The self as a transitional object: its relationship to narcissism and homosexuality.

Men with significant narcissistic and homosexual trends can show a tendency to experience a part of the self as a transitional object. Specifically, these men regard themselves as dolls. Excerpts from three psychoanalytic cases are offered to demonstrate this clinical finding. It is believed that this occurrence represents a significant arrest in the development of the self. The recognition, interpretation, and reduction of this transitional phenomenon constitutes a significant therapeutic experience in the analyses of these men.

Development of body image, eating disturbance, and general psychological functioning in female adolescents: covariance structure modeling and longitudinal investigations.

OBJECTIVE: Covariance structure modeling (CSM) and longitudinal analyses were used to identify a possible causal sequence for factors that might lead to the development of body image and eating disturbance in adolescent females. METHOD: In Study 1, subjects were measured for level of obesity, perceived weight status, maturational timing, history of being teased about weight/size, body image, eating disturbance, and global psychological functioning. Study 2 was a systematic replication of Study 1 using different indices of body image and a more comprehensive assessment of eating disturbance. In Study 3, subjects from Study 1 were reassessed at a 3-year follow-up. RESULTS: The findings from Study 1 indicated that level of obesity and perceived weight, but not maturational timing, had a directional influence on the other variables. In addition, teasing history was significantly related to the development of body image and eating disturbance. CSM and path analyses with Studies 2 and 3 data generally replicated and extended the results from Study 1. DISCUSSION: These findings offer important insights into possible causal sequences for the development of body image and eating disturbance.

Sexuality, body image, and self-esteem: the future after trauma.

Traumatic injury can drastically change a person's body appearance and function. The author explores the effect traumatic injury has on a person's self-esteem, body image, and sexuality. The cyclical theory of development provides the framework for a review of two core human values--intimacy and sense of self--adults use to organize and reorganize their lives following a traumatic event. Implications for trauma nurses are included.

Defensive anality and anal narcissism.

This paper aims at demonstrating a currently beleaguered assumption: the central importance, the continuing vitality, and the appropriate complexity of Freud's theory of the drives and of his idea of the primacy of the body ego. It is not enough to consider man a thinking machine or a social being; his animal nature must be given a central place in psychology. The paper postulates that 'anal or sphincter defensiveness' is one of the precursors of the repression barrier. Anality has been comparatively neglected in recent psychoanalytic literature, and so has its explorer, Karl Abraham. The paper's thesis is that there is a special defensive importance to anal erogeneity and libido, and to those aspects of ego and superego that are functionally operative (as the 'sadistic-anal organization' (Freud, 1917)) during the so-called 'sadistic-anal' developmental phase. Any of the psychic danger situations can evoke regression to manifestations of 'anal narcissim'--an attempt to master overwhelming feeling by a kind of emotional sphincter action, narrowing down the world to the controllable and the predictable. The basic assumption here is Fliess's idea that the attainment of anal sphincter control functions--with, as-it-were, 'psychic resonance'--as a means to master primal (murderous, cannibalistic) affect. For optimal psychic development, a proper balance must be attained between anal control of, and anal expression of, instinctual derivatives--especially of affect laden with aggression.

[Problems and results of an investigation about sexual development and sexual education of blind people (author's transl)].

The article underlines the necessity of preparing young blind people for partnership relations, married life and family life. It report on an investigation into the problems of psycho-sexual development and education of blind people. Unlike the psycho-sexual development, which proceeds similary to that of people who can see, the sexual development is somewhat delayed. The article presents the results of the investigation and analyses the questioning of blind people, their parents and teachers. It offers advice on the sexual education of young blind people.

Is early effeminate behavior in boys early homosexuality?

This report continues the study of early effeminate behavior in boys. A previous article reported on the long term follow-up of 55 boys and the resulting outcome in homosexuality. In this paper an effort is made to explore further the nature of the connection between childhood effeminate behavior and adult homosexuality. Individual signs are discussed with regard to relevant aspects of their development. It is concluded that early effeminate behavior is not merely a forerunner of homosexuality in that it forecasts homosexuality, but that is is in fact the earliest stage of homosexuality itself.

