Diagnosing breast lesions with contrast-enhanced 3-dimensional power Doppler imaging.

OBJECTIVE: To compare mammography with contrast-enhanced 2- and 3-dimensional power Doppler imaging for the diagnosis of breast cancer. METHODS: Fifty-five patients, who underwent breast biopsies with histopathologic assessment, participated in a study of mammography and contrast-enhanced sonography. Levovist (Berlex Laboratories, Montville, NJ) and Optison (Mallinckrodt, St Louis, MO) were administrated to 22 and 33 patients, respectively. Precontrast and postcontrast 2-dimensional power Doppler data of the lesion were obtained with an HDI 3000 system (Philips Medical Systems, Bothell, WA), and 3-dimensional data were acquired with an LIS 6000A system (Life Imaging Systems Inc, London, Ontario, Canada). Two independent and blinded readers assessed diagnosis. Receiver operating characteristic curves were computed individually and in combination for mammography and 2- and 3-dimensional sonography (before and after contrast). Histopathologic and imaging parameters were compared by Mann-Whitney statistics. RESULTS: Mammographic findings were available for 50 patients, biopsy for 54, and 2- and 3-dimensional sonographic images for 53 and 52, respectively. Of the 50 patients who had all 4 measures, 15 (30%) had malignancies. The areas under the receiver operating characteristic curve for the diagnosis of breast cancer were 0.51 for 2-dimensional contrast-enhanced imaging, 0.60 for 3-dimensional power Doppler imaging, and 0.76 for 3-dimensional contrast-enhanced imaging (P < .01). Mammography produced an area of 0.86, which increased when combined with 3-dimensional contrast-enhanced imaging to 0.90 and with all sonographic modalities to 0.96 (P < .001). The histopathologic diagnosis of benign or malignant correlated with the presence or absence of anastomoses and with the degree of vascularity assessed with contrast-enhanced 3-dimensional power Doppler imaging (P = .007 and .02). CONCLUSIONS: Contrast-enhanced 3-dimensional power Doppler imaging increases the ability to diagnose breast cancer relative to conventional 2- and 3-dimensional sonographic imaging.

Sonography of the gastrointestinal tract.

OBJECTIVE: The purpose of this presentation is to provide an overview of sonographic manifestations of various gastrointestinal diseases. METHODS: Patients were scanned with an HDI 3000 or HDI 5000 system (Philips Medical Systems, Bothell, WA) with both a 2- to 5-MHz curvilinear transducer and a 5- to 12-MHz linear transducer. The patients were asked to fast from the previous night. For accurate visualization of the stomach, a cup of water was given to patients thought to have stomach or upper gastrointestinal disease. RESULTS: A satisfactory sonographic examination could be performed in all patients. CONCLUSIONS: With recent technical advances, the increasing use of sonography in initial evaluation of patients with abdominal pain may allow earlier detection of bowel disease.

A computer module for the continuous monitoring of cardiac output in the operating theatre and the ICU.

A new pulse contour method to determine stroke volume and cardiac output continuously in patients on a beat-to-beat basis from the aortic pressure wave has been implemented in the form of a simple, inexpensive, fully automatic computing module for a commercially available patients monitoring system (Philips Medical Systems). Its reliability has been tested and shown in a computer analog, in experimental studies in 10 dogs (not reported here), in 22 hemodynamic studies on 20 young healthy volunteers and during 41 days in 20 postsurgical patients in the ICU, the most important result being that erros figures (15 and 19% respectively in the two human studies) are of the same order as when two standard methods, Fick and dye dilution are compared. The clinical studies have further indicated the easy applicability of the module 1 degree in the monitoring of critically ill patients in ICU's, 2 degrees as a monitor of the systemic circulation during anesthesia, and 3 degrees as a tool for studying the hemodynamic effects of pharmacological agents. The instrument consitutes no burden to the patients and has, several times during the course of the evaluation, provided an early warning of a deteriorating hemodynamic status of the patient to the physician.

Survey of small hospitals.

The CSHP Saskatchewan Branch created the position of Small Hospital Representative on their Executive Committee. This appointed person's major function was to improve representation on the Executive Committee for hospitals outside of Saskatoon and Regina. The representative sent a survey to these hospitals to establish the status of pharmacy practice in these hospitals and how to best serve them. There were 90 of 128 (70.3%) surveys completed. Survey results were separated into hospitals which employed pharmacists and those which did not. Elements of practice reported by hospitals with a pharmacist, which showed some degree of deficiency, included drug distribution systems, medication profiles and clinical pharmacy programs. Hospitals which did not employ a pharmacist obtained their pharmacy services from either a larger hospital or a local retail pharmacist. There were 11 hospitals which had no access to a pharmacist, depending on the Director of Nursing for medication-related activities. The Small Hospital Representative of CSHP Saskatchewan Branch, in co-operation with the professional associations used this study as a starting point in attempts to improve pharmacy practice in Saskatchewan in hospitals outside of Saskatoon and Regina.

