Implementation of exact and approximate randomization tests for polynomial growth curves.

Two stand-alone, menu-driven PC programs, written in GAUSS386i, which compare groups of growth curves in a completely randomized design using either (a) exact or (b) approximate randomization tests, are described, illustrated, and made available to interested readers. The programs accommodate missing data in the context of studies planned to have common times of measurement, but where some of the measurement sequences are incomplete. The measurement whose growth is being monitored need not have a Gaussian distribution. We consider the hypothesis that the mean growth curves in G groups are the same; and either compute the exact P value (exact test), or estimate, and provide a confidence interval for, the P value (approximate test).

Preventing disability through community-based health coaching.

The California Public Employees Retirement System (CalPERS) Health Matters program is a randomized controlled trial of a community-based health coaching program operating in Sacramento, California, since January 2001. It is modeled after the Health Enhancement Program and Senior Wellness Program of Seattle, Washington. Like the Health Enhancement Program, this program incorporates a menu of disability-prevention strategies, with health coaching, patient education on the self-management of chronic illness, and fitness forming the program's core. Unlike the Health Enhancement Program, the Health Matters program focuses as much attention on linking participants to existing community, health plan, and self-directed programming as it does encouraging them to participate in programming developed especially for the project. All participants and controls continue to receive their usual medical care from their managed care providers. Eligibility criteria for the program include having one or more qualifying chronic health conditions, being aged 65 and older, being a member of a participating health plan, and being accepted into CalPERS' Long Term Care Insurance Program. Baseline exclusions include being cognitively impaired or qualifying for long-term care insurance benefits due to deficiencies in two or more activities of daily living. The project has been successful in its enrollment strategy. It has also been successful in recruiting participants into project-sponsored, community-based, and self-directed disability-prevention programming. This is particularly true for programming related to diet, exercise, and various aspects of disease management.

Regression imputation of missing values in longitudinal data sets.

A stand-alone, menu-driven PC program, written in GAUSS, which can be used to estimate missing observations in longitudinal data sets is described and male available to interested readers. The program is limited to the situation in which we have complete data on N cases at each of the planned times of measurement t1, t2,..., tT; and we wish to use this information, together with the non-missing values for n additional cases, to estimate the missing values for those cases. The augmented data matrix may be saved in an ASCII file and subsequently imported into programs requiring complete data. The use of the program is illustrated. Ten percent of the observations in a data set consisting of mandibular ramus height measurements for N = 12 young male rhesus monkeys measured at T = 5 time points are randomly discarded. The augmented data matrix is used to determine the lowest degree polynomial adequate to fit the average growth curve (AGC); the regression coefficients are estimated and confidence intervals for them are determined; and confidence bands for the AGC are constructed. The results are compared with those obtained when the original complete data set is used.

Empowering health care improvement: an operational model.

BACKGROUND: The shift from compliance to continuous improvement requires a bridge--empowerment--that enables the creation of an environment in which employees can contribute the full range of their expertise in service to the organization. Leaders are responsible for creating this empowering environment. MODEL: An operational model that describes the nine aspects of empowerment (for example, mission and values, access to information, avenues of influence) can serve as a template for leaders to use to promote an atmosphere of empowerment in their organizations. METHOD: The authors adapted this model as a survey instrument to uncover some hospital success stories that demonstrate several aspects of empowerment in action. The survey was administered at two hospitals. CASE STUDY 1: At Beth Israel Hospital (Boston, Mass), Nutrition Services rated the "mission and vision" aspect of empowerment in their department at an average of 3.5 out of a possible 4.0. The department attributes this high rating to their focus on specific goals that reinforce the larger mission and vision of the hospital as a whole. These goals form the basis for annual employee evaluations. For example, a 1992 patient-focused goal was to provide nutrition counseling and education. The department approached this goal by developing a daily format ("Eat Smart") for the daily hospital cafeteria menu that provided nutritional information. The rationale was to educate employees, who could then bring back information to their patients. Alignment of the department's efforts with the larger mission of the hospital enabled Nutrition Services to extend its influence beyond departmental and professional boundaries. Implementation of other plans at the hospital demonstrate a sustained process of vision development on the part of the executive leadership. CASE STUDY 2: At Robert Wood Johnson University Hospital (New Brunswick, NJ), Support Services gave the "environment of trust and respect" aspect an average rating of 3.7. The key to this success story lies in the upgraded role of the housekeepers as "hosts." The "Pro-Act" restructuring model for patient care delivery creates and upgrades positions to relieve nurses of their task loads. The three nursing services that participated in the survey gave their highest rating to the "access to the education and training" aspect of empowerment. The high quality of professional excellence is supported by tuition reimbursement, incentives for credentials, the extern program and scholarships provided to attract outstanding nursing students to their staff and encouragement of nursing staff at all levels for publication, presentations, and poster sessions. CONCLUSION: These case studies demonstrate how leaders created an environment in which collective learning has become a way of doing business. Given the multifaceted nature of the empowerment model, some may wonder where to begin to focus leadership efforts. It is important to emphasize that the constellation of empowerment aspects constitutes an integral system; the full potential for empowerment arises from synergy among all nine aspects.

