Hospital ergonomics: a qualitative study to explore the organizational and cultural factors.

The primary objective was to identify the characteristics of the health care industry with respect to organizational and cultural factors and consider how these might impact on the practice of ergonomics. Qualitative methodology was chosen as a suitable approach. This was supported by a middle ground philosophical position. Twenty-one interviews were carried out with academics and practitioners using a questionnaire proforma which developed iteratively over the 18 months of the project. A progressive four stage sampling strategy was used starting with purposive sampling to spread the net. Suggested contacts were then followed up (snowball sampling), before the third stage of intensity sampling to focus on participants with specific experience in hospital ergonomics. A final strategy of analysis sampling sought extreme and deviant cases to achieve theoretical saturation. The analysis resulted in three categories: organizational, staff and patient issues. The organizational issues included both the size and complexity of the National Health Service. For example, three hierarchical lines were identified in the management structure: an administrative line, a professional line and a patient-focused clinical management line. One of the surprising findings for the staff issues was the perceived lack of ergonomic information about female workers as a population group and traditional female employment sectors. The patient issues incorporated three dimensions associated with the caring role: the type of work; expectations; and possible outcomes. The work tends to be dirty and emotional, with a professional subculture to allow the handling of other peoples' bodies. This subculture was linked to a 'coping' attitude where staff put the patients' needs and well-being before their own. The change in patient expectations (from being apologetic through to demanding their rights) is mirrored in a changing model of care from paternalism to partnership. A lack of ergonomic research was identified for female workers in the health care industry relating to both the type of work and gender issues.

Future directions in electronic image handling.

After a relatively slow start compared with the United States and Japan, several projects are now being established in Europe that are aimed at the development of prototype systems for medical image processing and management. Frequently, this includes aspects of multimedia communication, as well as legal, ethical, and economic issues. Consideration is also often given to systems security, reliability, and data protection. All these projects are based on the application of modern computer and communication technologies. The following interesting conclusions can be drawn from these preliminary activities: 1. PACS and IMAC systems should not be regarded as products or devices, but as a means to improve the infrastructure in a given medical care environment. Sometimes this activity is also referred to as knowledge business. Individual components of these systems, for example image acquisition devices, networks, storage facilities, and medical workstations, should be provided with standard interfaces allowing a modular build-up and an easy adaptation to the specific conditions of clinical departments. 2. Digital luminescence radiography will further establish itself as a method for image acquisition and increasingly will replace analog radiologic methods. Consequently, digital processing, archiving, and communication will be a necessity for optimal patient care. 3. New network technologies and magnetic-optical storage media offer the possibility of an improved cost-effectiveness for communication and storage. They should therefore be considered an important factor in future economic considerations regarding health care services. 4. Statistical estimation of atomic positions from exit wave reconstruction with a precision in the picometer range.

The local structure of Bi4W2/3Mn1/3O8Cl is determined using quantitative transmission electron microscopy. The electron exit wave, which is closely related to the projected crystal potential, is reconstructed and used as a starting point for statistical parameter estimation. This method allows us to refine all atomic positions on a local scale, including those of the light atoms, with a precision in the picometer range. Using this method one is no longer restricted to the information limit of the electron microscope. Our results are in good agreement with x-ray powder diffraction data demonstrating the reliability of the method. Moreover, it will be shown that local effects can be interpreted using this approach.

Early work with synchrotron radiation at stanford.

The use of synchrotron radiation in the soft and hard X-ray spectral region received major impetus with the start of parasitic operation of the Stanford Synchrotron Radiation Project (SSRP) in 1974. This was the first time that synchrotron radiation from a multi-GeV electron storage ring was made available in a user facility for studying the structure of matter. Here we review the early work at SSRP as well as the activities that preceded it, highlighting the scientific accomplishments (soft X-ray photoemission, EXAFS, protein crystallography), beamline instrumentation developments and source improvements. The early work using bending-magnet radiation led to the funding of several dedicated facilities in the US and elsewhere in the world - the so-called second-generation light sources. Early work with wiggler and undulator insertion devices led to funding of third-generation sources better optimized for insertion device sources, particularly undulators.

