Development and validation of the Ontario acute myocardial infarction mortality prediction rules.

OBJECTIVES: To develop and validate simple statistical models that can be used with hospital discharge administrative databases to predict 30-day and one-year mortality after an acute myocardial infarction (AMI). BACKGROUND: There is increasing interest in developing AMI "report cards" using population-based hospital discharge databases. However, there is a lack of simple statistical models that can be used to adjust for regional and interinstitutional differences in patient case-mix. METHODS: We used linked administrative databases on 52,616 patients having an AMI in Ontario, Canada, between 1994 and 1997 to develop logistic regression statistical models to predict 30-day and one-year mortality after an AMI. These models were subsequently validated in two external cohorts of AMI patients derived from administrative datasets from Manitoba, Canada, and California, U.S. RESULTS: The 11-variable Ontario AMI mortality prediction rules accurately predicted mortality with an area under the receiver operating characteristic (ROC) curve of 0.78 for 30-day mortality and 0.79 for one-year mortality in the Ontario dataset from which they were derived. In an independent validation dataset of 4,836 AMI patients from Manitoba, the ROC areas were 0.77 and 0.78, respectively. In a second validation dataset of 112,234 AMI patients from California, the ROC areas were 0.77 and 0.78 respectively. CONCLUSIONS: The Ontario AMI mortality prediction rules predict quite accurately 30-day and one-year mortality after an AMI in linked hospital discharge databases of AMI patients from Ontario, Manitoba and California. These models may also be useful to outcomes and quality measurement researchers in other jurisdictions.

[Platelet reference values in healthy children living in Mexico City].

INTRODUCTION: There are few studies on normal values of platelet parameters (number of plate-lets, mean platelet volume [MPV], thrombocytocrit, and platelet distribution width [PDW]) in children. The objective of this study was to determine the normal values of these platelet indices in healthy children who live in Mexico City, located 2240 meters above sea level. MATERIAL AND METHODS: 642 healthy children, between 2 and 16 years old, who had normal growth and developmental features and no history of treatment with drugs that alter platelets, were selected. The children fasted for 12 hours before their blood was collected by venipuncture of the arm. Platelet values, determined in an automatic counter (Cell Dyn 3000), were used in the statistical analysis to calculate the mean, standard deviation, and the 3, 50, and 97 percentiles of each data distribution, stratified by age and sex. Correlations between platelet count, MPV, PcT, and PDW, according to age and sex, were evaluated with linear regression statistics. RESULTS: The number of platelets presented a negative correlation with age (r = -0.31, p = 0.001), whereas the mean platelet volume showed a positive correlation (r = 0.29, p = 0.001). The platelet distribution width correlated positively (r = 0.13, p = 0.001) and thrombocytocrit, negatively (r = -0.08, p = 0.05), with no evident variation in relation to age or sex. CONCLUSIONS: The decrease of platelet number with the subsequent increase of mean platelet volume in relation to age possibly resulted from the platelets having become more efficient in carrying out their haemostatic function. The values of platelet indices obtained in this study were similar to those of children living at sea level and, therefore, may be used as reference values in pediatric patients with clinical problems related to these cells.

Dietary sodium, an independent determinant for urinary deoxypyridinoline in elderly women. A cross-sectional study on the effect of dietary factors on deoxypyridinoline excretion in 24-h urine specimens from 763 free-living healthy Japanese.

