The dangers of feeling like a fake.

In many walks of life-and business is no exception-there are high achievers who believe that they are complete fakes. To the outside observer, these individuals appear to be remarkably accomplished; often they are extremely successful leaders with staggering lists of achievements. These neurotic impostors--as psychologists call them--are not guilty of false humility. The sense of being a fraud is the flip side of giftedness and causes a great many talented, hardworking, and capable leaders to believe that they don't deserve their success. "Bluffing" their way through life (as they see it), they are haunted by the constant fear of exposure. With every success, they think, "I was lucky this time, fooling everyone, but will my luck hold? When will people discover that I'm not up to the job?" In his career as a management professor, consultant, leadership coach, and psychoanalyst, Manfred F.R. Kets de Vries has found neurotic impostors at all levels of organizations. In this article, he explores the subject of neurotic imposture and outlines its classic symptoms: fear of failure, fear of success, perfectionism, procrastination, and workaholism. He then describes how perfectionist overachievers can damage their careers, their colleagues' morale, and the bottom line by allowing anxiety to trigger self-handicapping behavior and cripple the very organizations they're trying so hard to please. Finally, Kets de Vries offers advice on how to limit the incidence of neurotic imposture and mitigate its damage through discreet vigilance, appropriate intervention, and constructive support.

[The burden caused by injury in countryside residents in Dongying, Shandong Province].

OBJECTIVE: To make clear the burden of injury in residents living in countryside of Dongying, Shandong Province. METHODS: A retrospective questionnaire survey was conducted among 15,276 residents in 20 villages of Dongying Municipality of Shandong Province with a multiphase cluster randomly sampling on cases of injury from March 1, 2002 to February 28, 2003. The injury was decided with three principles which had been carried out through the nation. The standardized incidence and mortality rates were calculated on base of the population data of Shandong Province in 2000. The burden of injury was evaluated by incidence rate, mortality rate, years of potential life lost (YPLL) system, disability adjusted life year (DALY), direct and indirect economic costs, et al. RESULTS: Totally 902 were injured cases in which the crude incidence rate was 5.90%, and the standardized incidence rate was 5.93%. It was shown higher in men (7.79%) than in women (4.03%). The crude death rate was 12.438 per million. And the standardized death rate was 11.257 per million. 27 cases were crippled. The years, the working years and the valued years of the potential life lost were 24, 19.6 and 8.7 years respectively for each death case. Disability adjusted life years was 566.61 years. Direct economic cost for medical expenses of all cases was 816.3 thousand RMB Yuan and 200.1 thousand RMB Yuan for else costs. Indirect economic cost was 5,492.9 thousand RMB Yuan according to DALY. CONCLUSION: Injury was common incidence in rural residents in Dongying which caused a heavy burden on them. Prevention and control of injuries would be enhanced.

Image-guided stereotactic management of non-AIDS-related cerebral infection.

Every neurosurgeon can appreciate Dandy's recognition that the drainage of brain abscesses causes trauma to the delicate parenchyma. Over the years, brain surgery has evolved toward management of problems by using less and less invasive techniques and thus gaining ever lower morbidity. Clearly, the advent of better imaging techniques has improved the outcome in patients afflicted with intracerebral infections. The combination of stereotaxy with these imaging techniques is contributing a "zero mortality" in the treatment of these infections. In our series of 29 consecutive patients with non-AIDS-related infections, no patient died as a direct result of a stereotactic surgical procedure. Two patients (7%) had new neurologic deficits after surgery. The only patient left with a permanent disability had a kidney allograft and subacute bacterial endocarditis. His condition deteriorated 6 hours after aspiration of a sterile abscess, when an intra-abscess hematoma was diagnosed and evacuated. In retrospect, this complication may have been avoided by less vigorous aspiration. Three of the four patients with nonviral infections who died were iatrogenically immunosuppressed for their organ transplants. These patients are difficult to treat, and given the current popularity of transplantation procedures, neurosurgeons will face more and more opportunistic infections. In general, the patients with abscesses did well. On the other hand, nonoperative mortality was extremely high for patients with viral encephalitides. This high mortality may have resulted from a delay in diagnosis and treatment or from the unavailability of highly effective antiviral agents at the time the biopsies were performed. The importance of early diagnosis and treatment of infection cannot be overemphasized. T.H. Flewett's warning about the management of HSE applies to the management of all cerebral infections: "It seems clear from everybody's published results [in the papers already given] if we wait to do biopsy until the clinical indications are unmistakable, we have waited so long that the patient, if he survives, will be left a severe neurological cripple." Because it is relatively noninvasive, stereotactic neurosurgery has been used increasingly to diagnose brain masses in patients with AIDS. We recommend its use for establishing diagnoses in all suspected cases of cerebral infection. We agree with Rosenblum et al: Empiric treatment of brain infections should be regarded as "radical." Such treatment should be reserved for patients who have an identifiable source of infection and causative organism or for patients who are clinically too unstable to undergo surgery.(ABSTRACT TRUNCATED AT 400 WORDS)

[Apoptosis mononuclear cells in sarcoidosis].