Activity-based cost management in health care--another fad?

By providing improved information for strategic planning purposes activity-based cost management (ABM) systems can help hospitals and other health care providers improve the quality and efficiency of the care they provide control costs and manage their resources better. The NHS is starting to evaluate the ABM approach. Describes a research project in one specialist hospital in Sheffield UK which found that conventional approaches to costing can be inaccurate by as much as 100 per cent; conventional approaches will consistently undercost complex, specialist, infrequent episodes of care and overcost straightforward frequent episodes; and the resulting information from ABM about costs and processes significantly improves the decision-making capabilities of managers particularly in relation to ¿what if¿ situations.

Assessment of erectogenic properties of apomorphine and yohimbine in man.

1. In a four period, double-blind, placebo-controlled study, apomorphine (0.009 mg kg-1 subcutaneous route) and yohimbine (0.30 mg kg-1 i.v. route) were administered to 10 male healthy volunteers. Penile circumference was monitored using plethysmography and subjective sexual arousal was self-assessed. Data were collected before, during and after a stimulation session during which 50 erotic slides were projected to the subjects. 2. Apomorphine induced an erection starting from the fourth minute post-injection, and potentiated the visually-induced response. Self assessment showed increased tumescence and rigidity without modifications of sexual arousal. 3. Yohimbine did not modify penile diameter without stimulation and did not affect the physiological response to erotic stimuli. 'Sexual excitement' was decreased by yohimbine during the post-stimulation phase. One subject experienced severe anxiety while he was infused with yohimbine. 4. These results are discussed with reference to animal data and to recent studies carried out in impotent patients.

Pattern recognition techniques in microarray data analysis: a survey.

Recent development of technologies (e.g., microarray technology) that are capable of producing massive amounts of genetic data has highlighted the need for new pattern recognition techniques that can mine and discover biologically meaningful knowledge in large data sets. Many researchers have begun an endeavor in this direction to devise such data-mining techniques. As such, there is a need for survey articles that periodically review and summarize the work that has been done in the area. This article presents one such survey. The first portion of the paper is meant to provide the basic biology (mostly for non-biologists) that is required in such a project. This part is only meant to be a starting point for those experts in the technical fields who wish to embark on this new area of bioinformatics. The second portion of the paper is a survey of various data-mining techniques that have been used in mining microarray data for biological knowledge and information (such as sequence information). This survey is not meant to be treated as complete in any form, since the area is currently one of the most active, and the body of research is very large. Furthermore, the applications of the techniques mentioned here are not meant to be taken as the most significant applications of the techniques, but simply as examples among many.

Cost benefit analysis of Haemophilus influenzae type b vaccination programme in Israel.

STUDY OBJECTIVE: The recent availability of Haemophilus influenzae type b (HIB) conjugate vaccines prompted an examination of the costs and benefits of four and three dose HIB prevention programmes targeting all newborns in Israel. MEASUREMENTS AND MAIN RESULTS: A four dose programme would reduce the number of childhood (aged 0-13) HIB cases from 184.2 to 31.3 per year, yielding a benefit ($1.03 million) to cost ($3.55 million) ratio of just 0.29/l for health services only, based on a vaccine price of $7.74 per dose. When benefits resulting from a reduction in mild handicaps and severe neurological sequelae are included, the benefit ($4.48 million) to cost ratio rises to 1.26/l and it reaches 1.45/l when the $0.66 million indirect benefits of reduced work absences and mortality are also included. Break even vaccine costs are $2.24 when health service benefits only are considered and $11.21 when all the benefits are included. CONCLUSION: In the absence of other projects with higher benefit to cost ratios, Israel should start to provide a nationwide HIB vaccination programme since the monetary benefits to society of such a programme will exceed the costs to society. A barrier to implementation may occur, however, because the costs of the programme exceed the benefits to the health services alone.

