Microlens arrays for integral imaging system.

When designing a system capable of capturing and displaying 3D moving images in real time by the integral imaging (II) method, one challenge is to eliminate pseudoscopic images. To overcome this problem, we propose a simple system with an array of three convex lenses. First, the lateral magnification of the elemental optics and the expansion of an elemental image is described by geometrical optics, confirming that the elemental optics satisfies the conditions under which pseudoscopic images can be avoided. In using the II method, adjacent elemental images must not overlap, a condition also satisfied by the proposed optical system. Next, an experiment carried out to acquire and display 3D images is described. The real-time system we have constructed comprises an elemental optics array with 54 H x 59 V elements, a CCD camera to capture a group of elemental images created by the lens array, and a liquid crystal panel to display these images. The results of the experiment confirm that the system produces orthoscopic images in real time, and thus is effective for real-time application of the II method.

Integrated waste management for rural development in Egypt.

Rural areas generate a large amount of plant and animal residues that can be recycled and utilized instead of relocation and/or burning. This will lead to increasing the benefits from agricultural sector in rural communities and ensuring a better environment. To increase the economic output and environmental benefits of recycling agricultural residues, integrated system should be considered, e.g., energy--compost-recycled water system; composting--co-composting system; food-feed compost system, ensilage of crop residues. The present work was a pilot study for optimizing integrated systems for bioconversion agricultural residues completed by establishing a Training Center for Recycling Agricultural Residues (TCRAR) thereby ensuring the dissemination of the technical, environmental, and socioeconomic aspects to farmers, live stock producers, extensions service staff, and private sector. Three integrated subsystems for bioconversion of agricultural residues were developed. They were based on (i) energy--manure-recycled water system, (ii) composting and co-composting system, and (iii) food-feed/compost system.

Physiological and technological aspects of large-scale heterologous-protein production with yeasts.

Commercial production of heterologous proteins by yeasts has gained considerable interest. Expression systems have been developed for Saccharomyces cerevisiae and a number of other yeasts. Generally, much attention is paid to the molecular aspects of heterologous-gene expression. The success of this approach is indicated by the high expression levels that have been obtained in shake-flask cultures. For large-scale production however, possibilities and restrictions related to host-strain physiology and fermentation technology also have to be considered. In this review, these physiological and technological aspects have been evaluated with the aid of numerical simulations. Factors that affect the choice of a carbon substrate for large-scale production involve price, purity and solubility. Since oxygen demand and heat production (which are closely linked) limit the attainable growth rate in large-scale processes, the biomass yield on oxygen is also a key parameter. Large-scale processes impose restrictions on the expression system. Many promoter systems that work well in small-scale systems cannot be implemented in industrial environments. Furthermore, large-scale fed-batch fermentations involve a substantial number of generations. Therefore, even low expression-cassette instability has a profound effect on the overall productivity of the system. Multicopy-integration systems may provide highly stable expression systems for industrial processes. Large-scale fed-batch processes are typically performed at a low growth rate. Therefore, effects of a low growth rate on the physiology and product formation rates of yeasts are of key importance. Due to the low growth rates in the industrial process, a substantial part of the substrate carbon is expended to meet maintenance-energy requirements. Factors that reduce maintenance-energy requirements will therefore have a positive effect on product yield. The relationship between specific growth rate and specific product formation rate (kg product.[kg biomass]-1.h-1) is the main factor influencing production levels in large-scale production processes. Expression systems characterized by a high specific rate of product formation at low specific growth rates are highly favourable for large-scale heterologous-protein production.

Online motion-gated dynamic three-dimensional echocardiography in the fetus--preliminary results.

The aim of this study was to visualise the fetal heart in dynamic three dimensions (4-D) during an ultrasound (US) scan (online), rather than after (offline). With special pairing and sequential setting to minimise interference between two scanners, umbilical arterial Doppler waveforms (UADWs) from one scanner were used as an online motion gating source to trigger simultaneous 3-D cardiac structural data acquisition by another. Of 25 data sets from 10 fetuses, 18 were acquired in 15 to 30 s per set with > or = 50% Doppler waveforms efficiently converted to triggering signals. Of 15 valid 4-D data sets, 10 were reconstructed in 2 to 20 min, compared to over 2 h previously reported (mainly for offline gating). Fine structures (including chordae tendinae and trabecular muscles) were depicted in six sets. The main problems in degrading 4-D images were extensive shadowing (6) from bony structures during rigid mechanical scanning, and random motion artefacts (6) from prolonged setting-up time with a complex combination of several systems. Integration of these systems is, therefore, recommended.

Receptors that mediate cellular dependence.

