Brachydactyly type B with its distinct facies and 'Cooks syndrome' are the same entity.

A sibling pair with brachydactyly type B born to a normal non-consanguineous couple are described and the severity of their condition discussed. It is proposed that a subgroup of individuals with brachydactyly type B principally involving the nails and distal phalanges, and also having distinct facies, might be identical to individuals having 'Cooks syndrome'.

Acceptability of extrusion cooked cereal/legume weaning food supplements to Ghanaian mothers.

Six cereal/legume mixtures were developed with the aid of computer-assisted optimization software from cereal and legume staples indigenous to the West African sub-region. The mixtures had 45-50% maize, 35-40% decorticated cowpeas and either blanched peanuts or decorticated soybeans as a source of lipid and complementary amino acids. Three processing schemes involving roasting, amylase digestion and extrusion cooking were employed. The proportion of ingredients in each cereal/legume blend was based on meeting the nutrient requirement of the 0.5-0.9-year-old infant and cost considerations. Nutrient composition of the blends (proximate, amino acid, mineral and vitamin composition) indicated that these formulations were adequate nutritionally as weaning supplements (Mensa-Wilmot et al, 2000a,2000b). These formulations were evaluated by mothers of weanling children based on their preferences with respect to color, flavor, texture and willingness to purchase the product assessed. A total of 133 one-on-one interviews and 23 group discussions were conducted (involving 6-12 respondents) with selected Ghanaian women. The mothers found the convenience of a weaning food made from local staples that could be processed on village/market scale very attractive.

Determination of rendering plant sterilization conditions using a commercially available ELISA test kit developed for detection of cooked beef.

A commercially available enzyme-linked immunosorbent assay (ELISA) test kit developed for detection of cooked beef in meat samples was used to determine appropriate heat treatment of rendered materials. An improved extraction procedure increased the absolute difference in R-values between 2 rendered materials treated under different conditions (average temperature 129 and 134 degrees C, respectively). To evaluate the influence of the main sterilization parameters on ELISA results, a factorial design approach was used. The parameters investigated were temperature, time, particle size, and meat composition. Lean meat samples containing beef and pork were sterilized under strictly controlled conditions in a laboratory autoclave. The experiments demonstrated that the R-values obtained with the ELISA test kit for beef are strongly influenced by temperature and time, whereas particle size has a minor influence. The proportion of bovine material did not have any impact on R-values. Autoclave-processed lean meat samples were analyzed by using an ELISA test kit for pork, which was validated in a collaborative trial. The ELISA test kit for pork proved to be more sensitive for the investigated parameters, thus verifying and extending previous investigations.

Inefficacy of cooking methods on mercury reduction from shark.

Shark and other carnivorous fishes present high potential risk of excessive contamination by mercury. The distribution of mercury throughout the body of blue shark--Prionace glauca--was analysed, and the effects on mercury levels by frying and baking in a laboratory oven, and in a microwave oven, were measured. There was no significant statistical difference in mercury levels in the samples taken from regions near the head, or from central and tail parts, indicating homogeneous distribution of the metal in muscles throughout the body. Frying and baking did not affect original mercury levels present in blue shark. This study indicates that specific studies are needed to define the efficacy or inefficacy of the cooking methods on mercury reduction from fish, in order to clearly resolve divergent opinions in the literature.

Prevalence of filarial antigenaemia in Papua Nnew Guinea: results of surveys by the School of Public Health and Tropical Medicine, James Cook University, Townsville, Australia.

During the period from 1991 to 1997 the School of Public Health and Tropical Medicine, James Cook University carried out filariasis surveys in several parts of Papua New Guinea using the newly introduced Onchocerca gibsoni monoclonal (Og4C3) and immunochromatographic test (ICT) antibody-based assays for filarial antigen and, in some cases, a Knott's test for microfilariae. The average prevalence of filarial antigenaemia and microfilaraemia was 56% and 35% respectively confirming earlier survey results that filariasis is hyperendemic in many parts of the country. The antigen tests detected 25% more cases than the Knott's test and the simplicity of the ICT and its capacity to produce almost instant results make it an ideal tool for surveys.

Rural initiatives at the James Cook University School of Medicine: a vertically integrated regional/rural/remote medical education provider.