A new approach to the design of information systems for foodservice management in health care facilities.

An organizational framework for integrating foodservice data into an information system for management decision making is presented. The framework involves the application to foodservice of principles developed by the disciplines of managerial economics and accounting, mathematics, computer science, and information systems. The first step is to conceptualize a foodservice system from an input-output perspective, in which inputs are units of resources available to managers and outputs are servings of menu items. Next, methods of full cost accounting, from the management accounting literature, are suggested as a mechanism for developing and assigning costs of using resources within a foodservice operation. Then matrix multiplication is used to illustrate types of information that matrix data structures could make available for management planning and control when combined with a conversational mode of computer programming.

[Menu design for total parenteral nutrition: simplified model derived from a computerized system].

After the design and use of a computerized system for the prescription of parenteral nutrition, the authors drew up clinical choice tables, as well as others of a pharmaceutical elaboration, based on the same algorithm of the computer program. The conclusion reached was that the nutrition selection process based on these tables is simpler, saves more time, is based on a logical plan of decision making and does not need a computer support.

[The iodine allowance and the prevalence of goiter among the children in organized collectives in the city of Kemerovo].

Nutritional value of diets given to children attending kindergartens and schools in Kemerovo according to specified menu (proteins, vitamins, minerals) has been studied. The unbalance in diet ration leading to development of iodine deficiency in children of these institutions is responsible for high incidence of goiter. The level of iodine deficiency is 34.0% in kindergartens and 78.2% in boarding schools. High occurrence of goiter in children's institutions should be considered in planning antigoiter preventive measures.

A brief description of I.C.E.: the integrated crystallographic environment.

We describe the current status of the program I.C.E., the Integrated Crystallographic Environment. I.C.E. is a windows-based, menu-driven image processing package mainly aimed toward crystallographic image processing, although some of its functions have been used in analyzing single particle data and helical objects. The system can take individual images of crystals through the entire process of data analysis, from windowing the raw image through to the final evaluation of defocus, symmetry and creation of projection reconstructions. Current plans are to incorporate a three-dimensional database of images and electron diffraction patterns, and 3-D merging and visualization.

The AFFIRM trial: main trial and substudies-what can We expect?

The clinical categorization of patients who present with atrial fibrillation is a major determinant of the most appropriate strategy for rhythm management. For those patients with recurrent atrial fibrillation that has not become permanent the two available strategies are rhythm control and anticoagulation or rate control and anticoagulation. There is no clear evidence that one of these strategies is superior to the other. In the AFFIRM trial these two strategies are being compared to one another in a randomized trial. Patients are randomly assigned to one of the two strategies and the treating physician then uses therapies from an approved menu as clinically indicated. Both pharmacologic and nonpharmacologic therapies are used. An overview of the main study protocol is presented. The primary endpoint is total mortality but there are a number of clinically important secondary endpoints. Several substudies will explore important ancillary questions and some of these are also described. At this time over 3000 patients have been enrolled and the planned enrollment is 4300. Enrollment will end late in 1999 and the last patient enrolled will be followed for two years. The AFFIRM Trial will provide important information concerning the management of atrial fibrillation in a large portion of the patients who have this arrhythmia.

