[Analysis of volatile oil in pericarpium Zanthoxyli].

Essential oils were extracted from the pericarp of Zanthoxylum bungeanum and Z. schinifolium. The chemical constituents were analysed by means of GC-MS and 33 compounds were identified. Their contents in the essential oil were also determined.

[Chronic obstructive bronchitis. Effect of Gelomyrtol forte in a placebo-controlled double-blind study].

In a double-blind study involving two groups of ten patients with chronic obstructive bronchitis, the expectorant Gelomyrtol forte was tested against placebo for its effectiveness and tolerance. The parameters: amount of sputum, Rt and IGV, together with all the usual clinical laboratory parameters, were determined. In addition, the color of the sputum was always noted. On the basis of patient scores, daily entries on the ability to expectorate, attacks of coughing, general coughing, and shortness of breath were made by the patients for the duration of the 14-day treatment period. On conclusion of the study, the patients were asked to assess the effectiveness of the supplementary medication, and the care-providing physician also assessed effectiveness. All score parameters related to coughing improved, in some cases appreciably, relative to the placebo group. The findings in term of sputum volume and color, were also distinctly better in the Gelomyrtol forte group. Correspondingly, both patients and physicians assessed the effectiveness of Gelomyrtol forte to be distinctly better than that of the placebo. Although the groups are relatively too small and heterogeneous, to establish statistically significant differences, the results do strongly suggest a favorable and major effect in patients with relevant cough symptoms. Both subjective and objective tolerance was excellent.

[Treatment of pulp diseases in milk teeth with chlorocamphomenthol].

The results of treatment with chlorcamphomenthol are assessed clinically in cases of grade II pulpopathy in milk molars of children aged from 3 to 9 years. Out of 64 teeth examined after 1 to 4 years in 60 cases (93.7%) the clinical condition of the tooth was satisfactory, and in 4 cases the tooth was lost. A high percent of good results encourages to the use of chlorcamphomenthol in endodontic treatment of milk teeth.

[Determination of eucalyptole in eucalyptus oil by gas chromatography].

This paper reports the determination of eucalyptole in eucalyptus oil by gas chromatography. The results were similar to the specifications in the Chinese Pharmacopoeia. The method is simple, rapid, accurate and sensitive and requires only small amount of samples. The coefficient of variation is less than or equal to 0.52%.

Menthofuran-dependent and independent aspects of pulegone hepatotoxicity: roles of glutathione.

Pulegone, a monoterpene that protects source plants against predators, is a hepatotoxic constituent of the folklore abortifacient pennyroyal oil. In the rat, pulegone extensively depleted glutathione measured in both liver tissue and plasma, and its toxicity was markedly enhanced in animals treated with buthionine sulfoximine. The glutathione-depleting effect of pulegone was compromised following inhibition of cytochrome P-450 by piperonyl butoxide. In addition, we found no evidence for conjugation of glutathione to unchanged pulegone in vitro. Administration of menthofuran, a known oxidative and hepatotoxic metabolite of pulegone, only marginally affected glutathione levels in plasma and liver, and toxicity was not augmented by buthionine sulfoximine. These results provide indirect evidence for cytochrome P-450-catalyzed bioactivation of pulegone via at least two independent pathways: 1) the formation and subsequent activation of menthofuran from pulegone; and 2) the formation of reactive intermediate(s) from pulegone, but not menthofuran, which can be detoxified through a mechanism requiring reduced glutathione.

Efficient and effective root canal retreatment without chloroform.

A root canal retreatment technique is described which eliminates the need for chloroform as a gutta percha solvent. This technique utilizes the gutta percha softening actions of both a self-heating instrument and the heat-potentiated solvent action of eucalyptol. In addition, sonics/ultrasonics and/or Gates-Glidden drills are suggested as useful adjuncts to canal debridement during re-preparation of the canal system. Given the controversy regarding the toxicity of chloroform, both for dental patients and dental personnel, practitioners may wish to consider using this retreatment technique to eliminate chloroform from endodontic retreatment procedures, while still achieving effective, efficient root canal retreatment.

