[Epidermoid cyst of the mouth floor].

The epidermoid cysts are frequent during childhood, however mouth floor location are very unusual, because of their more difficult diagnosis and therapeutic approach. We present a 5 years old male, symptoms free until a week before, when his parents noticed a well defined mass in the mouth floor. A physical examination leaded to the diagnosis of possible epidermoid cyst. The tumor was excised through an introral approach. A review of different diagnostic means and surgical management are undertaken.

Ciguatera neurotoxin poisoning mimicking burning mouth syndrome.

Burning mouth syndrome is a condition in which the patient perceives a sensation of intraoral burning, typically of the anterior tongue. This article presents a case report of a patient presenting for orofacial pain evaluation in whom ciguatera neurotoxin poisoning is diagnosed. The clinician should be aware of neurotoxin poisoning as a possible cause of symptoms of burning mouth, especially among patients who have recently traveled to a tropical area. Recognition of this condition in this case highlights the need for a detailed and accurate patient history.

Kinetics of humoral immune response in pigs vaccinated against foot and mouth disease.

The present investigation was conducted to study the foot and mouth disease virus (FMDV)-specific humoral immune response (HIR) in pigs, following vaccination with oil adjuvanted foot and mouth disease (FMD) vaccine, upto 90 days post vaccination (dpv). For this, 40 Large White Yorkshire (LWY) pigs (20; one-year old female (gilts) and 20; three-month old piglets) were vaccinated @ 2 ml/animal, subcutaneously. Sera samples were collected at fortnight interval from all the animals. The log10 SN50 antibody titres against all the serotypes (Type O, A and Asia-1) were detected in both gilts and piglets from day 7 to 90 dpv indicating the persistence of HIR up to the last day of sampling. The maximum antibody titres were observed on 28 dpv, thereafter, titres started declining, but were present till 90 dpv against all the three FMDV serotypes. HIR was more pronounced in piglets in comparison to gilts, as group mean SN antibody titres against all the three FMDV serotypes were found to be more maintained and significantly higher in piglets.

Spindle cell lipoma of the floor of the mouth. A case report.

A sessile swelling in the floor of the mouth appeared three years ago in a 29-year old Caucasian female located laterally to the opening of the right duct of Wharton. The mass covered by normal looking mucosa exhibited slight growth since then. After excision, histological examination revealed the presence of a tumor formed by areas of mature adipose cells interspersed with extensions of tightly disposed fusiform fibroblasts immunoreactive for vimentin and CD-34. There were foci of concentric fibroblasts forming dense whorls. When large these conglomerates exhibited chondrocytes in the center. Mast-cells were not rare throughout the fibroblastic areas. No signs of malignancy occurred. These features led to the diagnosis of a spindle cell lipoma with chondrous metaplasia. The relative rarity of such a tumor in this location, especially in the chondrous variety, was considered worth of presentation, while the differential diagnosis with other intraoral tumors, namely mucoceles, dermoid cysts, mesenchymomas, fibromas and myxomas of the floor of the mouth, is discussed.

[Local hyperthermia by means of ballon-type electrodes in combined therapy of patients with locally spread cancer of the mouth mucosa and lower lip].

Modification of the standard chemo- and radiotherapy with local hyperthermia gives evident positive results in treatment of patients with local cancer of tunica mucosa of mouth and lower lip. Since this area having uneven complex surface needs to develop radiofrequency emitting special applicators. We have used catheters with ballons filled with hypertonic solution as electrodes to adapt the mouth cavity. An electric field creating between these ballons and high surface adjustability makes this system come in useful.

[Computerized tomography in the staging of carcinoma of the oropharynx, the tongue and the mouth floor].

Eighty-seven patients with carcinoma of oropharynx, tongue and floor of the mouth were examined by means of CT; (TNM criteria); tumor staging was reviewed and compared with that obtained by clinical examination, by endoscopy, at surgery and histology. CT proved to be a reliable technique to detect both the presence of neoplasms, with the exception of very superficial ones, and their deep spread to parapharyngeal space, to muscles of floor of the mouth and prevertebral pterygoid muscles. Lymph node metastases, especially to retropharyngeal nodes, were also clearly demonstrated on CT scans, which indicates this technique as the examination of choice in oropharyngeal and oral cavity tumor staging, for it yields valuable information which can integrate clinical findings.

[Burning mouth syndrome].

Burning mouth syndrome is an intraoral pain disorder, stinging and/or itching sensation of the tongue or oral mucous membranes. Numerous causes have been suggested, including local factors such as candidiasis and denture trauma, systemic factor such as nutritional deficiencies, diabetes and climacteric, xerostomia, miscellaneous conditions such as psychogenic factors. A systematic approach for the success in diagnosis and treatment is included history taking, complete mouth examination and laboratory investigation.

[Intra-oral sonography in neoplasms of the mouth and base of the tongue].

Sonography has proved superior to CT and MRI for imaging T1 or T2 carcinomas of the oral cavity and base of the tongue. However, in examining the anterior half of the tongue and the floor of the mouth problems arise due to absorption of sound by the mandible and air inclusion. Ultrasound examination can be performed end-orally by means of a miniaturised transducer. End-oral sonography produces images of the anterior parts of the tongue and the floor of the mouth without artefacts. This method is superior to conventional transcutaneous sonography for this region.

[Dysontogenetic cysts of the mouth floor].

Congenital cysts of the floor of the mouth are rarely seen. We investigated in the years 1987 and 1988 four patients with swellings in the floor in the mouth. The histological examination results dermoid and epidermoid cysts. A teratoma was not seen. Sonographic investigation and computertomography were both helpful by getting the correct diagnosis of these cysts and their location. The treatment was the exstirpation from the outside of the throat because all cysts were located under the geniohyoid muscle.