MICRO-IDEA: improving decisions in epidemiological analysis by a microcomputer.

A microcomputer system has been designed to promote uniform and correct epidemiological data handling in the Local Health Units of the National Health Service in Italy. The system allows to define files of any format, to perform the common data manipulation procedures and to make simple statistical analyses. Two modules, based on artificial intelligence techniques, give the possibility to input from 'experts' and to update domain-specific knowledge concerning facts which characterize the application area covered by the file, and some consultation models which represent the reasoning to follow when solving problems of epidemiological data analysis. A decision support module allows an inexpert user to have access to these models, and to be guided in the choice of the appropriate procedure. The menu-driven interactive approach insures a friendly user-to-system interface, and entails a little training for effective use by health workers inexpert in informatics.

Microcomputer use in an oral biopsy service.

The need for rapid and accurate retrieval of the data generated by an oral biopsy service and the adjacent medical center was met with the purchase and programming of a microcomputer and hard disk drive. The planning phase involved an assessment of the needs of the department, creation of an ideal form to be displayed on the video screen that can be easily used to enter the information, selection of coding systems, and selection of compatible hardware and software. Customized in-house programming using a commercially available database management system has created an entry form and menu-driven information retrieval system tailored to the needs of the department.

Setting up and auditing guideline- and evidence-based cardiac rehabilitation.

BACKGROUND: Rehabilitation is available to only a minority of post-myocardial infarction (MI) patients and is of variable quality. Guidelines now recommend individualised care delivered by hospital, primary care and community services, but there is little evidence of the feasibility, acceptability or effectiveness of this approach. AIMS: To demonstrate the feasibility of guideline-based rehabilitation, to audit delivery and outcome and to identify problems. METHODS: A four-phase stepped programme for post-MI patients was developed based on individualised in-hospital care and aftercare from a menu of options. Delivery involved co-ordination between hospital services, primary care and community services. Self-report audit data were collected in hospital and at 3 months post-discharge by postal questionnaire and telephone calls. Clinical information was recorded from hospital, telephone and outpatient contacts. Delivery of care to patients receiving interventions was recorded. RESULTS: It was possible to negotiate individualized plans for all patients and to monitor progress for 3 months after discharge. The rehabilitation team achieved high rates of delivery of agreed interventions, considerably better than delivery by primary care. Problems largely related to difficulties and failures in communication. Patients with major social or psychological difficulties were the most difficult to treat. CONCLUSION: Guideline-based rehabilitation is feasible, but there is a need to improve the coordination of delivery of later steps of care and also to refine specialist interventions.

Accessing NCI's SEER cancer data base with SeerQuery and CD-ROM.

The National Cancer Institute operates the Surveillance, Epidemiology, and End Results (SEER) cancer data base. SEER data are obtained from participating population-based registries that monitor cancer incidence and patient survival in a representative 10 percent sample of the general population. The data cover all cancers (except superficial skin cancers) in the defined regional populations. SeerQuery is a personal computer program for accessing that data on IBM-compatible personal computer compact diskettes in read-only memory (CD-ROM) form. SeerQuery facilitates rapid access to cancer data at minimal cost and effort to the user. SeerQuery is menu-driven, enabling physicians and other health care professionals to query the data base directly. They can use the data to determine cancer frequency, perform cross-tabulation, determine incidence, and calculate survival using such variables as primary cancer site, histologic type, stage, sex, age, and race. The comprehensive data base lacks many selection biases that are inherent in data reported from other sources. SeerQuery has applications in professional education and in cancer control program planning and resource allocation.

Survey of computer technology in foodservice management education.