[Information by authorities on hospital performance].

Information gathered in hospitals in Belgium by the authorities or their intermediaries is of three types: financial, medico-administrative, and quality of care assessment. From these computerized date bases, the public authorities can now measure socalled hospital "productivity". The aim announced by the authorities is to simultaneously combine increased economic effectiveness with improved quality of care. The use of this new bank of knowledge aims to enforce a sense of responsability to medical care. Regulation of the medical system by "medical management" can apply at various level depending on whether it works "a priori" or "a posteriori" and which or without financial incentives. There is however the risk of creating inaccuracies which must be taken into account when adjusting costs to each particular patient group. The use of all these tools should enable distinguishing between economic and qualitative objectives.

[Tumor ablation with MRI navigation--a novel method of microwave coagulation therapy for hepatic tumor].

Fifty-eight patients with hepatic tumor which consisted of 22 hepatocellular carcinomas and 36 metastatic liver tumors were treated by microwave coagulation therapy with MRI navigation. The tumors were located in all segments of liver except S1. In 24 cases among them, the abdominal approach was difficult, because the tumors were located just below the diaphragm. These cases were selected for thoracoscope-assisted microwave ablation under MR-guidance across the diaphragm. All MR data were collected on a vertically oriented open MRI system (0.5 T SIGNA SP/i system: GE Medical Systems). The microwave electrode was introduced into the liver through a 14G needle via a percutaneous puncture with real-time MR image navigation. Microwave ablations at 60 W for 60 seconds were repeated several times depending on the tumor size. MR imaging may be employed as a reliable guide for percutaneous puncture. Moreover, sufficient safety margin could be obtained for hepatic tumor ablation. MR-guided microwave thermoablation therapy is a feasible method of treatment for hepatic tumors.

[Physical imaging properties of a flat panel X-ray detector system].

We report the physical imaging properties of a flat panel detector (FPD) designed for radiographic imaging applications (Revolution XQ/i digital chest imaging system, G.E. Medical Systems). The imaging properties of the detector were evaluated through measurements of the characteristic curve, modulation transfer function (MTF), and Wiener spectrum. The digital characteristic curves of the FPD system were measured for the two tube voltages (80 kV and 120 kV). They showed that the correlation between the pixel values of FPD and the incident exposure to the FPD was a linear correlation. The dynamic range of characteristic curves had a range from about 0.003 to 2 microC/kg at the exposure. The presampling MTFs for different tube voltage were almost the same, when we measured the MTFs at 80 kV and 120 kV. And also, there was no significant difference between the MTFs measured with the slit in the direction parallel to the horizontal direction and with the slit in the perpendicular direction. The relative resolution of the FPD system was a 46% higher than that of ST-V imaging plate of FCR (Fuji Computed Radiography) system at 2 cycles/mm. The digital Wiener spectrum of the FPD system was about 1/10 lower than that of the CR system. Thus the FPD system can provide superior imaging performance due to both high resolution and low noise.

[Anaerobic threshold in the evaluation of heart function in patients with rate-responsive pacemaker].

It is very difficult to evaluate the ability of carrying out physical exercise in patients with rate responsive (RR) pacemaker (PM). However, the anaerobic threshold (AT) proved to be a useful parameter in the evaluation of cardiac function. The AT can be easily reproduced and not influenced from emotional aspects of both the patient and the physician, moreover being under maximum and then easy to achieve. Aims of our study were: to evaluate if the cardiopulmonary stress test can represent a method to be used for a more correct rate responsive pacemaker programming; to compare the data obtained of 3 rate responsive pacemakers steered by different sensors. We have studied 24 patients, of whom 10 with Activitrax (A), 8 with Meta (M), and 6 with Phymos MPT (P) pacemakers. Patients were submitted to symptoms limited cycloergometer stress test at 2 different settings: fixed rate at 70 b/min; increasing rate at until 85% of maximum heart rate for each patient, with range 0 + 10 W/min. Gas exchange data were continuously collected using an automated system (Medical Graphic System 2001) based on Whipp and Wasserman's method.(ABSTRACT TRUNCATED AT 250 WORDS)

The demise of vaginal operative obstetrics: a suggested plan for its revival.