Cells depend for their survival on stimulation by trophic factors and other prosurvival signals, the withdrawal of which induces apoptosis, both via the loss of antiapoptotic signaling and the activation of proapoptotic signaling via specific receptors. These receptors, dubbed dependence receptors, activate apoptotic pathways following the withdrawal of trophic factors and other supportive stimuli. Such receptors may feature in developmental cell death, carcinogenesis (including metastasis), neurodegeneration, and possibly subapoptotic events such as neurite retraction and somal atrophy. Mechanistic studies of dependence receptors suggest that these receptors form ligand-dependent complexes that include specific caspases. Complex formation in the absence of ligand leads to caspase activation by a mechanism that is typically dependent on caspase cleavage of the receptor itself, releasing proapoptotic peptides. Cellular dependence receptors, considered in the aggregate, may thus form a system of molecular integration, analogous to the electrical integration system provided by dendritic arbors in the nervous system.

Multipoint videoconferencing in health: a review of three years' experience in Queensland, Australia.

The objective was to review multipoint videoconferencing in Queensland Health from July 1996 to June 1999. Most videoconferencing has been conducted using desktop systems connected by integrated systems digital network (ISDN) at 128 kbps. Data on utilization and problems were extracted from monthly reports and a survey was conducted. Multipoint videoconferencing increased steadily over the 3 years from just and handful of conferences per month to 101 conferences and 703 hours of bridge use per month. Primary uses were education and administration. Relatively few technical failures and operator errors were recorded. But by 1999, late connection and low attendance were major problems. Survey responses indicated that multipoint videoconferencing met expectations at the great majority of sites. Most respondents were satisfied with the level of administrative and technical support provided. In this large and decentralized state, multipoint videoconferencing has proven a useful and effective means of bringing healthcare workers together for a common purpose, supplementing face-to-face events and other encounters mediated by communications technology such as satellite broadcasts and audioconferences.

Clinical process improvement: engage first, measure later.

The Institute for Clinical Systems Integration (ICSI) is a non-profit quality improvement organization that bridges 20 medical groups and integrated health care systems in the Minneapolis-St. Paul area. Created in 1992 by HealthPartners, HealthSystem Minnesota, and Mayo Clinic as a part of a contract awarded by the Buyers Health Care Action Group, a purchaser coalition, ICSI's aim is collaborative, continual improvement of the clinical care provided by its member institutions. ICSI's main focus is health care guideline developement and implementation. This article describes this process, as well as evidence of program effectiveness and "lessons learned."

Nurses' concerns about using information systems: analysis of comments on a computerized nursing care plan system in Taiwan.

AIMS AND OBJECTIVES: The purpose of this study was to pursue a deeper understanding of nurses' perceptions of a computerized nursing care plan as written in comments on a survey. BACKGROUND: Studies have examined nurses' perceptions of computer use, but comments written on surveys of computerized nursing care planning have never been reported and the pattern and meaning of such feedback have not been systematically analysed. DESIGN: Of 738 nurses in Taiwan who completed a two-part survey to evaluate a computerized nursing care plan, 202 responded on part II to an open-ended question about overall perceptions of a nursing information system. METHODS: A sample of 10 questionnaires with comments was subjected to qualitative content analysis. Key words and phrases were identified, then grouped into categories, which were used to code the remaining respondents' comments. Another 20 were used to pilot test these coding categories. The final coding categories were applied to all 202 questionnaires with comments for data analysis. RESULTS: Nurses were concerned about inconvenient access to computers; reduced work efficiency; inability to individualize patient care and nursing specialty deficiencies; poor content design, system function, and system integration; using the system as a policy requirement; and privacy and legal issues. CONCLUSIONS: Hardware availability, content design and user training/education programmes are critical issues that affect nurses' use of computers in their daily practice. RELEVANCE TO CLINICAL PRACTICE: These comments can be applied in strategic planning and staff development programmes to further use of information systems in patient care.

Oral health in Iran.

The health network in the Islamic Republic (I.R.) of Iran is an integrated public health system with a four-level Dental Health Care System integrated into it by 1997. The first level is one of primary prevention at 'health houses', at the next, oral hygienists and dentists in health centres perform basic oral health care services such as fillings, scaling and extractions. At the third level, dentists manage and treat oral diseases in 'urban health centres, while the last level is for advanced treatment by specialists in university health centres in the big cities. There are about 13,000 dentists nationwide (1 dentist: 5,500 population) and nearly 1,200 specialists in universities and private practices. Data from surveys in the past two decades, show a marked decline in dental caries from DMFT of 4 to 1.5 in 12-year-old children. However, the general level of oral health is still not satisfactory, particularly among children. The percentage of caries-free children (deciduous and permanent teeth) among 6- and 9-year-olds is 13.8 and 11.5 respectively and more than 50% of 12-year-old children have caries experience, with the decayed component being the greatest component. The main objective would be to cope with the dental caries problem in primary teeth and, in this respect, the national oral health plan should be aimed at developing oral hygiene skills, reducing the frequency of sugar intake, instituting water fluoridation, improving access to fissure sealants and regular dental care, and finally promoting dental health services toward minimum treatment intervention and effective preventive strategies and health promotion.

