Intranasal administration of alpha-human natriuretic peptide produces a prolonged diuresis in healthy man.

The effects of an intranasal administration of synthetic alpha-human natriuretic polypeptide (alpha-hANP) were studied in 8 healthy male volunteers. They were given an intranasal administration of alpha-hANP at doses of 0.56-0.62 microgram/kg twice in the same day, the first and second administrations being separated by 180 min. Urine volume and urinary sodium excretion increased 2 to 3-fold 60 min after each administration. There were no significant changes in blood pressure, heart rate, glomerular filtration rate, plasma renin activity and plasma concentration of aldosterone, cortisol and catecholamines. The intranasal administration of alpha-hANP was well tolerated by the volunteers, and no untoward effect was observed. The results suggest that alpha-hANP can be used as a nasal spray in patients with overhydration.

The relationship of urinary prostaglandins and plasma renin to sodium balance and diuresis in normal man.

The significance of changes in sodium balance and urinary sodium excretion for renal PG excretion was studied in normal man. In protocol B a strongly negative sodium balance was produced in 5 healthy young subjects by a low sodium diet given for 7 days (lo mmol Na/day) with 80 mg furosemide p.o. added on the last two days. 24 hour urinary PGE2 excretion remained constant, while plasma renin increased. In protocol A the effect of i.v. furosemide (1 mg/kg bwt) on urinary PGE2 and PGF2 alpha excretion rates was examined in 5 healthy young subjects. Rapid but short-lasting increases in PG excretion rates ran in parallel with the changes in urine flow rate. The study suggests that PGE2 is not of importance for the sodium homeostasis in normal man. Renal prostaglandins may play a modifying role for the renal response to loop diuretics but are hardly instrumental for the diuretic effect.

Serum lithium minimum and diuresis.

Serum lithium was analyzed over a 24-hour period in patients who were receiving lithium in one daily dose. Linear regression was performed with urine volume as the independent variable and lithium dose, maximum serum lithium concentration. 12-hour serum lithium, minimum serum lithium, length of treatment, and age of patients as the dependent variables. Only minimum serum lithium and urine volume showed a good positive correlation.

Effects of Visnea mocanera extracts on the bleeding time, gastrointestinal transit and diuresis in rodents.

The present study examined the effect of several extracts of Visnea mocanera L. on the bleeding time and gastrointestinal transit in mice using the Duke test and charcoal meal method, respectively; they were also investigated in order to examine their renal effects. Results showed that V. mocanera extracts significantly inhibited both the bleeding time and gastrointestinal transit in mice. No diuretic activity of these extracts were detected. In contrast, they significantly reduced the urine volume. The study provides experimental support to the therapeutic advocation of V. mocanera in ethnomedicine for the treatment of diarrhoea and the healing of wounds.

Abnormal circadian rhythm of diuresis or nocturnal polyuria in a subgroup of children with enuresis and hypercalciuria is related to increased sodium retention during daytime.

PURPOSE: In a subgroup of children with enuresis an increase in nighttime water and solute excretion has been documented. To investigate if modifications in renal function are involved in nocturnal enuresis, we assessed circadian variation in natriuresis and tubular sodium handling in polyuric hypercalciuric children. MATERIALS AND METHODS: A total of 10 children with proved hypercalciuria and nocturnal polyuria and 10 age matched controls were included in the study. A 24-hour urine collection was performed in 8 sampling periods for measurement of urinary sodium excretion. Segmental tubular sodium transport was investigated during a daytime oral water load test and calculated according to standardized clearance methodology. RESULTS: The children with enuresis showed a marked increase in the fractional excretion of sodium during the night (0.93% +/- 0.36%), while daytime sodium excretion was decreased (0.84% +/- 0.23%). Analysis of segmental tubular sodium transport revealed decreased delivery of sodium to distal tubule (C(H2O) + C(Na) = 10.7 ml/100 ml glomerular filtration rate), indicating increased proximal tubular sodium reabsorption but also stimulation of distal sodium reabsorption as demonstrated by increased fractional distal sodium reabsorption (92.9% +/- 2.2%, controls 90.5% +/- 2.9%). Increased distal reabsorption was associated with increased fractional potassium excretion (17.5% +/- 2.7%, controls 13.6% +/- 6.4%), indicating increased distal tubular sodium/potassium exchange. CONCLUSIONS: No intrinsic defect in renal tubular sodium transport was found, but during the day increased sodium reabsorption in proximal and distal tubules was observed, suggesting extrarenal factors to be involved in altered circadian variation in solute and water excretion by the kidney.
