Malignant Leydig cell tumor of the testis associated with Klinefelter's syndrome.

We reported the case of a 35-year-old man with Klinefelter's syndrome and a malignant Leydig cell tumor of the testis. Bilateral gynecomastia and right testicular enlargement led the patient to seek medical assistance. Despite initial orchidectomy two years later the patient developed lung and iliac lymph node metastases. The tumor appeared to be refractory to chemotherapy and to hormonal treatments including op'DDD. Finally, the patient died within 20 months of developing metastases. Leydig cell tumor is an exceedingly rare tumor, especially when associated with Klinefelter's syndrome. This association as well as presentation, pathologic features, hormonal abnormalities, clinical course and response to therapy of malignant Leydig cell tumors are discussed.

Malformations of angiogenesis in the low differentiated human carcinomas. immunohistochemical study.

Our previous observations showed that the perivascular mesenchyma of the thin-walled vessels (capillaries) in cancers may be the source of organ-specific stem cells. We suggested that the cells forming vascular channels in altered stroma participate in the tumor development. This study was designed to examine the distribution of the vessels and their appearance in the breast, lung and colon cancers. Using immunohistochemical methods, we have shown that in the low differentiated tumors both CD31 and factor VIII antigens may be expressed in capillaries chiefly on the periphery of neoplastic foci. Many of these vessels were discontinuous, with interruptions or unformed tubules. Sporadically, CD31 protein and factor VIII antigens were not expressed in capillaries inside the very low differentiated cancer cases. It is difficult to assess by immunohistochemical means whether the vascular malformations are the primary or secondary phenomena in the malignancy and why these abnormalities were especially visible in some low differentiated cancers.

[The expression of the epidermal growth factor-receptor mRNA in laryngeal squamous cell carcinoma and its correlation with clinical pathological characteristics].

OBJECTIVE: To study the expression of the epidermal growth factor-receptor(EGF-R) mRNA in laryngeal squamous cell carcinoma and its correlation with clinical characteristic and difference of tissues. METHOD: All 20 samples of laryngeal squamous cell carcinoma were determined by dot blot. RESULT: The EGF-R mRNA expression was evaluated in 20 laryngeal squamous cell carcinomas and 15(75%) were finded with EGF-R mRNA expression. When the mRNA of the laryngeal squmous cell carcinoma was decreased, the expression of the EGF-R mRNA was decreased. The EGF-R mRNA expression did not correlated with age or tumor localization (P > 0.05) but correlated with differentiated tumors (P < 0.05) and the stages of TNM according to classification (P < 0.05). Four patients with recuence cancers and six patients with the metastatic cervical-lymph-node involvement had an enhanced expression of EGF-R mRNA. CONCLUSION: The uncontrolled growth and proliferation of the laryngeal squmous cell carcinomas may be related to the abnormal EGF-R mRNA expression. The enhanced expression of EGF-R mRNA was correlated with the differentiation and stages of laryngeal squmous cell carcinomas.

Immunological and chromosomal studies in a patient with Sézary syndrome.

A patient with the small-cell variant of Sézary syndrome was studied before and during treatment with low-dose chlorambucil. He had depressed responses to phytohemagglutinin (PHA) and concanavalin A that returned to normal with clinical improvement. Spontaneously dividing cells were found in the peripheral blood by culturing lymphocytes without PHA for 24 hours. Seventy-two-hour PHA-stimulated lymphocyte cultures revealed predominantly hypodiploid chromosome numbers. Both abnormalities improved during treatment. Lymphocytes that were studied for sister chromatid exchanges had an increased number of exchanges per metaphase. Immunofluorescence studies revealed immunoglobulins and C at the dermoepidermal junction and in the dermal vessel walls. Our findings demonstrate that the depression of mitogen responsiveness that is sometimes seen in Sézary syndrome can be favorably affected by chemotherapy. Likewise, some chromosomal abnormalities may regress during successful therapy. These findings may provide a way of measuring the response to therapy.

[Diagnostic use of neutrophil segments negative for peroxidase, chloroacetate esterase and sudanophilia in peripheral blood in leukemia].

The paper contains the results of three cytochemical reactions used for detection of myeloid differentiation (peroxidase, sudanophilia, chloroacetate esterase) in neutrophil segments of the peripheral blood stream in 107 patients with acute myeloid or lymphatic leukaemia. Enzymatically deficient segments were detected in 23 (34.8%) patients with acute myeloid leukaemia. They were not found in any patients lymphatic leukaemia not in healthy subjects. The concurrent deficit of all three reactions was found in 69% of the cases with defective neutrophil segments. However, also isolated affection of any of these reactions was found. In acute leukaemias, not differentiated from the cytochemical and immunophenotypical aspect, we may consider simple evidence of the presence of the mentioned abnormal neutrophil segments in peripheral blood as a highly probable sign of myeloid differentiation of acute leukaemia.

Fine needle aspiration cytopathology of retroperitoneal lymph nodes in the evaluation of metastatic disease.