Sarcoidosis (SA) is a systemic granulomatous disorder of unknown etiology. The diagnosis is firmly established when clinicoradiological findings are supported by histological evidence of noncaseating epithelioid cell granulomas found on tissue biopsy Diagnosis of SA requires exclusion of other causes of granuloma formation, first of all Mycobacterium tuberculosis. Sarcoidosis is a disease known to be associated with various immunological alterations, including depression of cutaneous delayed-type hypersensitivity and an activation of monocytes/macrophages and a heightened helper T cell type 1 immune response at sites of disease but the mechanism leading to the persistent accumulation of inflammatory cells is not understood. In some studies, apoptosis is considered as factor in pathogenesis of sarcoid and tuberculous granuloma. Apoptosis--programmed cell death is a genetically encoded cell elimination program which ensures equilibrium between cell proliferation and cell death and by which crippled, useless or infected cells are eliminated as part of normal physiology. Aberrations in signalling, in receptors or alterations in mechanism of apoptosis are involved in the occurrence of tuberculosis. To date, there are a few of papers concerning alternations of apoptosis of peripheral and alveolar mononuclear cells in sarcoidosis have been published but the results have been controversial.

Orthopedic surgical management of hip and knee involvement in patients with juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis is the most common arthritic disease of childhood and a leading cause of childhood disability, affecting an estimated 300,000 US children and adolescents aged < or =16 years. Approximately 10% to 30% of patients experience functional deficits resulting from both the articular and systemic manifestations of their disease, including leg length inequality and deformity, that are often more crippling than joint destruction. Surgical intervention to treat bone and soft-tissue deformity, leg length inequality, and joint destruction is indicated when medical therapy has failed. Synovectomy, soft-tissue release, osteotomy, and epiphysiodesis are used to treat deformity and early joint destruction. Arthroplasty remains the primary therapy for joint destruction, although it is fraught with complications specific to this young patient population.

[Following up investigation of forensic psychiatry judicial expert mental testimony].

OBJECTIVE: To study different viewpoints to the conclusion and treatment, And the following up factors concerning social effect after the judgment of forensic psychiatry. METHODS: By letters, calls, visits and by investigation forms made by myself, 208 testimonied cases were followed up separately from judicial organ or other organizations and institutes, individuals testimonied or their family members, victims and their family members. RESULTS: Most testimonied individuals were married man of 30 years old or so whose average education were 4.84 years and testimonied by the public security organs. In criminal cases, violental criminals (129 cases, 83.77%) were much more than non-violental criminals (25 cases, 16.23%) and homicide criminals (44 cases, 28.57%) were most common in the former but civil cases were few. The rate of retstinvony was 2.93%. The testimonied individuals and their family members thought the condemn were appropriate (76.47%) for the people without criminal capacity and thought the condemn were appropriate (41.94%) or not appropriate (41.94%) for the people with criminal capacity. The opinions of the condemn for partial criminal capacity were between the former two cases. And they thought the condemn for 28 cases of non-guilty were appropriate (71.43%) and not appropriate (10.7%). 7 victims were dead. 10 victims were crippled. 10 victims restored to health. In 41 persons testimonied and set free with a verdict of "not guilty", 4 homicided again (4.87%), 2 set on fire (4.87%), 2 stealed (4.87%) and 3 had wrecked behavior (7.31%). CONCLUSION: So it is suggested that the department concerned should keep criminals with mental disorder under control and treatment.

[Fluorine as a factor in premature aging].