The status of the utilization of the computer as a tool in teaching foodservice management concepts in undergraduate dietetic education programs was determined. A questionnaire was administered to directors of Plan IV, V, and coordinated (undergraduate) programs at universities approved or accredited by The American Dietetic Association. Statistical analysis of the 137 usable responses (57%) showed that computers were used as a teaching tool in 85% of the programs surveyed. The majority of programs (58%) utilized microcomputers with floppy disk systems. Teaching laboratories equipped with computers were available in 83% of the programs represented by the respondents. Concepts most often taught with computer assistance included menu and recipe analysis and food cost accounting techniques. Ninety-four percent of the dietetic educators surveyed reported a need for increased utilization of the computer as a tool in teaching foodservice management concepts. Most respondents (92%) indicated that foodservice management educators need additional training in using the computer as a tool.

A program for the graphic representation and manipulation of DNA sequences.

We have developed a program for the graphic representation and manipulation of DNA sequences. The program (named CARTE from the French for 'map') is intended as a tool in the planning and analysis of recombinant DNA experiments. DNA sequences are represented as standard restriction maps, using any desired combination of restriction enzymes. Features of interest, such as promoters or coding sequences, can be highlighted. The sequence can be manipulated to mimic cloning, using deletions, insertions or replacements at specified sites. This process is facilitated by the simultaneous display of a graphic map of the entire sequence, a detailed picture of the work in progress, and a menu of functions.

Ultrasound-based stereotactic guidance in prostate cancer--quantification of organ motion and set-up errors in external beam radiation therapy.

OBJECTIVE: A mobile transabdominal ultrasound-based targeting system (BAT(R)) has been developed which can stereotactically localize the position of the prostate each treatment day and directly integrate this information into the treatment planning system. Daily target verification facilitates a marked reduction in planning treatment margins by correcting potential organ-motion and set-up errors. Previous studies have been performed to establish the precision of ultrasound localization. This report quantifies the magnitude of the patient isocenter shift parameters encountered during clinical implementation of this system. MATERIAL AND METHODS: After five weeks of conformal external beam radiation therapy, 54 patients underwent a second CT simulation. Prostate-only fields based on this scan were created with no PTV margin beyond the CTV. For each of the final conedown treatments (2-4 fractions), patients underwent ultrasound-based stereotactic prostate localization at the treatment machine. The portable system, which electronically imports the CT simulation target-contour and isocenter information, is situated adjacent to the treatment couch. Transverse and sagittal suprapubic ultrasound images are captured, and the system electronically couples this data to the baseline isocenter. The CT contours are maneuvered in three dimensions by a touch-screen menu to visually overlay the ultrasound images. The system then displays the three-dimensional (3D) couch shifts required to produce field alignment. RESULTS: One hundred and eighty-nine daily ultrasound prostate position shifts were recorded for 54 patients. The isocenter field misalignment between the baseline CT and ultrasound ranged from -26.8 to 33.8 mm in the anterior/posterior (A/P) dimension, -10.2 to 30.9 mm in the lateral dimension, and -24.6 to 9.0 mm in the superior/inferior (S/I) dimension. The corresponding directed average disagreements were -3.0 mm (SD 8.3 mm) A/P, 1.86 mm (SD 5.7 mm) lateral, and -2.6 mm (SD 6.5 mm) S/I. The magnitudes of undirected misalignments were frequently larger than 5 mm (51% of A/P, 31% of lateral, and 35% of superior measurements) and oftentimes larger than 10 mm (21% of A/P, 7% of lateral, and 12% of superior measurements). CONCLUSIONS: Organ motion and set-up uncertainties limit optimization of 3D treatment planning by expanding the width of PTV margins required to ensure target coverage. Transabdominal ultrasound-based stereotactic guidance is a safe and direct method for correcting patient positioning. Our experience with the BAT system in a large cohort of prostate cancer patients revealed that substantial daily isocenter corrections were encountered in a large percentage of cases. This data would suggest that daily clinical isocenter misalignments are greater than would be expected from published data on organ motion and set-up variations encountered in the study setting.
