[The determination of plasma volume using indocyanine green in man].

The importance of circulating blood (BV) and plasma volume (PV) in critically ill patients and physiological research is unchallenged. Recently, Evans blue (EB) [8, 25] and radioactively labelled serum albumin (RIHSA) [20] have mostly been used as tracers for PV determination. However, the disadvantages of radioactive contamination (RIHSA) and dye accumulation (EB), especially in repeated measurements, are obvious. In addition, recent reports show a possible carcinogenic potential for EB [15, 21]. This has prompted us to examine the feasibility of indocyanine green (ICG), a tricarbocyanine dye currently used for cardiac output and liver blood flow measurements, for the determination of PV. The volume of distribution of ICG has been reported to represent PV [5, 26]. METHODS. In 23 healthy volunteers (19 men and 4 women), PV was determined in duplicate (PV1, PV2) with an interval of 30 min. Before injection a tourniquet was put around the arm and a pressure above the systolic arterial pressure was applied for 2 min. During recirculation, ICG (2.5 mg/ml) was administered in a dose of 0.25 mg/kg as a bolus injection over 5 s via an antecubital vein. Blood was drawn from an antecubital vein of the contralateral arm at 1 min intervals. After centrifugation, the optical density (corrected for blank) was read in a densitometer. Third- to ninth-minute plasma samples were used to calculate monoexponential plasma decay curves. The ICG concentration at injection time was achieved by extrapolation. A calibration curve was generated using 5 different known ICG concentrations. PV was calculated from injected ICG dose divided by ICG concentration at injection time. BV and red cell volumes (EV) were derived from measured PV and hematocrit (hct). RESULTS. Between minutes 3 and 9, tracer decay was monoexponential in all but 1 subject. From minute 10 on the plasma decay of ICG represented another, slower compartment (Fig. 1). The plasma half-life of ICG was 3.2 +/- 0.6 min (mean +/- SD). Mean PVs per body weight and body surface area (BSA) were 44 +/- 5 ml/kg and 1662 +/- 176 ml/m2, respectively. Linear regression revealed PV2 = 0.92.PV1 + 226 (r = 0.92) (Fig. 2). The mean percentage of difference (D) was -0.6%, the methodologic error (SD) +/- 5.7% [27]. Linear regression of PV and BSA revealed PV = 1885.BSA -416 (r = 0.71, P less than 0.0001) (Fig. 3). BV and EV estimates (Table 2) obtained from PV and hct showed reproducibility in the range of the PV determination because of excellent reproducibility of hct measurements. DISCUSSION. ICG plasma half-life times in our experiments were comparable to those reported by other authors [18, 19, 24]. Reproducibility of PV determination was good and was well within the limits of other tracer methods (EB, RIHSA) [17, 27]. Using exclusively peripheral veins for ICG injection and blood withdrawal did not seem to affect the accuracy of PV determination. PV estimates obtained by the ICG method showed good agreement with those known from the literature [7, 10, 25]. Our results correspond especially well with the data reported by Hurley [14] obtained from 481 healthy men using different methods (Evans blue, RIHSA, or labelled red cells).

Subjective criteria and probing attachment loss to evaluate the effects of plaque control and root debridement.

11 adult patients with moderate to advanced periodontitis were treated with oral hygiene instruction and an initial, single episode of root debridement. Before therapy, 3 independent clinicians examined all patients and identified sites that in their opinion probably would not respond to the therapy and would continue to lose attachment. On 2 occasions, 3 and 12 months later, the clinicians re-examined and re-evaluated all patients and all sites. The results of therapy were also monitored by probing attachment level measurements performed every 3rd month. All 11 patients completed 24 months of follow-up, and 6 subjects were available until 36 months. Sites with probing attachment loss after 12, 24 and 36 months were identified using linear regression analysis and compared to the clinicians' prediction of probing attachment loss. The results demonstrated a limited agreement between probing attachment loss determined by linear regression and the clinicians' predictions of probing attachment loss. It appears that the traditional clinical signs and factors used to forecast and identify periodontal disease activity are only moderately associated with probing attachment loss. This suggests that attachment loss may be caused by several factors, at least following initial therapy. The progression of an inflammatory disease of microbial etiology may be only 1 of such causes. Further studies are needed to clarify the nature and cause of probing attachment loss.

