Bifid condyle: case report.

The double headed mandible condyle is a rare alteration that is frequently diagnosticated as an incidental finding in a panoramic radiograph. The different theories about its etiology are explained in this article. Symptoms described with bifid condyles vary from case to case, but in most instances are absent. Computer tomography is the ideal imaging method to evaluate the condyle morphology and to role out any degenerative process to achieve the differential diagnosis. Treatment is conservative for symptomatic patients with TMJ disorders, surgical treatment is described in literature in TMJ ankylosis that develops in bifid condyles secondary to trauma.

Functional and esthetic rehabilitation in amelogenesis imperfecta with all-ceramic restorations: a case report.

Amelogenesis imperfecta is a hereditary condition resulting in poor tooth development, severe anomalies, or complete absence of enamel. Enamel lesions may be the only characteristic of this condition or may be part of a generalized syndrome. Amelogenesis imperfecta can be characterized by enamel hypoplasia and/or hypomaturation or hypocalcification of the existing teeth. Restoration for patients with this condition should be oriented toward the functional and esthetic rehabilitation and the protection of the existing teeth. This paper presents a description of a patient's oral rehabilitation with all-ceramic veneers and crowns after extensive crown lengthening. The diagnostic procedure is also reported in detail. A microscopic examination was also performed on an extracted third molar under polarized light.

The treatment of masseter hypertrophy with botulinum toxin type A.

Masseter hypertrophy is a benign condition with variable causative factors, such as bruxism, temporomandibular disorders, malocclusion and others, but has an unclear etiology in the majority of cases. Surgical masseteric resection was the conventional method of treatment for the asymmetric swellings over the ramus and angle of the mandible on one or both sides, in addition to the generally unsuccessful treatments of occlusal adjustment, splint therapy, tranquilizers or others. An effective alternative was the local injection of very small doses of botulinum toxin type A into the masseter. The toxin binds permanently to the motor end plate preventing acetylcholine release causing pre-synaptic neuromuscular blockade, the muscle can be selectively weakened and local paralysis is followed by atrophy of the muscle. Four cases with their follow-up are reported. This technique provided a predictable and conservative method of treatment for this type of facial asymmetry.

Migraine with aura and dental occlusion: a case report.

Migraine is defined as a primary headache; however, some reports may suggest a relationship with dental occlusion and dental parafunction (clenching or grinding of the teeth). A patient diagnosed with migraine with aura, with concomitant temporomandibular joint and masseter muscle pain, was treated by the use of a dental appliance. The treatment succeeded in eliminating headache and visual aura, and significantly reducing the other symptoms. A headache related to dental occlusion and dental parafunctions seems to be able to mimic a primary migraine headache. Therefore, dental evaluation is always advised for headache diagnosis.

New directions in dentistry.

What an exciting time to be in dentistry! The ongoing changes are enormous in quantity. The variety of procedures is overwhelming! The continuing entry of new technology into the profession is stimulating, if not daunting. The public need and demand for dental services is at an all-time high! However-keep in mind that as a profession, we have a service responsibility, and that means we should strive for an altruistic, honest orientation to serve the public and yet make a reasonable and respectable income for ourselves and our families.

Diagnosing and treating the patient with restricted mandibular opening: a new approach.

Patients with a restricted mandibular opening and related pain require a systematic process for differential diagnosis and treatment planning. This article considers the differential diagnostic process, treatment planning, and treatment delivery for a patient seeking a second opinion before surgery for a closed lock. A closed lock occurs when the disc in the joint has been pulled off the head of the condyle and forward, blocking the condyle from translating forward.

IPR: intraoral pressure-dependent registration in Cerec application.

Cerec 3D software is now able to restore occlusal tooth surfaces safely and reproducibly. With an existing morphology worth adopting and/or by making use of an opposing bite registration, the user can reproduce the central contacts to the opposing jaw with the aid of the correlation mode. However, this assumes that the existing maxillo-mandibular relation is physiological. In prosthetic treatment, it is essential to differentiate between a healthy patient and patients with functional disorders. In the first case, the possibilities of performing an accurate and exact reconstruction of the healthy parameters are given and can be myocentrically controlled and corrected with suitable means. However, in the presence of functional disorders, the pathological situation should not serve at the basis for the reconstruction. Adjustment and, if necessary, stabilization of the function is essential. The following article illustrates by reference to a complex patient case how Cerec restorations can be produced correctly chairside with the aid of the IPR technique even in the case of functional disorders.
