Lymphocytes, chronic bronchitis and chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease (COPD) is a cytotoxic T lymphocyte (CD8)- and macrophage (CD68)-predominant chronic inflammatory disorder of the conducting airways and alveoli. This is often associated with a neutrophilia, inflammation of small airways and destruction of tissue beyond the terminal bronchiolus, i.e. emphysema. In contrast, asthma is a helper T cell (CD4; type 2)-predominant chronic inflammatory disorder of the conducting airways in which there is T lymphocyte-derived gene expression for interleukin (IL)-4 and IL-5 but not interferon gamma. There is fragility of airway surface epithelium, thickening of the reticular basement membrane, bronchial vessel congestion and (when severe) an increase in the mass of bronchial smooth muscle. This is usually (but not always) associated with tissue and peripheral blood eosinophilia rather than a neutrophilia and there is exudative plugging of the airways. These differences of inflammatory profile, remodelling and lung function are seen when smokers with COPD are compared with non-smoking mild asthmatics. However there may be important similarities and overlap, particularly in more severe asthma when neutrophils predominate and in the older and or smoking asthmatic when reversibility of airflow is less obvious. We have recently demonstrated gene expression for IL-4 and IL-5 in and around the mucus-secreting glands of airways resected from smokers without a history of asthma. Also exacerbations of bronchitis may be associated with a tissue eosinophilia. On examination of bronchial biopsies from these patients we show surprisingly strong gene expression for IL-4, IL-5 and even human eotaxin and RANTES (regulated on activation normal T cell expressed and secreted). Whilst CD4 T lymphocytes of the Th2 phenotype might be expressing these cytokines in bronchitis, CD8 T lymphocytes are also capable of secreting IL-4 and IL-5. Viruses may modulate these changes in distinct lymphocyte functional phenotypes. The relevance and importance of CD4/CD8 T lymphocyte ratio to the development of COPD is discussed.

[Clinical course of chronic bronchitis in Chernobyl wreckers].

AIM: To characterize clinical symptoms of chronic bronchitis (CB) in Chernobyl wreckers. MATERIAL AND METHODS: 71 CB male Chernobyl wreckers exposed to low-dose radiation and 43 control patients were examined and treated in a specialized hospital in 1988-1996. Clinical features of CB were investigated by clinical (complaints, history, objective examination), laboratory (total blood count, biochemical and immunological blood picture, coagulogram) and instrumental (fibrobronchoscopy, x-ray, radiometry, spirography) findings with further statistical processing. RESULTS: Chernobyl wreckers more frequently had neurocirculatory asthenia and hemoptysis. The latter was caused by bronchial mucosa atrophy. In chronic non-obstructive bronchitis they exhibited mild bronchoobstruction on the distal level. Laboratory indices showed no essential changes. CONCLUSION: Chernobyl wreckers have a more severe course of CB.

[Indications for antibiotic long term treatment of chronic and recurring bronchitis].

The formerly used long-term therapy with antibiotics in chronic bronchitis in adulthood has been nearly completely replaced by an intermittent treatment of the single attack. Therefore it seems necessary to check the appropriateness and the indications for an antibiotic long-term therapy in childhood. In the light of the greater importance of viral and bacteriological infections of the airways in children and in the light of the frequency of confirmed malformations of the tracheobronchial tree the further appropriateness for an antibiotic long-term therapy after a thorough bronchological examination is established. Also firstly established bronchiectases or severe cases of deforming bronchitis initially mostly require such a long-term treatment. Among the cases of a specialized dispensary care for chronic and recurrent bronchopulmonary diseases such an antibiotic long-term therapy is indicated in nearly 8 to 10 per cent of all patients.

Effects of oral doxofylline on inflammatory changes and altered cell proliferation in chronic obstructive bronchitis.

The effects of oral doxofylline (400 mg BID) on bronchial airway mucosa were investigated in 14 patients with chronic obstructive bronchitis. The eligible patients had to have forced expiratory volume in 1 second > 60% of the predicted value and oxygen partial tension > 55 mm Hg. At the onset and the end of the study, bronchial biopsies were performed via a flexible fiberoptic bronchoscope. Chronic inflammation of the airways was graded according to absence of lesions = 0, involvement of 1-10% = 1, 10-50% = 2 and > 50% = 3. After three months of treatment, 57% of patients in the doxofylline group presented absence of lesions, while the remaining 43% exhibited advanced lesions. In the control group, absence of lesions was observed in 14%, while lesions of grade 1, 2 and 3 were visible in 14%, 29% and 43%, respectively. At histological examination, a significant difference in the degree of structural changes was observed in the doxofylline group between baseline and the end of the study (p < 0.03). In conclusion, doxofylline may induce favorable effects on inflammatory changes and altered cell proliferation of the airway mucosa in patients with chronic obstructive bronchitis.

