Effects of aerosolized and/or intravenous lidocaine on hemodynamic responses to laryngoscopy and intubation in outpatients.

A randomized, double-blind study was carried out on 40 unpremedicated, ASA I-II adult surgical outpatients to assess the effects of aerosolized lidocaine, intravenous lidocaine, both, or neither, on circulatory responses to laryngoscopy and intubation. Lidocaine (4 mg/kg) or saline was given by nebulizer in the holding area beginning at -15 minutes. The patient underwent a standardized induction of anesthesia that included IV curare (3 mg) and O2 by facemask at minute 2, followed by IV thiopental (5 mg/kg) and succinylcholine (1.5 mg/kg) at minute 5. Lidocaine (2 mg/kg) or saline was given by IV push at minute 4. Laryngoscopy was begun at 5 minutes and continued for 45 seconds before intubation. Heart rate and systolic, diastolic, and mean blood pressures were automatically recorded at 1-minute intervals from 0 to 11 minutes. The four treatment groups included: group 1, aerosolized and IV saline; group 2, aerosolized saline, IV lidocaine; group 3, aerosolized lidocaine, IV saline; and group 4, aerosolized and IV lidocaine. There were no differences among the four treatment groups (n = ten per group) in any of the four hemodynamic variables before laryngoscopy and intubation. Within each group, after intubation all four hemodynamic variables increased significantly over the corresponding baseline values for that group. However, the maximum values attained after intubation did not differ significantly among the four treatment groups for any of the four hemodynamic variables, whether those maxima were expressed as absolute values or as a percentage of baseline.(ABSTRACT TRUNCATED AT 250 WORDS)

[An analysis of methods for controlling and regulating the concentration of vaccinal preparations during aerosol immunization].

In this work the methods for the passive control and automatic regulation of the concentration of vaccinal aerosols in static chambers are analyzed. For such analysis, a method for the calculation of the parameters of vaccinal aerosols, taking account of the law of the distribution of particles by their sizes and of the processes of sedimentation and inactivation of microbial aerosols, has been developed. To make empirically established coefficients more precise, the necessary experimental studies have been carried out. High effectiveness of the method providing for the regulation of aerosol mass concentration has been demonstrated, and conditions necessary for effective aerosol immunization have been established.

[Pharmacokinetic research and the ultrastructural changes in the lungs of white rats after aerosol administration of a high dose of kanamycin sulfate].

Studied is kanamycin sulphate "Pharmachem", substance with activity 750 UI/mg, applied on albino rats in the form of water aerosol. The experiments are carried out in aerosol camera of flow-dynamic type with capacity 450 l at exchange of the air 60 l/min with aerosol apparatus of pneumatic type, for 10 min and staying in the aerosol cloud another 10 min. It is established that the kanamycin sulphate, after aerosol application on albino rats in dose of 50000 UI/m3, passes in the blood and is found out in the serum yet in the first hour (1.75 +/- 0.23 mg/cm3 serum). The maximal quantities of antibiotic are established in the first hour in the lungs (52.1 +/- 11.4 micrograms/g) and the antibiotic is detained in therapeutical concentrations for 240 h (0.1 +/- 0.03 micrograms/g). The content of the kanamycin in the kidneys is in the limits from 0.1 +/- 0.02 to 1.69 +/- 0.17 micrograms/g. At the electronic-microscopic tests of the lungs is observed hyperemia and destruction of the lamella corpuscles, which processes up to the 120th hour gradually calmed down.

Retinopathy from inhaling 4,4'-methylenedianiline aerosols.