Cyclosporin A-induced cholestasis. The mechanism in a rat model.

Cyclosporin A (CyA) causes cholestasis in a significant proportion of transplant patients. Doses of 5, 10, and 15 mg CyA/kg body wt or the Miglyol 812 vehicle were administered intraperitoneally for 1, 2, and 3 wk to separate groups of rats to investigate the mechanism of this cholestasis. At 1 wk a dose-response relationship between serum CyA levels and increasing CyA doses was noted. A maximum CyA blood level was achieved by 2 wk with the 10- and 15-mg/kg doses. Subsequent studies were performed using the smaller (10 mg/kg) dose administered for 3 wk. This dose resulted in a marked increase in serum bile acid levels compared with vehicle-treated controls (24.6 +/- 4.0 vs. 4.3 +/- 1.2 mumol/L, p less than 0.001) without inducing significant changes in serum glutamic oxaloacetic transaminase, serum glutamic pyruvic transaminase, bilirubin, alkaline phosphatase, and albumin levels or hepatic architectural alterations. With CyA treatment, baseline bile flow decreased by 35% and bile salt secretion decreased by 25% compared with vehicle-treated animals. Cyclosporin A and vehicle-treated rats were infused intravenously with taurocholate (4 mumol/min X kg) for 2 h and then depleted of bile salts over the next 24 h. Bile samples collected over this period were graphed as bile salt secretion versus bile flow. The mean slope of the linear regression for the CyA-treated rats was 62% of the control, demonstrating a decrease in bile salt-dependent flow. Extrapolation of the linear regression to the ordinate demonstrated a 22% decrease in bile-independent flow with CyA treatment. Therefore, in our experimental model of CyA-induced cholestasis, the decrease in flow observed was the result of a decrease in both bile salt-dependent and bile salt-independent flows and occurred in the absence of significant biochemical or histologically evident hepatotoxicity.

Age-specific changes in the regulation of LH-dependent testosterone secretion: assessing responsiveness to varying endogenous gonadotropin output in normal men.

Pulsatile and thus total testosterone (Te) secretion declines in older men, albeit for unknown reasons. Analytical models forecast that aging may reduce the capability of endogenous luteinizing hormone (LH) pulses to stimulate Leydig cell steroidogenesis. This notion has been difficult to test experimentally. The present study used graded doses of a selective gonadotropin releasing hormone (GnRH)-receptor antagonist to yield four distinct strata of pulsatile LH release in each of 18 healthy men ages 23-72 yr. Deconvolution analysis was applied to frequently sampled LH and Te concentration time series to quantitate pulsatile Te secretion over a 16-h interval. Log-linear regression was used to relate pulsatile LH secretion to attendant pulsatile Te secretion (LH-Te drive) across the four stepwise interventions in each subject. Linear regression of the 18 individual estimates of LH-Te feedforward dose-response slopes on age disclosed a strongly negative relationship (r = -0.721, P < 0.001). Accordingly, the present data support the thesis that aging in healthy men attenuates amplitude-dependent LH drive of burst-like Te secretion. The experimental strategy of graded suppression of neuroglandular outflow may have utility in estimating dose-response adaptations in other endocrine systems.

Distortion-product otoacoustic emission spectra and high-resolution audiometry in noise-induced hearing loss.

Distortion product otoacoustic emissions (DPOAE) elicited by 60dB SPL pure tones at f1 and f2 were collected at 2f1-f2, in 1/10th octave steps, in a sample of 36 ears from 27 patients with noise-induced hearing loss (NIHL). They were analyzed in the frequency domain against the outcome of high-resolution pure-tone audiometry, performed with the help of a Békésy sweep-frequency automatic audiometer. The characteristics of DPOAE level plots as a function of frequency (the so-called DP-grams), relative to the DPOAE levels of a control age-matched group, were compared to their alleged counterparts on the audiograms, i.e., the lower and upper frequency boundaries of the interval with hearing loss. Ears with NIHL split into two subgroups, one (n = 25) with a notch in the DP-gram such that its lower boundary matched the lower limit of the audiometric notch (linear regression with a slope of 0.91, r(2) = 0.644, p < 0.001). Likewise, when it existed, its upper boundary matched its upper counterpart on the audiogram (linear regression with a slope of 0.96, r(2) = 0.89, p < 0.001). In this respect, DP-grams performed better than transient-evoked OAE spectra, which exhibited poor correlations with audiogram patterns. The second subgroup (n = 11) exhibited normal DPOAEs at all frequencies despite audiometric losses similar to those of the first subgroup. In all cases, DPOAE levels were poor predictors of the degree of hearing losses. It is hypothesized that NIHL in the second subgroup involves inner hair cells or auditory neurons, instead of outer hair cells in the first subgroup. Provided NIHL affected outer hair cells, DP-grams provided a comparatively accurate predictor of the spectral extent of hearing loss.

