Group treatment of secondary erectile dysfunction.

The effects of three group treatment formats on 20 men with secondary erectile dysfunction and their partners were contrasted. After a comprehensive medical and psychological screening, each couple was assigned to one of three treatment groups (Communication Technique Training, Sexual Technique Training, Combination Treatment) or to one of two control groups (Attention-Placebo, No-Treatment). Couples in the three treatment groups and the attention-placebo group participated in their respective formats in twice-weekly sessions for a total of 20 hours. The no-treatment control group received sex education and treatment after a 5-week waiting list period. All three treatment groups fostered substantial gains so that between-format differences were not statistically significant. Subject variables which predicted success/experience ratio gains included age of the male partner, perceived level of relationship adjustment, and the male partner's success/experience ratio prior to treatment. Eighty-one percent of the treated men reached the criterion of 80% or greater success/experience ratio (successful penetration and subsequent ejaculation) at the 6-month follow-up. Good nocturnal tumescence prior to treatment was correlated with a better treatment outcome than poor tumescence.

Expression of chimeric connexins reveals new properties of the formation and gating behavior of gap junction channels.

Direct intercellular communication occurs through specialized channels, which are formed by the interaction of two half-channels, or connexons, contributed by each of the two participating cells. The ability to establish intercellular communication is specified, in part, by the expression of different structural proteins, termed connexins. Connexins can control the establishment of intercellular communication by selectively pairing with some but not other family members. To characterize the protein domains that allow connexins to recognize and discriminate between alternative partners, we have created chimeras composed of selected regions of rat connexin43, which forms channels with Xenopus connexin38, and rat connexin32, which cannot. Pairs of Xenopus oocytes were used to test the ability of the chimeras to form homotypic channels with themselves, and heterotypic channels with the parent connexins or with endogenous Xenopus connexin38. While all hybrid molecules tested were efficiently expressed by oocytes, most were devoid of functional activity. A chimera consisting of connexin32 from the N terminus to the second transmembrane domain, fused to connexin43 from the middle cytoplasmic loop to the C terminus, designated as 3243H4, was able to pair functionally with Xenopus connexin38 and one of its parent connexins, connexin43. Voltage-dependent closure of heterotypic channels containing 3243H4 was asymmetric, exhibited novel characteristics that were not predicted by the behavior of the parent connexins and was dependent on the type of connexin with which 3243H4 was paired. In contrast, 3243H4 was unable to form functional channels with either itself or the other parent, connexin32. Together, these results suggest that these connexins are not composed of functionally exchangeable regions and that multiple domains, namely the middle cytoplasmic portion and the second extracellular domain, can influence the interactions between connexins present in adjacent cells. Furthermore, they indicate that voltage gating is not strictly intrinsic behavior for a given connexin, but can be modulated by the partner connexins to which they are paired. Finally, the finding that 3243H4 is functional only in heterotypic configurations, and cannot form homotypic channels, suggests the existence of a novel form of selectivity: self-discrimination. The latter property may represent another mechanism that operates to control the extent of communication between cells.

Romance and sex: pre-marital partnership formation among young women and men, Pune district, India.

Using qualitative and survey data in a rural and an urban slum setting in Pune district, India, this paper describes patterns of pre-marital romantic partnerships among young people aged 15-24, in spite of norms that discourage opposite-sex interaction before marriage. 25-40% of young men and 14-17% of young women reported opposite-sex friends. Most young people devised strategies to interact with others, largely from the same neighbourhood. There were wide gender differences with regard to making or receiving romantic proposals, having a romantic partner and experiencing hand-holding, kissing and sexual relations. For those who engaged in sexual relations, the time from the onset of the partnership to having sexual relations was short. Sex most often took place without protection or communication, and for a disturbing minority of young women only after persuasion or without consent. Among those who were unmarried, a large percentage had expected to marry their romantic partner, but for a third of young women and half of young men the relationship had been discontinued. Partnership formation often leads to physical intimacy, but intimacy should be wanted, informed and safe. Findings call for programmes that inform youth in non-threatening, non-judgmental and confidential ways, respect their sexual rights and equip them to make safe choices and negotiate wanted outcomes.