[Functional disorders following resection of the tongue and mouth floor].

On observing 20 patients who had underwent total tongue and floor-of-the-mouth resection, the authors stated difficulties in swallowing and breathing that in certain cases called for laryngectomy. To counteract these disorders, floor-of-the-mouth and tongue reconstruction was performed with the aid of an arterialized frontal flap, which resulted in essentially undisturbed functions.

[Evaluation of real-time sonography in the pretherapeutic staging of malignant tumors of the tongue and mouth floor].

Based on our three years experience with about 200 transcutaneous real-time sonographies of the tongue and the floor of mouth, an assessment of the determination of size and site of 38 pretherapeutically examined malignant tumours (35 squamous cell carcinomas) was made. The patients were examined in reclined position, the head retroflected. Curved array transducers of 5 or 7.5 MHz and a silicon elastomer block as interface turned out to be optimal. All tumours could be detected by sonography as hypoechoic, more or less homogeneous, ill-defined areas. Two thirds of the tumour ulcerations could be seen as hard hyperechoic reflexes within the hypoechoic areas. When comparing the maximal diameter of tumours of the 20 operated patients, we found an agreement (5 mm tolerance) of sonography with the surgical specimen in 14 of 20 tumours (70%), of sonography with the clinically estimated diameter in 10 of 20 tumours (50%), whereas the clinically estimated diameter was in agreement with the surgical specimen in only 8 of 20 tumours (40%). A peritumoral inflammatory infiltration can simulate a larger tumour size in sonography. The extent of tumours within the tongue, to the floor of mouth, the lateral pharyngeal wall and the preepiglottic space was documented correctly in most cases. In three cases a previously unknown spread across the midline was found sonographically. Including the sonographic findings in the TNM classification, 5 T1 tumours would have been staged as T2 and one T3 tumour as T4. Inflammatory diseases of the tongue can show the same sonomorphology as malignant tumours.

T stages of tumors of the tongue and floor of the mouth: correlation between MR with gadopentetate dimeglumine and pathologic data.

PURPOSE: To define MR accuracy in the evaluation of the T stages of tumors of the tongue and floor of the mouth. METHODS: Fifty-two patients affected by squamous cell carcinoma were studied with a superconductive system at 1.5 T. The study was performed with spin-echo T1- and T2-weighted images before contrast and short spin-echo T1-weighted and gradient-echo sequences after gadopentetate dimeglumine administration. The results obtained with MR were compared with pathologic data. RESULTS: Good correlation of T stages (TNM system) was obtained in 45 of 52 cases. MR did not show four superficial lesions. In one case, infiltration of the cortical bone of the mandible was not demonstrated (MR, T2; pathologic, T4), and in another the lesion was classified as T2 instead of T3, as it was pathologically. One lesion was classified as T4 on MR because of infiltration of the alveolar ridge but was classified as T2 at surgery. In 46 patients who underwent surgery, the accuracy of MR was excellent for predicting the relationship of tumor to midline and oral floor musculature. The results obtained with gadopentetate dimeglumine were better than those obtained in noncontrast studies in 32 (62%) of 53 cases. CONCLUSION: MR showed high accuracy in the study of tumors of the tongue and floor of the mouth.

[Mucous membrane of the mouth and prostheses in old people (author's transl)].

The majority of people of advanced age at present have either very many gaps in their dentures or they have no teeth at all. For this reason they can only wear broad-base plate-type bridged, which can lead to pathological changes of the mucous membrane. The author uses the complex etiology of prothetic stomatopathy, in which not only changes in the mucous membrane of the mouth associated with ageing but also hormonal changes (menopause), general diseases and oral candidiasis play a role, to illustrated the necessity of collaboration between stomatologists and geriatricians. Oral and prosthetic hygiene degenerates with increasing age. The average period of wearing prostheses also increases with the age and the danger of chronic mucous membrane damage also increases. But since old people seldom if ever consult a stomatologist, every geriatrician should also have a look at the mucous membrane of the mouth when examining old patients.

Management of dry mouth.

Dry mouth is a common complaint and, when associated with salivary gland dysfunction, a significant problem. Accurate and complete diagnosis of the patient with complaints of xerostomia is essential. Management should be directed to relief of symptoms, control of oral disease, and improvement in salivary function. With a systematic approach and aggressive management, most dry mouth patients can achieve oral comfort and adequate oral function.

A mouth protector for an edentulous patient: a case report.

Most mouthguards made for edentulous mouths lack the stability to prevent dislodgement. In a case reported in the literature where stability was achieved the appliance was bulky, difficult to insert and remove and lacked permanence. To surmount these deficiencies one was designed in two halves which, when placed into the mouth, slotted together to give the stability of the one piece construction but at the same time allowed easy insertion and removal. This paper describes the construction of such a mouthguard and reports on one case.

Swelling of the floor of the mouth: a clinical dilemma.

Swelling of the floor of the mouth is sometime difficult to diagnose clinically. Lithiasis and neoplasm are the most frequent causes of intraoral swelling. Also after plain radiology and computed tomography the diagnosis is often dubious. A case of swelling of the floor of the mouth due to an adenoid cystic carcinoma of the sublingual gland is reported.

Lipoma of the floor of the mouth: report of an unusually large lesion.

Although lipomas are common, benign tumours found in any part of the body, their occurrence in the oral cavity is relatively rare. An unusual case of a large lipoma appearing on the floor of the mouth in a 77-year-old male, is presented in this paper. The unusual appearance in this case suggests that this tumour should be included as a rare possibility in the differential diagnosis of swellings in the floor of the mouth.
