Family planning KAP survey in Gaza.

This study explores the reproductive attitudes, contraceptive use, demand for family planning and related topics of a representative sample of the female population of reproductive age resident in a Refugee Camp in the Gaza Strip. A cluster sample of 841 resident women of reproductive age (15-49 years) was interviewed in their homes. Univariate and multivariate statistical analyses were performed using BMDP software. 98% of the interviewees favour family planning and 88% plan to use a contraceptive in the future. However, 52% of the women at risk do not use any contraception because of their husband's opposition, fear of side effects or lack of knowledge. The risk of having seven or more children is positively associated with a woman's low educational level and husband's desire for more than seven children. Despite favourable attitudes regarding family planning, there is ignorance and the prevalence of contraception use is low. There is a gap between fertility preference and achievement.

Gene expressivity is the main factor in dictating the codon usage variation among the genes in Pseudomonas aeruginosa.

Codon usage biases of all DNA sequences (length greater than or equal to 300 bp) from the complete genome of Pseudomonas aeruginosa have been analyzed. As P. aeruginosa is a GC-rich organism, G and/or C are expected to predominate in their codons. Overall codon usage data analysis indicates that indeed codons ending in G and/or C are predominant in this organism. But multivariate statistical analysis indicates that there is a single major trend in the codon usage variation among the genes in this organism, which has a strong negative correlation with the expressivities of the genes. The majority of the lowly expressed genes are scattered towards the positive end of the major axis whereas the highly expressed genes are clustered towards the negative end. This is the first report where the prokaryotic organism having highly skewed base composition is dictated mainly by translational selection, though some other factors such as the lengths of the genes as well as the hydrophobicity of genes also influence the codon usage variation among the genes in this organism in a minor way.

Characterization of canine superficial tumors using gray-scale B mode, color flow mapping, and spectral doppler ultrasonography--a multivariate study.

Superficial tumors are not routinely evaluated by two- or three-dimensional diagnostic imaging methods as part of the staging of canine cancer patients, although superficial tumors are readily imaged by ultrasound. The objectives of this study were to characterize the ultrasonographic patterns of superficial tumors and to evaluate whether ultrasound can help discriminate between benign and malignant tumors in dogs. Superficial tumors (n=132) in 86 dogs were evaluated by B mode, color flow mapping, and spectral Doppler ultrasonography. Size, echogenicity, tumor border definition, invasiveness, acoustic transmission, presence and distribution of vascular flow to and within the tumor, as well as perfusion indices were measured. The tumors were classified as lipomas, benign tumors, atypical mammary tumors, and malignant tumors. Multivariate statistics using discriminant analysis was used to determine which parameters may be used to predict the status of the tumor. Tumor echogenicity, border shape, acoustic shadowing, total number of vessels to the tumor and the total flow amount are the parameters that in combination resulted in the lowest classification error (24%), meaning that on average three out of four tumors were correctly classified using these parameters. All the lipomas and atypical mammary tumors were classified correctly by ultrasonography. The results of this study show that ultrasonography has an important role in the evaluation of canine superficial tumors, particularly in the evaluation of tissue homogeneity and tumor vascularity.

Morphometry and classification of the cat hypothalamic capillaries using multivariate analysis.

Multivariate statistical methods were used to do morphological determination of the amount and the relation of the astrocytic foot processes (AF), astrocytic lamina (AL) and dense zones (DZ) on hypothalamic capillaries. We corroborated that 100% of the perivascular membrane is surrounded by glial astrocytic cytoplasm. Principal component analysis shows that the original variables are independent of one another. Cluster analysis confirms the individual situation obtained through the principal component analysis. Discriminant analysis shows that the discriminant capacity of the 3 variables is very significant. For the potential interrelations of astrocytic foot processes, astrocytic lamina and dense zones we have clearly differentiated four groups of hypothalamic capillaries.

Multivariate discriminant analysis of normal, intraepithelial neoplasia and human papillomavirus infection of the uterine cervix samples.