Simultaneous determination of carvedilol and ampicillin sodium by synchronous fluorimetry.

The carvedilol and ampicillin sodium were simultaneously determined by the synchronous fluorimetry. With excitation wavelength at 254 nm, the maximum emission wavelengths of carvedilol and ampicillin sodium were at 357 and 426 nm, respectively. Because the emission spectra of carvedilol and ampicillin sodium were overlapped partially, carvedilol and ampicillin sodium cannot be determined directly by normal fluorimetric method. However, the synchronous fluorimetry can be used for determining both drugs simultaneously without separation procedure. The (Delta)(lambda) = 80 nm was used. Iso-propanol was selected as sensing reagent. Effects of pH, organic solvents and foreign ions on the determination of both drugs were studied. The linear relationship was obtained between the relative fluorescence intensity and concentration of carvedilol and ampicillin sodium in the range of 0.005-0.1 and 5.0-70.0 microg ml(-1), respectively. The linear regression equation of calibration graph for carvedilol is C = 0.000151F - 0.00210, and for ampicillin sodium is C = 0.0770F - 2.62. The correlation coefficient of linear regression equation is 0.9995 for carvedilol and 0.9998 for ampicillin sodium, respectively. The detection limit is 1 ng ml(-1) for carvedilol and 1 microg ml(-1) for ampicillin sodium. The relative standard deviations of carvidelol and ampicillin sodium are 2.47 and 1.61%, respectively. The recovery is from 96.0 to 103.0% for carcvedilol and from 98.0 to 105.0% for ampicillin sodium. This method was rapid, simple and highly sensitive for the determination of carvedilol and ampicillin sodium without pre-separation. The results obtained by this method agreed with those by the official methods. This method can be used for the determination of carvedilol and ampicillin sodium in the medicine dosage.

Soluble interleukin 6 receptor: A novel marker of moderate to severe sleep-related breathing disorder.

BACKGROUND: Given the previously described association between sleep-related breathing disorder (SRBD) and markers of inflammation, we assessed the relationship of SRBD with levels of both interleukin 6 (IL-6) and soluble IL-6 receptor (sIL-6R), a marker with more expansive physiologic effects than IL-6. The objectives were to explore the relationship between moderate to severe sleep apnea with IL-6 and sIL-6R levels and to examine morning and evening variability for each cytokine. METHODS: A total of 385 adult participants (> or =18 years of age) in the Cleveland Family Study, Cleveland, Ohio, underwent sleep studies and determination of IL-6 and sIL-6R levels in samples obtained in the evening and morning of polysomnography. Moderate to severe SRBD was defined as a respiratory disturbance index greater than or equal to 30. RESULTS: The subjects were aged 44.9 +/- 16.7 (mean +/- SD) years, 44% were male, and 48% were African American, with a body mass index (weight in kilograms divided by the height in meters squared) of 32.5 +/- 8.1 (mean +/- SD). Linear regression analysis showed that after adjustment for subject characteristics, waist circumference, and comorbidities, SRBD was not significantly associated with morning IL-6 levels. In contrast, linear regression analyses showed that, compared with the participants without SRBD, those with SRBD had significantly higher morning sIL-6R levels (mean +/- SD, 4.60 +/- 1.42 ng/mL [P = .001]), even after adjustment for subject characteristics, waist circumference, and comorbidities, which persisted after adjustment of evening sIL-6R levels. CONCLUSIONS: Morning sIL-6R levels demonstrated stronger associations with moderate to severe SRBD than morning IL-6 levels. Associations with SRBD and morning sIL-6R levels persisted even after adjustment for waist circumference, cardiovascular disease, and evening sIL-6R levels, suggesting the potential utility of sIL-6R as a marker for measuring overnight SRBD stresses. Further investigation of this biomarker may provide insight into SRBD-related inflammation.

