Bioadhesive oesophageal bandages: protection against acid and pepsin injury.

The rate of acid and pepsin diffusion through solutions of sodium alginate was measured using in vitro techniques. Previous work has demonstrated that solutions of alginate may adhere to the oesophagus for up to 60 min; this work measured their ability to protect the oesophageal epithelial surface from damage caused by refluxed acid and pepsin. Franz diffusion cells were used to measure the rate of acid and pepsin diffusion through an alginate layer. The effect of the type of alginate, alginate concentration and depth of alginate applied were investigated. The rate of both acid and pepsin diffusion was significantly reduced (ANOVA analysis; P<0.05) in the presence of an alginate solution compared to the control. A 2% (w/v) alginate solution with a high guluronic acid component, in a layer of 0.44 mm depth, demonstrated the greatest reduction in acid diffusion with a permeation coefficient 14% than that of a control value. All three alginates demonstrated significant reductions in acid diffusion with both increasing depth and increasing concentration, as expected. Pepsin diffusion was also significantly reduced as the depth and concentration of applied alginate increased. This study demonstrates that an adhesive layer of alginate present within the oesophagus will limit the contact of refluxed acid and pepsin with the epithelial surface.

Functional fracture bracing in metacarpal fractures: the Galveston metacarpal brace versus a plaster-of-Paris bandage in a prospective study.

A total of 133 patients with fractures of the second through the fifth metacarpal bones were randomized to receive either a functional brace (the Galveston metacarpal brace) or a dorsal/ulnar plaster cast. Only 42% of the patients in the metacarpal-brace group completed the treatment, in contrast to 81% of the patients in the plaster-cast group. Complications with the brace were due to 60% of the exclusion. No difference according to gender, age, fracture type, hand affected (right/left), or mechanism of injury was observed between the patients who completed the treatment and those who were excluded. Reduction of fractures could not be demonstrated. Reduction of mobility was more frequent in the plaster-cast group, but three months postinjury no reduction of mobility was observed in either group.

[Preventive stabilization of the knee joint by a bandage (brace) in an ice hockey player. Sense or nonsense?].

Braces for the knee are used by more and more First League Players to prevent injury to the knee, whether or not the knee has been injured previously. One player of the National Team wearing braces since last season complained of restricted efficiency. Electromyographical examination of his thigh muscles showed severe neurogenic changes. We concluded to refuse that so-called preventive braces.

[Experience with Baycast plastic bandages and their possibilities].

A new method of manufacturing temporary orthoses is described. Immobilization--as much as necessary and as little as possible--is attainable with these orthoses, which reduce damage by immobilization and simplify rehabilitation. Recommendation to the manufacture regarding the characteristics of the material and accessory aids are presented.

Bandaging a "broken heart": hypnoplay therapy in the treatment of multiple personality disorder.

Hypnoplay therapy is beneficial in two stages of the treatment of multiple personality disorder, as illustrated in two cases. First, when applied to child alters, this method provides an alternative route to the disclosure of secrets, the nonverbal expression of thoughts and feelings, and the safe abreaction of powerful emotion. Second, during postfusion, hypnoplay therapy can help the patient to establish a reliable self-object and to complete developmental periods that were absent or severely curtailed.
