Growth of intraperitoneally transplanted ascites hepatoma, AH39 cells, in the area of gelatin sponge inoculation into the abdominal wall of Donryu rats.

A piece of gelatin sponge was inoculated submesothelialy in the abdominal wall of female Donryu rats, and 1, 2, 4, 7, 14, or 21 days later, 1 x 10(5) cells of ascites hepatoma AH39 were intraperitoneally transplanted. Tumor cells were detected in the area of gelatin sponge inoculation in each group 2 to 5 days after the transplantation. Generally, the number of rats in which tumor cells were detected was larger in those transplanted with tumor cells within a short period after the gelatin sponge inoculation. Changes in the area of gelatin sponge inoculation and nontreated area in the rats which received intraperitoneal transplantation of the tumor cells on the 1st day after gelatin sponge inoculation were studied. Tumor cells were detected in the area of gelatin sponge inoculation as early as 2 days after the tumor transplantation, whereas they were detected on the 14th day in nontreated areas. Tumor detection was preceded by inflammatory reaction in both cases.

Anterior tension-free repair under local anesthesia of abdominal wall hernias in continuous ambulatory peritoneal dialysis patients.

BACKGROUND: A 10-year experience of abdominal wall hernia repair performed with anterior tension-free mesh or plug technique under local anesthesia in end-stage renal failure patients submitted to continuous ambulatory peritoneal dialysis (CAPD) is described in order to assess the safety and effectiveness of this approach. METHODS: Between January 1993 and December 2002, 18 hernia repairs were performed under local anesthesia in 16 patients (14 males and two females) with a mean age of 70 years (48-78). One umbilical and three unilateral inguinal hernias were observed and repaired before starting peritoneal dialysis (PD), while two umbilical, eight unilateral, and two bilateral groin hernias developed and were then treated during PD. Repairs were performed electively in all but one case, which was an emergency operation for strangulation. An ipsilateral scrotal swelling was also present in two indirect unilateral inguinal hernias. In these cases, the hernia sac was ligated before entering, while in the others it was simply dissected and inverted. RESULTS: Patients were discharged the same day or the day after surgery. No local or general immediate or late complications occurred. CAPD in subjects operated on during PD treatment was resumed the same day of surgery. In no instance was hernia recurrence or leak of dialysis solution observed at follow-up examinations. CONCLUSIONS: The absence of surgical and general complications and the nearly immediate resumption of PD indicate the anterior tension-free repair under local anesthesia as a safe and effective technique for CAPD patients even in an ambulatory or day-surgery setting.

Use of the LAP DISK (abdominal wall sealing device) in laparoscopically assisted myomectomy.

OBJECTIVE: To evaluate the efficacy of an abdominal wall sealing device (the LAP DISK) used during laparoscopically assisted myomectomy (LAM). DESIGN: Retrospective study. SETTING: Tottori University Hospital, Yonago, Japan. PATIENT(S): All 43 patients who underwent LAM using the LAP DISK. INTERVENTION(S): Ultrasonography and magnetic resonance imaging. MAIN OUTCOME MEASURE(S): Treatment strategy, operative outcome, and postoperative pregnancy rate. RESULT(S): Weight and size of the myomas removed ranged from 40-700 g (mean: 208.0 g) and 2-10 cm (mean: 5.4 cm). Mean blood loss was 42.3 mL. Half of the 18 patients who had been diagnosed with primary infertility for >2 years became pregnant without postoperative assisted reproductive techniques. CONCLUSION(S): The LAP DISK, a useful device for LAM, allows surgeons to remove myomas safely and repair uterine defects effectively while minimizing blood loss and trauma.

A "plastic" sutureless abdominal wall closure in gastroschisis.

BACKGROUND/PURPOSE: Several techniques are described for closure of the gastroschisis abdominal wall defect. The authors describe a technique that allows for spontaneous closure that is simple, cosmetically appealing, and minimizes intraabdominal pressure after bowel reduction. METHODS: Under either general anesthetic or analgesia with sedation, the gastroschisis bowel is decompressed, and the bowel is primarily reduced. The gastroschisis defect is covered with the umbilical cord tailored to fit the opening, and 2 Tegaderm (3M Healthcare, MN) dressings reinforce the defect ("plastic closure"). Intragastric pressure is monitored during and after the procedure. If primary reduction is not possible, the bowel is reduced daily via a spring-loaded silo (Bentec Medical, CA). After reduction of the bowel, the defect is allowed to close spontaneously using the "plastic closure" technique. The authors prospectively treated a cohort of patients with gastroschisis that included simple to complex cases using this technique. RESULTS: Ten children with gastroschisis were treated; 6 of these children had a primary reduction and simple closure of their defect using the "plastic closure." In the remaining 4 children, the plastic closure was used either primarily or secondarily to silo placement, despite the need for repair of complex intestinal anomalies. The average times to first feeding and discharge were 12.5 and 28.3 days, respectively. Six of the 10 children (60%) had small umbilical hernias, and only 1 underwent operative repair at 13 months of age. CONCLUSIONS: The plastic closure of gastroschisis is simple, safe, and cosmetically appealing. Intraabdominal pressures are well controlled, and the umbilical position remains centrally located in this sutureless technique. Umbilical defects can occur but are observed for spontaneous closure like most primary umbilical hernias.