The use of fluorine compounds in various areas of medicine, particularly in dentistry, as well as in agriculture and industry became very popular in the second half of the 20th century. Fluorine owed this widespread acceptance to observations that its compounds stimulate ossification processes and reduce the prevalence of caries. Unfortunately, growing expectations overshadowed the truth regarding interactions of fluoride on the molecular level. The fact was often ignored that fluoride is toxic, even though laboratory data stood for a careful approach to the benefits of usage. Excessive exposure to fluoride may lead to acute poisoning, hyperemia, cerebral edema, and degeneration of the liver and kidneys. Acute intoxication through the airways produces coughing, choking, and chills, followed by fever and pulmonary edema. Concentrated solutions of fluorine compounds produce difficult to heal necrotic lesions. In spite of these dramatic symptoms, acute intoxications are relatively rare; the more common finding is chronic intoxication attributable to the universal presence of fluorine compounds in the environment. The first noticeable signs of excessive exposure to fluoride in contaminated water, air, and food products include discolorations of the enamel. Dental fluorosis during tooth growth and loss of dentition in adulthood are two consequences of chronic intoxication with fluorine compounds. Abnormalities in mineralization processes affect by and large the osteoarticular system and are associated with changes in the density and structure of the bone presenting as irregular mineralization of the osteoid. Fluorine compounds also act on the organic part of supporting tissues, including collagen and other proteins, and on cells of the connective tissue. These interactions reduce the content of collagen proteins, modify the structure and regularity of collagen fibers, and induce mineralization of collagen. Interactions with cells produce transient activation of osteoblasts, stimulate fibroblasts to produce collagenase, and trigger toxic reactions in osteocytes and chondrocytes of trabecular bone. Growing deformations of the skeleton reduce mobility and result in permanent crippling of the patient. Fluoride increases the mass of non-collagen proteins such as proteoglycans and glucosaminoglycans, accelerating skin aging even though protein biosynthesis is generally suppressed. The final outcome includes progressive vascular lesions and disorders of energy metabolism in muscles. In conclusions, the use of fluoride, particularly by dentists and pediatricians, must be controlled and adapted to individual needs. It is worth remembering that fluoride: is the cause of disability due to bone deformations and abnormalities in the musculoskeletal system; reduces the incidence of caries but do not protect against tooth loss; exerts an adverse effect of metabolic processes in the skin; accelerates calcification of vessels and thus reduces their elasticity; inhibits bioenergetic reactions, in particular oxidative phosphorylation, reducing physical activity of muscles. These findings suggest that fluorine may be yet another factor in accelerated aging and revive the dispute started more than two and half thousand years ago whether aging is a physiologic or pathologic process. The understanding of factors modifying the process of aging is the basis for preventive measures aimed at extending life and maintaining full psychosocial activity.

[Complication after vertebroplasty in patient with senile osteoporosis: strategical therapy to revise?].

Compression fractures are common in patients with osteoporosis and they are crippling. By 1892, Julius Wolff et al. realized that mechanical loads can affect bone architecture in living beings, but the mechanisms responsible for this effect were unknown, and it had no known clinical applications. In 2003 we know how this effect occurs and some of its applications. Vertebroplasty and kyphoplasty are relatively new techniques used to treat painful vertebral compression fractures. Percutaneous vertebroplasty is a therapeutic, interventional radiologic procedure. The technique consists of the percutaneous puncture of the affected vertebral body, followed by injection of bone cement into a vertebral body for the relief of pain, and the strengthening of the bone. This procedure also carries a risk of complications, and the fracture risk of vertebrae adjacent to cement vertebrae may be increased. The authors describe a case of vertebrae's fractures adjacent to cement vertebrae that are occurred in a patient with senile osteoporosis.

Socio-economic factors in obesity: a case of slim chance in a fat world?

The global obesity pandemic has been well-documented and widely discussed by the public, the media, health officials, the food industry and academic researchers. While the problem is widely recognised, the potential solutions are far less clear. There is only limited evidence to guide decisions as to how best to manage obesity in individuals and in populations. While widely viewed as a clinical and public health problem in developed countries, it is now clear that many developing countries also have to grapple with this problem or face the crippling healthcare costs resulting from obesity-related morbidity. There is also abundant evidence that obesity is socio-economically distributed. In developed countries persons of lower socio-economic position are more likely to be affected, while in developing countries, it is often those of higher socio-economic position who are overweight or obese. The aim of this paper is to briefly review the evidence that links socio-economic position and obesity, to discuss what is known about underlying mechanisms, and to consider the role of social, physical, policy and cultural environments in explaining the relationships between socio-economic position and obesity. We introduce the concept of 'resilience' as a potential theoretical construct to guide research efforts aimed at understanding how some socio-economically disadvantaged individuals manage to avoid obesity. We conclude by considering an agenda to guide future research and programs focused on understanding and reducing obesity among those of low socio-economic position.

Juvenile hyaline fibromatosis--a rare case report.