OBJECTIVE: To investigate the effect of sodium, protein and calcium in habitual diets on bone resorption. DESIGN: A cross-sectional study. SETTING: A community-based study. SUBJECTS: Healthy free-living male (n=342) and female (n=421) volunteers aged 20-79 y recruited for a health and nutrition examination survey conducted by a local government. METHODS: Bone resorption was assessed by the measurement of free deoxypyridinoline in 24 h urine. Dietary assessment was by one-day dietary record method. Sodium and urea in 24 h urine were also measured for assessment of sodium and protein intake. The relationships between deoxypyridinoline excretion and the dietary factors were examined using correlation and multiple regression statistics. RESULTS: In women aged 50-79 y, protein intake was positively associated with deoxypyridinoline excretion (P<0.05); and the urinary deoxypyridinoline/creatinine ratio was positively associated with both the urinary calcium/creatinine ratio (P<0.01) and sodium/creatinine ratio (P<0.05). In the multiple regression model, the association of the deoxypyridinoline/creatinine ratio and sodium/creatinine ratio was still significant (P<0.05) after adjustment for possible confounders, in this sex and age group. CONCLUSION: In elderly women, habitual excess sodium in diets may result in bone loss through accelerated bone resorption.

Adjustment of cardiac repolarization to changing cycle length in healthy infants.

This study describes the relation of cardiac repolarization (QT interval) to a changing heart rate (RR interval) in healthy infants, utilizing abbreviated electrocardiographic (ECG) recordings. A total of 105 limb lead ECG recordings were made in 53 infants of postconceptional age 30 to 52 weeks over a 5- to 15-minute period in order to obtain a heart rate variation of at least 20%. The QT and preceding RR intervals were measured during one cycle from each 3 second ECG segment (minimum 40 cycles) and the log QT-log RR relation was examined by linear regression statistics. Average RR was 387 ms (SD 42) and the average log QT/log RR slope 0.39 (SD 0.16). The slope correlated negatively with RR and was unassociated with age or sex. In 7 of the infants a single recording showed a QT-RR regression that failed to meet the 5% level of significance; 6 of these infants had additional tracings with a significant regression. When the log QT-log RR regression was studied in 20 continuous rhythm strips with spontaneous rate variation, the analysis of 9-32 consecutive cycles yielded an average slope of 0.13 (SD 0.07); in more than one-half of these strips the QT-RR association lacked significance at the 5% level. Medical treatment to prevent recurrent calcium urolithiasis. A guide to critical appraisal.

Among patients with urolithiasis, the recurrence rate is 10-23% per year. We have applied guidelines for critical appraisal to 46 publications addressing the efficacy of thiazides, orthophosphates, cellulose phosphate, allopurinol, magnesium and citrate as prophylaxis against recurrent urolithiasis. The 34 studies which do not have a randomly allocated control group are subject to methodologic deficiencies such as co-intervention, variable outcome measures, variable natural history, statistical regression to the mean, selection bias and incomplete follow-up of patients. These deficiencies make conclusions regarding the efficacy of an intervention suspect. Among the 12 randomized clinical trials are 5 thiazide, 2 orthophosphate, 4 allopurinol and 1 magnesium intervention. The methodologic and statistical questions addressed were: adequacy of randomization, clinical relevance of outcomes, description of patients, clinical and statistical significance, and completeness of follow-up. Based on these methodologic considerations, one could not conclude that orthophosphates, cellulose phosphate, magnesium or citrate were efficacious in preventing recurrent urolithiasis. Two of the 5 thiazide and 1 of the 4 allopurinol randomized clinical trials demonstrate convincing evidence for efficacy of these interventions. With the exception of pilot studies of new interventions, conclusions about efficacy of interventions claimed to decrease the urolithiasis recurrence rate should be based on methodologically sound randomized clinical trials.

A non-parametric method of reconstructing single-dose survival curves from multi-fraction experiments.

PURPOSE: A method of estimating the single-dose curve from designed multifraction experiments is described and applied to three datasets. MATERIALS AND METHODS: The method, which is non-parametric and based on standard statistical regression techniques, can be used for functional endpoints which are either continuous or binary. The datasets are concerned with wound healing on mice, myelopathy in guinea pigs and spermatogenesis in mice. The results are compared with the results from fitting the linear quadratic model. The statistical methods of Bootstrapping and residual plots are illustrated. RESULTS: The method is based in part on an assumed statistical model, however, exact knowledge of the correct statistical model is not necessary to obtain an estimate of the shape of the single-dose survival curve. We find no good evidence from the reconstructed single-dose survival curve of an "induced repair" phenomena at low doses for the wound healing and spermatogenesis experiments. For the myelopathy experiment the data are consistent with the LQ model with a low alpha-beta ratio down to doses of at least 1.5 Gy per fraction. CONCLUSIONS: A robust statistical method of estimating the shape of the single-dose survival curve is demonstrated using standard statistical software on three datasets.

