Walking versus breathing: mechanical differentiation of sea urchin podia corresponds to functional specialization.

The podia of sea urchins function in locomotion, adhesion, feeding, and respiration; but different podia on a single urchin are often specialized to one or more of these tasks. We examined the morphology and material properties of podia of the green sea urchin, Strongylocentrotus droebachiensis, to determine whether, despite apparent similarities, they achieve functional specialization along the oral-aboral axis through the differentiation of distinct mechanical properties. We found that oral podia, which are used primarily for locomotion and adhesion, are stronger and thicker than aboral podia, which are used primarily for capturing drift material and as a respiratory surface. The functional role of ambital podia is more ambiguous; however, they are longer and are extended at a lower strain rate than other podial types. They are also stronger and stiffer than aboral podia. In addition, all podia become stronger and stiffer when extended at faster strain rates, in some cases by nearly an order of magnitude for an order of magnitude change in strain rate. This strain-rate dependence implies that resistance to rapid loading such as that imposed by waves is high compared to resistance to slower, self-imposed loads. Thus, the serially arranged podia of S. droebachiensis are functionally specialized along an oral-aboral axis by differences in their morphology and mechanical properties.

The need for special foods and sugar substitutes by individuals with diabetes mellitus.

The need for special dietary products marketed for use by individuals with diabetes mellitus and the safety and efficacy of certain nutritive sweetener substitutes for sucrose are reviewed. Special foods for individuals with diabetes mellitus are not necessary to achieve the dietary objectives recommended by leading United States and European authorities. They can be achieved conveniently and at minimum expense through enlightened choices of commonly available food items. At present, specific and unique characteristics of food products with special therapeutic properties for diets of diabetic individuals cannot be delineated or defined on rational nutritional grounds. Such terms as "diet", "dietetic", and "diabetic" on food labels have no uniform meaning for consumers, and diabetologists have observed that patients tend to consume such foods without regard to their energy content. Some consumers regard the reduced-calorie and low-calorie prepared food products as convenient in diets for weight reduction and diabetes although their use in dietary management of diabetes has no therapeutic basis other than weight reduction and maintenance. When fed as pure substances to fasted subjects, the nonglucose carbohydrate nutritive sweeteners, fructose, xylitol, and sorbitol, are absorbed relatively slowly and produce less postprandial hyperglycemia and insulin response than sucrose or glucose. Adequate studies of their long-term effectiveness when ingested as part of mixed meals have not been conducted. Although these sucrose substitutes are generally considered safe, the significance of recent information on possible carcinogenicity of oral xylitol in long-term feeding studies has not been fully evaluated. In view of the lack of certain essential information on the long-term effectiveness of various diets in preventing or mitigating the chronic debilitating complication of diabetes, suggestions for future research are included.

Health performance of feeder calves sold at conventional auctions versus special auctions of vaccinated or conditioned calves in Ontario.

OBJECTIVE: To compare health performance during the first 28 days in the feedlot for vaccinated or conditioned feeder calves sold through special auctions in Ontario with health performance for calves sold through conventional auctions in the province. DESIGN: Cohort study. ANIMALS: 12,313 calves sold through conventional and special auctions at the Keady Livestock Market during the fall of 1999 and 2000. PROCEDURE: Buyers of calf groups were approached at the auction market or contacted by telephone and asked to record the number of calves requiring treatment for bovine respiratory tract disease (BRD) during the first 28 days after purchase. RESULTS: 211 calf groups (> or = 20 calves/group) were followed up for 28 days after purchase. Multivariate logistic analysis indicated that vaccinated calves purchased through special auctions were 0.68 (95% confidence interval, 0.50 to 0.93) times as likely to receive treatment for BRD as were calves purchased at conventional auctions and that conditioned calves were 0.22 (95% confidence interval, 0.12 to 0.38) times as likely to receive treatment. Groups that received antimicrobials by injection on arrival at the feedlot were 0.64 (95% confidence interval, 0.43 to 0.96) times as likely to be treated as were groups that did not. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that vaccinated and conditioned calves were less likely to receive treatment for BRD during the first 28 days in the feedlot; however, there was no difference in mortality rate.

Grip strength of Texas Special Olympians.