Contraception survey: Denmark 1988.

In 1988 a survey** was conducted in 1,033 women living in Denmark, whose ages ranged from 15 to 44 years. The main aims of the study were to identify the influence, if any, of demographic and socioeconomic factors on contraceptive use and to assess the women's knowledge of selected facts concerning their own fertility. The study's additional objectives were to gain insight into factors influencing communication on the subject of contraception, such as advice obtained from professional counsellors, general information dissemination and discussion between partners, and to investigate women's attitudes and opinions regarding contraceptive methods and their use. The results are discussed in relation to the findings of other studies on the use of contraceptives both worldwide and in a number of European countries. The Danish findings are also examined in the context of possible action to reduce the relatively high rate of legally induced abortions in Denmark.

[Sexuality in older women].

OBJECTIVES: To determine the prevalence of sexual relations in women >or=65 years. To determine factors that affect their activity and interest in sex. To evaluate the impact on their quality of life. DESIGN. Descriptive, cross-sectional study by means of survey. SETTING: Palacio de Segovia Health Centre, Madrid, Spain. Primary care. PARTICIPANTS: Women >or=65 years old. Criteria of exclusion: bed-ridden women, with psychiatric disease and/or moderate-severe cognitive deterioration. Sequential selection from 10 medical lists. MAIN MEASUREMENTS: Social and demographic, medical, geriatric (Katz and Lawton-Brody index), quality of life (WONCA sheets), variables on sexuality. RESULTS: One hundred and sixty two women. Mean age, 74.7 (7.8). Sixty one women with a stable partner (37.7%). Thirty four maintained sexual relations (20.98%), 61.8% (21) of them with coitus: these were satisfactory for 29 women (85.3%). For 50 (30.8%), communication was the most important aspect of sexuality. The lower their age, the more sexual relations they had (P<.022; odds ratio [OR]=0.85; 0.74-0.97). Of 115 women satisfied with the relations of their youth, 28.7% maintained sexual relations now (P<.003; OR=17; 1.7-178.8). The 23.9% maintained their sexual desire, though their partners had more interest in sex (50.9%). Women with stable partners and high self-esteem had more sexual relations than those who had neither, with a significant difference in both cases (P<.001). Maintaining sexual relations is a dependent factor on the Katz index (P<.007). The 32.2% of women thought that their sex life had changed for the worse, while 30.12% (20.54+19.17) thought their sex life had not changed or had improved. CONCLUSIONS: One of every 4 women >64 years old maintain sexual relations. Factors that determine sexual activity are: self-esteem, having a partner, sexual desire, previous sexual history, and the Katz index. Sexual relations are independent of quality of life.

Lipopolysaccharides as a communication signal for progression of legume endosymbiosis.

Establishment of a successful symbiosis between rhizobia and legumes results from an elaborate molecular dialogue between both partners. Bacterial nodulation (Nod) factors are indispensable for initiating plant responses, whereas bacterial surface polysaccharides are important for infection progression and nodule development. The mutant ORS571-oac2 of Azorhizobium caulinodans, affected in its surface polysaccharides, provokes a defective interaction with its host Sesbania rostrata. ORS571-oac2 induced structures with retarded development and continued generation of infection centers and organ primordia, leading to multilobed ineffective nodules. Bacterial development throughout the interaction occurred without major defects. A functional bidirectional complementation was obtained upon coinfection of ORS571-oac2 and a Nod factor-deficient mutant, indicating that the Fix- phenotype of ORS571-oac2-induced nodules resulted from the absence of a positive signal from ORS571-oac2. Indeed, the Fix- phenotype could be complemented by coinoculation of ORS571-oac2 with lipopolysaccharides (LPSs) purified from A. caulinodans. Our data show that Nod factors and LPSs are consecutive signals in symbiosis. Nod factors act first to trigger the onset of the nodulation and invasion program; LPSs inform the plant to proceed with the symbiotic interaction and to develop a functional fixation zone.

Lengthening shortened penis caused by Peyronie's disease using circular venous grafting and daily stretching with a vacuum erection device.