Body image and weight concerns among Southwestern American Indian preadolescent schoolchildren.

This study was undertaken to determine whether perceptions about body image and size vary by body mass index (BMI) and demographic factors among American Indian children in the Southwestern United States. We surveyed 1,996 American Indian schoolchildren in the fifth grade (mean age 10.5 +/- 0.7 years) at eleven rural elementary schools. Heights and weights were measured using standard protocol. Multiple linear regression models were developed for preferred body shape, desired body shape, and body satisfaction to determine association with demographic and physical factors. Results from multiple linear regression modeling indicated that BMI and gender influence body satisfaction, with heavier children and girls being less satisfied with their bodies (P<.001 and P<.05, respectively). Geographic isolation and lack of amenities in the home did not appear to affect body satisfaction or messages from others about being too fat or thin; however, geographically isolated students were more likely to choose a larger body shape as best looking. Girls of appropriate weight were 2.2 times more likely than boys to have been told they were too fat. Sixty-one percent of all students surveyed had tried to lose weight. These findings, although similar to those for non-Hispanic Whites of the same age, are contrary to prevailing ideas that American Indians value obesity. In fact, we observed a high level of body dissatisfaction among children of appropriate weight, particularly girls, and prevalent dieting across all weight categories and both genders.

Selection for lambing rate and clean fleece weight in sheep.

This study was designed to compare the effectiveness of selection for lambing rate (LB) and clean fleece weight (CFW) separately or by a combination of both traits at once. Two flocks of approximately 200 western white faced ewes per flock were maintained at two locations. Each flock was divided into four equal selection groups of similar initial birth type and age. The basis of selection in each flock was as follows: Line I, single trait selection for LB; Line II, single trait selection for CFW; Line III, multiple trait selection for LB and CFW; and Line IV; visual selection based on conformation and general fleece quality. Repeatability and heritability estimates were .18 and .16 +/- .04, respectively, for LB and .66 and .60 +/- .09, respectively, for CFW. The genetic correlation between LB and CFW was .26 +/- .16. Linear regression coefficients (number born/year) of annual response for LB in Lines I to IV were .018 +/- .008, .009 +/- .006, .011 +/- .005, and .009 +/- .005 (P > .05). Linear regression coefficients (kg/year) of annual observed response for CFW in lines I to IV were -.001 +/- .006, .027 +/- .009, .019 +/- .006, and .008 +/- .006. Response in Lines II and III was significant for CFW (P < .05) compared with Lines I and IV. In this study, CFW was increased by selection in white-faced sheep with little effect on lambing rate.

Semiparametric regression analysis of longitudinal data with informative drop-outs.

Informative drop-out arises in longitudinal studies when the subject's follow-up time depends on the unobserved values of the response variable. We specify a semiparametric linear regression model for the repeatedly measured response variable and an accelerated failure time model for the time to informative drop-out. The error terms from the two models are assumed to have a common, but completely arbitrary joint distribution. Using a rank-based estimator for the accelerated failure time model and an artificial censoring device, we construct an asymptotically unbiased estimating function for the linear regression model. The resultant estimator is shown to be consistent and asymptotically normal. A resampling scheme is developed to estimate the limiting covariance matrix. Extensive simulation studies demonstrate that the proposed methods are suitable for practical use. Illustrations with data taken from two AIDS clinical trials are provided.

Apolipoprotein B, ratio of total cholesterol to HDL-C, and blood pressure in abdominally obese white and black American women.

