Symptomatic hypokalemia resulting from surreptitious diuretic ingestion.
We report five cases of symptomatic hypokalemia caused by surreptitious diuretic ingestion. A positive diagnosis was made in four patients by a colorimetric detection of the diuretic in urine by simple chemical means. Patients who surreptitiously ingest diuretics appear to be mainly women, among whom there appears to be high prevalence of diagnoses involving the reproductive system. Many of these patients have health-related occupations. They seem to be very concerned with appearing obese or edematous. A high urine potassium level in the presence of a low serum potassium level can suggest this diagnosis.
rCBF measurements by 133Xe inhalation: applications in neuropsychology and psychiatry.
This review describes recent advances in the application of rCBF measurements by the 133Xe-inhalation method to neuropsychological and psychiatric research. Studies mapping cortical activity, as indicated by flow variations during visual, auditory and tactile tasks of varying complexity are described. The bilateral rCBF measurements do also offer information concerning lateralization of mental processes. Some clinical applications in psychiatry are finally reviewed demonstrating the use of the technique in differential diagnosis of dementia of organic ethiology (Alzheimer's disease, multi-infarct dementia) against pseudodementia due to psychosis. The method is also very useful in the study of cases with toxic influence by alcohol and other agents.
Depressive illness in old age: getting behind the mask.
Different presentations of depression may occur in the elderly. Complaints of physical illness are especially common and pseudodementia may be the presenting picture. Overtreated hypertension which can be well controlled with diuretics is a common cause of depression in the elderly. Taking the patient off the drug known to cause depression will often solve the problem.

The clinical features, radiographic findings, histopathologic features, and treatment options are reviewed.

[Psychoanalysis of self injury].
Psychoanalytic aspects of diseases with self-inflicted mutilation (factitious diseases and Munchhausen syndrome) have been investigated increasingly within the last years in the setting of long term ambulatory and hospital treatments. The author describes in this report how these disease states may also be interpreted as equivalents of remembrance and can be understood as inadequate attempts for self assistance. He outlines possibilities for psychotherapy arising in view of this background.