Abortion in adolescence: the ethical dimension.

The purpose of this essay is to present a bioethical approach to the abortion dilemma in adolescence. It is addressed to health workers involved in the medical care of adolescents and to those involved in adolescent counseling. The introduction presents an overview of the subject in the U.S.A., its medical significance (obstetric, gynecologic, psychologic) as well as some anthropological historico-legal data relating to adolescent abortion. This is followed by a practical description of ethical theory. The main body of the presentation consists of a review of the most important contributions of bioethical thought as they relate to abortion. Adolescent abortion in particular is addressed in the context of issues such as autonomy, rights and responsibility. This includes a description of moral development in adolescence and an analysis of moral aspects of treatment of adolescents. Some personal comments and propositions are made, with the aim of generating further contributions.

Psychological insights in Shakespeare's final play, The Two Noble Kinsmen.

Shakespeare's final play, The Two Noble Kinsmen, contains profound psychological insights. Like all of Shakespeare's reworkings of old material, the result is not merely a variation on a theme but a psychological statement in and of itself, which respects and revisits the past even as it presents a new and original statement. In this paper I argue that the transformation of the Chaucerian into the Shakespearean has a premonition of the Freudian in it also: Shakespeare not only delivers insights on development and sexuality, he anticipates an important Freudian concept in his introduction of the theme of the jailer's daughter whose "love-sickness" requires on understanding of transference before sense can be made of it!

And they lived happily ever after: marriage concepts of older adolescents.

Children of divorced parents are themselves more likely to divorce. The transmission hypothesis has been variously explained by the role model learning, high risk mate selection, increased individual psychopathology and more recently by the explanation of the family's positive sanction of divorce. Conversely, studies without matched controls of adolescents' reaction to parents' separation and their own future marriage indicate that these adolescents are wary of marriage, planning to delay marriage until their late 20's or 30's and highly concerned with partner choice. A study of 18 and 19 year old college students (N = 140) was undertaken to see if the attitudes expressed by those from divorced families are unique to this group or representative of the attitude of the general teenage population towards marriage. The students completed the Tamashiro Marriage Concepts questionnaire and a demographic/social questionnaire. The majority, over 90%, scored on the first two stages of marriage concepts (magical or idealized conventional) consistent with previous findings for this age group. A sub sample of 17 adolescents with divorced parents was compared with 17 matched controls. No significant difference was found in marriage concepts, attitudes or social variables including predicted marital age, dating patterns and sexual experience. Thus older adolescents despite their previous experience tend to be less mature in cognitive style in their attitude to marriage than one would expect at this stage of ego development and, therefore, are at high risk for divorce with early marriage. Further, those adolescents whose background includes the experience of a parental separation and divorce show no difference in their attitude to marriage from their peers.

One of the gang: sexual development of adolescents with physical disabilities.

The social implications of disability may affect the sexual development of adolescents with physical disabilities. Using the Peer Similarity Scale to measure the perceived similarity to or difference from nondisabled peers, this study surveyed adolescents with physical disabilities about their perceptions of their sexual development. The data indicate that while, in general, the adolescents see themselves as similar to nondisabled peers, adolescents with certain diagnoses may be more likely to anticipate difficulty in achieving desired adult sexual relationships and future goals.

Similarities in becoming: transsexuals and adolescents.

The Gender Identity Service (located in Boston, Massachusetts) is an evaluative and counseling unit designed to provide full services including sexual reassignment to transsexuals. In that capacity, since the spring of 1972, we have seen over 120 transsexuals. During this time we have been impressed with behavioral similarities between our population of transsexuals and adolescents. This emerging observation provided impetus for the present paper. After carefully reviewing our caseload material it is our contention that indeed psychological, biological and sociological similarities do exist. Through a comparison of these, we hope not only to make transsexual behaviors more comprehensible but also to underscore the importance of the adolescent years for the development of adult gender identity. Using primarily an ego psychoanalytic approach, the paper attempts to demystify the diagnosis of transsexual by pointing to its dynamic similarities with adolescence.