Building on the success of the former North Queensland Clinical School/Royal Australian College of General Practitioners collaboration in North Queensland, the James Cook University School of Medicine continues to develop a model of close collaboration with stakeholder groups that provides medical education services to dispersed communities in northern Australia. The flagship themes of the programme (rural & remote, Indigenous and tropical health) are essential to the regional mission to improve the health care of people in northern Australia. Selection processes target regional and rural background students and Aboriginal and Torres Strait Islander students. An innovative curriculum design reinforces interest in and relevance to regional health needs. The present paper provides an overview of the roles and early progress of the new School of Medicine, with the aim of keeping rural doctors informed about the continuing development of a successful 'national standards/local collaboration and control' model of education service delivery.

[Clostridium perfringens in raw and cooked meats and its relation with the environment in Costa Rica].

The presence of Clostridium perfringens in eight slaughter houses from Costa Rica was analyzed using the Most Probable Number (MPN) technique, in order to assess the risk of acquiring a food borne intoxication due to consumption of contaminated meat. C. perfringens was detected in 29 (88%) out of 33 soil samples collected from the slaughter house surroundings (average 6.7 x 10(2) MPN/g), as well as in 70 (93%) out of 75 intestinal contents of slaughtered animals (average 3 x 10(4) MPN/g), in 42 (55%) out of 76 samples of slaughtered meat (average 2.2 x 10(4) MPN/g) and in 30 (61%) out of 49 retail meats (average 8 x 10(3) MPN/g). In addition, the presence of this bacterium was evaluated in ten retail meat markets located in the Metropolitan Area of Costa Rica, where it was isolated from 15 (75%) out of 20 samples of ground meat and from 28 (36%) de 78 stew meat samples (average 1.9 x 10(3) and 7.5 x 10(2) MPN/g, respectively). Only one out of 35 samples of cooked meat obtained from 32 restaurants that utilize heated water baths (average temperature of 82 degrees C) was positive for C. perfringens (4 MPN/g, temperature 72 degrees C). Out of 1121 bacterial isolates obtained, 250 were evaluated for enterotoxigenicity. Only 3 (1.2%) of these tested positive for enterotoxin production, probably because most wild strains are not toxin producers, even though they can be induced to produce it as a result of repeated thermal shocks. The present results urge the adoption of adequate preventive measures and high sanitary standards in the meat processing industry in Costa Rica, in order to minimize the risk of food-borne intoxications caused by C. perfringens, due to its widespread distribution and potential human health hazard.

Use of the Cook airway exchange catheter in "bridging" the potentially difficult extubation: a case report.

The Cook airway exchange catheter (CAEC) is a long, flexible, hollow tube designed to facilitate the exchange of an in situ endotracheal tube. The primary use of the CAEC had been as a tube exchanger in the critical care setting. The unique characteristics of this device led us to use and report the use of the CAEC in the anesthetic management of a morbidly obese patient with significant tracheal deviation secondary to an enlarged thyroid gland. The potential strategies that we considered in this case were (1) extubating the patient in a deep plane of anesthesia; (2) extubating conventionally, with the patient awake; and (3) extubating the patient awake, with a "bridge" to full extubation. After carefully weighing the options, we decided to use the CAEC as a tool that would allow us, if the patient did well, to go forth to full extubation, or, if the patient's condition deteriorated, to return to reintubation, or even oxygen insufflation, which would create a "bridge" to extubation.

[Toxic effects of cooking oil fumes on testis of rats].

To investigate the toxic effects of inhaling cooking oil fumes (COF), 30 male SD rats were submitted to inhaling COF at a constant concentration and duration (35-53 mg/m3, 30 min/d) for different length of time (20, 40 and 60 days), and examined for change in body weight, spermtogenesis and content of trace elements in their testes. The results indicated that the body weight and the ratio of gonad to body weight in rats inhaling COF for 40 and 60 days were lower than those in the controls (P < 0.05). Fewer alive sperms and more malformed or dead sperms were fond in the epididymis of rats exposed to COF for 40 and 60 days. Among the four trace elements(Zn, Mn, Cu, Fe) studied, the content of Zn in the testis was found to be significantly decreased and that of Mn significantly increased in the rats exposed to COF for longer periods. There existed a time-effect relationship in all of the above findings. The results suggested that inhaling COF was significantly toxic to male gonads, and the imbalance of Zn, Mn might be contributive to the damage of testis.

[Effects of cooking oil fume condensate on cellular immunity and immunosurveillance in mice].