The present investigation studies the role of multivariate statistical methods on quantitative histopathological features of cells in uterine cervix epithelium to discriminate between normal and abnormal uterine cervix samples. 143 histological specimens were included in the study involving normal cervix, cervical intraepithelial neoplasia (CIN) lesions and cervical human papillomavirus (HPV) infection with and without CIN (condyloma-CIN and condyloma-NCIN groups, respectively). Deep, middle and superficial regions of the cervical squamous epithelium were morphometrically analyzed. Identification of normal cervix from pathological cases was highly achieved with a specificity of 100%. The application of discriminant statistical method within pathological specimens showed an acceptable percentage of cases correctly classified; thus, an efficiency of 83.0% and 74.6% was obtained in order to discriminate within CIN and condyloma-CIN grades respectively. These percentages increased when differentiation between each grade of CIN versus condyloma-CIN were considered, using only 1-3 morphometrical parameters. Our findings indicate that the combination of nuclear and cytoplasmic quantitative features, specially size parameters, permit a high correct percentage classification of cervix samples. The discrimination process was better when few diagnostic categories were included; however, 100% specificity for normal samples was always reached.

Spiritual care activities of nurses using Nursing Interventions Classification (NIC) labels.

PURPOSE: To describe the spiritual care activities of nurses as subsequently identified in the Nursing Interventions Classification (NIC) labels. METHODS: Data were taken from a larger study that used a multiple triangulation research design to describe spiritual perspectives, interventions, and attitudes of 1,000 Sigma Theta Tau International members. Data analysis included descriptive and multivariate statistics for quantitative items, and content analysis for responses to questions. FINDINGS: 97 respondents reported providing 32 spiritual care activities. Ten NIC labels actually mapped the nurses' spiritual care activities. CONCLUSIONS: Spiritual care activities involve a broad spectrum of interventions that may be unique to each patient. The 32 spiritual care activities described by the nurses provide new knowledge regarding core spiritual care activities. The use of NIC labels can facilitate documentation of spiritual care activities in diverse practice settings. PRACTICE IMPLICATIONS: This study supports greater specificity in describing spiritual care interventions to a level that allows replication and advancement of knowledge.

Quantitative approaches to primate morphology.

Quantitative morphometric methods are focused mainly on dental morphology, skull shape, shapes of body segments, body proportions and overall size. In the majority of cases simple measurements of linear dimensions or angles have been used. Only rarely are the possibilities inherent to a quantitative description of shape fully exploited. Nevertheless, the utility of morphometrics for taxonomic studies is now generally accepted, in particular if the measurements provide the data base for multivariate statistics. For an understanding of the selective value of the characters studied, a different kind of quantitative data is necessary. As most of the observed variation is in locomotor and jaw structures, which serve to fulfil mechanical functions, quantitative data from the field of mechanics are required: moments of force, moments of resistance, velocities, accelerations, forces, stresses. Even more important than these empirical data is precise knowledge of the way in which these factors are connected in mathematical equations which describe the mechanical laws governing the animal and its motions. If the basic constraint which directs the phylogenetic development of forms is indeed the saving of energy as is commonly accepted, the mechanical approach will open up a causal understanding of evolution.

Defensive characteristics in individuals with amalgam illness as measured by the percept-genetic method Defense Mechanism Test.

Twenty patients complaining of symptoms deriving from their amalgam fillings and a non-patient group were assessed by means of the perceptual projective Defense Mechanism Test (DMT). The test protocols were scored for 130 DMT variables and analyzed by means of the multivariate statistical method Partial Least Squares discriminant analysis. The objective was to try to distinguish the group with amalgam illness from the non-patient group by means of the DMT. The results showed that it was possible to distinguish the two groups significantly from each other. The most characteristic traits of the patient group were a general lateness in perception and few emotional responses compared with the non-patient group and, especially, an inability to perceive the aggressive component in the stimulus picture. The DMT seems to be a powerful method in the effort to understand the mechanisms underlying the problems of amalgam illness.

Correlates of species richness in mammals: body size, life history, and ecology.

We present the most extensive examination to date of proposed correlates of species richness. We use rigorous phylogenetic comparative techniques, data for 1,692 mammal species in four clades, and multivariate statistics to test four hypotheses about species richness and compare the evidence for each. Overall, we find strong support for the life-history model of diversification. Species richness is significantly correlated with shorter gestation period in the carnivores and large litter size in marsupials. These traits and short interbirth intervals are also associated with species richness in a pooled analysis of all four clades. Additionally, we find some support for the abundance hypotheses in different clades of mammals: abundance correlates positively with species richness in primates but negatively in microchiropterans. Our analyses provide no evidence that mammalian species richness is associated with body size or degree of sexual dimorphism.

Radical radiotherapy for localized adenocarcinoma of the prostate. A report of 191 cases.