PURPOSE: We evaluated the results of chronic intermittent stretching with a vacuum erection device after circumferential tunical incision and circular venous grafting in 4 patients with penile shortening from severe Peyronie's disease. MATERIALS AND METHODS: We performed complete circumferential tunical incision and covered the defect with a circular venous graft in 4 patients with shortened penis as a result of Peyronie's disease. Preoperative evaluation included determination of patient and partner expectations, potency status, measurement of penile length after intracavernous injection and color duplex ultrasonography to determine possible vascular communication. Lower saphenous, upper saphenous and deep dorsal veins served as graft materials. We advised patients to use a vacuum device on a daily basis for 6 months starting 1 month after surgery. Postoperative evaluations were done at 6 and 18 months postoperatively. RESULTS: At 6-month followup 1 patient who did not use the vacuum device gained 1 inch in penile length and was not available for further followup. The other 3 patients each gained 2 inches but had decreased erectile rigidity due to narrowing in the grafted area (hourglass deformity). One patient who wanted a more natural erection elected penile prosthesis implantation about 1 year after grafting. The remaining 2 patients gained 3 inches at 18-month followup and regained partial penile rigidity similar to preoperative erections when the hourglass deformity improved. All patients were satisfied and indicated that surgery improved psychological well-being as well as relationships with partners. CONCLUSIONS: The results in this small group are satisfactory. Our technique offers a reasonable solution for correction of penile shortening in patients with Peyronie's disease.

Effects of a pragmatic teaching strategy for requesting information by communication board users.

The effects of a pragmatic teaching strategy on 4 nonspeaking children's abilities to initiate requests for information (i.e., who, what, and where) using manual communication boards were investigated. A teaching strategy based on milieu teaching and time-delay procedures was developed and embedded in naturally structured communication tasks to evoke information requests. Results indicated that information requests using the communication board occurred only after the pragmatic teaching strategy was applied sequentially to the three request types during the experimental condition. This effect was demonstrated repeatedly across the 4 subjects. Although generalization across request types was not demonstrated, substantial generalization was found across partners and environments. A social validation measure confirmed the treatment effectiveness. Independent raters perceived significant differences when viewing before- and after-training videotapes on 7 of 10 communication variables for all subjects.

The impact of life skills education on adolescent sexual risk behaviors in KwaZulu-Natal, South Africa.

PURPOSE: To assess the impact of exposure to life skills education by youth in KwaZulu-Natal Province (KZN, South Africa) on knowledge and behaviors associated with the spread of HIV/AIDS. METHODS: Data come from a panel study of 2222 youth from several population subgroups in KZN. The youth were aged 14-24 years when interviewed in 1999 and 2001. The intervention was a full coverage life skills education program for all students in middle and secondary schools. Impact was measured as net dose-response relationships between life skills exposure and outcomes. Econometric methods were used to control for nonrandom exposure to life skills education. Outcomes included sexual behavior and condom use indicators. RESULTS: Significant effects, albeit modest in magnitude, were observed on selected areas of sexual-reproductive health knowledge and perceived condom self-efficacy, along with larger effects on condom use at first and last sex. No consistent effects on age at sexual initiation, secondary abstinence, or partnering behaviors were observed among these youth. CONCLUSIONS: School-based life skills education appears capable of communicating key information and helping youth develop skills relevant to reducing HIV risk. However, the South African national program has yet to be fully implemented, and whether this initiative will result in sustained behavior modification among youth on a sufficient scale to affect the HIV/AIDS epidemic is uncertain.

Identification of best practices in the delivery of patient food services through public/private sector partnerships.

This article describes the evolution of public/private sector partnership in patient food service delivery and identifies skills required by executives to manage partnerships effectively, features that may be generalized to other areas; and the importance of labour relations. Site visits conducted across Canada demonstrate that when partners understand each other's objectives, commit to providing strong leadership, and create meaningful labour relations and communications strategies, partnerships can be successful.

Suzuki cross-couplings of unactivated secondary alkyl bromides and iodides.