Fibromatosis is a group of disorders characterized by infiltrating fibroblastic proliferation. Among them Juvenile hyaline fibromatosis (J.H.F) is a rare, progressive, crippling autosomal recessive disorder diagnosed based on the characteristic clinicopathological findings of generalized cutaneous nodular lesions, gingival hypertrophy, flexion contractures of large joints with osteolytic lesions, and proliferating fibroblasts set within a hyalinized stroma. This disorder commonly manifests in childhood with family history of consanguinity and in siblings. A case of seven year old boy born to consanguinous parents is reported who presented with multiple subcutaneous nodules and gingival hypertrophy. Histopathological examination revealed proliferating fibroblasts embedded in an abundant homogeneous eosinophilic hyalinized matrix. The matrix showed PAS stain positivity, supporting the pathogenesis of this disorder as an inborn error of glycosaminoglycans metabolism. The differential diagnosis is discussed and the literature reviewed.

[French sanitary cooperation in developing countries].

In the face of a worsening public health situation throughout the world, French know-how in the field of sanitary cooperation warrants a far more active commitment from our country. Instead there have been only a few initiatives and we are witnessing a programmed decline of our presence. The statement issued by the Economic and Social Council in 2006--the highlights of which will be presented at this meeting--strongly advocates a voluntary program to revive sanitary cooperation by encouraging civic-spirited young people to express international solidarity through public service in impoverished areas. By compensating the shortage of human resources in developing countries, this program would allow immediate implementation of measures to promote action, training and research not as a replacement operation but rather as a genuine partnership. Deployment of our public aid for development in the field of health-care must not have a detrimental effect on the bilateral dimension of the relationship that indeed requires urgent strengthening. In addition tighter coordination of all parties involved in cooperation activities is needed to ensure greater efficiency and consistency. New funding sources (starting with the airline ticket tax) and greater mobilization of the European Union is necessary to ensure that developing countries have more financial resources to wage the "health-care battle". Reducing the "health-care gap" that cripples development and destabilizes societies must be the first priority of French Cooperation.

[Thrombo-embolic events in patients with mechanical prosthetic valves--echocardiography in diagnostic and therapeutic decision making].

Mechanical prosthetic valves (PV) are prone to thrombosis which may result in death or cripple hood due to PV blockade and embolisation (EM). In most pts with clinical symptoms of PV thrombosis (TPV) and in pts with a history of EM transthoracic echocardiography (TTE) and Doppler echocardiography are sufficient in diagnosing but not in stating the exact mechanism of PV stenosis. Transesophageal echocardiography (TEE) is a method of choice in the therapeutic decision making in this group of pts. PV replacement is indicated in pts with a thrombus floating in the left atrium (LA), large, peduncle-like thrombus in LA, TPV in mitral or aortic position with blockade of the disc and symptoms of heart failure < III NYHA, TPV with a high risk of spontaneous or post-thrombolytic EM (large thrombus on the PV annulus >5 mm or >0.8 cm2, highly mobile thrombus, history of EM). Thrombolysis is recommended in tricuspid TPV, TPV in mitral or aortic position with blockade of the disc and symptoms of heart failure III/IV NYHA. Intravenous heparin infusion can be effective as a short-term bridging to PV replacement or an alternative for thrombolysis in case of TPV with no signs of disc blockade. The results of thrombolysis and heparin infusion should be controlled by TEE. In pts with proper function of PV on clinical examination and negative history of EM, TTE/TEE should be considered if the risk factors of EM are present: incorrect INR control, atrial fibrillation, left ventricular dysfunction, III/IV NYHA, diabetes, certain types of PV cage PV, pivoting-disc PV other than Medtronic-Hall valve.

[Amiodarone for long-term treatment of ventricular arrhythmias].

As one of the causes of sudden cardiac death, recurrent ventricular arrhythmia represents a potentially life-threatening rhythm disorder. amiodarone is an effective anti-arrhythmic drug, the effectiveness of which is based on a prolongation of the action potential and thus a lengthening of the refractory period. In a multicenter, noncontrolled observational study of 482 patients aged between 21 and 89, it was found that amiodarone effectively suppressed life-threatening or crippling arrhythmias in a majority of the patients, even when they had previously been unresponsive to other anti-arrhythmic agents. Physical performance improved under the treatment. An analysis of the side effects observed revealed good tolerance at a low maintenance dose.

[Joint status after prophylactic substitution. An investigation in hemophilic children and adolescents in the DDR].