4,4'-Methylenedianiline (MDA) is an important chemical intermediate in the production of isocyanates and polyurethane elastomers and polymers. The health hazards from acute inhalation exposure to the aerosols were evaluated. Guinea pigs of albino and pigmented strains were exposed nose-only to the aerosols of MDA in polyethylene glycol 200 (PEG) solution. The exposure was 4 hr per day, 5 days per week for a total of 10 exposures in 2 weeks. The time-weighted average aerosol concentration was 0.44 +/- 0.09 mg/liter and the optical number length mean diameter of the aerosol particle was 2.4 micron with sigma g of 2.1. During exposures, no overt respiratory distress was observed. Two weeks after the exposures, the guinea pigs were tested for possible dermal sensitization by being challenged with dermal application of MDA-PEG solutions at concentrations of 0, 2, 20, and 200 mg/ml. Neither dermal irritation nor allergic response was detected under this experimental condition. Thereafter, the animals were tracheostomized for measurements of changes in lung insufflation pressure for detecting possible changes in the distensibility of the lungs from a challenge dose of an aerosol of MDA-PEG at a concentration of 200 mg/ml. No significant changes were observed under this testing condition. Finally, the animals were euthanized for histopathologic examinations of eye, lung, liver, and kidney. The most remarkable findings was the degeneration of the inner and outer segments of the photoreceptor cells and the pigmented epithelial cell layer of the retinas of both albino and pigmented strains of guinea pigs.(ABSTRACT TRUNCATED AT 250 WORDS)

[Physical basis and therapeutic range of application of ultrasonic aerosols (author's transl)].

An aerosol of an aqueous solution still changes its size from development up to alveoli; therefore only conclusions can be made about possible sizes of particle spectrum. Volatilisation, condensation, coagulation and sedimentation are the important interfering factors. To characterize an aerosol one should know the aerosolvolume, the quantity of nebulised substance and the relative size distribution. A method for measurement of size distribution -- a radioactive aerosol was deposited in a spiral centrifuge -- is described and the aerosol of two ultrasonic nebulizers were compared. Because of their physical properties aerosols of ultrasonic nebulizers play a main part in prophylaxis and therapy of functional lesion of bronchopulmonary affections.

[Medical aerosols devices. Technical questions and regulations (author's transl)].

In today's practice, the medical aerosols are most often composed of soluble and hygroscopic particles emitted by the evaporation of a primary aerosol of solution droplets. The instruments on the one hand and the formula of the aerosoltherapeutical solution on the other should be worked out so that the fixation of aerosol in the lung be most efficient. The main data in view of this improvement are studied: --the curve of fixation of aerosols of sodium chloride in lung and the recall of the standard curve for insoluble and inert particles; --the characteristics of medical aerosol devices subjected to approbation or still unregulated (spray can). The calculation of the doses determined in standard conditions are illustrated together with the parameters which can make them vary.

[Investigations on the carcinogenic burden by air pollution in man. XVI. Formation of combined carbon black and benzo(a)pyrene aerosol (author's transl)].

Two methods for the preparation of carbon black aerosols have been investigated: incomplete combustion of acetylene, acetylene + benzene and other hydrocarbons as well as a "resublimation" of amorphous carbon. The first method was developed for generating soot aerosols in animal experiments, but the latter method needs more basic investigation. Using radioactive acetylene and benzene the produced soot aerosol could be labelled by 14C. Benzo(a)pyrene aerosol was prepared by means of a vapour condensation and was also radioactive labelled. With a combination of both generators, a combined carbon black and benzo(a)pyrene aerosol was prepared. The benzo(a)pyrene amounts bound to the soot were in the range of from 1 ng to 50 microgram per 1 mg soot. Experiments dealing with adsorption and desorbtion of benzo(a)pyrene on soot in the gas phase have shown, that benzo(a)pyrene is relatively tightly adsorbed and cannot be easily or completely desorbed.

Absorption of aerosolized drugs from the rat lung.

