Replantation of avulsed teeth after long extra-alveolar periods.

In 11 patients, 18 teeth, with extra-alveolar periods ranging from 6 h to 48 days were replanted after root-filling with chloroform resin and guttapercha. After observation periods from 1 to 7 years, only 1 tooth was lost (after 5 years), but the remaining 17 teeth were clinically well functioning. All teeth were ankylotic and subjected to replacement resorption of various extent. Inflammatory root resorption and periradicular osteitis were not observed. Marginal bone loss up to 5 mm was noticed, but was accompanied by gingival retraction and thus pathological periodontal pockets (5 mm) were not observed. Consequently, teeth replanted after long extra-alveolar periods (even several days) can function for many years and thus postpone prosthetic treatment.

Endodontic management of trauma to permanent teeth.

Oral trauma is a common event. It has been estimated that up to 20% of the United States population has sustained injuries to the teeth. In a perspective study, Andreasen reported that one of every two Danish children had sustained an oral injury by age 14. This article will discuss the different types of injuries that can happen to permanent teeth, endodontic management of these injuries, and possible complications. Exarticulations (totally avulsed teeth) will not be thoroughly discussed.

Complete replacement resorption after replantation of maxillary incisors: report of case.

This article describes the treatment of a 17-year-old patient with complete root resorption of the maxillary permanent central and lateral incisors following avulsion and replantation seven years ago. The most important factor influencing the prognosis of replanted teeth is the status of the periodontal ligament (PDL). As a result of replantation, the PDL cells necrosed and tooth replacement resorption occurred. The main factors, which affected the resorption after replantation and survival of PDL cells, could be summarize as; dry extraoral time greater than 30 minutes, the kept tooth in a dry environment, touching the cementum surface, and splint treatment for a prolonged time. Since complete root resorption was found in our case, these factors probably also occurred.

Modern treatment of avulsed teeth by emergency physicians.

New research has shown that the incidence of success of replanted avulsed teeth can be greatly increased by differentially diagnosing and treating them according to the length of extraoral time and the physiological width of the apical foramen. A specific treatment regimen has been presented that will give the physician the information necessary to maximize the success of replanted avulsed teeth that may be seen.

Therapeutic protocols for avulsed permanent teeth: review and clinical update.

Traumatic injuries to permanent teeth include coronal and root fractures, subluxations, luxations, and avulsions. The most common complications after avulsions are necrosis of the pulp and root resorption. Treatment is often complex, time consuming, and expensive and requires multidisciplinary approaches such as endodontic and periodontal treatments, surgery, orthodontic movements, as well as esthetic coronal restoration. This review describes new clinical modalities for the treatment of avulsed teeth and discusses the rationale for their use.

[The reimplantation of teeth avulsed by trauma].

The paper underlines the importance of the reimplantation of teeth following injury in young subjects. The positive results achieved within the short term encourage the extension of this method even to those cases with a poor prognosis (unsuitable means of conservation, long interval between injury and reimplantation). The tooth with in any case serve to retain the space until a prosthetic replacement is inserted when growth has ended.