OBJECTIVE: To compare the association of apolipoprotein B (ApoB) and total cholesterol to high-density lipoprotein cholesterol (TC/HDL) with blood pressure in abdominally obese white and black American women. We also sought to determine if there are ethnic differences in blood pressure values that could be explained by differences in mean values of ApoB and TC/HDL. METHODS: Data (n = 1844) from the Third US National Health and Nutrition Examination Survey were used in this study. Abdominal obesity was defined as waist circumference (WC) of > or =88cm or having WC greater than what was expected as predicted from residuals obtained from linear regression of WC on BMI. Bi-variate Pearson's correlation analysis was used to quantify the degree of association of ApoB and TC/HDL with blood pressure and other lipids. Multiple linear regression analysis was used to assess the independent contribution of ApoB and TC/HDL to blood pressure, adjusting for age, total cholesterol, alcohol intake, and smoking. To determine ethnic differences in blood pressure values associated with ApoB or TC/HDL, dummy variables were used to compare blacks with whites fitted in multiple regression models, while adjusting for age, total cholesterol, alcohol intake and smoking. RESULTS: Elevated ApoB was positively associated with diastolic and systolic blood pressure (DBP/SBP) in blacks and whites, independent of age, total cholesterol, alcohol intake and smoking (P < 0.01). Elevated TC/HDL was also positively associated with increased DBP and SBP in whites (P < 0.05). For the same value of ApoB and TC/HDL whites had higher values of DBP and SBP than blacks, adjusting for confounding variables. CONCLUSIONS: Compared with TC/HDL, ApoB was more strongly associated with DBP and SBP in both abdominally obese white and black women. Since ApoB is associated with hypertension, the combination of elevated ApoB and hypertension may identify a group of patients with more marked risk of vascular disease, thus, warranting further investigation.

Quantitative characterization of hormone receptors.

Most workers characterize steroid (and other hormone) receptors by graphical analysis of Scatchard plots or by simple linear regression. Unfortunately, these methods are suboptimal from a statistical point of view. The Scatchard plot, B/F vs. [Bound], does not satisfy the assumptions underlying simple linear regression: both variables are subject to error, and these errors are intimately interdependent. Accordingly, nether B/F nor [Bound] is an appropriate independent variable. Furthermore, both variables (B/F and [Bound] show non-uniformity of variance. Thus, even when the Scatchard plot is liner, one should estimate the binding parameters (affinity, K, and binding capacity, R) by means of weighted nonlinear least-squares regression, using the Total ligand concentration as the independent variable, and either B/T or [Bound] as the dependent variable. In the case of a nonlinear Scatchard plot, one should also use weighted nonlinear least-squares curve fitting to estimate the K and R values for the high and low affinity classes of sites. Allowing the computer program to provide the best estimate of the nonspecific or nonsaturable binding is also desirable. The program should provide estimates of the standard errors and/or 95% confidence limits for the estimated parameters, and the joint 95% confidence limits for K and R. One should routinely attempt to fit several models of varying degrees of complexity (e.g., 1,2, or 3 classes of sites), provide estimates of the goodness-of-fit for each, and then select the best model by statistical criteria. Sometimes, we encounter Scatchard plots that are obviously nonlinear but provide insufficient information within any one experiment to permit reliable characterization of two or more classes of sites. In this case, we may employ any of several alternative techniques, including 1) use of the " limiting slopes" technique to obtain approximate estimates of parameters; 2) use of a Continuous Affinity Distribution, with consideration of only the receptors with an affinity above an arbitrarily selected cutoff value of k; 3) use of a Discrete Affinity Distribution, by assigning values to the affinities (K19 K2) based on prior information, and then estimating the binding capacities; 4) pooling information over several specimens within an assay or over several assays by use of normalizing or scaling factors. The best estimates of these scaling factors can be obtained by the use of a general least-squares method for pooling data from different specimens or experiments. A series of computer programs to perform these analyses has been developed. They have been applied successfully to analysis of steroid receptors in specimens from breast carcinoma.

Clinical approach to renal study incidental to 99mTc-MDP bone scintigraphy.

In order to investigate the feasibility of the assessment of renal function with 99mTc-MDP, we compared renographical images, renogram patterns and the glomerular filtration rate (GFR) obtained by means of a modified Gates' method and 200 MBq of 99mTc-MDP with those obtained by means of 99mTc-DTPA. Because 19 of 20 patients had malignant tumors in the genitourinary tract, there was no difference between the two tracers in identifying a parenchymal defect corresponding to renal cancer. Of eight patients with hydronephrosis, four had a defect or decreased uptake with a dilated pelvis, whereas the other four had marked radioisotope retention in the renal pelvis or the whole kidney on serial images. There was also no difference between the two tracers in identifying hydronephrosis. Of 38 paired renograms 35 showed the same renogram patterns with both tracers. Of three patients with different renogram patterns, two had hydronephrosis. In 20 patients including three patients with bone metastasis, total GFR and split GFR obtained with both tracers correlated with a correlation coefficient of r = 0.920 (p < 0.001) and r = 0.944 (p < 0.001), respectively. Excluding bone metastasis from the analysis, a linear-regression analysis showed excellent agreement between the two measurements with a correlation coefficient of r = 0.960 (p < 0.001) and r = 0.963 (p < 0.001), respectively. The linear regression equations were Y = 1.009X - 0.111 and Y = 1.034X - 0.714, respectively. In conclusion, 99mTc-MDP can be used as a supplement to evaluate renal function incidental to the survey of bone metastases in patients with malignant tumor.