Use of regression analysis and flow cytometry for determining levels of mixed semiallogeneic immune chimerism.

It has been shown that tolerance or specific immunologic nonresponsiveness in various lymphohemopoietic transplant models can be associated with the development of mixed lymphoid chimerism. As a specific example, composite tissue (limb) allografts were studied as a model for vascularized bone marrow transplantation (VBMT) and it was demonstrated that development of stable cellular immune chimerism is associated with long-term allograft survival. Recently, studies were initiated using a new parental to hybrid VBMT model, but the detection of donor cells is complicated, due to the fact that they share one parental allotypic determinant. Therefore, regression analysis with a flow cytometric immunofluorescent staining assay was evaluated for the assessment of cellular lymphoid chimerism in donor parental to hybrid (P-->F1) lymphohemopoietic transplant models. Standard curves consisting of known mixed populations of parental donor (Lewis, LEW) and hybrid host F1 (Lew x BN, LBN) lymphocytes were established. Standard curves were analyzed by linear regression statistics and excellent coefficients of determination (r > .881) were obtained for all standard curves. A highly statistically significant (p < .016) linear relationship between level of donor cell chimerism (independent variable) and percent stained (dependent variable) was determined. The technique was then evaluated using the parental to hybrid VBMT model. Levels of donor LEW lymphoid chimerism in all VBMT LBN recipients were successfully assessed by regression analysis and inverse prediction using distinct recipient allodeterminant markers. In conclusion, this technique was proven to be reliable and accurate for the detection of of chimerism in parental to F1 lymphohemopoietic allograft models.

Geographical variation of seropositivity to Ehrlichia risticii (equine monocytic ehrlichiosis) of horses in New York state.

A total of 2,579 serum samples from horses in New York state during 1985-1986 were examined for seropositivity to Ehrlichia risticii using the indirect fluorescent antibody technique. Cluster analysis statistical technique was used to group counties according to their estimated EME-disease rate (seropositive proportion of sampled horses). Counties were clustered into 4 groups of different EME-disease rates, representing high (86% seropositive), medium (66% seropositive), medium-low (47% seropositive) and low (6% seropositive) risk regions. The logistic regression statistical technique was used to examine whether the geographic variation of E. risticii seropositivity is due to other EME-risk factors. Risk factors known to be associated with EME (age, breed, sex, month of sample submission) have not contributed to the apparent geographical variation of the disease in New York state. This lack of an association between equine host factors or temporal factors with the geographical variation of EME suggests that conditions necessary for establishment of EME endemic foci involves other factors. The county equine population density and presence of a major equine racetrack were not associated with the geographical variation of the disease in New York state. The high risk regions for EME were located predominately along river valleys, but an explanation of why EME activity varies geographically remains speculative.

Changes in renal vein, renal surface, and urine oxygen tension during hypoxia in pigs.

