Effect of Mirazid (Commiphora molmol) on experimental heterophyidiasis.

Mirazid (MZ), an oleo-resin extract derived from Myrrh was reported in several experimental and clinical trails to be safe and effective against other trematodes like schistosomiasis and fascioliasis. This experimental work aimed at investigating the possible efficacy of MZ against heterophyids (Pygidiopsis genata), using praziquantel as a therapeutic control. Results showed that MZ in emulsion form is a promising drug for the treatment of heterophyidiasis, as proved by significant reduction of worm count, overt surface tegumental changes like deformity and erosion of tegumental spines observed by scanning electron microscope (SEM). The effective dose regimen was 500 mg/kg/d for 3 successive days, produced 100% reduction in worm load. The proved efficacy of the drug, together with reported low toxicity, relative to praziquantel, favours its use as a natural new alternative therapy for the treatment of human heterophyidiasis.

[Evaluation of three different ELISA kits for the diagnosis of trichinellosis].

Human trichinellosis caused by Trichinella spp. is a widely distributed parasitic disease, acquired by ingestion of undercooked or raw meat containing larvae of the parasite. In January 2004, a trichinellosis outbreak following the consumption of raw minced meat ball, occurred in Izmir, Turkey. In the present study, an in-house excretory/secretory (ES) IgG ELISA and two commercial IgG ELISA kits (Kit # I; IBL, Hamburg, kit # II; Cypress, Belgium) have been used to evaluate their diagnostic efficiencies in this outbreak. Serum samples were collected from 100 patients with trichinellosis, from 16 patients with other parasitosis and autoimmune diseases (2 toxocariasis, 8 hydatic cyst, 1 fascioliasis, 5 rheumatoid arthritis) and from 20 healthy subjects. While the sensitivity of in-house ELISA has been found as 100%, the sensitivities of commercial kits number I and II have been found as 75% and 48%, respectively. The specificities of these three tests have been detected as 93.7%, 87.5% and 100%, respectively. The results obtained in this study suggest that in-house ELISA is of significant diagnostic value for the diagnosis of trichinellosis.

Negative-ELISA using native and filtrated cystic fluid antigens to rule out cystic echinococcosis.

An increasing number of cases of echinococcosis in Thailand have been imported, probably native infections and medical transfers. Serodiagnosis is one diagnostic choice for interpreting infections before a further step is done. Due to limited antigen, indirect ELISA has been used as a negative screening test for IgG-detection to rule out echinococcosis. Native hydatid cystic fluid (HCF) antigen from Belgium was used for such testing, in which the ODs-ELISA of samples were compared with those of two positive controls. Subsequently, hydatid cyst fluid from a Thai patient was obtained and the filtrated cyst fluid antigen [(<30)-(>10) kDa, HCF30.10] was prepared to develop negative screening results for the serum samples. By using HCF, three of twenty-four samples resulted in higher ODs-ELISA than the controls. In an attempt to observe the cross-reactivity of this native antigen, IgG-antibodies from many helminthiases cross-reacted and showed high ODs-ELISA. The HCF30.10 Ag was used to develop the test and analyze IgG-antibodies from 5 positive controls (2 parasite-confirmed and 3 positive-serodiagnosed), 183 heterologous cases of 29 diseases and 50 healthy control sera. At a cut-off value of 0.484, the test had 100% sensitivity and 42% specificity. Only Malayan filariasis, onchocercosis, fascioliasis, amebiasis, giardiasis and blastocystosis gave true negatives. Antibodies from nematodiases strongly cross-reacted with HCF30.10 Ag. Nine of fifty (18%) healthy serum controls produced higher OD-values than the cut-off. The routine ELISA uses the HCF30.10 Ag to produce a negative result to echinococcosis, because limited cystic fluid antigen (Thai patient) for test improvement, a lot of cross-reactions and only two protoscolex-positive controls are available.

Triclabendazole and its two main metabolites lack activity against Schistosoma mansoni in the mouse model.

Some have claimed that triclabendazole, a safe and efficacious drug for the treatment of fascioliasis, also exhibits antischistosomal properties, but results are conflicting. We assessed the effect of triclabendazole and its two main metabolites against two different strains of Schistosoma mansoni harbored in mice. Low worm burden reductions (18.6-35.9%) were observed in mice infected with an Egyptian strain of S. mansoni and treated with a single dose of 120 mg/kg 3 days before infection or single/double doses of 120-200 mg/kg 7 weeks after infection. Triclabendazole failed to significantly reduce hepatic and intestinal tissue egg loads, and eggs of all developmental stages were observed. Administration of 400 mg/kg of either triclabendazole, triclabendazole sulphone, or triclabendazole sulfphoxide to mice infected with a Liberian strain of S. mansoni resulted in worm burden reductions < 10%. In comparison, high worm burden reductions (82-100%) were observed in S. mansoni-infected mice treated with single oral doses of 400, 500, or 500 mg/kg twice a day praziquantel, regardless of the S. mansoni strain. We conclude that triclabendazole and its main metabolites display weak and inconsistent schistosomicidal activities.