Remnant-like lipoproteins, hormone therapy, and angiographic and clinical outcomes: the Women's Angiographic Vitamin & Estrogen Trial.

BACKGROUND: Little is known about the impact of post-menopausal hormone therapy on remnant-like particle (RLP) concentrations and about the relationship between RLP concentration and angiographic progression of coronary artery disease and clinical events in women. METHODS: RLP cholesterol and triglyceride levels were measured at baseline and 3 months after randomization in 397 post-menopausal women enrolled in The Women's Angiographic Vitamin & Estrogen (WAVE) trial. Correlates of baseline RLP levels and changes in levels with post-menopausal hormone therapy were determined with multiple linear regression. Coronary angiography was performed at baseline and after a mean of 2.9 years. Changes in minimal and average luminal diameter were modeled with multivariate linear regression, clinical outcomes (non-fatal myocardial infarction, stroke, or cardiovascular death) with multiple logistic regression. RESULTS: The mean subject age was 65 years, 66% of subjects were white, 18% of subjects smoked, most subjects were overweight or obese, and 35% of subjects had diabetes mellitus. RLP cholesterol (0.277 +/- 0.254 mmol/L) and triglyceride (0.386 +/- 0.552 mmol/L) levels corresponded approximately to the 90th percentile in women in the Framingham study. RLP levels did not change significantly with hormone therapy. RLP levels at baseline, changes in RLP levels, and on treatment RLP levels did not relate to angiographic changes or clinical outcomes (non-fatal myocardial infarction, stroke, or cardiovascular death). CONCLUSIONS: RLP levels were high among post-menopausal women enrolled in the WAVE study, were not affected by hormone therapy, and did not relate to angiographic progression of coronary artery disease or clinical outcomes.

High circulating vascular endothelial growth factor (VEGF) is related to a better systolic function in diabetic hypertensive patients.

BACKGROUND: VEGF seems to have a protective role on cardiac microcirculation, but no data are available on its action on cardiac function and morphology in diabetic patients. We sought to test the hypothesis that circulating VEGF levels could influence the cardiac performance in type 2 diabetic hypertensive patients. METHODS: We studied 30 patients with type 2 diabetes and hypertension, without severe cardiac, retinal, renal and peripheral vascular damage. Ten non-diabetic hypertensive patients represented the control group. VEGF plasma levels (ELISA) and echocardiographic parameters were evaluated. RESULTS: Diabetic patients had VEGF plasma levels higher than hypertensive non-diabetic subjects [median 82 (IQR 12-190) vs 50.5 (IQR 28-77) pg/mL, p=0.05]. Simple linear regression analysis showed that VEGF levels are related to relative wall thickness (RWT) and both endocardial and midwall systolic parameters in the diabetic patients. Multiple linear regression analysis showed that RWT and ejection fraction (EF) were the only independent correlates of VEGF (r2=0.274, p=0.03, p=0.05; respectively). CONCLUSIONS: Our data showed that high VEGF plasma levels are associated to a better systolic function in diabetic hypertensive patients with cardiac remodeling. VEGF may play a role in the improvement of cardiac performance in diabetes.

A new method for the prediction of peripheral vascular resistance from the preoperative angiogram.