Little is known about the physical fitness parameters of individuals who participate in the Special Olympics. The purpose of this study was to assess the grip strength of 104 Special Olympians participating in the winter Olympic Games. Men were significantly stronger than women across age groups 20 to 59 yr. Women's right hands were stronger than their left, while the opposite was true for men, despite the fact that 78.8% of the subjects reported right hand dominance. Mean right and left grip strengths were 25-40% less than the 1985 normative data of Mathiowetz, et al. by age groups. Special Olympian men and women have grip strengths comparable to elderly people rather than age-matched men and women.

Hemispheric specialization in normally and slowly developing children: a tachistoscopic and dichaptic evaluation.

The purpose of the study was a description of hemispheric specialization characteristics of normally developing right- and left-handed children and determination of what differences in such characteristics, if any, existed between young normally developing and older slowly developing children. With dichaptic and tachistoscopic methods, hemispheric specialization characteristics of 15 slowly developing children (5-0 to 9-6 yr.) and 25 normally developing children (6-0 to 6-11 yr.) were assessed. Latencies and the number of correct responses were analyzed. Both right- and left-handed normally developing 6-yr.-olds showed considerable evidence of bilateralization of hemispheric functions for spatial and verbal information processing; slowly developing children exhibited unusual patterns of hemispheric specialization usually opposite those typically expected in children or adults. Response latency measures of performances of slowly developing children suggested a bilateralization of hemispheric function for processing of verbal and spatial information; number of correct responses indicated that lateralization of such functions was present.

Hemispheric specialization for tactile-spatial processing in preschool children.

Witelson (1976) found that boys but not girls showed right-hemisphere specialization for tactile-spatial processing as early as 6 yr. of age. In the present study, Witelson's task was administered to 46 normal 4- and 5-yr.-old girls and boys. Both sexes showed right-hemisphere specialization. There were no sex differences either in specialization or in over-all performance.

Regulation of emotional security by children after entry to special and regular kindergarten classes.

In this paper early adaptation after children's entry to kindergarten is conceptualized as a process of achieving emotional security. It was hypothesized that children adapt to school by means of security seeking from the teacher and behavioral inhibition. 30 normal children from regular classes and 36 children with a variety of problems, e.g., behavioral, emotional, and family problems, from special classes were rated by their teachers on the Inhibition Scale and Security Seeking Scale on 5 occasions during the first 3 mo. at school. By the end of this period teachers judged the intensity of behavior problems on the Internalizing Problem Scale and the Externalizing Problem Scale. Analysis showed that initial high scores on the Security Seeking Scale and Inhibition Scale decreased sharply during the first weeks, and that children from special classes scored consistently higher on the Security Seeking Scale and more variable on the Inhibition Scale than children from regular classes. Girls had higher scores than boys on both scales. Recent stress in the family as rated by the teacher was positively related to both scores on the Inhibition and Security Seeking Scales after entry. Finally, scores on the Security Seeking and Inhibition Scales over the first three months predicted scores on the Internalizing Problem Scale by the end of this period, especially for children in special classes. We conclude that understanding adaptation after school entry as a process of obtaining emotional security is productive, providing a means to link entry behavior to precursors and consequences.

Morphometric comparison of human nerve cells: special sensory system.

We compared morphometric data on various nerve cells in the human special sensory system using the modified Klüver-Barrera staining method with an extremely minimized shrinkage ratio and an image-analyzer. According to the measurement data of cell-body sizes, we classified nerve cells of the various nerve nuclei in the special sensory system into three groups. These data are of interest to better understand the process of nerve conduction in the special sensory system.

Choice of handedness measures in studies of hemispheric specialization.

Left-and right-handers have been reported to differ in lateral hemispheric specialization for cognition. We compared different methods of dividing subjects into handedness groups: preference(a 12-item questionnaire), performance(speed, strength, dexterity), and preference plus performance, and determined which method of handedness classification indicated the greatest group differences on EEG and dichotic measures of lateral specialization. All handedness measures were significantly intercorrelated. These handedness measures were significantly correlated with the dichotic test over the whole population but were significantly correlated with EEG asymmetry only in females. Scoring degree of handedness showed significant relationships to lateral specialization which were not seen when subjects were classified into discrete handedness groups. Ambidextrous subjects performed as well as right- or left-handers on unimanual tasks despite a lack of hand preference. The hand used for writing was shown to be too limited to be used as the sole index of handedness in studies related to cognition.