[Use of the Western Blot assay in the diagnosis of trichinosis.].

Trichinosis is a worldwide parasitic disease acquired by the consumption of raw or uncooked meat containing encysted larva. The western blot (WB) technique has an important role in the serological diagnosis of trichinosis as a confirmatory test. In our study, IgG antibodies specific for Trichinella were found in the sera of 25 patients with trichinosis seropositivity by In house ES ELISA. Of 10 control individuals, 2 were found to be positive for toxocariasis, 2, positive for fascioliasis and 6 were found to be healthy by the WB test. In WB analysis, serum samples from the 25 persons suffering from trichinosis showed common profile bands with the band size ranging from 14 to 123 kDa. These serum samples showed the following important three antigenic bands with different sensitivities of 45 kDa (88.0%), 55 kDa (60%), and 88-90 kDa (64%). As a result of this study, these three antigenic bands in WB test are considered to be valuable in the diagnosis of human trichinosis.

[The external and internal morphology of the intermediate hosts of Fasciola hepatica].

The main morphological traits of Fossaria cubensis and Pseudosuccinea columella, the two intermediate hosts of fascioliasis in Cuba, were studied. Differences may be seen regarding some aspects of the external morphology, as well as in the internal morphology, mainly in the reproductive system. The radular teeth have differences which are rather intangible, not allowing for their taxonomic isolation on the basis of this criterion alone. These studies suggest to take into account several criteria in order to determine the taxonomic position of each species.

Food-borne parasitic zoonoses in the Philippines.

A number of food-borne parasitic zoonoses have been recorded in the Philippines and include echinostomiasis, artyfechinostomosis, fascioliasis, heterophydiasis, carneophallosis, clonorchiasis, paragonimiasis, taeniasis, echinococcosis/hydatidosis, diphyllobothriosis/spirometrosis and sparganosis, intestinal capillariasis, gnathostomiasis, angiostrongylosis, toxoplasmosis and sarcosporidiosis. Some are now rarely observed while others continue to be public health problems. Many are endemic in certain areas of the Philippines because of the habit of consuming raw or partly cooked fish, snails, crustaceans, and meat. Artyfechinostomosis caused by Artyfechinostomum malayanum is a recently recognized problem in the Philippines and is reported in man and pigs. Human infection results from ingesting raw or partly cooked the freshwater snail, Bullastra cumingiana which serves as second intermediate host. More information on the epidemiology, transmission including the animal hosts involved locally are still needed for some of these problems. Human infection with many of these diseases can be prevented by changing the food habits, but this requires aggressive health education campaigns.

Food-borne parasitic zoonoses in the People's Republic of China.

The People's Republic of China has a large number of animal parasites transmitted through the ingestion of food products. Approximately 20 species of parasites in animals and man are reported from most of the provinces and autonomous regions of the country. The major food-borne parasitic zoonoses are reported from pigs, cattle, fish, crabs and crayfish, snails, frogs, snakes, and aquatic plants. The most important diseases are toxoplasmosis, taeniasis, cysticercosis, sparganosis, clonorchiasis, fascioliasis, fasciolopsiasis, paragonomiasis, echinostomiasis, trichinosis, gnathostomiasis, and angiostrongyliasis. There is concern that some of these parasitoses may develop into more serious problems because of the increase in free-market commerce and the lack of adequate inspection of food products. Furthermore, many cultures in China enjoy eating raw or inadequately cooked food products. Control programs have been initiated utilizing newer drugs, but there must also be educational programs, as well as an improvement in sanitation.

Current status of food-borne parasitic zoonoses in West Germany.

The present status of food-borne parasitic zoonoses in West Germany is characterized by a relatively high frequency of toxoplasmosis, sarcocystosis and Taenia saginata infections. From empirical data it can be estimated that 4% of the German population become infected per year by Toxoplasma gondii as well as by Sarcocystis species (S. hominis, S. suihominis). The number of T. saginata infected persons is reckoned at 900,000, which is equal to an average prevalence of 1.5%. Due to meat inspection and to modern methods of breeding and keeping pigs trichinellosis and Taenia solium infections have become rare diseases. According to eating habits there are only sporadic cases of fascioliasis and anisakiasis. Whether Diphyllobothrium latum and Opisthorchis felineus are still endemic, is unknown; it seems that the influx of refugees from areas of high endemicity never created new foci in West Germany.