The capacity to employ unactivated alkyl electrophiles as coupling partners will markedly increase the already exceptional utility of metal-catalyzed cross-coupling processes. This communication describes the development of a method that achieves the first Suzuki reactions of unactivated secondary alkyl bromides and iodides. The ability to couple readily available, easy-to-handle boronic acids is an attractive feature of this catalyst system.

Generalized effects of enhanced Milieu teaching.

The primary and generalized effects of Enhanced Milieu Teaching were examined with six preschool children with significant language delays. In a multiple baseline design across children, trainers implemented the naturalistic language intervention during play-based interaction sessions in the children's preschool classrooms. Children systematically increased their use of targeted language skills during the intervention sessions, and these changes were maintained when the treatment was discontinued. Generalized changes in children's communication resulting from the intervention were examined with untrained teachers, peers, and parents. Some generalization to untrained partners was observed for all children. Correlational analyses indicated that greater numbers of child utterances and greater diversity in vocabulary were associated with increased talking and mands for verbalization presented by partners.

Rapid redistribution of CD20 to a low density detergent-insoluble membrane compartment.

CD20 is a B cell integral membrane protein capable of initiating growth-modulating signals in human B lymphocytes upon its engagement with monoclonal anti-CD20 antibodies. In this report, we demonstrate that treatment of B cells with CD20 antibodies induces rapid redistribution of CD20 into a detergent-insoluble membrane compartment. Redistribution is detected as early as 15 s, following antibody addition, and involves up to 95% of CD20 molecules, depending on the antibody used. All of the detergent-insoluble CD20 was found in the low density fractions of sucrose density gradients, indicating that CD20 redistributes to glycolipid-rich membrane domains, analogous to caveolae in some cell types. As CD20 has previously been shown to associate with Src family tyrosine kinases, their co-existence in these compartments suggests a link to the role of CD20 in signal transduction. This study provides insight into the mechanism by which CD20 communicates signals to the cell interior and indicates that the search for membrane-proximal intracellular signaling partners should be directed to the Triton-insoluble fraction.

A framework for modelling fertility motivation in couples.

We present a theoretical framework that organizes individual-level fertility motivations into a couple-level model. One feature of this framework is the Traits-Desires-Intentions-Behaviour (TDIB) sequence through which the fertility motivations of individuals produce instrumental behaviours that are designed to promote or prevent childbearing. A second feature of this framework is the cognitive capacity of individuals to perceive a partner's motivational structure. We combine these two features into a dyad-level model that addresses interactions between partners at each step of the motivational sequence. We elaborate this model first with respect to the perception of partner's motivational structure and second with respect to the combination of partner's and own motivational structure. In the process we consider how couple-level processes of communication, influence, and disagreement can be measured and studied through these interactions. We conclude with a summary discussion of the framework and a consideration of the implications it has for a theory of reproductive psychology, population surveys, and family planning services.

Therapy with women with metastatic breast cancer.

OBJECTIVE: To describe some clinical issues encountered in individual and group therapy with women with metastatic breast cancer. CONCLUSIONS: The pressure to 'keep positive' poses dilemmas for patients and therapists alike, whether in individual or group therapy. The basis of skilled supportive care rests on the ability of therapists to assist patients and their families to face death rather than avoid it. Paradoxically, facing issues of death and dying leads to a greater sense of control, and empowers patients to reorder life's priorities, thus improving quality of life and relationships. In addition to the obvious need to detect and treat illness such as depression, working with women with metastatic breast cancer requires flexibility on the part of the therapist to adapt to her physical state of health, to include partners and family members where possible, and to liaise with colleagues in the treating team to facilitate communication and understanding of specific difficulties that may be encountered.

Exploring diagnostic processes: social science perspectives.

This review explores social science analyses of diagnosis of childhood neurological disabilities. The paper moves through three sections, which capture the historical and conceptual trends within the literature. The first focuses on work identifying the need to communicate effectively with parents when giving a diagnosis, the second explores the role parents can play as "partners" or contributors to diagnosis, and the final section goes further in exploring the social complexity of diagnoses in order to examine the embedded nature of social practices, power relations and hierarchies, and institutions in the diagnosis encounter.