Fine needle aspiration biopsy of retroperitoneal lymph nodes was performed in 47 patients with abnormal lymphograms during the course of evaluation for metastatic disease. The biopsies were positive for malignant tumor in 11 patients. Utilizing established cytopathologic criteria a definitive diagnosis could be established. Fine needle aspiration of abnormal lymph nodes detected by lymphography is an easily performed, accurate diagnostic technique.

[Usefulness of various echographic examinations in the study of extrapulmonary lesions of sarcoidosis].

Echocardiographic study performed by Ohmori and others revealed a statistically significant difference between sarcoidosis patients with and without left ventricular perfusion defects in regards to LVDd, LVDs, LVEF, %FS, WT and EPSS. Echocardiography is a useful noninvasive tool in addition to Thallium-201 myocardial scintigraphy, in detection of myocardial involvement of sarcoidosis. Marked swelling of abdominal lymph node and splenomegaly was found by abdominal echography, in the sarcoid patients at onset as well as in the chronic stage. Both abdominal echography and CT revealed a hepatic sarcoid mass lesion in patients with and without hepatic dysfunction. Improvement of these abdominal echographic abnormalities correlated well with clinical improvement of sarcoid lesion. In addition, muscular echographic abnormalities were found in muscular sarcoid lesion. It can be said that echography is useful in the detection and follow up of extrapulmonary lesions of sarcoidosis.

[A rare case of intrapulmonary sequestration in left upper lung field diagnosed before surgery].

This case is a 30-year-old male who was indicated abnormal shadow in the left upper lung field by chest X-ray film. The aortography was carried out because pulmonary sequestration was suspected by chest CT-scan etc. From the result, rare pulmonary sequestration in the left upper lung field was diagnosed because patterns with inflow of an abnormal artery from descending aorta into the left upper lung field and perfusion of it into the upper pulmonary vein were observed. The wedge resection was performed. The specimen was slightly hard on the whole, and many cartilages or multiple nodules were observed on the cut surface. Histopathologically, normal pulmonary structure was disappeared, and hyperplasia of the lymphatic nodule was remarkably observed in the interstitium, and cystic wall was covered by ciliated cylindrical epithelium.

CT of the pulmonary hilum: evaluation with thin-section ultrafast CT.

The recent progress of CT technology, such as spiral and ultrafast CT, has made it possible to obtain contiguous thin-section CT images of the pulmonary hilum during rapid injection of contrast material. These CT images consistently provide clear images of the pulmonary hilum, including normal peribronchovascular interstitium, bronchi, and pulmonary vessels, enabling a more precise and detailed evaluation of various pathological conditions than CT previously allowed. This article illustrates high-quality CT images in normal and abnormal conditions occurring in the pulmonary hilum and discusses the present CT diagnosis of various pulmonary hilar diseases.

Significance of mouse red cell rosette-forming lymphocytes in chronic lymphocytic leukaemia.

Increased mouse red cell (M) rosetting lymphocytes were demonstrated in the peripheral blood of patients with chronic lymphatic leukaemia. The range was wide, and patients showed considerable variation not only in the number of M cells but also in T and B rosetting lymphocytes. Treatment reduced M rosette lymphocytes proportionately as the total white count fell, and differential removal occurred only when the patients became leucopaenic. If we assume the M rosetting cells are the abnormal 'leukaemic' cells, treatment does not preferentially remove these. The M rosetting capacity appeared to be related to the presence of an immunoglobulin factor previously demonstrated on the cells and in the serum of patients with CLL which enhances in vitro viability of the leukaemic cells.

The sonographic appearance and significance of cervical metastatic nodes following radiotherapy for nasopharyngaeal carcinoma.

Serial ultrasound (US) was performed on 36 patients with palpable malignant nodes in the neck who had radiotherapy for nasopharyngeal carcinoma (NPC). All patients had a preradiotherapy baseline US examination, and follow-up studies at least 8 weeks after radiotherapy (RT). After RT there was diminution in size and disappearance of some of the nodes. However, in the residual nodes, US criteria for malignant nodes were still present in many of the post RT nodes. i.e. 140/143 (97.9%) residual nodes had an absent hilus and all the nodes were poorly reflective, 122 (85.3%) nodes still had sharp borders, 53 (37.1%) had a short to long axis (S/L) ratio greater than 0.5 and 11 (7.7%) nodes were greater than 8 mm in size. These parameters therefore are not of value in determining the persistence or recurrence of disease in a previously abnormal node. Appreciation of this fact should prevent unnecessary fine needle aspiration of these nodes up to 3 months following radiotherapy.

Klinefelter's syndrome and extragenital seminoma.

An extragenital metastatic seminoma in a patient with Klinefelter's syndrome is described. The increased incidence of many types of malignancies in patients with this chromosomal abnormality and the increased susceptibility of XXY cells to transformation by oncogenic SV-40 virus are discussed. A relationship is suggested circumstantially by these observations.

Rheumatoid arthritis: clinical features and pathogenetic mechanisms.

Rheumatoid arthritis is a disorder characterized by chronic inflammation affecting predominantly articular tissues, leading in some instances to disruption of the normal structure and function of the joint. In some patients extra-articular manifestations are also present. In the joints, the pathologic lesion consists of synovial cell proliferation with infiltration by inflammatory cells. The complex interaction among these cells is likely responsible for the connective tissue abnormalities that characterize the rheumatoid lesion. The factors responsible for inducing the inflammatory process are unknown.