Sensitivity and specificity of the ViroSeq human immunodeficiency virus type 1 (HIV-1) genotyping system for detection of HIV-1 drug resistance mutations by use of an ABI PRISM 3100 genetic analyzer.

The ViroSeq human immunodeficiency virus type 1 (HIV-1) genotyping system is an integrated system for identification of drug resistance mutations in HIV-1 protease and reverse transcriptase (RT). Reagents are included for sample preparation, reverse transcription, PCR amplification, and sequencing. Software is provided to assemble and edit sequence data and to generate a drug resistance report. We determined the sensitivity and specificity of the ViroSeq system for mutation detection using an ABI PRISM 3100 genetic analyzer with a set of clinical samples and recombinant viruses. Twenty clinical plasma samples (viral loads, 1,800 to 10,500 copies/ml) were characterized by cloning and sequencing individual viral variants. Twelve recombinant-virus samples (viral loads, approximately 2,000 to 5,000 copies/ml) were also prepared. Eleven recombinant-virus samples contained drug resistance mutations as 40% mixtures. One recombinant-virus sample contained an insertion at codon 69 in RT (100% mutant). Plasma and recombinant-virus samples were analyzed using the ViroSeq system. Each sample was analyzed on three consecutive days at each of three testing laboratories. The sensitivity of mutation detection was 99.65% for the clinical plasma samples and 99.7% for the recombinant-virus preparations. The specificity of mutation detection was 99.95% for the clinical samples and 100% for the recombinant-virus mixtures. The base calling accuracy of the 3100 instrument was 99.91%. Mutations in clinical plasma samples and recombinant-virus samples were detected with high sensitivity and specificity, including mutations present as mixtures. This report supports the use of the ViroSeq system for identification of drug resistance mutations in HIV-1 protease and RT genes.

Development of the mammalian urethra is controlled by Fgfr2-IIIb.

Development of external genitalia in mammalian embryos requires tight coordination of a complex series of morphogenetic events involving outgrowth, proximodistal and dorsoventral patterning, and epithelial tubulogenesis. Hypospadias is a congenital defect of the external genitalia that results from failure of urethral tube closure. Although this is the second most common birth defect in humans, affecting one in every 250 children, the molecular mechanisms that regulate morphogenesis of the mammalian urethra are poorly understood. We report that mice lacking the IIIb isoform of fibroblast growth factor receptor 2 (Fgfr2) exhibit severe hypospadias. Urethral signaling regions, as indicated by Shh and Fgf8 expression, are established in Fgfr2-IIIb null mice; however, cell proliferation arrests prematurely and maturation of the urethral epithelium is disrupted. Fgfr2-IIIb-/- mutants fail to maintain the progenitor cell population required for uroepithelial renewal during tubular morphogenesis. In addition, we show that antagonism of the androgen receptor (AR) leads to loss of Fgfr2-IIIb and Fgf10 expression in the urethra, and an associated hypospadias phenotype, suggesting that these genes are downstream targets of AR during external genital development. Genitourinary defects resulting from disruption of AR activity, by either genetic or environmental factors, may therefore involve negative regulation of the Fgfr2 pathway. This represents the first example of how the developing genitourinary system integrates cues from systemically circulating steroid hormones with a locally expressed growth factor pathway.

Can mental health integration in a primary care setting improve quality and lower costs? A case study.

OBJECTIVE: To describe the successful implementation of an evidence-based, integrated quality improvement mental health program in a primary care setting. SUMMARY: Intermountain Healthcare (IHC) has aligned resources around a conceptual framework that emphasizes clinic and community accountability, family and consumer health focused on recovery rather than disease, and enhanced decision making through partnerships and automation. The mental health integration system includes an integrated team led foremost by the patient and family with vital defined roles for primary care providers, care managers, psychiatrists, advanced practice registered nurses, support staff, and the National Alliance for the Mentally Ill. Pharmacists have assumed training functions on the team and have the potential to play more vital roles. CONCLUSION: The IHC experience demonstrates that mental health services can be effectively integrated into everyday practice in a primary care setting. Clinical and financial burden can be decreased for the health care team, patients, and family.

An integrated clinical computer system: implications for a medical information services department.

The hybrid record--both paper and electronic--is a cumbersome byproduct of the evolution to the computer-based patient record. Nancy Walker describes how William Beaumont Hospital managed its information services with this mixed media record. Her article provides practical solutions for health information managers including downtime procedures, report distribution plans, release of information steps, and audit procedures for maintaining confidentiality.
