[Primary malignant lymphomas of the digestive tract. Morphologic and clinical analysis of 100 cases].

The morphologic and clinical evaluation of malignant lymphomas having the primary localization in the digestive tract has been presented. The analysis of particular cases involved the localization of tumors, the histological types, age, sex and clinical symptoms. It has been stated that malignant lymphomas are rare primary malignancies of the digestive tract which develop most frequently in the stomach and their clinical course may be masked by other diseases of digestive tract.

Primary small bowel malignant tumors. Unrecognized until emergent laparotomy.

Forty-five consecutive cases of primary small bowel malignant tumors treated at our institution from 1969-1983 were reviewed. Sixty-four per cent of the patients had surgical emergencies. Fourteen patients had intestinal obstruction, 11 had gastrointestinal (GI) hemorrhage, and 4 had perforation of the small bowel. The remaining 16 patients were explored for persistent symptoms, an abnormality on GI series, or a mass lesion on CT scan. Thirty-eight patients underwent resection of the tumor and seven had palliative bypass procedures. This study suggests that a high index of suspicion and early diagnostic evaluation including a small bowel series is necessary to prevent the large number of small bowel tumors presenting as surgical emergencies. In particular, small bowel tumors should be suspected in patients with abdominal pain of unknown etiology, unexplained weight loss, or occult GI bleeding.

Some aspects of abdominal surgery in the elderly patient.

Most operations on elderly patients are of an urgent nature. If time permits, measures to improve cardiac, respiratory and renal functions in the appropriate ways should be undertaken. During recovery from anesthesia careful but adequate sedation will reduce the risk of myocardial ischemia. Throughout the postoperative period constant encouragement of the older patient is particularly helpful.Transverse abdominal incisions and the frequent use of temporary gastrostomy are advocated.External hernia, hiatus hernia, peptic ulcer, carcinoma of the stomach, biliary disease, appendicits, intestinal obstruction, and carcinoma of the large intestine are discussed specifically, with special reference to the practical details of management in the elderly patient.

[Changes in lactate dehydrogenase isoforms in the process of oncogenesis].

Isoenzymes of lactate dehydrogenase were studied by disc-electrophoresis in polyacrylamide gel, and in the clinic--in 1% agar gel. Oncovirus A12 invasion of the culture of rat embryo fibroblasts (REF) was found to result in the increased percentage of the cathode fractions activity (LDG-4 and LD-5) and in the disappearance of LDG-1 yet during the first day of the experiment prior to hypoxia and enhanced proliferation, i. e. it is most likely to be primary. In the homogenates of cancerous tumor and large intestine polyps of man also a reliable increase of the cathode and a decrease or disappearance of the anode fractions accur. A correlation of the experimental and clinical data allowed a suggestion to be made that LDG isoenzymes changes are genetically conditioned and play an important role in the process of oncogenesis, providing conditions for the increased intensity of glycolysis and proliferation.

[Effect of the joint use of BCG and antithymocyte serum on the growth of a syngeneic tumor in mice].

The growth of the syngeneic tumor Acatol in BALB/c mice was retarded if the animals were pretreated with BCG or antilymphocyte serum (ATS). Combined use of BCG and ATS led to a significantly more powerful retardation as compared to the effect produced by each factor alone. Using the adoptive transfer of splenocytes from treated mice it was shown that tumor growth suppression is effected by the cell types other than T lymphocytes and macrophages. It is probable that the effector cells within the given system are K cells and natural killer cells. The results attest to a possibility of search for a two-directional action on the immune system of the tumor host, which would stimulate antitumor effector cells and inhibit the activity of suppressors, particularly that of T suppressors.

[Malignant melanoma of the small intestine].

Three patients with intestinal metastases from a malignant melanoma are presented. The pathological and clinical findings are reviewed. The symptoms are variable , but usually include unexplained anemia, gastrointestinal bleeding, abdominal pain and intestinal obstruction. X-ray examination of gastrointestinal tract can reveal multiple metastatic lesions with so-called typical "Bull's eye" sign. Surgical excision is indicated and extended survival due to improved chemotherapy and immunotherapy is stressed.