[Correlations between electrophysiological parameters of the heart, blood gas composition and hemodynamics in patients with chronic obstructive bronchitis].

AIM: To elucidate correlations between gas composition of blood, hemodynamic and electrophysiological parameters of the heart and effect of these indices on arrhythmia onset in patients with chronic obstructive bronchitis (COB). MATERIAL AND METHODS: "Rhythm" ECG, 24-h ECG monitoring, transesophageal pacing, heart ultrasonography were performed in 160, 67, 160 of 160 patients with chronic obstructive bronchitis (COB), respectively. 76 patients suffered from respiratory insufficiency (RI) degree 1 and 84 of RI degree 2. 58 patients free of RI served control. Gas composition of the blood and acid-base balance of the capillary blood were examined in 50 patients. RESULTS: Shortening of effective refractory atrial period and prolongation of interatrial conduction predispose to emergence of suptaventricular arrhythmia in COB patients with RI. With the onset and development of RI in COB patients, heart chamber size and myocardial function become more and more essential for cardiac electrophysiological parameters. For COB patients free of RI, gas composition of capillary blood is of key importance in this respect. CONCLUSION: The proposed complex clinicofunctional method for assessment of electrophysiological condition of the heart with evaluation of intracardiac hemodynamics and gas composition of the capillary blood is recommended for registration of signs of the onset and unfavorable progress of cardiac arrhythmia in COB patients with RI as well as for objective and safe control over effectiveness of treatment of such patients.

[Characteristics of expression of active oxygen forms by blood cells in patients with chronic bronchitis].

AIM: To study generation of active oxygen forms in the whole blood of patients with chronic bronchitis and correction of the detected disorders. MATERIAL AND METHODS: By means of registration of spontaneous and staphylococcus-induced luminol-dependent chemiluminescence (LDCL) of the whole blood, generation of active oxygen forms in 73 patients with chronic bronchitis (CB) has been studied. RESULTS: By the intensity of luminescence, all the examinees were divided into two groups. In group 1 LDCL was high, in group 2 LDCL was under the control level. Combined treatment of group 1 patients included inhalations of ceruloplasmin antioxidant. Patients of group 2 were administered sodium hypochlorite that possesses prooxidation properties. Compared to conventional treatment, the above two treatments produced marked positive changes in the studied indices and clinical symptoms. CONCLUSION: The method of chemiluminescence registration of the whole blood allows validation of pathogenetic therapy of patients with CB and improves outcomes of the treatment.

Study of protection by recombinant fowl poxvirus expressing C-terminal nucleocapsid protein of infectious bronchitis virus against challenge.

A stable recombinant fowl poxvirus (rFPV) expressing the C-terminal region (119 amino acids) of the nucleocapsid (N) protein of an infectious bronchitis virus (IBV) strain Ch3 was constructed by inserting the coding sequence within the thymidine kinase gene of fowl poxvirus (FPV) by homologous recombination. The N protein was expressed under control of the vaccinia virus promoter P7.5 in chicken embryo fibroblast cell cultures as seen in immunofluorescence assay and in rFPV-inoculated specific-pathogen-free (SPF) chickens by detecting antibodies with enzyme-linked immunosorbent assay (ELISA). A homologous IBV strain (Ch3) and two heterologous IBV strains (Ch5 and H4) were used to inoculate SPF chickens in a challenge to examine the protective efficacy of the rFPV. When the chickens were challenged with IBV Ch3 or Ch5, the control birds had respiratory signs of infections bronchitis, whereas all the vaccinated birds were clinically normal although low levels of the IBV infection were detected by a differential ELISA. In contrast, in the chickens challenged with IBV H4, all control birds and vaccinated birds suffered from the highly lethal IBV H4 infection. Our results suggest that the C-terminal 119 amino acid of the nucleocapsid expressed by FPV is a host-protective antigen and may induce cross-protective immunity against illness among some IBV strains.

Characterization of three infectious bronchitis virus isolates from China associated with proventriculus in vaccinated chickens.

