by Schrader, with substantial modifications. As in Schrader's method, we used a fat-based ointment to emulate "dirt." The washing process was performed by placing the examined hand in a rotating soap solution for 5 minutes. The capacity of various soaps to remove the "dirt" was assessed by comparing the sebumeter readings before and after the washing process. The difference between the two readings provided a quantitative estimate of the percentage of "dirt" (ointment) that was washed off during the process. RESULTS: The cleansing capacity of two soaps was compared to that of water. Soap 1 showed a cleansing of 81.7 +/- 2.4%, soap 2 a cleansing of 75.3 +/- 2.9%, as compared to water of 29.7 +/- 3.4%. The curve representing the distribution of the data was very smooth, bell-shaped and symmetric about its mean. The difference between the cleaning activity of the two soaps tested was statistically significant (P < 0.0001). CONCLUSIONS: We have presented a new method for testing the cleansing capacity of soaps. We believe, that our method gives better results than that of Schrader; in particular, it enables us to discriminate more effectively among the various soaps. Our results indicate that the method is reliable and reproducible. It is also practical, easy to perform, does not require an expensive and complex washing machine, and can be carried out in every laboratory.

BACKGROUND: Acne is a disease that affects almost all teenagers (60-90%), and it is the pathology exhibiting the highest morbidity in the 10-24 year-olds age group. There are no consistent data at national level by which we can evaluate the preponderance of this pathology in the Portuguese population. OBJECTIVE: To estimate the prevalence of acne in teenage school students (10-12 year-olds). STUDY DESIGN: A study was carried out involving all 8 regions in the north of Portugal. One school per region was selected at random. RESULTS: As expected, the figure we obtained for the prevalence of this pathology was high (82.4%) with males being the most affected. Nevertheless, only 44% of the affected treated their acne, although the face is the part of the body most affected. In the females high androgenization (which is manifested by the presence of hair on the upper lip and on the side of the face) doubles the median of the degree of acne registered. Regional asymmetry was found in relation to information about the disorder. The degrees of acne are higher in schools in the interior. In the more rural areas, the pharmacist still plays an important role in prescribing treatment. CONCLUSION: As a rule, individuals are correctly informed about acne, confirming that the search for treatment and awareness of its value are generally helping to gradually eliminate some of the stigmas from the past.
Using micro-X-ray fluorescence (MXRF), a novel means of detecting fingerprints was examined in which the prints were imaged based on their elemental composition. MXRF is a nondestructive technique. Although this method requires a priori knowledge about the approximate location of a print, it offers a new and complementary means for detecting fingerprints that are also left pristine for further analysis (including potential DNA extraction) or archiving purposes. Sebaceous fingerprints and those made after perspiring were detected based on elements such as potassium and chlorine present in the print residue. Unique prints were also detected including those containing lotion, saliva, banana, or sunscreen. This proof-of-concept study demonstrates the potential for visualizing fingerprints by MXRF on surfaces that can be problematic using current methods.
