Pseudoaneurysm following femoral fracture.

If the physician is aware of this diagnosis and maintains an appropriate level of suspicion and low threshold to commence duplex evaluation. The potential morbidity of a fracture-induced traumatic pseudoaneurysm can be minimized.

[Hip protectors: what is their role in the prevention of femoral fractures?].

In elderly persons, hip fracture is frequent and associated with serious physical, functional and psychological consequences. Hip protectors seem promising for preventing hip fractures, especially in frail, institutionalized elderly persons. Despite methodological limitations, five randomized trials have reported positive results, with up to 85% risk fracture reduction in some trial. Interestingly, although 13 hip fractures occurred in subjects assigned to wear hip protector, only one occurred while the protector was actually worn. Adherence with wearing hip protectors remains a problem. Several ongoing trials will further enhance our knowledge on hip protectors effectiveness and cost-effectiveness to better define their role in hip fracture prevention.

[The treatment of proximal femoral fractures using the interlocking nail].

The good result hoped for with interlocking nailing of the proximal part of the femoral shaft is only possible when precise diagnosis is of the fracture form is made with reference to informative X-rays and a meticulous operative technique is observed. Therefore, positioning of the patient on an extension table, the repositioning of the bone, the choice of the insertion point for the nail, and careful drilling are of the utmost important. All follow-up examinations conducted in 42 patients, we found good or very good results in 32 cases, satisfactory results in 7, and poor results in 3.

The effect of age on gene expression in adult and juvenile rats following femoral fracture.

OBJECTIVE: To compare mRNA gene expression during fracture healing in young and adult rats. DESIGN: Gene expression was measured at zero, 1, 2, 4, 6, 8 and 10 weeks after fracture (6 rats/age/time point) in rats at 6 and 26 weeks of age at surgery. SETTING: AAALAC-accredited vivarium of an independent academic medical center. ANIMALS: Female Sprague-Dawley rats at 6 and 26 weeks of age. INTERVENTION: An intramedullary rod was placed retrograde in the left femur, and a simple transverse closed middiaphyseal fracture was induced. MAIN OUTCOME MEASUREMENTS: mRNA gene expression was measured for 34 genes for extracellular matrix, osteoblasts, bone morphogenic protein, inflammation, cytokine, and receptor genes. RESULTS: The young rats reached radiographic union by 4 weeks after fracture, whereas the adult rats took 8 to 10 weeks to unite. All genes studied increased in mRNA expression with a peak at 1 to 2 weeks after fracture. All genes in the young rats then subsided to baseline by 4 weeks after fracture. However, during the longer period needed for radiographic union in the adult rats, only genes related to bone matrix, osteoblastic markers, angiogenesis, and the fibroblast growth factors remained significantly up-regulated at 4 and 6 weeks after fracture. Genes related to cartilage, Indian hedgehog, the bone morphogenetic proteins, and transforming growth factor-beta came to undetectable baseline values in the adult rats prior to radiographic union. CONCLUSIONS: Most stimulators of bone healing are not expressed during the later stages of fracture repair in adult rats. Other genes must control bone growth to bridge the fracture gap.

Severe tetanus complicating an open femoral fracture successfully managed using intrathecal human tetanus immunoglobulin and external fixation.

We report here a case of severe tetanus, complicating a compound fracture of the femur, successfully managed without intensive care facilities, using intravenous sedation and intrathecal Human Tetanus Immunoglobulin (HTI). Substitution of conventional traction (Transtibial Steinman Pin and Thomas Splint) for a Day Frame External Fixation Device (EFD) provided excellent fixation and stability of the fracture in spite of severe tetanic spasms.

Callus formation and the rate of healing of femoral fractures in patients with head injuries.

Callus formation and the rate of union of nailed fractures of the femur has been determined in 22 patients with associated head injuries and compared with that in a group of patients with similarly treated fractures but without head injuries. The comparison confirms the widely held view that, in patients with head injuries, fractures frequently heal with excessive callus and at a faster rate than normal.

[Osteosynthesis of unstable pertrochanteric femoral fractures with the compression hip screw].

Injuries of unstable pertrochanteric fractures of the upper femur represent a contingent with high multi-morbidity at highest average age. The only successful procedures within biologically set boundaries, would be those of an early and weight-bearing resistive osteo-synthesis. Starting in 1979 till the middle of 1982, we treated 70 unstable pertrochanteric fractures out of 350 proximal femur fractures, with this compression hip screw. Because of its underlying bio-mechanical principle, its advantageous operative handling, as well as high durability of the material used, this version of an osteosynthesis showed few complications. We found neither head penetration and pseudoarthrosis, nor did we find any implant bending and breaking so far. Although averaging almost 80 years, 63% of our patients could be released after hospitalization for an average of 39 days, leaving at full mobility. The set goal of an early weight-bearing with elderly patients was fully achieved by means of this version of osteo-synthesis, since dynamic compression of even comminuted fracture zones permitted a statically sound and unhindered possibility at weight-bearing.