[Psycho-gynecology for children and girls].

The childhood is the most important period of human life. It is linkage between maternity and ability of giving the life to the next generation. In women life that is, first of all, preparing for the maternity. The medicine is health keeping and optimal human development oriented. In the process of treatment the psychology does not make the most. Effective treatment created by paediatric gynaecologists must be agreed with the basic rules of psycho-medicine, which equalizes bringing up, teaching and treatment.

Qualitative change from quantitative change: mathematical catastrophe theory in relation to psychoanalysis.

The relation of qualitative change to quantitative change is important to psychoanalytic theory. Until recently no tools were available for dealing with the problem, but the development of catastrophe theory has provided methods for studying it. Psychoanalytic investigators might benefit from the methods of catastrophe theory, and dilemmas arising from the supposed impossibility of relating quantity to quality are seen to allow of solution.

Adolescents with Down syndrome.

Adolescence is a period of transition that can create stress for both adolescents and parents. Adolescents with Down syndrome (DS) go through the same stages as other adolescents, but due to lack of cognitive and behavioral factors they and their parents may find this period particularly challenging. This paper reviews several studies, especially from the United Kingdom, of groups of adolescents with Down syndrome and their controls followed from childhood, through adolescence into adulthood. There are special medical problems for this population that require annual medical examinations and surveillance, but the focus has shifted from health problems to social maturation, developing independence, and transition from school to employment or work activity. Medical transition from a pediatric to family physician provider is mentioned with recommendations as to how that transition can be made as smooth as possible.

Sexual behaviors in retarded children and adolescents.

Literature reports on the sexual behaviors of mildly retarded adolescents are reviewed. Retarded adolescents often participate in masturbation and homosexual exploratory behavior. The retarded adolescent's heterosexual interests are of great concern to parents. The retarded adolescent is vulnerable to suggestibility, poor judgment and a failure to foresee the consequences of his actions. Parents are usually acutely distressed by the retarded youth's sexual behaviors, and they may develop an attitude that these behaviors are "bad." There is a need to provide appropriate sex education for retardates and to counsel their families about the management of sexual behaviors which occur during the adolescent years.

Individual and familial influences on the onset of sexual intercourse among urban African American adolescents.

A sample of 198 African American families, living in urban poverty, participated in a longitudinal study of adolescent sexual development beginning when children were in the 4th or 5th grade. Self-reports of family conflict and pubertal development and videotaped family interaction data were collected at 2 time points approximately 2 years apart. Youths reported on sexual debut at each time point. More boys than girls reached sexual debut early. Greater levels of family conflict predicted early sexual debut. Observational data indicated more developed preadolescents with greater family conflict and less positive affect were least likely to delay debut. Changes in pubertal development and observed family conflict were associated with early debut. Possible mediating mechanisms and implications for preventive interventions are discussed.

[Obscene telephone calls--relations to paraphilias, paraphilia related disorders and stalking].

Little is known about the aetiology, course and therapy of obscene telephone calls as a paraphilic behaviour. Overlaps to other paraphilias like voyeurism, paraphilia related disorders and the concept of stalking are reported in the literature. In case reports we describe the paraphilic development and course with a focus on the relationship to the victim. We discuss this on the background of different concepts regarding risk assessment and therapy.

Infrequent orgasms in women.

Out of a sample of 2,425 gynecological patients aged 21-40 and married for at least one year, three groups were selected according to the frequency of coital orgasm. The first, orgastic, group contained 1,266 (52.2%) orgastic women, the second group included 466 (19.2%) patients with infrequent orgasms, and the third group consisted of 151 (6.2%) patients whose infrequent orgasms were felt by the examinees as distressing. Significant differences were found between the three groups concerning family environment and childhood, level of education and professional standing, sexual development and life, and in the incidence of psychopathological symptoms. It appears that the insufficient capacity of many women to attain regular orgasms in sexual intercourse is caused by several factors of both biological and psychosocial nature.
