Prevention of child sexual abuse.

Child sexual abuse prevention programs are a growing phenomenon addressing an expanding number of topics raised by a variety of violent and perverse acts. Currently programs tend to focus on a fairly narrow age group and use an educational model for children. There is a need for broad-based programs with a strong focus on the adults around children. Perinatal prevention programs require an adult focus and must take advantage of a special time in child and family development to address lesson of health and intimacy in a way that may decrease the incidence of future child sexual abuse.

Lust, lack of desire, and paraphilias: some thoughts and possible connections.

How one develops specific sexual interests (desires, eroticisms, orientation, lust, preferences, etc.) is a basic and essentially unanswered question in sexology. The present paper explores a theoretical relationship between the the development of specific sexual interests and those individuals complaining of lack of desire. The ability of traditional sex therapy to intercede successfully in these problems is discussed.

[Pedophilic-voyeuristic behavior].

It is reported on a 26 year-old man with a paedophilic-voyeuristic behavior in the frame of a sexual abnormal development on the basis of a primary sexual pathological personality structure (Szewczyk). The maldevelopment has be expressed monosymptomaticly in the sphere of sexuality.

Enhancement of self-esteem among female adolescent incest victims: a controlled comparison.

A therapeutic intervention consisting of homogeneous group therapy and sexual education with same-sex therapist/clients was instituted with 15 female incest victims, compared to a matched control group; all the subjects had experienced intrafamilial incest and were court-placed in a Southern California resident facility. A series of behavioral markers and changes in self-esteem are reported by use of pre- and posttests of the Coopersmith Self-Esteem Inventory, behavioral observations by resident professionals, and checklists/daily behavior logs. The latter were evaluated for signs of further victimization of both groups. Findings show that female incest victims in the experimental group showed a significant increase in positive self-esteem as measured by the inventory, and developed a significantly increased knowledge of human sexuality, birth control, and venereal disease when compared to the control group.

The emergence of lesbian sexuality and identity cross-culturally: Brazil, Peru, the Philippines, and the United States.

Questionnaire data were collected from 205 lesbians and 224 heterosexual women in Brazil, Peru, the Philippines, and the United States between 1981 and 1988. Items include direction of childhood sexual interests, age of first awareness of sexual attraction, cross-attractions, age of first sexual contact, sex of partner in first sexual contact, age of awareness that sexual attractions were serious, and age of realization of adult sexual orientation. In general, similarities in the development of lesbian sexuality and differences between lesbians and heterosexual women were found, regardless of culture. Findings include: Lesbians were more sexually active as children and displayed more sexual interest in girls than heterosexuals did in boys; lesbians tended to become aware of their attractions to women later than heterosexuals realized attractions to men; lesbians reported considerably more early attraction to men than heterosexuals did to women; lesbians reported earlier sexual contact than did heterosexual women; the sex of the partner of first sexual contact for lesbians was split roughly equally between males and females whereas heterosexuals reported contact with males almost exclusively; childhood awareness that sexual attractions were serious were similar in lesbians and heterosexuals; lesbians defined themselves as homosexual later than heterosexuals defined themselves as heterosexual. We conclude that norms for heterosexual behavior impact the development of lesbian sexuality and identity, impeding, but not obliterating, their development. Lesbian sexuality and identity eventually emerge regardless of culture.

Childhood sexual histories of women with somatization disorder.

The author interviewed 60 women with somatization disorder and 31 women with primary affective disorders who were matched for race, age at interview, and level of education to obtain their childhood sexual development histories. The two groups of women reported similar sexual experiences at similar ages, except that significantly more women with somatization disorder had been molested as children.

Sexuality as a treatment issue with a special population of adolescents.

The handicapped adolescent population heretofore has not been properly identified or serviced. These adolescents have to deal with body image and sexual identity problems. Working with the adolescent individually, in groups, and in a therapeutic community setting, we have developed methods of dealing with the issue of sexuality.

More than playing house: a developmental perspective on the strengths in teenage motherhood.

Historically, the literature on adolescent pregnancy and motherhood has focused primarily on negative outcome. A review of current research reveals that outcomes for many teenage mothers and their children are not necessarily bleak. Indeed, becoming a parent can stimulate positive change in the adolescent's life. It is suggested that developmental level interacts with psychological, sociocultural, economic, family, and health variables to create a context in which teenage pregnancy can be an inhibitor or an enhancer of maturation.

Common psychological defenses seen in the treatment of sexually abused adolescents.

The negative psychological aftermath of sexual abuse is complex and has a direct correlation with the establishment of maladaptive defense mechanisms. Long-term psychotherapy can allow the adolescent patient to establish the cognitive and affective connections to the original trauma and work through the residue of guilt and shame. As mastery is achieved through the treatment process, higher level defenses should become more pronounced. Because the repressed memories related to the sexual trauma need to be unfolded slowly, so that the defensive capacities are not overwhelmed, treatment may need to be continued into adulthood. This paper proposes a theoretical discussion to assess the psychological defenses commonly seen in adolescents with a history of sexual abuse. Further research is needed on the sexually abused patient's intrapsychic experience and the development of defenses used to ward off painful memories related to the sexual trauma.

Narcissism and superego development.

To elucidate the role of the superego in th maintenance of narcissistic equilibrium, we reviewed Freud's ideas about narcissism and the superego as well as the relevant theories of Kohut, Kernberg, and certain ego psychologists. These latter authors offer an alternative mode of understanding narcissism more consistent with Freud's structural theory, one in which signal affects and superego functioning play a central role in normal development and in the pathogenesis of narcissistic disturbances. Early steps in superego formation were then examined schematically to elucidate the interaction of environmental influences and emerging psychic structure. We suggested that the first step in a developmental line toward superego formation is based on the affective qualities experienced in the course of self-object differentiation. Subsequent steps examined were introject and ego-ideal formation; compliance with th object; compliance with the introject; identification with the introject and the ego ideal; and finally, with oedipal resolution, the integration of superego nuclei into a progressively structuralized autonomous superego system. This system achieves growing independence from the drives and from pressures from early introjects during the course of latency, and functions to maintain the demands of the conscience and the standards of the ego ideal; rewards or punishments result when these demands and standards are or are not met. The final stage briefly considered here was the revision, modification, and elaboration of moral codes and the ego ideal as part of the adolescent process. Narcissistic vulnerabilities at various stages were pointed out in an attempt to stress that a particular clinical picture in later phases of development or adulthood may derive from any of several development points of origin and from one or more etiological factors.

Theme-focused group therapy on a pediatric ward.

Hospitalization for children may be a traumatic experience which can result in varying degrees of depression and anxiety. To help children deal with their negative reactions to illness and hospitalization the departments of psychiatry and pediatrics at Beth Israel Medical Center in New York introduced a program of theme-focused group sessions for pediatric patients. The group concerned itself with issues of the hospital experience including 1) lack of, or distortion of, factual information, and 2) fantasies, fears and anxieties related to hospitalization. The group also served as a diagnostic tool by allowing the observation of children's reactions to their hospitalization; as an educational experience for non-psychiatric hospital personnel to learn about children's development and psychological functioning; and to reduce the anxiety level of the children, lessen hyperactivity, aggressiveness, and resistance to hospital procedures.

[Psychoanalysis of feminism between ideology and science].

Separation from one's primary maternal love object is a crucial prerequisite for the acquisition of female sexual identity, for becoming a productive person and for loving. Using case vignettes, this paper attempts to show that the experience of separation can effectively be developed in the therapy process by means of six forms of experiencing active and passive abandonment.