In the context of a German-wide project establishing oncologic palliative clinics, a discussion has been started dealing with the specific requirements of the health Community. As a result of this discussion, the question as to the possible differences in quality and quantity of stress has arisen. An empiric case study has been undertaken in a Special oncologic clinic with a palliative ward. The following phenomenon were investigated: stress level according to the Maslach Burnout Inventory twice within a period of two years, coping strategies, and the design of the work place. The differences in stress between the groups resulted in only specific differences, but a distinct drop in levels of stress was recognized in proportion to the level of professional competence. Relations between conditions in the work place, coping skills, and subjective perception of stress offer many opportunities for health promoting interventions in palliative wards, namely increasing Special competences of the nursing staff and improving work conditions.

Postnatal development of the retinal projection to the nucleus of the optic tract and accessory optic nuclei in the hooded rat.

Retinal projections to the nucleus of the optic tract (NOT) and accessory optic nuclei (AON) were studied in the postnatal hooded rat after monocular injection of cholera toxin B subunit (CTB) into the vitreous chamber of the eye. At all postnatal ages, retinal axons were labeled sensitively; they revealed dense projections to the contralateral, and sparse but distinct projections to the ipsilateral, NOT and AON. The CTB labeling enabled the first delineation of the complete morphology of developing retinal axons in the ipsilateral NOT and AON. From postnatal day (P) 1 to P3, axons with complex growth cones were seen, and unbranched collaterals with simple growth cones increased and extended gradually. At P6, complex growth cones disappeared while branched collaterals with simple growth cones as well as small-sized varicosities increased. By P12 (two days before eye-opening) the adult-like pattern of terminal arbors appeared. The branched collaterals with tiny, small-sized varicosities present probably represented developing synaptic boutons. At P16 (after eye opening), the pattern of terminal arbors was well developed, almost to the same extent as in the adult. By contrast, a broadly distributed, transient retinal projection around NOT and AON was gradually eliminated; it started to disappear during the first few postnatal days, and was fully retracted by the time of eye-opening time to a pattern normal for the adult.

The pattern of early neuronal differentiation in Drosophila melanogaster.

Based on the staining of Drosophila embryos with neuron-specific monoclonal antibodies we describe the differentiation of the earliest neurons in the central nervous system. The metameric array undergoes a number of changes during development that distinguish several morphological units: metameres, neuromeres and ganglia. Neural landmarks in the CNS and the periphery are identified on the bases of their developmental history, axonal projection and segment specificity. A time sequence of differentiation starting in the posterior gnathal and anterior thoracic anlage has been found.

Dental care and the health maintenance organization concept.

The principal dental diseases, caries and periodontal disease, affect almost the entire population and result in considerable pain and discomfort, with eventual tooth loss, if untreated. These disorders can either be prevented or their effects minimized through dental intervention so that intact functioning dentitions can be maintained. Despite the fact that enough practicing dentists probably exist in the United States to achieve these results, the majority of the population lose all of their natural teeth if they live out their normal life spans. The solo-practice fee-for-service system, even with third-party payment, may reduce the difficulties somewhat but cannot solve the problem. Prepaid dental group practice, either independently or as part of a general health care system, has the potential of virtually eliminating edentulism in populations for which it has responsibility. At present, it is possible to project costs and resource needs with sufficient accuracy to start viable entities. The "health maintenance organization" concept, therefore, applies to dental care.

Communication between the higher education and industry.

The paper presents the starting premise and the general frame of the Tempus Project CME-02555-96, entitled "Know How Transfer form University to Industry". It is intended to be a study for establishing a frame and methods for communication between academic, scientific and research bodies and industry. This project continues the efforts made in the RENEHEMA Tempus project aiming to support the improvement of communication between higher education and industry. Following the conclusions of the RENEHEMA conference the strategy adopted by the management board focused on the Internet technology and free market mechanisms to fulfill the project objectives.