Outcome for fetuses with abdominal wall defects detected by routine second trimester ultrasound.

The outcome for 70 fetuses with abdominal wall defects detected by routine second trimester ultrasound during the years 1983-90 was investigated. In 65 of the 70 cases the prenatal diagnoses were correct. Thirty-two women had their pregnancies terminated by legal abortions. Thirty-eight pregnancies continued. Six of these ended with spontaneous abortion or intrauterine fetal death. Thirty-two infants were born alive. Nearly all cases with associated malformations and all cases with chromosomal defects were in the omphalocele group. The neonatal survival for infants with gastroschisis and infants with omphalocele without associated malformations or chromosomal defects was high (96%).

[Classification of postoperative hernias of the anterior abdominal wall].

The author has developed and suggested a classification of hernias of the anterior abdominal wall. The new classification is grounded on observations over numerous clinical manifestations and peculiarities of the disease and subdivides these hernias according to the condition and shape of the hernia sac, localization, number and size of the hernias, time of the occurrence and incidence rate of the hernia following an operation, presence of concommitant diseases and the degree of disability of the patients.

Prophylaxis with enoxaparin can produce a giant abdominal wall haematoma when associated with low doses of aspirin among elderly patients suffering cough attacks.

We report three patients aged 75-80 years observed in the Emergency Room with severe anaemia (requiring transfusion) due to a large abdominal wall haematoma while receiving standard prophylaxis against venous thromboembolism (40 mg/day enoxaparin for 6 days on average). All of them concomitantly received 100 mg/day aspirin because of previous ischaemic heart disease and presented similar clinical features: sudden onset of abdominal pain during a severe cough episode due to bronchial infection. A giant haematoma in the rectus abdominis muscle was recognized (by computed tomography) in every case. The cough has been related with this complication in some reports but its association with antiplatelet drugs and low molecular weight heparin could increase the risks in older patients. Sudden abdominal pain must alert the clinician to the rectus muscle sheath haematoma in order to avoid the risks of an exploratory laparotomy.

Fungal abdominal wall abscess in a renal transplant recipient.

The incidence of fungal infection is significantly higher in patients with endstage renal disease and renal transplant recipients than in normal individuals. Candidia Albicans is an uncommon cause of abdominal wall abscess. We describe a 37 year-old renal transplant recipient with such an infection. He presented with atypical clinical manifestations and an insidious course, but was successfully treated with antifungal therapy.

Paraffinoma in anterior abdominal wall mimicking liposarcoma.

Paraffinoma of breast is a recognized complication of paraffin injection for breast augmentation. Liquid paraffin can extend along fascial planes to involve adjacent tissues. A rare case of paraffinoma in anterior abdominal wall, which was misdiagnosed as a soft tissue liposarcoma before surgical excision, is reported. It was heterogeneous with marked posterior acoustic shadowing and small peripheral cysts on ultrasound. On MRI, it had ill-defined margins and was heterogeneous in signal intensity. Small round components which were hypointense on all sequences were demonstrated. There is significant overlapping of imaging features between paraffinoma and soft tissue liposarcoma. Histological differentiation from well-differentiated liposarcoma may also be difficult. A detailed clinical history of previous paraffin injection for breast augmentation is very important for correct interpretation of imaging and histopathological findings.

[Gastrointestinal stromal tumor of the abdominal wall. An unusual localization of a rare tumor].

Gastrointestinal stromal tumors (GIST) are the most common mesenchymal tumors of the gastrointestinal tract. We describe here for the first time a patient with a huge GIST of the abdominal wall without any relation to the gastrointestinal tract, omentum, or mesentery. With regard to the size of 24 cm and a low mitotic index, this GIST is considered an intermediate risk for metastasis. Radical surgical resection was performed with negative pathologic resection margins. The classic immunohistochemical phenotype of the tumor described facilitates the differential diagnosis to exclude abdominal desmoid tumor and solitary fibrous tumor (SFT). In the case of metastasis, therapeutic nihilism no longer seems justified with the availability of imatinib, a tyrosine kinase inhibitor, which shows encouraging results in the therapy of metastatic GIST.

[Imaging of connective tissue formations of the sub-umbilical anterior abdominal wall].

New high resolution imaging modalities (CT and MRI) can distinguish between bladder and anterior abdominal wall pelvic connective tissue. The main structure is the umbilicovesical fascia located anterior to the peritoneum and posterior to the transversalis fascia. This layer divides the space between an anterior prevesical space and a posterior perivesical space. An understanding of these spaces facilitates the clinical identification and topography of pelvic the fluid collections.