To investigate the absorption of aerosolized drugs from the rat lung, anesthetized animals were allowed to inhale through a tracheal cannula liquid aerosols of drug solutions generated with an air-jet nebulizer. The aerosols had a mass median aerodynamic diameter of 2.81 +/- 0.08 micron and a geometric standard deviation of 2.53 +/- 0.05. At various times after aerosol inhalation, the lungs were removed and assayed for the amount of compound that had not been absorbed. The times (min) necessary for 50% absorption were: antipyrine, 0.25; salicylic acid, 0.67; barbital, 0.93; amitrole, 1.3; urea, 1.4; procainamide, 2.3; guanidine, 3.1; erythromycin, 6.3; benzylpenicillin, 20.5; p-aminohippuric acid, 21.7; mannitol, 26.5; and N-acetylprocainamide ethobromide, 34.5. Comparison with previously reported absorption rates measured after intratracheal injection of 0.1 ml of drug solution showed that drug inhaled as an aerosol was absorbed roughly two times more rapidly than when administered by intratracheal injection. The results suggest that drug absorption may be more rapid from the alveolar region than from the tracheobronchial region of the lung.

Dose response relationship of Haemophilus pleuropneumoniae aerosols in pigs.

The virulence of Haemophilus pleuropneumoniae was quantitated for ten and 12 week old pigs following aerosol exposure. The volume and concentration of culture aerosolized, the estimated numbers of organisms inhaled by the pigs and the mortality rates at 72 hours postexposure were computed and used to calculate the LD50. There was correlation between the concentration of culture aerosolized, the amount of the estimated inhaled dose and the mortality rates. The ten week old pigs were apparently more susceptible to aerosols of H. pleuropneumoniae than the 12 week old pigs. The LD50 value or a multiple of it appears to be a reasonable basis for a standardized aerosol challenge of the immunity of pigs vaccinated with experimental or commercial H. pleuropneumoniae vaccines.

Aerosol vaccination of calves with pasteurella haemolytica against experimental respiratory disease.

Three experiments were conducted on calves in which the efficacy of vaccination with live Pasteurella haemolytica in aerosol was tested by challenge with sequential aerosol exposure to bovine herpesvirus 1 and P. haemolytica. Neither single nor multiple aerosol vaccinations protected against the experimental disease. Macroscopically recognizable rhinitis, tonsillitis, tracheitis and pneumonia occurred in both controls and vaccinates. In one experiment as many as three aerosol vaccinations with live P. haemolytica for up to 20 minutes failed to elicit clinical signs in exposed calves. Pasteurella haemolytica was isolated less frequently from tissues of vaccinated calves than from those of nonvaccinated calves. Pasteurella haemolytica was isolated from deep nasal swabs of 4/14 vaccinated calves five and six days after viral exposure. It was concluded that although bovine herpesvirus 1 vaccination has been shown previously to prevent the experimental disease produced by bovine herpesvirus 1-P. haemolytica, live P. haemolytica vaccination by aerosol will not provide the same protection.

[Stability of Aujezky's disease virus strain MK25 in an aerosol].

Twenty-two tests were carried out to evaluate the stability of the virus of Aujeszky's disease in aerosol. Use was made of a aerosol chamber of a flow dynamic type, 450 l, of 60 l/min of passing air, 5 min. exposure, 80-100 per cent relative humidity, and temperature range of 14-21.2 degrees C. The size of aerosol particles surpassed 5 micron, and changed according to the composition of the dilution medium of the vaccine. The aerosol was sampled via impinger at the rate of air flow of 5.2 l/min. Tested were the stabilizing properties of 2% glycerin, 10% pepton, 0.8-2.0% saccharose, 2% gelatin, and 5% skimmed milk added to the extender consisting of a 0.2 M phosphate buffer saline, pH 7, specifically for the MK25 strain of the Aujeszky's disease virus in a dispersed status. Highest stability, reaching 11.2% survival rate of the virus in aerosol was recorded at the dispersion of the vaccine in a 0.2 M buffer saline with the addition of 10% pepton and 0.8% saccharose at pH 7.

The relationship between the pulmonary distribution of inhaled fine aerosols and tests of small airway function.