Occult supraclavicular lymph node metastasis identified by FDG-PET in patients with carcinoma of the uterine cervix.

PURPOSE: The objective was to evaluate the frequency and prognostic significance of occult supraclavicular lymph node metastases identified by 2-[(18)F]fluoro-2-deoxy-D-glucose-positron emission tomography (FDG-PET) in patients with cervical carcinoma. PATIENTS AND METHODS: Between March 1998 and January 2002, 186 patients with a new diagnosis of cervical cancer underwent whole-body FDG-PET before therapy. Fourteen patients had abnormal FDG uptake in left supraclavicular lymph nodes without palpable disease. All 14 patients underwent sonographically guided fine-needle aspiration of the left supraclavicular lymph nodes. One patient refused therapy, 6 were treated with palliative intent, and 7 received definitive irradiation and concurrent chemotherapy. Survival was calculated by the Kaplan-Meier method. RESULTS: The overall frequency of FDG-positive left supraclavicular lymph nodes was 8% (14/186). Metastasis was pathologically confirmed in all 14 patients. Therefore, the positive predictive value of abnormal FDG uptake in left supraclavicular lymph nodes was 100%. Nineteen percent of all patients (35/186) had abnormal FDG uptake in para-aortic lymph nodes. The frequency of positive FDG uptake in the left supraclavicular lymph nodes was 40% (14/35) in those with para-aortic lymph node uptake and 15% in those with stage IIIb disease. The median overall survival was 7.5 months. At last follow-up, 11 patients were dead and 3 were alive with disease. All patients developed metastatic disease, most commonly to bone and lung. CONCLUSION: The positive predictive value of abnormal FDG uptake in left supraclavicular lymph nodes was 100%. Prognosis for these patients was dismal despite aggressive therapy.

Cytogenetic analysis of invasive breast cancer: a study of 27 Asian patients.

Tumor cytogenetic analysis from 27 patients with breast cancer diagnosed at the Singapore General Hospital revealed complex karyotypic aberrations in 12 cases. The study group comprised 25 women and 2 men, ranging in age from 33 to 78 years (median 52 years). Ethnic distribution consisted of 22 Chinese, 3 Malaysian, and 2 Indian patients. Pathologic assessment disclosed 24 invasive ductal, 2 invasive mucinous, and 1 mixed invasive mucinous and ductal carcinomas. Histologic grading showed 3 grade 1, 10 grade 2, and 12 grade 3 tumors; 2 cancers were not graded, because they had been subjected to prior chemotherapy. Tumor sizes ranged from 1.5 to 10 cm (median 3 cm). Eleven cases were axillary node negative, whereas the remaining 16 node-positive cancers affected as many as 3 nodes in 8 cases and 4 or more nodes in another 8. Twenty cases demonstrated estrogen-receptor positivity, and 8 cases progesterone-receptor positivity. The spectrum of cytogenetic abnormalities involved chromosomes 1, 3, 6, 7, 8, 11, 16, and 17 and ranged from gains and deletions of both long and short arms, trisomy, monosomy, and other rearrangements. There was a trend toward the presence of karyotypic abnormalities in tumors of higher grade.

Thymic reticulum of autoimmune mice. I. Ultrastructural studies of the diabetic (db/db) mouse thymus.

The thymus of the db/db mouse, an autoimmune strain with insulin-dependent diabetes mellitus and manifesting hyperglycemia, polyuria, glycosuria and obesity, was observed by electron microscopy. Its comparison with normal thymuses and thymuses from non-diabetic obese mice has revealed two major differences: first, the presence of crystal-like structures in some of the numerous clear vacuoles in cells, and second, a modification of the cells forming the border of cystic cavities. These cells contain two types of granules which are quite plentiful. One variety is clear and big, and a second type is small and dense, with an aspect similar to that of secretion granules. It is hypothesized that the crystalloid formations found in epithelial cells, and the presence of small, dense granulations in cells bordering the cystic cavities, are a consequence of the abnormal storage of the thymic hormone, thymulin, which results from a secretory function defect. These observations suggest a possible role for the thymic reticulum in thymic function deficiencies.

[Unexpected findings during abdominal sonography. Their incidence and clinical significance].

19% of 1490 abdominal sonograms from 1192 patients led to findings which were not related to the original purpose of the sonogram or to the actual disease, and of which neither the treating physician nor the patient were aware. The most frequent findings were those in liver, kidneys and gallbladder. The majority of ultrasonographically detected lesions were benign; tumour diseases were suspected only in 26 cases. The incidence of such findings increased with the patients' age. The positive, predictive value of the findings was around 90%. In 20% of the patients with unexpected findings, further diagnostic measures were taken; therapeutic consequences were recorded in only twelve cases. Further measures were recommended in 20% of the patients, a majority of which had pancreatic and vascular abnormalities. Because such findings may considerably facilitate the diagnosis of subsequent complaints, all accessible organs should be examined during abdominal ultrasonography.