Diagnostic and interventional procedures for the biliary tract.

Various diagnostic imaging studies have been employed in the past year to evaluate the normal and abnormal biliary ductal system. Variations in the normal ductal drainage of the left lobe of the liver, in which the right lateral hepatic duct drained into the left hepatic duct, were studied because of the implications for the surgical resection of the left lobe. Choledochal cysts have been studied in adults using endoscopic retrograde cholangiopancreatography to evaluate the abnormal junction between the common bile duct and pancreatic duct and the long dilated common channel. An ultrasound study indicated that children with choledochal cysts also may have intrahepatic duct dilatation. An increased incidence of malignancy has been noted in adults with choledochal cysts, and their appearance on ultrasound, CT, and cholangiography were described. Cystic fibrosis produces abnormalities of the biliary tree, extrahepatic strictures, and more interestingly, intrahepatic ductal dilatation and abnormal contour without strictures. The usefulness of the preoperative ultrasonographic evaluation of the biliary tract in Oriental cholangiohepatitis was stressed. Bile duct abnormalities in fascioliasis were also noted on CT scans. Patterns of abnormality in the CT appearance of the thickened, contrast-enhanced, extrahepatic bile ducts were elucidated. Focal concentric, focal excentric, diffuse concentric, and diffuse excentric ducts were seen with various forms of pancreatic disease, choledocholithiasis, and various forms of cholangitis. An enhanced, thick-walled duct indicates disease, but is a nonspecific finding. Chronic cholecystitis was found to produce false-positive cholescintigram results in patients with suspected acute cholecystitis, but only in those with severe degrees of chronic cholecystitis. The results of multicenter trials using extracorporeal biliary lithotripsy in the United States have been published. They are somewhat disappointing and do not confirm the original excellent results reported in Germany for treatment of gallbladder stones. Much interest has been focused on the use of expandable metallic stents for the treatment of benign and malignant biliary obstruction. Preliminary data suggest good patency rates for benign lesions. Patency rates for malignant lesions are similar to those of previously available plastic stents.

Successful treatment of Taenia saginata and Hymenolepis nana by single oral dose of praziquantel.

Infections by adult cestodes are widely distributed in the world, and induced digestive and general disturbances. In this study, 40 patients with Taenia saginata and 15 with Hymenolepis nana were treated by one single oral dose of Praziquantel, at the dosage of 10 mg/kg/day and 20 mg/kg/day respectively. As side effects, no biological disturbances occurred, but 6 patients complained of abdominal pain and diarrhea. The efficiency was complete. Praziquantel, a wide spectrum anti-helminthic day drug, used in schistosomiasis, fascioliasis, cysticercosis, is the best treatment of adult cestodes.

[Comparison of the yield of indirect hemagglutination reaction and complement fixation reaction in the diagnosis of neurocysticercosis].

Two variations of an indirect hemagglutination test (IHAT) and a complement fixation test (CFT) for the diagnosis of human cysticercosis were compared and evaluated. For the IHAT, a cysticerci crude total saline extract (SE) and a cysticerci lyophylized and delipidized veronal bicarbonate saline buffer (VBS) extract were used, comparing their diagnosis yieldings with that of a CFT in 57 confirmed cysticercosis patients: 45 serum samples and 32 cerebrospinal fluid (CSF). Sera and CSF from 29 patients with other neurological diseases and 25 sera from healthy volunteers were also compared. Both types of methods presented an overall average concordance of 91.5% and 97.0% with CSF and sera respectively. With respect to the sensitivity observed with CFT was 85.2% and 93.3% for CSF and sera, whereas that of IHAT was 96.9% in CSF and 97.8% in sera, when SE antigen was used; with the VBS antigen for IHAT 96.9% and 95.6% were detected in CSF and sera respectively. In order to determine the specificity of the IHAT, besides the study in healthy volunteers, in patients with other neurological diseases and in 156 serum samples from individuals with other parasitoses, such as hydatidosis (43), trichinosis (56), fascioliasis (31) and Chagas' disease (26) were also tested. A high reactivity with the hydatidosis group was found. The specificity, using a titre > or = 1:16 as a diagnostic value and without considering hydatidic sera was 99.4% for RHAI (SE), 100.0% for RHAI (VBS). The use of IHAT and CFT in diagnosis of human cysticercosis is discussed.