To determine whether ureteral urine oxygen tension could serve as a monitor of renal hypoxia and its relationship to other renal O2 tension parameters, we simultaneously measured femoral artery (PaO2), renal vein (PrvO2), renal surface (PrsO2), and ureteral urine (PuO2) oxygen tensions in 8 anesthetized pigs while incrementally decreasing the inspired oxygen concentration (FiO2) from 21% to 12%. Renal artery blood flow, measured by transit time ultrasound, renal oxygen consumption, and thermodilution cardiac output, was constant. Changes in PaO2, PrvO2, PrsO2, and PuO2 caused by decreasing FiO2 were evaluated by one-way analysis of variance. The relationships between PuO2 and the other O2 tension parameters were evaluated by correlation coefficient and linear regression statistics. Of six possible O2 decrements (combinations of 3, 6, and 9%), only PrvO2 significantly decreased with all six decrements. PuO2 decreased when FiO2 decreased 6% or more. PuO2 is not a sensitive indicator of systemic hypoxia. Under constant renal perfusion and oxygen consumption, PuO2 had a correlation coefficient of 0.80 and a regression equation of PuO2 = 0.84 (PrvO2) + 11.6, with PrvO2. PuO2 is related to PrvO2 when renal perfusion is constant.

Multiple logistic regression modelling substantiates multifactor contributions associated with sick building syndrome in residential interiors in Mauritius.

This paper presents a mathematical model that depicts the relationship between the possibility of occurrence of common health problems and factors leading to Sick Building Syndrome symptoms in domestic interiors in Mauritius. The prevalence of upper respiratory symptoms (dry eyes, runny nose), central nervous system symptoms (headache, nervousness), and musculoskeletal symptoms (pain/stiffness in shoulders/neck) were found to be elevated when responses were statistically regressed to type of building and age of respondents. The model presented here will be useful in helping to identify and quantify the relative role of factors that contribute to Sick Building Syndrome. Thus it may be possible to evaluate the effectiveness of current building operation practices and to prioritise allocations of resources for reduction of risk associated with Indoor Environmental Air Quality.

Statistical analysis of productivity in one physical therapy department.

The purposes of this retrospective study were to document productivity-relevant measurements, describe the relationships between these measurements, and discuss the implications of the relationships in one physical therapy department. The department's inputs (clinician and staff hours worked), outputs (patient visits, patient procedures, hours charged, and dollars charged), and productivity (staff utilization, financially productive time, and dollars charged per staff hours worked) were monitored. Pearson product-moment correlations, partial correlations, and linear regression statistics were calculated to determine the relationships between the variables measured. During the 30-month period studied, average daily census of the institution decreased, clinician hours worked decreased, patient procedures and dollars charged increased, and the three productivity measures increased. The relationship between output measures and clinician hours worked was apparent only after the effects of increased productivity were controlled. Departmental outputs and productivity were independent of the institution's census. The findings provided no evidence that decisions regarding staffing should be based on institutional census.

Masculinity, femininity, type A behavior, and psychosocial adjustment in medical students.

The freshman class of a midwestern medical school completed measures of masculinity and femininity, Type A behavior, and a variety of dependent variables concerning psychological well-being, adjustment, and interpersonal satisfaction. Appropriate statistical treatment of the data revealed strong and consistent masculinity effects on neuroticism, depression, self-esteem, confidence, hedonic capacity, locus of control, and relationship satisfaction. Femininity main effects varied in number as a function of the statistical method employed and involved a more diverse group of variables than is typically reported. Additive androgyny formulations of mental health were supported; balance androgyny formulations were not. No evidence for a Type A X Masculinity effect on adjustment was found. Discussion focuses on the correct interpretation of masculinity and femininity scales, comparability of analysis of variance (ANOVA) and multiple regression statistical analyses, and the viability of the concept of androgyny.

Regression analysis of digital ridge counts on pattern intensity index: a developmental interpretation.

Regression analyses of digital ridge counts on pattern intensity index (PII) are performed on data from three populations. The results for two Papua New Guinean populations (608 subjects from Karkar Island and 397 from Lufa sub District) are compared with those for a dermatoglyphically very different population from the Faroe Islands (296 subjects). Digital relationships are examined within each population. Equating the statistical regression of digital ridge counts with the embryological regression of apical volar pads, some biological interpretations of the findings are suggested. The results indicate that developmental fields across the embryological hand interact with genotype to produce digital differences in dermatoglyphic phenotype.