[Studies on the essential oils of flos Magnoliae].

Essential oils were extracted from nine species of Flos Magnoliae. Their contents were determined. Analysis of chemical constituents in these oils was performed by means of GC-MS. 69 compounds were identified. The percentage of these components in oils were determined by GC.

[Therapy of chronic apical periodontitis in traumatically injuring front teeth with ongoing root growth].

Because of the patient's age and the width of the apical foramen, conventional or surgical root canal fillings are frequently not indicated for the treatment of chronic apical periodontitis in teeth whose roots have not yet fully grown. Therefore resorbable paste fillings are applied either to induce deposition of apical hard substance or to bridge the time until root canal filling is possible. In a comparative clinical and radiological study in 171 young patients, Walkhoff's iodoform paste and calcium hydroxide were examined with regard to their inductive effect on the formation of an apical hard substance barrier.

The metabolism of the abortifacient terpene, (R)-(+)-pulegone, to a proximate toxin, menthofuran.

(R)-(+)-Pulegone, the major monoterpene component of the abortifacient mint oil, pennyroyal oil, is metabolized by hepatic microsomal monooxygenases of the mouse to a hepatotoxin. The formation of a toxic metabolite is apparently mediated by cytochromes P-450 of the phenobarbital class inasmuch as phenobarbital pretreatment of mice increases, whereas beta-naphthoflavone pretreatment decreases, the extent of hepatic necrosis caused by pulegone. Furthermore, two inhibitors of cytochromes P-450, cobaltous chloride and piperonyl butoxide, block toxicity. An analog of (R)-(+)-pulegone that was labeled with deuterium in the allylic methyl groups was found to be significantly less hepatotoxic than the parent compound. The results indicate that oxidation of an allylic methyl group is required for generation of a hepatotoxic metabolite. Menthofuran was identified as a proximate toxic metabolite of (R)-(+)-pulegone, and investigations with (R)-(+)-pulegone-d6 and 18O2 strongly indicate that menthofuran is formed by a sequence of reactions that involve: 1) oxidation of an allylic methyl group, 2) intramolecular cyclization to form a hemiketal, and 3) dehydration to form the furan.

The effects of analgesics on pain-related somatosensory evoked potentials.

Human cerebral evoked potentials can be employed to test the effectiveness of analgesics. In the present study, pain-related cortical responses were elicited by chemical stimulation of the nasal mucosa in combination with electrical stimulation of dental pulp. In alternating sequence, 16 series of 6 chemical and 6 electrical stimuli each were presented. Interstimulus and interseries intervals were 2 s and 50-60 s, respectively. Stimulus intensities were 54% carbon dioxide (v/v) and 4 dB over the electrical current threshold of 21-45 microA for noxious chemical and electrical stimulation, respectively. Four volunteers participated in the study. The EEG was recorded at 8 sites of the international 10-20 system. After each series of stimuli, the test subjects, using visual analogue scales, rated pain intensity relative to a single standard stimulus they had received initially. After running through the procedure once, without their noticing it, the subjects intravenously received 5 ml (2.5 g) metamizol or 5 ml 0.9% NaCl, which were slowly (over a period of 12 min) added to an infusion. The entire stimulation procedure was then repeated 10 min later. Whereas the evoked potentials clearly exhibited habituation effects due to stimulus repetition within a series, the latter were absent in the subjects' pain ratings. Despite the comparatively high i.v. dose of 2.5 g metamizol, pain ratings markedly different from those given under placebo were not observed. In contrast, potential amplitudes after metamizol administration exhibited a clear tendency towards smaller amplitudes in both types of stimulation. In addition, metamizol effects on pain rating could be demonstrated successfully by applying continual chemical stimulation.
