Common problems in ophthalmic plastic surgery.

Many ophthalmic plastic surgical problems are ignored because the patient seems to be ""getting along." Frequently physicians are only vaguely aware of the constant daily discomfort their patients experience from seemingly minor problems such as endocrine exophthalmos, an ill-fitting prosthesis or baggy eyelids. As a result, too often the patient is unaware of the treatment possibilities that are available to relieve and often eliminate his problem. These conditions are often first seen by the family physician.

[Clinical and morphological bases of inferior exterior tarsopexy].

Involution processes caused by elongation of the lower eyelid are associated with loss of elastic fibers, destruction of collagen fibers, and fibrous degeneration of the distal fragment of the tarsal plate. The authors present a morphological validation of resection of the lower pedicle of the external ligament of the eyelid and shortening of the lateral part of the tarsal plate. Modified suture fixation to the orbital external wall periosteum and external retinaculum is proposed, ensuring adequate tension of the lower eyelid. High functional efficiency of the operation is analyzed on 66 clinical cases; the incidence of complications was low and good cosmetic results were attained (period of observation 3-52 months).

Reconstruction of the orbit following trauma.

An overview of the approach to the surgical management of orbital trauma used within the Department of Oral and Maxillofacial Surgery at The University of Washington is presented. In particular this paper discusses our experiences with the use of transconjunctival incisions, a modified technique for the harvesting of cranial bone grafts, and the use of intra-operative C-T scanning. Also presented are the results of two studies performed on aspects of the management of orbital trauma within the Department.

Total eversion of the upper eyelids in a newborn.

A case of bilateral congenital eversion of the lids in a newborn is presented. The baby presented at the Eye Clinic of Otibhor Okhae Teaching Hospital about four hours after a spontaneous vaginal delivery. Surgical correction was carried out one eye at a time with absorbable sutures. Recovery was uneventful and normal anatomical configuration was regained 8-10 days post operatively. The aim of this case report is to create awareness among workers in maternity homes, primary health centres and other health institutions about the existence of this congenital anomaly. Prompt referral to the Ophthalmologist is advised, as the condition is very amenable to treatment.

Complications of sinusitis in Ibadan, Nigeria.

Nowadays the complications arising from sinusitis are rare since the introduction of antibiotics. However sinusitis and its complications are still life-threatening and if neglected may result in high morbidity and mortality. It is the purpose of this study to draw attention to the effects of the spread of infection beyond the sinus wall and thus to influence the clinician in his management in order to prevent the subsequent spread of the disease, and secondly make him aware of the sinogenic origin of these complications when he sees them. A retrospective study of 90 patients with sinusitis seen over a ten year period between 1988 and 1997 in the ENT Department, University College Hospital, Madan was (lone and those that had complications were further analysed to meet the objectives of this study. There were 90 patients seen with sinusitis over the ten years period, out of which 33 (37%) cases had complications, 17 males and 16 females (M.F.1:1). The average age of cases with complications was 28 years (age range 2.5 - 75 years). 66.6% of the complications cases occurred in age group under 30 years of age. Thus children and young adults were mostly affected. Twenty-six cases (79%) of chronic sinusitis developed complications while in acute sinusitis there were 7 cases (21%). The site affected most commonly by complications was the orbit (41%) followed by Sinus wall (32%), orophyarynx (18%), intracranial (5%); no car complications were seen. The type of complications seen at various sites and their incidence were also highlighted. Thus with this study it is hoped that clinicians would be reminded that complications of sinusitis still occur and hence be aware of the necessity to recognize and prevent them as rapidly as possible to avoid serious consequences.

[Elastic fibers of the cervical stroma in precancer and cancer].

Along with the description of the architectonics of elastic fibres in normal cervicovaginal tissues the kinetics of their changes was followed in precancerous conditions and cancer of the cervix. It was found that during the process of growth and development of precancerous lesions and cancer elastic fibres would undergo a number of gradually increasing changes from surface, reversible to profound, irreversible with their ultimate total destruction.

Comparison of transconjunctival versus subciliary approaches for orbital fractures: a review of 60 cases.

The increasing emphasis on open reduction in the management of orbital fractures has led to an extensive debate as to which approach provides adequate exposure for these fractures. This retrospective study compares the exposure provided and the rate of complications between transconjunctival and subciliary incisions for orbital rim and floor fractures. The charts of 55 patients with orbital fractures, treated with open reduction and internal fixation, were reviewed. A total of 30 subciliary and 30 transconjunctival incisions had been performed, and the adequacy of exposure as well as intraoperative and postoperative complication rates were compared. The authors found a higher rate of complications with the subciliary approach and, therefore, advocate the use of a transconjunctival incision for the management of orbital fractures.

[Novel contribution of dynamic telethermography in the diagnosis of dysplasia and cancer of the uterine cervix (author's transl)].

At the present time when maximum information, obtained without practising an incision, on anomalies of the cervix take on the greater importance, thermivision provides novel data. Hot dysplasia was revealed to be true early cancer, cold dystrophy was near to hot subclinical cancer and "screened" in a way and picked up by routine thermovisual investigations. An elevated thermogenesis, even of a colposcopically benign lesion should maintain clinical alertness. Disagreement between the results of colposcopy, thermovision and cervico-uterine cytology, instead of leading to the rejection of these methods, must on the contrary make one look for the reasons for these divergences. D.T.T. in introducing new data, undoubtedly provides an element of uterine cervical physiology. Faced with an evident early cancer, study of the thermal emission enables one to precisely show the degree of local extension of this cancer and sometines its multifocal nature. It is an event, an approach to the physiology of malignancy. Furthermore it is remarkable to note the cooling after ionizing irradiation, rapid and complete cooling. D.T.T. is therefore called upon to take its place which is even greater as new non-mutilating therapeutic methods such as cryosurgery for example develop.

[Reverse flow axial island flap in the mastoid region].

Anatomical and clinical studies on the reverse flow axial island flap in the mastoid region were reported. The studies in the dissection of 9 cadavers with a total of 17 sides and the observations in 6 operations proved that the blood in the superfacial temporal artery could flow reversely into the postauricular artery through the anastomotic branches to nourish the mastoid island flap. So the island flap wit a fascia pedicle containing the superficial temporal vessels could be transferred to various region of the face through subcutaneous tunnel to repair deformities of the orbit, nose, eyes and cheek in one stage operation. This reverse axial island flap has been utilized in 18 cases. 16 flaps survived. All cases were followed up for 2 to 7 years. The results were satisfactory.

[Apert's syndrome: a case report].

Apert's syndrome is a type of acrocephalosyndactylia that is from part of the great group of craniofacial synostoses. It is characterized by craniofacial dysmorphia and syndactylia on hands and feet, which differentiates it from Crouzon's disease. It is a rare affection that is often transmitted through an autosome dominant mode, but sporadic cases exist. We report the case of a 15-year-old girl who presented characteristic clinical signs of Apert's syndrome with normal karyotype without parental consanguinity. The Ser 252 Trp mutation of the FGFR2 gene was found, confirming the molecular diagnosis. This study illustrates the severity of ocular and neurological problems of untreated Apert's syndrome. The presence of hemoglobinopathy (Hb AS) is also a mark of its originality.