Gamma-camera lung images obtained after aerosol inhalation (2.2 micron mean aerodynamic diameter) were compared with those obtained during inhalation of krypton-81m gas. The penetration of the aerosol to the periphery of the lung appeared in both smokers and nonsmokers to be similar to the penetration of krypton-81m. An index of the inhomogeneity of deposition of the aerosol (aerosol distribution index, ADI) was calculated by dividing the variance of the cell by cell radioactivity counts by the mean count. This index was expressed as the mean value for both lungs (ADIM) or the value for the lung with the higher value (ADIH). ADIM and ADIH were correlated with the FEV1 and maximum expiratory flow rates at 50% and 25% of vital capacity and on average showed higher values for smokers than non-smokers. ADIH, forced expired time and partial expiratory time from 50% to 0% of vital capacity were correlated with pack-years of smoking. It is concluded that the results of tests of small airway disease are related to inhomogeneities of airflow in the periphery of the lung and that measurements of aerosol deposition may provide a sensitive indicator of disease of small airways. The future potential of the test is discussed.

Physical properties of aerosols produced by several jet- and ultrasonic nebulizers.

In order to estimate the administered dose of inhaled drugs from aqueous aerosols, we have examined the output characteristics of eleven jet- and nine ultrasonic nebulizers. Under various operating conditions we measured driving pressure, airflow, solution- and solute output of the nebulizers, particle concentration and size distribution in the aerosol. The particle size distributions were determined of the dry particles from evaporated droplets of aerosolized 9 mg X ml-1 NaCl solutions by using a mobility aerosols spectrometer. These physical properties of the aerosols were found to vary considerably between nebulizers (even among specimens of the same type). Solution output per litre of air from the ultrasonic devices was 2 to 10 times larger than that from the jets, being dependent on electric power and driving pressure respectively. The jet nebulizers showed the largest particle concentration per litre of air. The volume median aerodynamic diameter of the droplets generated by the jet nebulizers was about 1.5-4 microns. With the exception of the Siemens TV 6000 and the Bosch Halomed, this diameter ranged between 3 and 7 microns among the ultrasonic nebulizers, being dependent on the frequency of the ultrasonic vibrations. The output characteristics appeared also to be affected by extension tubes, air vents, solute concentration and airflow. Solute output varied nonproportionally with solution output, which confirms the comparative merit of output measurements by weighing. The results of the present study can be used to estimate the administered dose of therapeutic or provocative agents by nebulizer equipment. Actual drug output can only be quantified by careful calibration of each separate set-up.

Mononuclear cells, mast cells and mucous cells as part of the delayed hypersensitivity response to aerosolized antigen in mice.

Mice (BALB/c) exposed to picryl chloride (PiCl) on their shaved abdomen and untreated animals were after 7 days exposed to daily aerosolized trinitrophenylated dog serum albumin (TNP-DSA). Mice exposed to PiCl tended to respond earlier and more strongly with both delayed hypersensitivity (DH) and IgG antibodies in serum and bronchial washings than did mice exposed to aerosol only. Picryl chloride sensitization resulted in spontaneously proliferating axillary lymph node cells, which could not be further stimulated with antigen or mitogen. Histological examination of lung tissue of aerosol-sensitized animals revealed an increase in mononuclear cells and mast cells around bronchioli and mucous cells, particularly in those animals exposed for prolonged periods and sensitized with PiCl prior to aerosol. Sensitization of mice with aerosolized TNP-DSA administrated in two 2- and 1-week periods with a 4-week interval responded with DH and IgG antibody in a dose dependent fashion irrespective of presensitization with PiCl. In bronchial washings IgG antibodies were found particularly after two 2-week periods of exposure. The cells taken from the axillary or brachial lymph nodes showed spontaneous proliferation. Culture of the cells to achieve mast cell maturation resulted in no or very low numbers of mast cells in the lymph nodes.

Airway responses in normal subjects following topical anesthesia with ultrasonic aerosols of 4% lidocaine.

