The development of a women's health center in an academic health center.

Women's health needs have been a special focus of health care for more than a century. In the early 1970s, a segment of the women's market began seeking changes in the health care system. Since then, the emergence of women's health centers has been a nationally growing trend. This professional paper will address the various elements in designing a women's health center in an academic health setting, beginning with the development of a concept and needs assessment and ending with the influence a women's health center has on health care for others.

Styles of reminiscence and ego development of older women residing in long-term care settings.

Styles of reminiscence used in life stories, rather than being outcomes of life review undertaken in old age, may be the characteristic ways in which individuals at particular levels of ego development, think about, relate to, and recount the stories of their lives. To investigate the contention that differences in the styles of reminiscence of older persons could be explained by their levels of ego development, seventy-five women residing in long term care facilities responded to the Washington University Sentence Completion Test and told the stories of their lives. A trend from higher to lower levels of ego development was associated with styles of reminiscence in the direction predicted. Women with Affirming styles had higher levels of ego development than women with the Negating and Despairing styles. Resilience seemed the most likely explanation for cases that did not fit predictions. Life stories are suggested for routine data gathering in long term care because they allow the essence of another person to be grasped and foster a different level of engagement than do conventional data gathering techniques.

Courage in the development of self in women.

The author discusses the development of self in women, explores the role of courage inherent in this movement, and, employing existentialist notions of courage as authenticity, explicates how these notions may lead to the development of a stronger sense of self. Notes as well that this development of self entails a dangerous complexity and that pastoral care providers will want to aid women in holding together both autonomy and connection.

Helping women help themselves: developing a menopause discussion group.

Menopause discussion groups are needed to provide women with accurate and up-to-date information, but--more importantly--to facilitate sharing experiences, to help women understand their choices, and to empower them to be informed and take care of themselves. Members of a group who are comfortable expressing their feelings and thoughts find a therapeutic benefit from group participation. Health care providers are urged to develop these groups in their communities to assist women as they approach menopause and beyond.

Maternal serum and amniotic fluid inhibin A levels in women who subsequently develop severe preeclampsia.

The purpose of this study was to evaluate whether maternal serum (MS) and amniotic fluid (AF) inhibin A levels are elevated in patients who subsequently develop severe preecalmpsia, and to investigate the correlation between MS and AF inhibin A levels in the second trimester. The study included 40 patients who subsequently developed severe preecalmpsia and 80 normal pregnant women. Inhibin A levels in MS and AF were measured with enzyme-linked immunosorbent assay (ELISA). The MS and AF inhibin A levels in patients who developed severe preeclampsia were significantly higher than those in the control group (both for p<0.001). There was a positive correlation between MS and AF inhibin A levels in patients who developed severe preeclampsia (r=0.397, p=0.011), but not in the control group (r=0.185, p=0.126). The best cutoff values of MS and AF inhibin A levels for the prediction of severe preeclampsia were 427 pg/mL and 599 pg/mL, respectively; the estimated ORs that were associated with these cut-off values were 9.95 (95% CI 3.8-25.9, p<0.001) and 6.0 (95% CI 2.3-15.8, p<0.001). An elevated level of inhibin A in MS and AF at the time of second trimester amniocentesis may be a risk factor for the subsequent development of severe preeclampsia.

Smoking as a risk factor in the health of women.

It is estimated that more than 200 million women smoke worldwide. It is also estimated that if current smoking rates among men and women persist worldwide, by 2025, 9% of the world's deaths and disabilities will be related to tobacco use. Smoking is associated with an increased risk of cardiovascular disease, cancer (lung, cervix, pancreas, kidney, bladder, esophagus, pharynx), pulmonary disorders, cerebrovascular disease, and many other health risks. Smoking also adversely affects fertility and reproductive outcomes. Although a greater percentage of women smoke in developed than in developing countries, women in developing countries appear to be initiating smoking habits in increasing numbers. If current trends persist, the adverse effects of smoking on women's health clearly will continue to escalate.

MMP-2 levels are elevated in the plasma of women who subsequently develop preeclampsia.

OBJECTIVE: To determine levels of matrix metalloproteinase (MMP)-2 and MMP-9, and the tissue inhibitors of metalloproteinases (TIMP)-1 and TIMP-2 in the plasma of women destined to develop preeclampsia prior to the onset of clinical disease. STUDY DESIGN: Plasma samples were taken from women whose pregnancies were subsequently complicated by preeclampsia and from normal pregnant women at 22 and 26 weeks and at delivery or diagnosis. Following equal protein loading, MMP-2 and 9 and TIMP-1 and 2 were quantified using zymography and Western blot analysis, respectively. RESULTS: Plasma MMP-2 levels were significantly elevated at 22 weeks (p = 0.02) and at diagnosis (p = 0.003) in the preeclampsia group, but there was no difference at 26 weeks. TIMP-1 levels were significantly reduced in the preeclampsia group at 26 weeks (p = 0.0002), but TIMP-2 levels were not quantifiable. CONCLUSION: At all three gestational time points an imbalance in the MMP-2:TIMP-1 ratio was found in patients who subsequently developed preeclampsia. We speculate that increased net MMP-2 activity may contribute to the endothelial dysfunction that is central to the pathophysiology of preeclampsia.