Rapeseed oil fume, soyabean oil fume and salad oil fume condensate were injected respectively to female Kunming mice. The results showed that, compared to control group, in mice exposed to low level of rapeseed oil fume condensate, the delayed hypersensitivity responses were inhibited, while in mice exposed to high level of that, the delayed hypersensitivity responses and the NK cells activity were inhibited. In mice exposed to high level of soyabean oil fume condensate the delayed hypersensitivity responses, the NK cells activity and CaM activity were inhibited. The results of these parameters were all statistically significantly different from those in control mice. In mice exposed to low level of salad oil fume condensation, the delayed hypersensitivity responses and the NK cells activity were significantly inhibited. The results indicated that cooking oil fume could affect immune system of animals.

[Multivariate analyses of causal factors included cooking oil fume and others in matched case-control study of lung cancer].

This paper reports the study of 1:1 matched case-control for eighty-three cases of primary lung squamo-carcinoma and seventy-one cases of primary lung adenocarcinoma from urban districts of Nanjing. The result of conditional analysis reveals that cooking oil fume is a common risk factor for lung squamo-carcinoma and adenocarcinoma. Its relative risk and population attributable risk (%) are 3.8138, 3.4466 and 51.56%, 60.99% respectively. Besides, smoking, family history of tumors, fuel types and keeping warm by coal stove in winter are selected as risk factors for lung squamo-carcinoma, while history of chronic bronchitis and family history of tumors are screened as risk factors for lung adenocarcinoma. Summary population attributable risk for all five factors of lung squamo-carcinoma and all three factors of lung adenocarcinoma are 99.47% and 76.65% respectively.

[Successful airway management using a Cook airway exchange catheter as an emergent dilator for life-threatening neoplastic stenosis of the trachea: a case report].

A 68-year-old male patient was referred to our hospital for his intractable and progressive dyspnea. Chest roentgenography and computerized tomography revealed severe lower tracheal stenosis due to neoplastic invasion. The tumor completely surrounded his trachea for 5 cm length and the remaining internal diameter of the trachea was less than 5 mm. To avoid fatal asphyxia, emergency airway security was required. After placing both femoral arterial and venous cannulae for percutaneous cardiopulmonary support system, we dilated the stenotic trachea using a Cook airway exchange catheter (CAEC), and then an armored tube of 7 mm inner diameter was introduced through the CAEC. The maneuver was smoothly completed within one minute without any complications. His dyspnea was completely relieved. Following chemotherapy and radiotherapy were so effective that the endotracheal tube was extubated two weeks later uneventfully. After several series of chemotherapy and 50 Gy irradiation, the patient was discharged three months later without symptoms. We concluded that CAEC could be one of the life-saving instruments for emergent airway management in case of severe organic tracheal stenosis.

Effectiveness of single detachable COOK coils in closure of the patent ductus arteriosus.

OBJECTIVE: To assess the efficacy and safety of single detachable coils in the closure of PDAs. METHODS: Review of cases in which a single detachable coil was implanted. The diameter of the ampulla was used to select coil size. Diameters of the PDA, ampulla and residual shunts were measured. RESULTS: There were 36 patients with a mean minimal ductal diameter of 2.2 +/- 0.6 mm. Immediately after implantation, 46% of the patients had residual shunts and after 24 hours, only 28%. A final spontaneous closure rate of 94% was observed after 21 months. There was a significant (p < 0.01) difference when minimal PDA diameter of those who had complete closure within 24 hours (median: 2 mm, interquartile range: 1.7-2.3 mm) was compared to those with a residual shunt (median: 2.5 mm, interquartile range: 2.3-3.2 mm). Negligible complications were experienced. CONCLUSIONS: Single Cook detachable coils are effective for PDA closure with a low complication rate if properly selected. There is a high rate of spontaneous closure of trivial residual shunts. In patients with a minimal ductal diameter > or = 2 mm and a residual shunt, more coils may be considered.

New gas chromatography-olfactometric investigative method, and its application to cooked Silurus glanis (European catfish) odor characterization.

A new gas chromatography-olfactometric method, gas chromatography-global olfactometry omission detection (GC-GOOD), was applied to dynamic headspace odor extracts of Silurus glanis (European catfish). The GC-GOOD method is based on the omission test theory and uses a gas chromatograph coupled with a three-way valve and an a flame ionization detector. The GC-GOOD method enabled the identification of key families of volatile compounds in the S. glanis global odor and the elucidation of the interactions occurring between these families. Significant main effects were observed for the families of volatile compounds exhibiting cooked odor, grassy odor and alcohol, solvent and plastic odors. Omission of these families involved a loss of odor similarity.