Outbreaks of an avian disease in infectious bronchitis-vaccinated chickens in China have led to the characterization of coronaviral isolates Q1, J2, and T3, which were isolated from proventricular tissues of the affected young layer flocks. Serologic analysis revealed that they could induce high titers of infectious bronchitis virus (IBV) antibodies in inoculated specific-pathogen-free (SPF) chickens in indirect enzyme-linked immunosorbent assay but were not neutralized by antisera specific to the IBV serotype M41 and the Australian T strain. In a pathogenicity experiment, the clinical signs and related gross lesions resembling those of field outbreaks were reproduced in SPF chickens, and viruses were reisolated from the damaged tissues, including trachea, proventriculus, duodenum, and cecal tonsil. Sequence data demonstrated the complete S1 amino acid sequences of these isolates were almost identical despite recovery from geographically different areas in China and had 47.3%-82.3% similarity in comparison with the 47 published S1 sequences. On the basis of genotyping and limited serology, the three isolates, which were responsible for field outbreaks of the disease, might be a new IBV variant.

[Clinical and epidemiological diagnosis of chronic dust-induced bronchitis].

A unified scheme for comprehensive interview of workers of a large metallurgic works who come in contact with occupational noxious agents that cause respiratory organs was used to select 300 workers (199 men and 101 women). They underwent a bicycle exercise test that involved the determination of forced vital capacity (FVC), forced expiratory volume within the first second (FEV1), peak expiratory air flow (PEAF), and the Tiffeneau index. Moreover, the flow/volume ratio was examined on a Flowscreen device (Erich Eger) and a test with a bronchodilator (ventoline, 200 mu/kg) was also made. The authors conducted 64 assays of eosinophilic cationic protein (ECP) and 64 assays of mucinic antigen (MA). Serum ECP and MA levels were measured by immunofluoresence and sandwich-immunofluorescence, respectively. Screening functional indices obtained from the flow/volume and bronchodilator tests are sensitive in both manifestative and latent dust-induced bronchitis so they permit identification of latent and obvious airways obstruction. ECP is a highly reliable diagnostic criterion for the asthmatic form of chronic occupational bronchitis and MA is for its inflammatory form.

[Dynamics of interferon production during different phases of the pathological process in children with acute pneumonia and relapsing bronchitis].

With the purpose of gaining further insight into regularities of changes that take place in indices for the interferon status in children with acute pneumonia and current bronchitis depending on the phase of the pathological process, 112 children were examined in whom the level of serum interferon was measured together with production of alpha- and gamma-interferon by leucocytes of the peripheral blood in vitro. It is shown that in the examined patients with acute pneumonia and relapsing bronchitis in the acute period and during the phase of reparation there are differences in functioning of indices for the system of interferon.

[Objectives for antibiotic therapy in acute exacerbations of chronic bronchitis].

ANTIBIOTIC EFFICACY: According to early studies, antibiotics have moderate efficacy in acute exacerbation of chronic bronchitis. The lack of efficacy is particularly clear for patent exacerbation with marked alteration of respiratory function. Recent studies have shown that newer compounds exhibit an efficacy similar (no proven superiority) to comparison compounds (75 to 95% favorable outcome with treatment). The recommendations of the IVth Consensus Conference on Anti-infectious Therapy thus propose first line antibiotic therapy for patients with a forced expiratory volume in 1 second (FEV1) between 80 and 35% and broader spectrum and new antibiotics in case of failure of the first line treatment for patients with severe obstruction or frequently recurrent exacerbation. EVALUATION CRITERIA: Using exacerbation-free interval, reduction in the number of exacerbations, duration of treatment and/or hospital stay as evaluation criteria, interesting results are obtained with amoxicillin/clavulanic acid, azithromycin, and ciprofloxacin. Independent factors predictive of therapeutic failure are, according to one study, FEV1 less than 35%, ambulatory administration of oxygen, more than 4 acute exacerbations within 24 months, history of pneumonia or sinusitis, and requirement for long-term corticosteroid therapy. Factors predictive of recurrence are, according to another study, dependence on oxygen therapy, prolonged corticosteroid therapy, smoking, and/or heart disease. Cost effectiveness is particularly interesting with ciprofloxacin, especially in more severe patients. PERSPECTIVES: It is important to target antibiotic therapy for acute exacerbation of chronic bronchitis specifically for patients who will truly benefit, adapting the prescribed compound to the bacterial target.

Comparative study of cefaclor AF vs. cefuroxime axetil in acute exacerbations of chronic bronchitis.