Generation 04 sodium slides and generation 04 reference fluid evaluated for use with the Ektachem 400 Analyzer.

Evaluation of a new generation of sodium slides and reference fluid for use with the Kodak Ektachem 400 Analyzer demonstrated improved performance in generation 04 over the previous generation 03 slide for sodium determinations. A decrease in between-replicate imprecision and a correction of the "serum" assigned values of the calibrators used to calibrate the slide used with the reference fluid account for the improvement. When generation 04/Ektachem results (y) were compared with results obtained with a Beckman ion-selective electrode (x), linear regression statistics were: slope = 0.88, y-intercept = 15.4 mmol/L, and Syx = 2.1 mmol/L. The average fixed bias in comparison with the Beckman electrode decreased from +1.9 mmol/L with generation 03 to -0.6 mmol/L with generation 04 materials. Random error is indicated by a decrease in Syx from 2.8 mmol/L for generation 03 to 2.1 mmol/L for generation 04. Application of Deming's regression, a more accurate portrayal of the generation 04 sodium data set, produces a slope of 1.0 and a y-intercept of -0.6 mmol/L.

Preoperative factors that predict hospital length of stay after distal arterial bypass.

PURPOSE: The purpose of this study was to identify the preoperative factors that are predictive of prolonged hospital length of stay (LOS) and to discuss strategies that might assist in minimizing LOS for this select group of patients. METHODS: Two hundred seventy-five arterial bypass procedures with the in situ technique were performed between 1986 and 1993. The relationship between 14 preoperative variables and hospital LOS was analyzed with both univariate (Kaplan-Meier) and multivariate (Cox regression) statistical techniques. A model was developed to determine the significant preoperative variables that were associated with prolonged LOS. RESULTS: The primary and secondary patency rates and limb salvage rates at 4 years were 73.3% +/- 3.2%, 78.9% +/- 2.9%, and 81.9% +/- 3.2%, respectively. The median postoperative LOS was 15 days, with a mean +/- SD of 17.8 +/- 12.3 days (range 4 to 93 days). With Cox regression analysis, the variables that were significant predictors of LOS (with a model p value < 0.002) were age (greater than 74 years vs less than 75), history of cerebrovascular disease (transient ischemic attack, stroke, past carotid endarterectomy vs nil), and operative indication (limb salvage vs disabling claudication). CONCLUSIONS: This study illustrates that certain preoperative variables are predictive of prolonged postoperative LOS after in situ bypass. The significant preoperative factors identified should be used to direct specific care and discharge planning for these individuals.

Multiwavelength discrimination and measurements of a two-gas mixture by use of a broadly tunable mid-infrared semiconductor laser.

Spectroscopic detection of gases can be achieved by measuring a few species-specific absorption lines, requiring very accurate wavelength control. Alternatively, it can be achieved by using many wavelengths spread over a wide range; each wavelength need not be optimal spectroscopically, but all collectively form a unique fingerprint for the species of interest. Statistical regression can be used to quantify their concentrations. An experimental evaluation of this concept involved using a 3.1 microm broadly tunable Sb-based mid-IR laser to discriminate and measure mixtures of acetylene and water vapor with absorption spectral overlaps. As many as 30 wavelengths from approximately 3200 to approximately 3280 cm-1 were used to measure 5 x 5 combinations of the two-gas concentration. Statistical analysis of the results validates the concept. Each gas concentration was consistently and reliably measured without any problem of interference from the other. In addition, the method was sufficiently sensitivite to detect unusual discrepancies by use of statistical analysis. Optimization of the system's detection capability and its receiver-operating characteristics is demonstrated. The results suggest that the statistical multiwavelength broadband approach to detection of gas mixture can be a highly effective alternative to species-specific single-line spectroscopy.

Non-infectious lung complications are closely associated with chronic graft-versus-host disease: a single center study of incidence, risk factors and outcome.