OBJECTIVE: To assess the results of radiotherapy in the treatment of localized prostate cancer. End points for assessment were survival, local control, development of metastases, and the toxicity of therapy. MATERIALS AND METHODS: The case notes of 191 patients who were treated with radical radiotherapy between 1982 and 1992 were reviewed. The pathology of 130 patients was reviewed by a single pathologist. A multivariate analysis was performed to identify significant prognostic factors with regard to survival and relapse. RESULTS: One-hundred and eighty-two patients were assessable. The minimum length of follow-up was one year (median = 40 months). The actuarial cause-specific 5 and 10 year survival rates were 63% and 35% respectively. Local progression occurred in 41% of patients with 37% developing metastases. Multivariate statistical analysis demonstrated that T stage and Gleason Score were significant predictors for survival. Late complications were usually mild, with only 4% developing serious bladder toxicity. CONCLUSION: Radical radiotherapy has a role in the curative treatment of prostate cancer. Survival is significantly related to T stage at the time of presentation, and to the Gleason Score of the tumour. Survival in this series was not as good as the best surgical series, but it is still not clear which patients should receive radiotherapy and which surgery as their primary management.

Risk factors for predicting mortality in a paediatric intensive care unit.

Rapid advances in critical care technology and rising cost of medical care have spurred the development of outcome analysis including mortality risk prediction. The main objective of this study was to assess the risk factors contributing to mortality in our paediatric intensive care unit (PICU). This is a cohort study, consisting of consecutive admissions to the PICU from 1 January to 31 December 1997. The factors studied included multi-organ dysfunction syndrome (MODS), Pediatric Risk of Mortality III (PRISM III) scores in the first 24 hours (PRISM III-24), mechanical ventilation, renal replacement therapy, age, and diagnosis-related groups. Univariate and multivariate statistical methods were used. Univariate analysis showed that need for mechanical ventilation, renal replacement therapy, presence of MODS involving 3 or more organs and PRISM III-24 scores were significantly associated with outcome (P < 0.0005). Relative risk of mortality in the presence of MODS and PRISM III-24 scores > or = 8 were 11.3 (95% CI: 3.3 to 38.3) and 15.8 (95% CI: 2.0 to 127.8), respectively. Using Cox Proportional Hazards model, the relative risk of mortality for any new admission could be calculated by the equation RR = e0.1032 x P, where P = PRISM III-24 scores.

Associations of insulin-like growth factors, insulin-like growth factor binding proteins and acid-labile subunit with coronary heart disease.

OBJECTIVE: IGFs and their binding proteins (IGFBPs) are produced both systemically and locally by cells of the cardiovascular system. As growth promoters, they may play a role in atherosclerosis. DESIGN: Case-control, cross-sectional. PATIENTS: A total of 95 nondiabetic male patients with coronary heart disease (CHD) and 92 probands from the Prospective Cardiovascular Munster (PROCAM) who were below the age of 60 years and matched by age, body mass index (BMI) and smoking habits. MEASUREMENTS: We analysed the strength and independence of associations of angiographically assessed presence of CHD with BMI, systolic and diastolic blood pressure, total, high-density lipoprotein (HDL) and LDL cholesterol, triglycerides, lipoprotein(a), apolipoproteins A-I and B, total and free IGF-I, IGF-II, IGFBP-1, IGFBP-3, IGFBP-5, acid-labile subunit (ALS), insulin, C-peptide, testosterone, DHEAS and sex hormone binding globulin. RESULTS: Using multivariate statistical analysis, the presence of CHD had significant positive associations with total IGF-I, IGFBP-5, ALS and IGFBP-3. These associations were independent of each other as well as of traditional risk factors, insulin and sex hormones. CONCLUSION: These observations may indicate a pathogenetic role of the GH/IGF axis in coronary atherosclerosis.

Comparative studies on the effects of the combination drug lorazepam plus diphenhydramine (Somnium) versus lorazepam on the noopsyche, thymopsyche and psychophysiology in nonorganic insomnia related to generalized anxiety disorder.

