[Effect of lumbar sympathectomy on tissue blood flow in the extremities in arteriosclerosis obliterans].

Lumbar sympathectomy in patients with obliterating atherosclerosis of lower extremities in the nearest postoperative period results in acceleration of the intracutaneous blood flow and of intramuscular blood flow as well as in greater extra-tissue blood shunting. At remote terms the intramuscular bloodflow is also accelerated. Patients with the IV degree of ischemia should be divided into two groups according to results of special investigations: patients with traumatic and with ischemic necroses. It is of importance for choice of rational surgical methods and for prognosis of outcomes of the disease.

[Effect of lumbar sympathectomy on orthostatic reactivity of the arteries of the extremities in thromboangiitis obliterans].

Lumbar sympathectomy in majority of patients with thromboangiitis was found to be followed by a reduced pathogenetically reliable orthostatic and post-orthostatic++ spasm of the diseased arteries. Patients with persistent and continuous remission had normo- and hypotonic orthostatic reaction of the extremity vessels. Remote clinical results of the intervention on lumbar sympathetic ganglia were dependent on the degree of correction of local angiotonic and angiospastic disorders.

[The blood supply and oxygen balance of the different tissues of the extremities following experimental sympathectomy].

The experiments in dogs suggested an essential difference in terms of the rearrangement of regional blood circulation in the skin and muscles of the extremity after experimental sympathectomy. Ganglionectomy induced a reduction of general peripheral arterial resistance on the operated side, an increase in the skin tissue blood flow and its oxygenation rate, a deceleration of the process of biological oxidation reducing the capability of muscles to use blood oxygen and cutting down the maximum of oxidase cytochrome (a3) activity. There was no essential change in the regional acid-alkaline balance. The above changes were due to a disturbance of the neurotrophic sympathetic effect on tissues.

Video-assisted endoscopic thoracic sympathectomy in the management of intractable palmar hyperhydrosis.

Minimally invasive endoscopic surgical techniques have revolutionised patient management. We present our findings in our first 10 cases of bilateral video-assisted endoscopic thoracic sympathectomy in the management of intractable palmar hyperhydrosis including the first such procedure in Singapore. We have found the procedure to have minimal morbidity, good patient acceptance and all patients have remained with dry palms.

[A preliminary observation of EcochG and cochlear morphology after cervical sympathectomy in guinea pigs].

In order to examine the effect of sympathetic nerve on the inner ear, we observed the changes of EcochG and ultrastructure of the hair cells before and after cervical sympathectomy in 13 guinea pigs. We also compared the results with those of the contralateral side (as a normal control group). We found that there were morphologic changes in the stereocilia of the hair cells in the surgical side. No changes were found in the thresholds, amplitudes and latencies of EcochG. It seems that the effect of the morphologic changes are too small to be detected in the electrophysiologic experiments.

[Thoracic sympathectomy via the thoracoscopic approach. The operative technic].

The authors describe the technique of thoracic sympathectomy by thoracoscopic approach in the variant in which the sympathetic chain is divided between T4 and T5 sympathetic ganglion associated with the division of the gray and white rami communicantes of the T2-T3-T4 sympathetic ganglions. The article emphasizes the importance of the 2nd sympathetic ganglion in the regulation of the blood circulation of the arm and the advantages of the thoracoscopic approach for this type of intervention.