Non-infectious lung complications (NILC) are frequent, influencing morbidity and mortality of patients after allogeneic BMT. Although the term NILC encompasses a number of different entities, an association with GVHD has been noted for almost all of them. Our study was directed towards assessing the incidence and risk factors for developing NILC, as well as the response to treatment and long-term outcome. Forty (14.7%) out of 272 patients surviving for more than 3 months after allogeneic BMT, developed lung complications fulfilling the criteria for NILC. The evaluation was based on clinical investigation, radiologic imaging, lung function tests, broncho-alveolar lavage and biopsies. Risk factors were assessed by univariate and multiple statistical regression models, where chronic GVHD proved to be the only significant risk factor for the development of NILC (P = 0.011). In three patients NILC developed in direct association with donor lymphocyte infusions. The majority of patients responded well to treatment with corticosteroids and immunosuppressive drugs. NILC had no adverse effect on survival. The frequency of NILC was low in autologous (5%) as compared with allogeneic transplants (14.7%) but this difference was not statistically significant. Representation of passive hindlimb postures in cat spinocerebellar activity.

1. We report here about the modulation of dorsal spinocerebellar tract (DSCT) activity by limb posture. In principle, DSCT activity could represent limb position in one of several ways. According to a classical notion of DSCT function, DSCT activity might be expected to correlate with changes in individual joint angles. However, given the evidence for extensive polysynaptic convergence onto DSCT units, it is reasonable to propose that DSCT activity represents more global variables such as the orientation of limb segments or the length and orientation of the whole limb. 2. In six anesthetized cats we recorded the activity of 96 antidromically identified DSCT neurons while a robot arm passively positioned the left hindfoot in 20 positions distributed in the sagittal plane, holding each position for 8 s. For each position we measured the joint angles, limb segment angles, and the length and orientation of the limb axis (defined as the line connecting the hip joint to the hindpaw). We used regression statistics to quantify 1) possible relationships among geometric variables of the hindlimb and 2) relationships between DSCT firing rate and limb variables. 3. First, we found a statistically significant relationship among the joint angles that could be described by a covariance plane accounting for approximately 70 percent of the total variance. Thus the 3 degrees of freedom represented by the joint angles in the sagittal plane are effectively reduced to only 2 by the coupling between joints. This finding resembles that described for the behaving cat during stance. However, the correlation between the hip and ankle angles in the passively displaced hindlimb was just the opposite of that observed during active stance. Moreover, we observed that the length and the orientation of the limb axis is determined simply by a linear combination of the three joint angles. 4. Most of the DSCT neurons (82 of 96) were significantly modulated by changes in foot position (1-way analysis of variance, P < 0.001). For those cells, we explored systematically how their activity was related to limb geometric variables. We found mostly linear relationships between individual joint or limb segments angles and DSCT firing rates. However, although these relationships were statistically significant, the random variance was often quite high. Moreover, approximately 70% of the cells were modulated by more than one joint or limb segment angle, suggesting that a model incorporating global geometric variables might explain a larger fraction of the variance in the neural data. 5. Consequently we tested how well DSCT activity was modulated by the length and the orientation of the limb axis with the use of a linear regression model with length and orientation (or the equivalent linear combination of joint angles) as predictors. We found that this model explained a larger fraction of the variability in the firing pattern of nearly every modulated cell than did any of the single joint models tested. 6. We also attempted to account for the effect of the mechanical joint covariance on this result by accounting for correlated independent variables in the analysis. We used a regression model incorporating all three joint or limb segment angles and performed a backward elimination of insignificant or redundant variables. The result was that 67% of the neurons were independently modulated by at least two joint angles, indicating that the modulation did not necessarily reflect the biomechanical constraint of joint angle covariation, but rather a central convergence of sensory information from more than a single joint. 7. From these results we conclude that the firing rates of a majority of DSCT neurons encode the position of the hindfoot relative to the hip joint.(ABSTRACT TRUNCATED)