[The indirect hemagglutination reaction in the diagnosis of trichinosis].

An indirect hemagglutination test (IHAT) for trichinosis using a Melcher's antigen was evaluated and compared with a precipitin test (PT) and a bentonite flocculation test (BFT). One hundred and forty eight serum samples from patients from the whole country confirmed or suspected to have trichinosis by clinical or epidemiological evidences were studied: 117 (79.1%) samples resulted positive by IHAT, 138 (93.2%) by PT and 65 (43.9%) by BFT. Sixty three serum samples from patients with strong clinical suspect of trichinosis presented the PT and the BFT positive and were compared with the IHAT for sensitivity study. IHAT was positive in 60 (95.2%) serum samples. In order to determine the specificity of IHAT 25 serum samples from healthy volunteers and 124 serum samples from individuals with other parasitoses, such as cysticercosis (48), hydatidosis (45) and fascioliasis (31) were studied. The specificity, using a titre > or = 1:16 as a possible diagnostic value was 96%. The use of IHAT with RP and BFT in the diagnosis of human trichinosis is discussed.

Immunocytochemical evaluation of Trichinella spiralis larval antigen.

The antigenicity of Trichinella spiralis muscle larvae was studied for the purpose of developing an immunodiagnostic test for trichinosis. Ultrathin sections of fixed larvae were treated with sera from patients with a variety of parasitic infections including trichinosis, anisakiasis, gnathostomiasis, fascioliasis, dirofilariasis, severe trichuriasis, moderate trichuriasis, paragonimiasis, cysticercosis cellulosae hominis and diphyllobothriasis, and then treated with protein A-tagged colloidal gold. The cuticle surface and stichocyte alpha 0- and alpha 1-granules reacted predominantly with sera from trichinosis and severe trichuriasis patients, and negligibly with the others. By contrast, the cuticle inner layer, hypodermis, and hemolymph reacted with all sera examined though there was some variability in the intensity of the staining. These results suggest that the cuticle surface and stichocyte alpha-granules possess a high degree of specificity for antibody-produced in T. spiralis infection, whereas the other larval components exhibit nonspecificity.

Serodiagnosis of human paragonimiasis caused by Paragonimus heterotremus.

Enzyme-linked immunosorbent assay (ELISA) and indirect hemagglutination tests (IHA) were evaluated for serodiagnosis of human paragonimiasis caused by Paragonimus heterotremus using homologous adult worm extract as antigen. IgG-ELISA was the most sensitive, being positive in all paragonimiasis sera tested while IHA and IgA-ELISA gave 88% and 59% positive rates respectively. Cross reactivity in IgG-ELISA was detected with fascioliasis sera, producing overall assay specificity of 97%. It is suggested that IgG-ELISA is a reliable serodiagnostic test for human paragonimiasis caused by Paragonimus heterotremus.

Geographical distribution and habitat preferences of the invader freshwater snail species Lymnaea columella (Mollusca: Gastropoda) in South Africa.

The present geographical distribution of Lymnaea columella, as recorded in the National Freshwater Snail Collection, is described and discussed. It appears that L. columella is the most successful colonist of all the freshwater snail species in South Africa, and, together with Bulinus tropicus and Lymnaea natalensis, it forms the most widely distributed freshwater snail species in the region. Data regarding the 2,341 discovery sites of L. columella recorded by collectors during surveys are analysed. L. columella cluster mainly in rivers or streams with perennial, slow flowing, or stagnant freshwater with many plants and a mud substrate. L. columella is regarded as a more effective intermediate host for the transmission of fascioliasis than the indigenous snail species. The economical implications of this are discussed briefly in the light of its success as an invader species.

[Population density of intermediary hosts of tropical diseases in watercress beds].

A study of the density of river mollusks in six watercress groves is made. It is seen how densities of F. cubensis, intermediate host of fascioliasis in Cuba, is maintained with relative stability. The poor effectiveness of P. duryi as an agent of biologic control is demonstrated. The high density of P. cubensis, which seems to exert a certain regulatory effect on F. cubensis--particularly in the months of reproductive peaks (August and September)--is emphasized. Densities of B. havanensis and B. helophila may be affected by the density of F. cubensis and P. cubensis, which may exert certain influence in the decrease of pulmonate density. F. cubensis and P. parvulus are morphometrically compared and a high correlation between length and width of the shell (r = 0.9) is emphasized; such correlation was smaller in P. parvulus (r = 0.7).