Building community-based service systems for children with special needs: the Michigan Locally Based Services program.

The philosophy of the Michigan Department of Public Health, Division of Children's Special Health Care Services (DCSHCS) program holds that children with special health care needs have a right to every opportunity for self-support and self-fulfillment. To assist families, the program historically has provided support with medical care and treatment payments. Likewise, local health departments have shared in the process of supporting families of children with special health care needs. A growing awareness that these families experience a variety of other needs not directly attended to by these mechanisms led the Division to respond further to those needs. The response was aimed toward increasing the capacity to locate clients and provide family assessments, and, where necessary, case management services at the local community level. The implementation, on-going program development, stabilization, and future trends of the Locally Based Services program are described from the varying perspectives of the state and a local health department.

Parent/professional partnerships in caring for children with special health care needs.

In order for children with special health care needs to be adequately cared for, a variety of integrated, coordinated services are needed to support the children and their families. To facilitate optimal coordination of care for these children, it is imperative that the professionals who serve them engage in a partnership of care with parents or other family members. To that end, the Division of Children's Special Health Care Services (DCSHCS), Michigan Department of Public Health has carried out a variety of efforts to foster such partnerships. This article describes the efforts that have been carried out over the last few years in Michigan to develop parent/professional partnerships. It is not intended to be a scholarly review of the literature, but rather an explanation of strategies that have been used successfully to develop this important interface. Particular emphasis is given to describing the establishment of partnerships between parents of children with special health care needs and nurses and physicians. It is hoped that the sharing of such information will assist others who are just beginning such efforts.

Tympanometry and otoacoustic emissions in a cohort of special care neonates.

Otoacoustic emission (OAE) screening and oto-admittance testing (678 Hz probe tone) were performed on both ears of 84 special care neonates, as part of a larger study of middle-ear effusion in neonates and infants. OAE results, tympanometry, and acoustic reflex results are all strongly and significantly associated. No evidence was found of any maturational effects in the results. Based on the findings, a tentative classification scheme for neonatal tympanograms is suggested. We conclude that 678 Hz tympanometry is a useful indicator of middle-ear status in very young babies, and that middle-ear effusion does strongly affect OAEs in neonates. OAEs are also strongly affected by negative middle-ear pressure (MEP), and mean MEP in ears failing OAE screens was significantly more negative than in those passing. The prevalence of abnormal tympanometry, which may indicate middle-ear effusion or dysfunction, was 20% of ears (29% of babies) in this group. It appears that middle-ear effusion could account for about half of the ears failing an OAE screen on the special care baby unit. We also find that length of stay on the special care baby unit is an important risk factor for development of middle-ear effusion: those on the unit for over 30 days have about four times the risk of bilateral abnormal tympanometry.

Development of functional specializations within the maturing rabbit flexor digitorum profundus tendon.

Along its length, the rabbit flexor digitorum profundus (FDP) tendon exhibits two functionally specialized regions: classical tendon (CT) and fibrocartilage (FC). We examined their development in rabbits, ranging in age from newborn to nine-months postnatal, using a combination of light microscopic, immunohistochemical and biochemical techniques. There is little histodifferentiation in newborn tendon. Both regions are composed of thin collagenous fibers, numerous fibroblast-like cells and a low molecular weight dermatan-sulfate proteoglycan. Regional specialization has begun by two-weeks postnatal and by three-months postnatal, FC regions have been transformed into a true fibrocartilage characterized by a complex collagenous and elastic fiber network, numerous chondrocytes and a matrix rich in a high molecular weight predominantly chondroitin-sulfate proteoglycan and type II collagen. These features are elaborated between three and nine-months postnatal. CT regions undergo little substantial change during growth and maturation. The rabbit is born altricial and incapable of adult patterns of locomotion. We propose that the developmental expression of functional specializations within the FDP tendon is closely linked with the onset of different physical demands arising from the adoption of adult patterns of locomotion.

Designing special switches and control systems for multiply handicapped young people--a problem-led approach.

This paper discusses a design strategy for assistive equipment for disabled people, in particular for special switches and control systems to enable the severely physically handicapped to control powered wheelchairs, computers, communication aids and environmental control systems. Special switch systems in this application must provide a complex human-equipment interface. Experience over a number of years with over 80 severely handicapped young people has refined the design strategy for both special switch systems and interfacing. Some examples are given of a problem-led approach, and the materials used and the method of manufacture are discussed.