Menstrual and premenstrual experiences of women in a developing country.

The menstrual and premenstrual experiences of black Zimbabwean women were examined. Twenty-five professional women and twenty-five domestic workers were interviewed using semistructured, open-ended interviews designed to explore their experiences. An analysis of consensual data indicated a number of predominant themes, including secrecy associated with menstruation, the negativity surrounding menarche, the breakdown of the traditional family network that passes on information about menstruation, and the acceptance of menstrual cycle experiences by the women. The main differences between the two groups concerned explanations of the functions and purpose of menstruation and the reporting of physical and affective symptoms. These differences are suggestive of the impact of educational level on experiences of menstruation.

[Blood pressure circadian rhythms during development of hypertension in healthy pregnant women].

Investigation of systolic (SBP) and diastolic blood pressures (DBP) circadian chronological structure changes by means of blood pressure monitoring in healthy pregnant women during development of gestational hypertension (GH) and pre-eclampsia (PE) was carried out. 72 hours blood pressure monitoring (TM-2421 device, Tokyo, Japan) was carried out in 34 healthy pregnant women. Parametric and non-parametric indices of circadian rhythms were determined, which allowed us to include in group A 17 women who had normal BP until delivery. Group B consisted of 17 women with hypertension in II and III trimesters. Increase of hyperbaric index (HBI) of SBP was revealed in the I trimester in women in whom hypertension was developed later in pregnancy. HBI was used as a prognostic criterion of developing MESOR-hypertension long before its clinical manifestation. In the II and III trimesters HBI, per cent time of elevation (PTE%) and MESORs of SBP and DBP circadian rhythms progressively augmented. Pregnant women with hypertension belonged to type non-dipper according to D/N% changes (less then 10%). This may be the result of insufficient decrease of night values of SBP in group B pregnant women, which is characteristic of GH and PE.

Assessment of a mobile training programme on nutrition for Bangladesh rural development board (BRDB) women's programme in Bangladesh.

About half of the total population of the country are women and without their active participation no development programme can be achieved. This study involved a total number of 690 trainees from 23 upazilas of different districts in Bangladesh. The results obtained from both pre and post tests were compared and analyzed to find out the impact of the mobile training programme on nutrition. The mean age of the trainees was 29.54. The mean number of children per trainee was 3.73 and the mean income was close to Taka 1048.26. Analysis of the results regarding knowledge of the training about nutrition at pre and post training stages reveal that at post training stage they were more aware of the importance of nutrition. The difference in knowledge level at pre and post training stages was statistically significant (P less than .001).

Women in child development: themes from the SRCD Oral History Project.

Research in child development has emerged as a major intellectual discipline and a topic of great interest and importance to society. The Society for Research in Child Development (SRCD) has been the leading scholarly society concerned with the advancement of this research. As part of its focus on a history of the field, the SRCD has undertaken an oral history project in which prominent members are interviewed. Of the 102 completed interviews, 47 are women's accounts, which document important contributions women have made to child development. The experiences reported by individual participants reflect themes in the general history of women in psychology and child development during the early and mid-1900s. Women encountered obstacles to their efforts, even as they were supported by others and struggled to achieve prominence amid the sometimes ambiguous or conflicting circumstances that characterized women's entry into child development.

Women's labour force participation and socioeconomic development: influences of local context and individual characteristics in Brazil.

We address several key hypotheses about the effects of socioeconomic development on women's labour force participation during the transition from agriculture to industrialism. To this end, we explore differences in women's labour force participation in Brazil by education, marital status, age, and urban or rural residence. We also show how socioeconomic development affects the overall level of women's participation and the differentials by education, etc. Our data are drawn from a large 1973 PNAD (Pequisa Nacional por Amostra de Domicilos) survey conducted by the Brazilian census bureau. Socioeconomic development in different parts of Brazil ranges from pre-industrial agriculture to heavy industry. Using logistic regression, we show that the general level of women's labour force participation does not change with the level of development. Highly educated women are much more likely than the less educated to be in the labour force (net of other influences); this difference is substantially greater than in post-industrial societies. Somewhat surprisingly, the influence of education is the same across the range of development levels in Brazil. Single women are more likely to be in the labour force than married women, and the difference grows during development. Age has a curvilinear relationship to labour force participation, and the old are much less likely to participate in more developed places. Rural women are slightly more likely to be in the labour force at all levels of development.