The preoperative angiogram is widely used to estimate runoff prior to infrainguinal bypass grafting, but the traditional method of angiographic scoring (0, 1, 2, or 3 based on the number of patent tibial vessels) correlates poorly with measurements of peripheral vascular resistance. We assigned a score of 0, 1, or 2 to each of four parameters (anterior and posterior tibial arteries [AT and PT], peroneal artery [PER], and plantar arch [ARCH]) on the preoperative angiograms of 39 patients admitted for elective femoral bypass grafting. These scores were then examined for their ability to correlate with the peripheral vascular resistance measured in each patient at the time of surgery. Multiple linear regression suggested that the scores assigned to the AT, PT, and ARCH were significantly correlated with observed resistance, but the PER subscore was not. Multiple linear regression of the AT, PT, and ARCH subscores suggested that resistance could be predicted from the equation: In (Resistance [mm Hg/ml/min]) = 0.786 - 0.268(AT) - 0.25(PT) - 0.358(ARCH), for which r = 0.78 and p less than 0.001. The ability of this relationship to predict resistance was tested by a division of the patient population into two roughly equal groups based on their observed resistance at surgery. When tested in this fashion, this equation had a sensitivity of 88% and a specificity of 82%. These findings suggest that the preoperative angiogram can be graded in a simple, yet unambiguous way, which allows a reasonable prediction of peripheral vascular resistance.

Probabilistic diffusion tractography: a potential tool to assess the rate of disease progression in amyotrophic lateral sclerosis.

The goal of probabilistic tractography is to obtain a connectivity index along a white matter pathway that reflects fibre organization and is sensitive to pathological abnormalities contributing to disability. Here, we present the development of voxel-based connectivity measures along the tractography-derived corticospinal tract (CST). We investigated whether these connectivity measures are different in patients with amyotrophic lateral sclerosis (ALS) and correlate with the rate of disease progression. We also investigated whether fractional anisotropy (FA), which reflects directional coherence of fibre tracts, is reduced in the CST of ALS patients and relates to disease progression rate. Thirteen patients with probable or definite ALS and 19 healthy subjects were studied. The probabilistic tractography algorithm segmented the bilateral CST, along which FA and connectivity values were obtained. To take into account the asymmetric distribution of connectivity values, two summary statistic measures that focused on voxels with higher connectivity values were selected and then used in the analysis, together with the mean connectivity and the mean FA. To complete the analysis, the same summary measures for FA were included. Differences in all these indices between patients with moderate or rapid disease progression rate and controls were investigated using linear regression, adjusted for age and white matter fraction. The association between FA or connectivity in the CST and the disease progression rate was assessed using linear regression. Patients with a rapid disease progression rate had significantly lower summary connectivity measures than controls in the left CST, but there was only a borderline statistical difference in mean connectivity. Patients with rapid progression had a significantly lower mean FA, and any other FA measure, in both CSTs than controls. When only patients were considered, strong associations between the rate of disease progression and all the connectivity measures in the left CST were found (P-values between P < 0.001 and P = 0.002, partial correlation coefficients between -0.90 and -0.82). However, there was no evidence of an association between disease progression rate and any of the FA measures in the bilateral CST. Our findings suggest that FA and connectivity provide complementary information, since FA is sensitive to the detection of all the group differences, whereas the summary connectivity measures correlate with disease progression rate. The development of such connectivity measures raises their potential as markers of disease progression in ALS, and provides guidance for their use in other neurological diseases.

Modeling the glottal volume-velocity waveform for three voice types.

The purpose of this study was to model features of the glottal volume-velocity waveform for three voice types: modal voice, vocal fry, and breathy voice. The study analyzed data measured from two sustained vowels and one sentence uttered by nine adult, male subjects who represented examples of the three voice types. The primary analysis procedure was glottal inverse filtering, which estimated the glottal volume-velocity waveform. The estimated glottal volume-velocity waveform was then fit to an LF model waveform. Four parameters of the LF model were adjusted to minimize the mean-squared error between the estimated glottal waveform and the LF model waveform. Statistical averages and standard deviations of the four parameters of the LF glottal waveform model were calculated using the data for each voice type. The four LF model parameters characterize important low-frequency features of the glottal waveform, namely, the glottal pulse width, pulse skewness, abruptness of closure of the glottal pulse, and the spectral tilt of the glottal pulse. Statistical analysis included ANOVA and multiple linear regression analysis. The ANOVA results demonstrated that there was a difference in three of the four LF model parameters for the three voice types. The linear regression analysis between the four LF model parameters and a formal rating by a listening test of the quality of the three voice types was used to determine the most significant LF model parameters for each voice type. A simple rule was devised for synthesizing the three voice types with a formant synthesizer using the LF glottal waveform model. Listener evaluations of the synthesized speech tended to confirm the results determined by the analysis procedures.