Predictions made more than a quarter of a century ago regarding excessive use of cesarean delivery for cases of dystocia have been realized. Breech presentation, correctable dystocia, and twin gestation are increasingly being delivered by the abdominal route. Recent studies have shown that neonatal morbidity is not inherently improved by cesarean birth, and in fact have reported comparable results with vaginal operative procedures in properly selected and managed cases. Pressure to control the cesarean birth rate is now being exerted by consumer groups, professional groups, and governmental agencies. The reintroduction of vaginal operative procedures to modern obstetrics would help reduce the cesarean birth rate. However, major changes in the medical liability system, medical education and training, and the method of certifying obstetricians will be required before vaginal operative obstetrics becomes a serious alternative to cesarean delivery. Without these changes, I believe there is little hope of reversing the trend toward cesarean birth.

A group orientation program for families of newly admitted cancer patients.

Diagnosis and treatment of cancer is traumatic for patient and family, and hospital admission is the climax of the crisis. The authors describe a program in which social workers and former cancer patients orient families to a cancer center at the time of patient admission. The goals of the program are to lessen anxiety and increase mastery at this entry point into the medical system. No medical information is given during the group sessions. Content includes information on hospital facilities, treatment equipment, common feelings that cancer patients and their families experience, ways that family members can support the patient, and the roles of various hospital staff. The sessions also serve as a screening tool for the hospital to identify families that may need additional intervention. The program lessens the trauma for patients and their families, engages family members in treatment, and facilitates positive interactions between the hospital staff and members of patients' support networks.

Temperature-induced change of thick filament and location of the functional sites of myosin.

The combination of the small-angle X-ray camera to use the synchrotron radiation (National Lab. of High Energy Physics, Tsukuba) and a sensitive X-ray detecting system (FCR System, Fuji Medical System Inc.) using the imaging plate enabled us to record a small angle X-ray diagram of liver rabbit muscle within few minutes. Live relaxed rabbit muscle kept at 25 degrees C gave clear relaxed pattern, in which myosin layer line can be observed up to 13th order. When it was cooled down to 5 degrees C, it gave the small-angle X-ray pattern which looks like that obtained from contracting frog muscle, whereas cooled muscle produced no tension. The 8th meridional reflection almost vanished. The pattern is different from that obtained from rigor muscle: actin layer line at 72 A spacing can not be observed. The "stiffness" and (1,1) reflection on the equator increased. This indicates that more crossbridges are at the vicinity of the thin filaments without developing tension at low temperature. This might be related to the marked decrease in the rate of the step from M*ADP to M.ADP at low temperature. The position of the ATP binding site, SH1, actin binding site(s) was determined by three-dimensional image reconstruction method. Actin-binding site was determined by comparing the three-dimensional image of actin-tropomyosin-S1 and that of actin-tropomyosin-troponin-Ca. The position of ATP binding site and SH1 was determined by three-dimensional reconstruction of the complexes of actin-tropomyosin and S1 of which the ATP binding site or SH1 was labelled with avidin. It was found that SH1 locates near the actin-binding site at the same side of S1. The distance from head-rod junction to ATP binding site and SH1 was similar. But they locate at the different side of S1 and the distance between two sites is about 5 nm, which is consistent with that obtained by energy transfer method.

Semen preparation with the Sperm Select System versus a washing technique.

OBJECTIVE: To compare sperm migration through sodium hyaluronate with simple washing as methods for preparing sperm for IUI. DESIGN: Ten normal semen specimens were prospectively collected and samples were prepared by simple washing and by migration into sodium hyaluronate using the Sperm Select System (Select Medical Systems, Williston, VT). The semen and each treatment group were evaluated for sperm concentration, percent motile, viability, acrosomal status, longevity, and computer-aided semen analysis (CASA) parameters. SETTING: University reproductive endocrinology facility. RESULTS: The recovery of motile sperm was significantly higher for the washing method (mean +/- SEM 75% +/- 7%) than for the hyaluronate method (10% +/- 1%). The number of motile sperm recovered by migration into hyaluronate was independent of the percentage of motile sperm in the semen specimen and positively correlated with sperm concentration. The hyaluronate method produced greater percentages of motile, viable, and morphologically normal sperm, with lower proportions of premature acrosome reactions, higher sperm velocity, and greater linearity. CONCLUSIONS: The Sperm Select System method of sperm separation provides a highly uniform specimen with improved sperm quality. However, the recovery of motile sperm is considerably lower than for simple washing methods.
