[Fibrosing mesenteritis--a surgical problem].

The authors describe three cases of rare fibrotizing mesenteritis in adult patients. In two the aetiological agent was not detected, in the third one it was obviously talcum powder. The disease was manifested as acute abdomen or was detected accidentally. Compression of the visceral vessels caused the development of ischaemic foci with intestinal fistulae. Surgical treatment failed.

[Fibrosing reaction of nutmeg liver].

Hypoxia and intrahepatic hemodynamic abnormalities in circulatory failure are important pathogenetic factors in the development of cardiogenic fibrosis. Glycosamine glycans and H-acetylhexosaminidase serve as markers for basic substance turnover in the connective tissue and may be used in clinical practice and as screening tests to detect liver cirrhosis. Haptoglobin and ceruloplasmin act as protectors to stabilise the fibrogenesis of nutmeg liver and exert an immunomodulating action. Glutamate dehydrogenase levels may be employed as an early diagnostic criterion for assessing hepatocytic dysfunction caused by hypoxia in patients with heart failure.

[Fibromyalgia syndrome (fibrositis): as frequent as it is unknown].

Fibromyalgia syndrome is characterized by generalized musculoskeletal pain and abnormal sensitivity to palpation of characteristic myofascial areas. We evaluated its prevalence in the 673 patients seen as first consultations in a hospital rheumatological outpatient clinic during one year, and we made an automatized review of the Spanish literature about this condition in the last 17 years. We found 70 cases (10.4%) of fibromyalgia syndrome, none of which had been referred to the hospital with this diagnosis. We report their clinical features. In our bibliographical search we found only one article describing this condition. We conclude that it is a common clinical syndrome, unknown in nonspecialized milieus. Generally, the patients are inadequately diagnosed and treated, and/or referred to hospital.

[Is fibrositis an immuno-rheumatologic disease?].

An attempt was made to test the hypothesis that immunological dysfunction occurs in primary polyenthesopathy (PP) by studying skin immunofluorescence, capillary microscopy, photoplethysmography and the lymphocyte populations in PP patients defined according to the usual criteria of Yunus. Skin immunofluorescence in 15 PP patients (14 women, 1 man, average age 50.9 years) failed to reveal any deposit of IgG, A, M, Clq, or C3c either at the dermoepidermal junction or in the vessels. 26 patients (24 women, 2 men, average age 50.5 years) were studied using capillary microscopy and 12 using digital photoplethysmography. The microvascularization abnormalities observed were mild and non specific. A study of the lymphocyte populations using Coulter flux cytometry in 35 PP patients (32 women, 3 men, average age 46 years) did not show any modification in the number of CD4 and CD8 lymphocytes by comparison with the controls. These results do not agree with the data given in the literature and this is probably due to the heterogeneity of the patients studied, with some studies showing a low concentration of antinuclear factors and an abnormal frequency of dry symptoms which, in the opinion of the authors, is considered as an exclusion criterion in the diagnosis of PP. In the author's view, polyenthesopathy is not an immune disease.

[Reaction of the fibrose eyeball covering upon the suture material synthetic and absorbable "Dexon". (Experimental study) (author's transl)].

The present paper deals with the results of 20 eyes of experimentally operated laboratory animals, whose perforating wounds (of cornea and those of sclerocorneal region) have been suturated with a new type of absorbable synthetic suture material "Dexon" thickness being 6-0. The synthetic absorbable suture material "Dexon" is made of polyglycol acid, and it has been introduced to market by the firm Davis and Geck (U.S.A.). The inflammatory tissue reaction to the presence of the suture material "Dexon" is prominent and can by compared to the reaction to chromic resorbable suture materials (catgut or collagen). The suture material "Dexon" start to resorb in the course of the 2nd-3rd weeks after operation. The suture material "Dexon" can be used at an advantage in the operation where it is possible to cover the knots by a conjuctival lobe (in sclerocorneal region or in strabismus surgery) in this way the tendency to overgrowing of epithelial cells along the suture channels can be prevented. The "Dexon" material is sufficiently flexible and firm and has no antigenic properties, therefore it appears very prospective for ophthalmosurgery.

[Increased oxidative metabolism of alveolar macrophages in idiopathic fibrosing alveolitis].

The results of the present investigation show that, in IPF, alveolar macrophages are preactivated, and are capable, both spontaneously and also after cellular stimulation with a variety of stimulants, of producing appreciably more cytotoxic oxygen metabolites as compared with alveolar macrophages from healthy control subjects. A striking finding is the fact that the production of oxidants following stimulation with the complement factors C3a-C5a and aggregated IgG is appreciably lower than following stimulation with PMA or zymosan. This might be evidence in support of the theory that the alveolar macrophages in IPF are activated by immune complexes.

Modulation of pain in fibromyalgia (fibrositis syndrome): cerebrospinal fluid (CSF) investigation of pain related neuropeptides with special reference to calcitonin gene related peptide (CGRP).

The cerebrospinal fluid (CSF) levels of calcitonin gene related peptide (CGRP) were 0.94 +/- 0.06 fmol/ml (mean +/- SEM), of substance P, 35.1 +/- 3.2 fmol/ml and of substance P (1-7), 10.8 +/- 1.2 fmol/ml, as measured by radioimmunoassay in 26 female patients with fibromyalgia. No correlation was found between the levels of CGRP and the substance P and substance P (1-7) levels (r = 0.316, p = 0.14). Our results show that the anatomical coexistence of pain related neuropeptides in neurons is not necessarily reflected by the levels of these peptides measured in the CSF. The presence of CGRP in the CSF could be important since it can enhance the nociceptive activity of tachykinins. This may be of importance in the pathogenesis of pain in fibromyalgia.

[Fibrosing lung process caused by TEL mineral wool].

The case of a 51-year-old man with pneumoconiosis caused by fibrous glass dust is reported. The diagnosis is confirmed by histological examination and by x-ray micro analysis using a scanning electron microscope. Pneumoconiosis following fibrous glass exposure should be taken into consideration in differential diagnosis of interstitial lung diseases.