In a double-blind, parallel-group design study on hypnotic and anxiolytic effects of the combination drug Somnium (lorazepam 1 mg + diphenhydramine 25 mg) (SOM) as compared with 1 mg lorazepam (LOR) alone, daytime behavioral effects were studied in 44 patients with nonorganic insomnia related to a mild generalized anxiety disorder. After a placebo run-in phase of 1 week, they received active treatment for 4 weeks. Psychometric evaluations included 6 thymopsychic variables (somatic complaints, state anxiety, trait anxiety, self-rating anxiety and depression scale and adjective mood scale), 6 noopsychic variables (general, associative, numerical and total verbal memory; correct reproductions and errors-Benton visual memory test), 4 pupillary measures (pupillary diameter, latency, relative change in % and half recovery time in pupillary response) and 6 skin conductance measures (baseline, latency, ascending time, time to peak, skin conductance response and skin conductance response maximum). Multivariate statistical analysis of the thymopsyche demonstrated significant improvement after both compounds, with the combination drug significantly superior to its single component lorazepam. Also regarding noopsychic measures, Somnium was significantly superior to the single component lorazepam, as specifically verbal memory was improved after Somnium, while opposite changes occurred after lorazepam alone. Pupillary measures revealed no significant overall inter-drug differences, whereas skin conductance findings demonstrated that the combination drug Somnium was superior to the single component lorazepam also in regard to the anxiolytic effect at the autonomic nervous system level.

The optimization of protein secondary structure determination with infrared and circular dichroism spectra.

We have used the circular dichroism and infrared spectra of a specially designed 50 protein database [Oberg, K.A., Ruysschaert, J.M. & Goormaghtigh, E. (2003) Protein Sci. 12, 2015-2031] in order to optimize the accuracy of spectroscopic protein secondary structure determination using multivariate statistical analysis methods. The results demonstrate that when the proteins are carefully selected for the diversity in their structure, no smaller subset of the database contains the necessary information to describe the entire set. One conclusion of the paper is therefore that large protein databases, observing stringent selection criteria, are necessary for the prediction of unknown proteins. A second important conclusion is that only the comparison of analyses run on circular dichroism and infrared spectra independently is able to identify failed solutions in the absence of known structure. Interestingly, it was also found in the course of this study that the amide II band has high information content and could be used alone for secondary structure prediction in place of amide I.

Functional and morphological affinities of the subadult hand (O.H. 7) from Olduvai Gorge.

Study of the O.H. 7 hand was based primarily on morphological comparisons with a large series of hand skeletons of extant hominoid primates. Most of the hand elements are fragmentary or have missing epiphyses and only comparisons based on qualitative morphological observations are possible. The distal phalanges are complete, however, and were analyzed metrically utilizing univariate and multivariate statistical techniques. To compensate for size differences among the Hominoidea a number of size adjustments were employed. None of the adjustments were totally satisfactory from theoretical and practical standpoints and none completely eliminated the influence of size. There is no entirely satisfactory procedure to eliminate size and it is advisable to use several techniques that are not closely related, to compare the results and interpret them with caution. In certain features the wrist and fingers resemble those of African apes; in others they are more like modern human hands; in still others they are unique. The scaphoid and the proximal articular surface of the trapezium retain ape-like features, as do the proximal and middle phalanges. The pollical carpometacarpal joint and the distal phalanges are closer in morphology to those of modern humans. The scaphoid, proximal phalanges and middle phalanges of rays II-V indicate a hand capable of a strong power grip. A number of features of the thumb and the distal phalanges suggest that the O.H. 7 individual was capable of more precise manipulation that extant apes. FLK NN-A, a first distal phalanx, does not closely resemble the first distal phalanx of any of the living Hominoidea. Multivariate distance analysis indicates, however, that it is closest in overall morphology to the pollical distal phalanx of modern humans. In some features not included in the metric analysis, FLK NN-A also resembles the hallucial distal phalanx of modern humans.

Influence of population on the classification of ECG-VCG's using linear regression techniques.

This paper describes the influence of different populations on statistical multivariate classification rules and classification results where the word "population" refers only to the frequency of diagnoses to be expected, the so-called prior probabilities. Using linear regression as a multivariate classification technique and six groups consisting of five pathological conditions and normals as test data, it has been shown: (a) That the population influences to a great extent the selection of the best ECG-VCG measurements for the classification rule. (b) That a mismatch of the populations in the learning and test sets can considerably decrease the number of correct classifications. (c) That a certain correction of the mismatch can be achieved when the prior probabilities in the learning and test sets are known, Further, the paper discusses the change of prior probabilities over the years at the Variety Club Heart Hospital in the University of Minnesota and its effect on the performance of the classification algorithm which has been used.

Atmospheric bulk deposition to the lagoon of Venice Part I. Fluxes of metals, nutrients and organic contaminants.