Airway dynamics were studied in seven healthy unanesthetized male subjects before and after topical anesthesia with ultrasonic aerosols of 4% lidocaine to assess the effects of this clinically useful technique on normal pulmonary function. Responses to the ultrasonic mist per se were evaluated by studies on separate days with physiologic saline. Significant increases in expiratory flow rates during maximal and partial expiratory flow-volume curves occurred in response to lidocaine. In contrast, flows during partial expiratory flow-volume curves were reduced following saline inhalation. Lung elastic recoil was unaffected by either aerosol. Results of single breath nitrogen washout and responses to breathing helium were unchanged following topical anesthesia with lidocaine, but the saline aerosol significantly increased the slope of phase III during nitrogen washout and volume of isoflow after breathing helium. These changes indicate that topical anesthesia of the airway with 4% lidocaine given as an ultrasonic aerosol produces mild bronchodilation in healthy subjects, whereas saline administered in the same fashion causes mild bronchoconstriction. The effects of both aerosols most likely represent direct action on the smaller peripheral bronchi.

[The stability of drugs under the conditions of the ultrasonic thermo aerosol (author's transl)].

Under conditions of the ultrasonic thermo aerosol the stability of selected drugs is investigated by estimation of their concentration and by spectrophotometric and thin-layer chromatographic analysis, respectively. Using an ultrasonic nebulizer USI 50 of the common production neither the spectrophotometric nor the thin-layer chromatographic analysis give any sign of destruction processes of the drugs used. As a rule the concentration of the drugs is lower in the condensed aerosol than in the stock solution. For clinical use this decrease in unimportant. Producing the stock solution of nystatin corresponding to the recommendations of the committee "Aerosols in the Medicine" of the Society of Bronchopneumonology and Tuberculosis of the GDR, only very low concentrations of nystatin can be detected in the aerosol. Producing the solution in another way--chosen by us--50% of the concentration in the stock solution can be fund in the aerosol. But this manner of production still needs the approval of the central expert evidence committee.

Reflex-mediated desquamation of bronchiolar epithelium in guinea pigs exposed acutely to sulfuric acid aerosol.

Terminal conducting airways are known to be vulnerable to direct injury by a variety of noxious aerosols. Sulfuric acid aerosol, a by-product of fossil fuel combustion, produces desquamation of terminal bronchiolar epithelium in guinea pigs that is believed to result from direct deep lung irritation, an effect separable from reflex airway constriction induced by sulfuric acid. To characterize desquamation of bronchiolar epithelium, 20 guinea pigs were exposed to 32.6 mg/cu m sulfuric acid aerosol with a mass median aerodynamic diameter of 1.0 micron for 4 hours. The guinea pigs were killed upon termination of the exposure, or 24 hours later, and airway alterations were evaluated by light and transmission electron microscopy. To test whether the development of bronchiolar epithelial desquamation is independent of reflex airway constriction, 24 guinea pigs were exposed to an identical aerosol for 4 hours after pretreating half with 5 mg/kg atropine sulfate intraperitoneally to inhibit airway constriction. Sulfuric acid produced diffuse pulmonary hyperinflation with areas of consolidation and atelectasis. Epithelial desquamation occurred in airways supplying regions of developing atelectasis and was most extensive in terminal bronchioles. Parasympathetic effector blockade with atropine eliminated epithelial desquamation. These results indicate that sulfuric acid-produced desquamation of terminal bronchiolar epithelium is not separable from reflex airway constriction and that terminal conducting airways are vulnerable not only to direct injury by noxious aerosols but also to indirect, reflex-mediated injury.

Aerosol application of cyanoacrylate adhesive.

BACKGROUND: Cyanoacrylate adhesive has been used to treat corneal perforations and stromal melting disorders. We examined the efficacy of aerosolization as a means of applying cyanoacrylate to the cornea. METHODS: Central corneal perforations were created with a 1-millimeter trephine in cadaver eyes. A small amount of N-butyl cyanoacrylate was delivered to the perforation site via aerosol. Experiments were also conducted with cadaver eyes which had received lamellar keratectomies to stimulate corneal thinning disorders. RESULTS: Adequate seal of the 1-millimeter perforation was achieved following aerosol application of cyanoacrylate adhesive. In experiments conducted with eyes which had been perforated and those which received lamellar keratectomies, smooth anterior surface contours were achieved using the aerosol technique. CONCLUSIONS: Aerosolization may be an effective method of applying cyanoacrylate adhesive.