BACKGROUND: Acute exacerbations of chronic bronchitis (AECB) are the most common complication of chronic bronchitis. The majority of AECB are caused by infection. The choice of appropriate antibacterial therapy for AECB is becoming more difficult and is usually empirically. Cefaclor and cefuroxime are used for ambulatory treatment of AECB. MATERIAL/METHODS: This multicenter, randomized, single blind study was undertaken in order to compare efficacy and safety of cefaclor AF (500 mg BID) and cefuroxime axetil (250 mg BID) in 10 days treatment of ambulatory patients with AECB. 170 adults were enrolled into the study. Clinical responses were assessed on 17th-24th day after randomization. RESULTS: Both antibiotics had high over 97% effectiveness in the treatment of AECB. There was statistically significant 1.7 times higher rate of patients with cough release after cefaclor treatment compared to cefuroxime (p<0.03). There was a significantly 2.25 higher rate of patients with AECB symptoms release like: increasing dyspnea, sputum volume and sputum purulence or cough in cefaclor group compared to cefuroxime (p<0.0187). Both treatments resulted in significant improvement of pulmonary peak expiratory flow (PEF). There were no differences between the rates of gastrointestinal and other side effects in both groups. CONCLUSIONS: 1. Cefaclor and cefuroxime have similar high efficacy and safety in 10 days treatment of patients with AECB. 2. Cefaclor treatment significantly higher 2.25 times reduces the rate of principle symptoms of AECB compared to cefuroxime. 3. Both antibiotics treatment significantly increase PEF, with higher tendency observed in after cefaclor treatment.

[Airway alveolar macrophages and LFA-1 expression in chronic bronchitis].

OBJECTIVE: To assess the relationship between the expressions of lymphocyte function associated antigen-1 (LFA-1) in macrophages in both BALF and bronchial mucosa and the severity of airway inflammation in patients of chronic bronchitis. METHODS: Bronchial alveolar lavage and bronchial mucosa biopsy were performed in 18 patients with stable stage of chronic bronchitis (CB). LFA-1 positive alveolar macrophage (AM) was measured by immunocytochemistry stain. The percentages and number of AM in BALF and bronchial mucosa were also calculated and the thickness of mucosa measured. RESULTS: (1) The numbers of AM and neutrophils of BALF in CB were significantly higher than those in healthy group (P < 0.01, respectively). So did the percentage of neutrophils of BALF (all P < 0.05). (2) The number [(4.36 +/- 2.11) x 10(4)/ml] and percentage [(24.6 +/- 5.8)%] of positive AM expressing LFA-1 in CB were significantly higher than those [(0.63 +/- 0.25) x 10(4)/ml, (10.6 +/- 2.7)%] in healthy group (all P < 0.01, respectively). The number of AM in BALF was positively correlated with that of positive AM expressing LFA-1 (r = 0.624 7, P < 0.01). The number of CD(68)(+) cells [(21.6 +/- 4.6)/HP] in mucosa and the thickness of mucosa [(0.20 +/- 0.09) micrometer] in CB were significantly greater than those [(10.0 +/- 3.4)/HP, (0.08 +/- 0.04) micrometer] in healthy group (P < 0.01, respectively). They were inversely corrlated with the value of FEV(1)%pre (r = -0.5875, P < 0.01; r = -0.655, P < 0.05). CONCLUSIONS: Increased expression of LFA-1 may be one of important causes leading to the accumulation and migration of AM in the airway of patient with CB. AM infiltration and the thickening of bronchial mucosa are closely related to the decline of FEV(1).

[Effect of anticholinergic therapy on myocardial reserve dynamics in patients with chronic obstructive bronchitis].

The aim of the work is to study the influence of a long-term ipratropium bromide on the character of the functional heart indices changes in patients with chronic obstructive bronchitis. Positive effects on the myocardial reserve are reported under a 2-week atrovent monotherapy. Atrovent favoured a positive trend in clinical symptoms, that determined the myocardial reserve. With the above drug, the frequency of the reduced myocardial reserve has come to be lowered by 34.5%. Based on the data presented recommendations are given for use of ipratropium bromide in chronic obstructive bronchitis patients with the incipient form of heart failure.

[Experience with rovamycin and roxithromycin in patients with chronic bronchitis in the presence of posttuberculous changes].

Rovamycin and roxithromycin were comparatively studied for their effects in 25 patients with chronic bronchitis and posttuberculous changes in the lung. Ten patients received oral rovamycin, 3,000,000 IU, twice daily and 15 had oral roxithromycin, 150 mg, twice daily. The therapy averaged 9 days. Clinical cure was observed in 90% of cases on rovamycin and in 86.7% on roxithromycin. There were no adverse effects of these drugs. The findings suggest that due to their high therapeutical and bacteriological activities, both rovamycin and roxithromycin show their good clinical efficacy in the treatment of exacerbations of chronic bronchitis in patients with residual posttuberculous changes.