Opportunities for improving quality in the health care industry.

This paper discusses the current state of health care quality and describes some of the factors that are hindering efforts to move toward evidence-based practice. The President's Advisory Commission on Consumer Protection and Quality spent one year looking at the state of health care quality and developed a set of recommendations to tackle the serious quality problem in the health care industry. The Institute of Medicine has started an initiative, Quality of Health Care in America Project, which is addressing many of the quality of care areas identified by the President's Advisory Commission on Consumer Protection and Quality. This paper discusses the work of these two groups and concludes with key issues in the advancement of quality health care.

Nutrition and cardiovascular disease in Finland since the early 1970s: a success story.

This paper describes the Finnish experience on the population strategy to prevent cardiovascular disease with special reference to diet. Systematic work to lower the risk factors of cardiovascular disease started in the early 1970s with the North Karelia project and rapidly expanded to cover the whole country. The main aims were to change the type of fats used, to lower sodium intake and to increase vegetable and fruit consumption. Concurrently, a comprehensive monitoring system was developed including regular population surveys every five years. During the period 1972-1997 major changes took place in the diet as well as in blood pressure and serum cholesterol levels. At the same time, cardiovascular mortality decreased dramatically. The Finnish experience shows that dietary changes are possible but require a persistent and comprehensive intervention.

Security development of a pocket-sized teleradiology consultation system.

Telemedicine widens the access to sensitive data and therefore data security development is an essential part of telemedicine projects. Publicly available security evaluation methods provide a good starting point for security work. We built a mobile teleradiology consultation system that is integrated into hospital information system and systematically evaluated possible security threats that may endanger it. Based on the analysis, a security model was designed and implemented. We conclude that systematic approach to data security development facilitates the process but does not give all answers to the practical questions.

Quality of life after multiple trauma--summary and recommendations of the consensus conference.

In October 1999, an international and interdisciplinary consensus conference was held about the assessment and application of quality of life (QoL) measures after multiple trauma. Four working groups represented the following patients: children with traumatic brain injury (TBI); adults with TBI, adults with multiple injuries (but without TBI), and adults with spinal cord injury. According to predefined questions, the groups tried to identify the relevant problems of the patients, at different time points after the traumatic event. A review of the existing instruments for quality of life assessment and the evidence of their application in trauma patients in the scientific literature was performed by each group. Based on the results of these literature reviews it was concluded that there are not enough data to establish "evidence-based" guidelines for QoL assessments in these patients. Nevertheless, the groups comprised of experts clinicians and methodologists, agreed on the Glasgow Outcome Scale and the SF-36 as generic tools for QoL assessment across all trauma patients. It was further recommended to use these generic tools in combination with condition-specific instruments to better reflect the specific problems of the patients. Finally, the whole group suggested that it was not appropriate to view this conference as a "final report" about QoL assessment in trauma patients, but rather it should be seen as a starting point for increased efforts to initiate clinical research projects using QoL as an outcome, to develop better instruments, and to include QoL assessments into daily routines.

[Proposal for first level nursing degree].

A project for the organisation of the new nursing degree program starting in Italy in 2001/02 is presented (national regulation published 5 June 2001). It is the fruit of the work of a group of nursing professors of the University of Insubria (Varese, Italy). After the explanation of the criteria which have led to the educative objectives construction, the choice of the scientific disciplines necessary to reach them has been made. The same educative objective have guided the amount of working hours assigned to each discipline and formative activity (according to the European credit transfer system). The characteristic elements of the project are: the presence of an introductory module among the so called "professionalising formative activities"; the centrality of these professionalising formative activities; the constant reference to a conceptual nursing model; the decision to keep the integrated courses modality beginning from the experience gained since 1992; the reduction in number of the integrated courses and therefore of the exams.