First available data on atmospheric fall-out were provided by sampling monthly bulk depositions in four sites inside the Lagoon of Venice (550 km2). Sampling was carried out monthly during the period July 1998-July 1999, in one site near an industrial area (Porto Marghera; site D), another site in the city of Venice (site A), and the remaining two in the southern- and northernmost ends of the Lagoon (Valle Figheri, site C; Valle Dogà site B). The following determinations were carried out for each samples: pH, conductivity, grain-size, particulate load, and dissolved nutrients (N, P). Samples were then subdivided into soluble and insoluble fractions, and Al, Ca, Na, K, Mg, Si, Mn, Fe, Zn, Ni, Cr, Cu, Pb, Cd, As, Hg, Ti, V, S, P, Se and Sb were analysed on both fractions. Total organic micropollutants (PAH, PCB, HCB, DDT, PCDD/F) were measured. As regards particle size distribution, there was great variability among sampling sites. The percentage of the < or =2 microm grain-size fraction was higher in the southern and northern ends of the Lagoon. Small differences were found among sites for major elements, whereas higher variability was observed for inorganic and organic micropollutants, with standard deviations between 20% and 60% of the fluxes measured. Major differences in annual fluxes between the most polluted sites (mostly D and A) and background (site B) were seen for Cd (0.26 vs. 0.06 mg m(-2) year(-1)), Hg (41 vs. 15 microg m(-2) year(-1)), PCB ( approximately 2500 vs. approximately 500 ng m(-2) year(-1)) and HCB ( approximately 8000 vs. approximately 1000 ng m(-2) year(-1)). Comparisons with previous data, collected in the periods 1993-1994 and 1995-1997, were only available for a few trace metals. A definite decline in the annual Pb flux in the city of Venice was detected, from 18 to 13 mg m(-2) in 1996/1997 and 1995/1996 respectively, to approximately 5 mg m(-2) in the present study. Total annual deposition was calculated by means of two different methods, which gave very similar results: (i) the mean value of deposition in the four sites was multiplied by lagoon area (550 km2); (ii) the monthly rain isopleths were combined to normalize deposition values. The figures are: 15-34 kg of Hg and Sb, approximately 200 kg of As, approximately 100 kg of Cd and PAH, 0.7-1.3 tons of Cr, Ni and V, more than 2 tons of Cu and Pb, 17 of Zn, 55 of total P, approximately 200 of Al, and 3900 of DIN. Total fluxes of organics inside the lagoon were: PAH approximately 100 kg; HCB approximately 1 kg; DDT approximately 0.4 kg. PCB and PCDD/F fluxes were approximately 500 g and approximately 10 g, corresponding respectively to 0.1 and 0.4 g I-TE. The correlations between fluxes of inorganic micropollutants and grain-size were significant. Multivariate statistical analysis was applied to investigate more accurately relationships between the insoluble and dissolved fractions of inorganic micropollutants and grain-size fractions. In particular, significant correlations were highlighted between the dissolved fraction of As and the < or =1 mum particle size fraction. Relations between levels of SigmaPCDDF, SigmaPCDD, PCB and PAH congeners and grain-size revealed significant correlation coefficients for the remote sites (B, C), and none in the urban and industrial sites (A, D). In particular, significant correlations were highlighted between SigmaPCDDF, SigmaPCDD and particle size fraction < or =2 mum, and between benzo(a)pyrene and PCB 167 and particle size fraction 4-8 mum.

MRI texture analysis on texture test objects, normal brain and intracranial tumors.

Texture analysis was performed in three different MRI units on T1 and T2-weighted MR images from 10 healthy volunteers and 63 patients with histologically confirmed intracranial tumors. The goal of this study was a multicenter evaluation of the usefulness of this quantitative approach for the characterization of healthy and pathologic human brain tissues (white matter, gray matter, cerebrospinal fluid, tumors and edema). Each selected brain region of interest was characterized with both its mean gray level values and several texture parameters. Multivariate statistical analyses were then applied in order to discriminate each brain tissue type represented by its own set of texture parameters. Texture analysis was previously performed on test objects to evaluate the method dependence on acquisition parameters and consequently the interest of a multicenter evaluation. Even obtained on different sites with their own acquisition routine protocol, MR brain images contain textural features that can reveal discriminant factors for tissue classification and image segmentation. It can also offer additional information in case of undetermined diagnosis or to develop a more accurate tumor grading.