[Changes of activities of daily living and quality of life in a special nursing home during a one-year period].

The aim of this study was to examine the changes of activities of daily living and with quality of life of patients in a special nursing home founded in 1997 in Tsuwano Town, Shimane. We studied 19 special nursing home patients (age 69 to 96 years, mean age 83.3 years) for one year. To evaluate activities of daily living, mental decline, and quality of life, the Tokyo Metropolitan Institute of Gerontology Competence Index. Hasegawa's dementia rating scale, self-rating depression scale (Zung), apathy scale (Starkstein et al) and PGC morale scale (Lawton, subjective well-being scale) were used. The total score of the index of activities of daily living, self-rating depression scale, and the apathy score did not significantly change during one year. Although the total score of Hasegawa's dementia rating scale decreased significantly, PGC morale scale significantly improved during one year. These findings demonstrate that daily active rehabilitation had a beneficial effect on PGC morale scale in a special nursing home.

Improvement of sensory quality of lowfat kefalograviera-type cheese by using commercial special starter cultures.

Two commercially available special starter culture systems, Alp DIP and a mixture of Alp DIP D and Joghurt V1, were compared with one commercial regular starter culture, CH-1, for their effects on the compositional, sensory and textural characteristics of lowfat (9.5%) high moisture (49.6%) Kefalograviera-type cheese during aging. A full-fat control Kefalograviera cheese (30.8% fat, 37.8% moisture) was also made with the regular starter culture. The results indicated that the type of starter did not affect the composition (moisture, fat, protein, salt and pH) of the lowfat cheese. Sensory analysis showed that the lowfat cheeses made with the special cultures received greater body and texture scores and significantly higher flavor scores than the lowfat control cheese after aging for 90 and 180 d. Moreover, the former cheeses received body and texture and flavor scores not significantly different from those of the full-fat cheese. Texture profile analysis by Instron showed that there were no significant differences in the textural characteristics (force and compression to fracture, cohesiveness, springiness, gumminess and chewiness) between lowfat cheeses made with the special cultures and that made with the regular starter, except for hardness which was significantly lower in the former cheeses.

Blood, growth, and other characteristics of special-fed veal calves in private cooperator herds.

The objectives of this study were to determine the mean values for mortality, blood, and growth traits and the correlations among blood and growth traits on privately owned farms producing special-fed veal. Mortality percentages were available on 13 groups of special-fed veal calves (n = 1403) from four farms; growth and some blood traits were available on 9 groups from four farms (n = 975). Mean mortality percentages for periods 0 to 2, 2 to 7, and 7 to 16 wk, and over 16 wk were 1.0, 1.0, 1.0, and 3.1%, respectively. Means for average daily gain for 2 to 7 wk were 1.37 +/- .008 kg and for 7 to 16 wk were 1.52 +/- .001 kg. When calves arrived at the veal farm, total plasma protein averaged 6.2 +/- .05 g/dl. Examination of total plasma protein upon arrival of calves indicated that 81% of the calves had received colostrum. The base level indicating colostral deficiency was < 5.5 g/dl of total plasma protein; 51% of the calves had total plasma protein > 6.0 g/dl at arrival. Mean hemoglobin concentrations at arrival and at 2, 7, and 16 wk were 11.1 +/- .15, 10.6 +/- .18, 9.2 +/- .14, and 7.8 +/- .13 g/dl, respectively. Means for hemoglobin were generally higher than previously reported for special-fed veal calves reared in individual stalls. Hemoglobin and red blood cell counts were correlated positively with 16-wk average daily gain and body weight, and midtrial white blood cell counts were correlated negatively with average daily gain and body weight from 2 to 7 wk; there were no other significant relationships between blood traits and growth performance measures.

William Osler and "the special field of neurological surgery".

Harvey Cushing's paper, "The special field of neurological surgery," published in the Bulletin of The Johns Hopkins Hospital in 1905, constitutes a recognized milestone in the establishment of neurological surgery as a separate surgical specialty in the United States. The main point the author wishes to make here is that the very special friendship of Sir William Osler, influencing, encouraging, stimulating Cushing at the particular time that it did (1901 to 1905), was probably the primary positive influence that made it possible for Cushing to achieve specialization in neurological surgery and to make his considerable contribution in this field.
