[Objectification of gingival blood-supply disorders by photoelectric plethysmography (author's transl)].

The measuring method described in this paper is used to photoelectrically detect the volume pulsation of the gingival border. The method, which is remarkable for its ease of application, enables both the original pulse and its first differential derivation to be recorded reliably. Evaluation of the pulse cure is by the time of inclination and by the recently introduced "plateau index". Results of studies conducted by Kötzschke et al. show that the plateau index allows to clearly distinguish between normal subjects and parodontopathy patients including those where the disease is in its early stage. This measuring method may be used for objectifying therapeutical measures. Using the "nitroglycerin test", it was possible to show that parodontopathies are very frequently accompanied by functional blood-supply disorders.

[D-SGP Duo--a new plethysmography for acral, arterial and venous blood pressure measurement].

This is to present a new portable plethysmograph using modern microprocessor technology, a two-channel digital strain gauge plethysmograph (D-SGP Duo). This system contains two strain gauges and a pressure gauge and has measuring programmes for thrombosis screening (venous occlusion test) and for the acral blood pressure measurement. For the first time it is possible to determine not only the acral systolic arterial pressure but also the acral venous blood pressure. Examples of application are given.

[Measurement accuracy of oscillatory and whole body plethysmography determination of airway resistance. Study of a mechanical model].

The degree of accuracy of the plethysmographic and oscillatory method in determining respiratory resistance has been examined on a mechanical lung model. At this model different levels of the resistance could be reproducibly adjusted and exactly determined with sensitive measuring instruments. The plethysmographic method allows a precise estimation of the resistance. It was found that the absolute variation of the plethysmographically measured values is not greater than 5%. The Ros pointer scale of the Siregnost FD 5 yields systematically incorrect curve diagrams. In the lower range of the resistance the measured values are to high while the measured results of the resistance become progressively to low with an increasing resistance. The reason is the Ros pointer scale which does not show the real component of the impedance at a phase angle of 0 degree. The values of the real component of the respiratory impedance (Rreal) which yields the Siemens standard set show a great coincidence with the lung model resistance (R(aw)). The coincidence could be even improved by use of electronic data processing. With a computer program developed by us it is possible for the first time to indicate and registrate consecutively individual and average values of the real component (Rrealcomp) and the reactance of the respiratory impedance as well as the phase angle between the alternating pressure delta p and the oscillating flow (V). Thereby the accuracy of measurement is improved and the long winded analysis with the "phase diagram" is not necessary anymore. Further experimental and clinical investigation have to show whether the oscillatory method in the way described above will offer new possibilities for the assessment of the pulmonary function. The phase angle and its course during the respiration cycle is in this connection of special importance as a possible new parameter.

Technical aspects of impedance plethysmography.

This paper describes the basic methods for measurement of body impedance, electrodes and their configuration, and the measuring instrument with its limitations. A microcomputer assisted impedance plethysmograph system, developed at BARC and different lead configurations for impedance plethysmographic investigation are also described. Typical impedance plethysmographic waveforms recorded from a normal subject and measurement of their amplitude and various time intervals are illustrated.

Diagnosis of peripheral arterial occlusive diseases using impedance plethysmography.

Impedance plethysmographic observations have been compared with arteriographic findings in 216 patients with peripheral arterial occlusive diseases. Impedance plethysmographic diagnosis in these patients was obtained by Parulkar's method without apriori knowledge of arteriographic diagnosis. But for minor discrepancy in the anatomical location of the block in few patients, impedance plethysmographic observations correlated very well with arteriographic findings. Impedance plethysmographic diagnosis was found to be correct in 312, wrong in 53, false negative in 8 limbs respectively, yielding a sensitivity of 97.5% and specificity of 98.1% of this technique.

Evaluation of state of circulation in radiation injury using impedance plethysmography.

Seven patients with history of radiation burn were subjected to impedance plethysmographic investigation at Non-invasive Vascular Laboratory, K. E. M. Hospital. Impedance plethysmograms were recorded from various locations in both the upper extremities in supine and digits of both the hands in supine as well as on hyper-abduction in sitting position. Control values of blood flow index (BFI) and differential pulse arrival time (DPAT) were derived from similar data in 15 normal subjects. It was observed that digits having thinning of epidermis of skin recorded marked decrease in values of BFI and significant change in value of DPAT and these observations correlated well with thermography and vascular scintigraphy. Digits recording significant decrease in BFI, which were clinically normal, were found to have changes in the skin during follow up examination. Thus IPG provided a sensitive modality for detecting ischaemia in early stages in patients with history of radiation injury.

[Methodologic comparison of the polyfrequency oscillation method, transcutaneous oxygen pressure measurement and body plethysmography in bronchial provocation with methacholine].

A bronchial provocation challenge test was conducted with 30 subjects using metacholin. In randomised sequence lung function analysis tests were carried out with the bodyplethysmograph (Raw, FEV1) and the polyfrequent oscillation method (resistance, reactance between 2 and 52 Hz), whereas the transcutaneous oxygen pressure (tc-PO2) was measured continuously. Correlations between the various parameters, the change of the values at PD60 sGaw in relation to the initial values, the interindividual variability and the reactivity were determined. Medium correlations were found for the oscillatory parameters and Raw, whereas for tc-PO2 and Raw the correlation was markedly lower. In terms of percentage the greatest change was found in reactance, in relation to the initial value, followed by Raw and FEV1. The interindividual variability resulted in the highest values for the reactance, followed by Raw and FEV1. Interindividual variability showed the highest values for the reactance before oscillatory resistance and Raw. In respect of reactivity, reactance also had the highest values. Overall evaluation showed that bodyplethysmography and the polyfrequent oscillation method (reactance or resonance frequency) are comparably sensitive.

[Noninvasive hemodynamic monitoring via the integration of data obtained by ECG, aortic flow by Doppler esophageal probe and by finger plethysmography].

The aims of this work are to describe the general and technical characteristics of a new device for the noninvasive monitoring of patients in intensive care and during general anaesthesia, and the results concerning the reliability of this method. An ultrasonic esophageal probe and an echo-Doppler device have been used to obtain continuous data of the aortic diameter and of blood velocity. Aortic output is calculated automatically. This method, together with other non-invasive monitoring techniques (blood pressure, heart rate, rhythm and cardiac conduction), gives on the one hand the data of aortic output, systemic peripheral resistance and stroke volume; on the other, through a computerized elaboration, the systolic time intervals (PEP pre-ejection period, LVET left ventricular ejection time, QS2 electromechanical systole, PEP/LVET ratio of PEP to LVET). The validation of STI data, has been obtained through 125 comparative measurements for each of the three parameters. The data obtained through the aortic velocity waveform in descending aorta (pulsed Doppler) have been compared with those obtained through the aortic pressure waveform (intra aortic catheter). The correlation was: PEP 0.92, LVET 0.95, QS2 0.93. The clinical application of this method supplies data concerning cardiac load, after-load and indirectly cardiac pre-load. This non-invasive procedure gives us a continuous measurement of hemo-dynamic situation, which allows the physician to plan and evaluate the therapeutical efficacy. Finally new pathologic events may be opportunely faced.