The joint condition in 98 children and adolescents with severe or moderate types of haemophilia participating in a summer camp between 1987 and 1989 had been analyzed. This was done by the centers' report and on the other hand with testing the patients by aid of the score system recommended by the World Federation of Hemophilia. According to the centers' reports more than 50% of all patients seem to be free of joint contractures. But according to the more sensitive score system more patients are affected by discrete symptoms of the chronic arthropathy. There had been discovered big differences between the centers and an increase of the patient's score by age. Not seldom an impairment of the findings within a single year was recognized. The joint problems in haemophiliacs are solved by the prophylactic treatment in an incomplete manner only. Lacks in the sophisticated evaluation of joints, with the management of prophylactic treatment as well as adjustment to prevention lead to avoidable damage crippling the patient on later age.

Does apoptosis contribute to CD4 T cell depletion in human immunodeficiency virus infection ?

Despite an extensive knowledge of the molecular characteristics of the human immunodeficiency virus (HIV) identified more than ten years ago as the cause of AIDS (acquired immune deficiency syndrome) (Barre-Sinoussi et al. 1983) some critical questions have not been answered yet: Is the progressive disappearance of CD4+ helper T lymphocytes, the hallmark of AIDS, directly related to the killing of infected cells by the virus? If not, how do CD4+T cells die? Is HIV using its viral factory to kill uninfected bystander cells? What causes the immune system collapse in HIV infection? In the past three years some important studies have provided stimulating clues suggesting that AIDS is not only related to the killing of host cells by HIV but is also a consequence of mechanisms of misactivation of the immune system, leading to anergy or apoptosis of non-infected effector cells. We discuss some of the in vivo and in vitro models providing evidence that HIV is able to kill and cripple the immune system either by acting directly on its targets or indirectly in bystander T cells keeping in mind that HIV disease must be considered as a multifactorial process.

Evaluation of a rapid test kit for detection of HBsAg/eAg in whole blood: a possible method for pre-donation testing.

In many developing countries where hepatitis B is endemic, positivity rate for HBsAg in donor blood is high, and in some places, up to 20% of donated blood has to be discarded for being HBsAg positive. This degree of wastage may be financially crippling for some developing countries. Pre-donation testing may be useful, so that donors who test HBsAg positive are deferred and wastage of costly blood bags is reduced. The study is to evaluate the suitability of the AMRAD kit, for pre-donation testing for HBsAg. One hundred and one (101) healthy blood donors were screened for HBsAg/eAg using the test kit. The same specimens were screened using Monolisa (ELISA) kits for HBsAg and eAg as the standard. True positive (TrP), False negative (FN), True negative (TrN) and, false positive (FP) values were then found, from which, sensitivity and specificity, were derived. The AMRAD test kit detected 93 specimens as negative and 8 specimens as positive for HBsAg as against 94 negatives (TrN) and 7 positives (TrP) by monolisa. Thus, one false positive (FP) result was found in using the kit while no false negative (FN) occurred. The findings in this preliminary study suggest that AMRAD kit may be a useful predonation screening test for HBsAg.

[Long-term results of peripheral vascular injuries in patients' limbs following reconstructive surgical procedures and influence on the quality of life].

Major vascular injuries in extremities are rare and constitute problems for surgeons and their sequelae strongly influence remote future of the patients. The aim of study is to evaluate surgical treatment of vascular injuries in extremities and some aspects of quality of life. Since 1983 until 2002 sixty four patients with vascular limbs injuries were treated in the department. Remote evaluation has been performed in 33 persons. Severity of limb injuries was measured by Mangled Extremity Severity Score (MESS). In 20 patients (60.6%) very good and good recent results were obtained, satisfactory in 24.2% and bad results in 15.2% persons. Evaluation of functional status has been made by means of locomotion test and Jebsen-Taylor's test, social approval questionnaire, depression Beck's scale, the scale of hypochondria and by an original questionnaire to evaluate the quality of life. As considerable interdependence has been found between the functional status and the following factors: injury severity expressed in MESS (p < 0.01), clinic reception procedure (p < 0.001), ischemia time (p < 0.01), coincidence of other injuries (fractures and dislocations, muscle, tendon and nerve lesions) (p < 0.01), limb amputation (p < 0.01). Hand function significantly influences the quality of life. Patients who were in shock after trauma in the remote assessment showed susceptibility to the lowered mood and depression (p < 0.05). Quality of life is strongly connected with the features of personality, correlating mainly with the mood and tendencies of concentration upon somatic symptoms. Function of the hand influences strongly the quality of life. Amputated patients may in spite of their crippling limitations are able to adapt to everyday life.
