Pancreatic cystadenoma of the spleen.

A cystadenoma of the pancreas within the spleen from pancreatic heterotopia is described. The tumor was located in the hilum of the spleen and completely separated from the normal pancreatic tail. This fact strongly supports the possibility that this tumor arose from pancreatic heterotopia.

Metastasis of a well-differentiated liposarcoma in a dog and a note on nomenclature of fatty tumours.

A slow-growing mass in the left thigh of a 7-year-old spayed female fox terrier dog was originally diagnosed as an infiltrative lipoma by surgical biopsy. Necropsy findings one year later revealed multiple masses made up of well-differentiated adipocytes in the spleen, liver, lungs and a lymph node. The final histopathological diagnosis of these masses and, in retrospect, for the initial leg mass was well-differentiated liposarcoma. This case illustrates some of the confusion in current nomenclature of fatty tumours. Tumours made up of well-differentiated adipose cells which show no tendency toward invasion of surrounding tissue should be designated lipomas. We suggest that fatty tumours characterized by local tissue invasion and/or metastasis should be classified as liposarcomas. Liposarcomas may be further subdivided into well-differentiated and poorly-differentiated types.

Laparoscopy in the abdominal staging of melanoma.

A retrospective study on laparoscopic results in the abdominal staging of 297 patients with melanoma, is reported. In 195 (99.3%) of patients, laparoscopy was technically feasible, and in 10% the clinical stage was changed on the basis of the findings. In 34% of patients with metastasis of the liver and peritoneum, the tumor had a diameter of under 1 cm and therefore was not detectable with non-invasive techniques. In the light of these results laparoscopy is advisable in patients with Stage I melanoma at a high risk of metastatization (Clark IV-V; Breslow greater than 0.76 mm) and in Stage II patients. In Stage III patients, laparoscopy seems indicated for an evaluation of the efficacy of systemic treatment.

Comparative carcinogenicity of o-toluidine hydrochloride and o-nitrosotoluene in F-344 rats.

o-Toluidine hydrochloride and one of its metabolites, o-nitrosotoluene, were administered in the diet (0.028 mol/kg diet) to 2 groups of 30 male F-344 rats for 72 weeks. o-Nitrosotoluene induced significantly more tumors of the bladder (16/30 rats) and liver (20/30) than did o-toluidine hydrochloride (bladder, 4/30; liver, 3/30. Both compounds induced comparable numbers of peritoneal tumors and fibroma of the skin and spleen. o-Toluidine hydrochloride induced more mammary tumors (13/30) than did o-nitrosotoluene (3/30). The results indicate tha N-oxidation is important in the induction of bladder and liver tumors by these single ring compounds but that other mechanisms could be involved in the induction of peritoneal, skin, spleen and mammary tumors.

Color Doppler imaging of focal splenic masses.

UNLABELLED: We report the sonographic findings in 43 patients with focal splenic masses. A differential diagnosis of splenic tumors was attempted on the basis of echotexture, size, vascularity and patterns of blood flow within and around tumors on color Doppler sonography. Using the surrounding splenic tissue as an in vivo reference, markedly hypervascular (n = 3), hypervascular (n = 2), isovascular (n = 4), hypovascular (n = 3), and avascular (n = 31) tumors could be differentiated. The final diagnoses were splenic infarction (n = 13), splenic rupture (n = 9), splenic metastasis (n = 7), splenic lymphoma (n = 3), splenic abscess (n = 4), hemangioma (n = 3), splenic cyst (n = 3) and splenoma (n = 1). In 17 cases (40%) color Doppler provides no further information for diagnosis, color Doppler was helpful in 21 patients (49%) and in four (9%) cases, Doppler sonography detects unexpected flow phenomena and was necessary for final diagnosis and treatment. In one patient (2%) sonographic diagnosis was incorrect. CONCLUSION: Characteristic patterns of blood flow were seen in different splenic tumors and can, therefore, aid in the differential diagnosis of focal splenic lesions, and identify patients at risk for serious complications.

Epithelioid hemangioendothelioma of spleen with intrasplenic metastasis: ultrasound and computed-tomography appearance.

The US and CT appearances of epithelioid hemangioendothelioma of the spleen with intrasplenic metastasis have not been previously reported. We described a 29-yr-old female with such a disease. Abdominal US study revealed a large mass in the upper pole and multiple small nodules in the rest of the spleen. CT scan also showed similar lesions of hypodensity which were not apparently enhanced by contrast medium.

Spinal and extracranial metastatic dissemination of malignant glioma.

Metastatic dissemination of malignant glioma is rare. Metastatic disease usually occurs within the neuraxis but extracranial spread to the liver, spleen, lung, peritoneum and lymph nodes can also occur. In most cases of metastatic disease, the patient has previously undergone a craniotomy. The prognosis is uniformly poor. The vast majority of patients do not survive beyond six months from diagnosis of metastatic spread of a primary intracranial tumour. The pathophysiology and natural history of this condition is still not fully understood, hence well-designed prospective studies are needed.

The spleen: development and functional evaluation.

Despite the fact that the spleen has multiple functions, only one has been widely used for evaluation of the organ by imaging techniques (phagocytosis of 99mTc sulfur colloid). The usual splenic uptake of this radiocolloid can by used to determine the size, location, and integrity of the organ. A major use of splenic radiocolloid imaging has been in the study of congenital defects. Thus, eventration of the diaphragm, accessory spleens, splenogonadal fusion, the asplenia and polysplenia syndromes, and the wandering spleen are amenable to study by means of intravenously administered radiocolloid. Interference with the splenic uptake of radiocolloid can be either focal or generalized (as in functional asplenia). Imaging of the spleen has a major role in evaluating suspected trauma of the organ and in following its clinical course. The return of splenic function after splenectomy (splenosis or accessory spleens) can be documented by radionuclide imaging, and likely by hematologic techniques when the volume of tissue is sufficiently large. The detection of intrasplenic lesions is important in tumor staging and as an alerting sign to an ongoing process.

Biological behavior and cell properties of new AKR lymphoma malignancy variants.

The AKR lymphoma-leukemia is a T lymphocyte neoplasm, most suitable as a model for human T cell malignancies. We have been interested in the process of tumor progression in the AKR lymphoma system. In the present study, two newly isolated variants, the TAU-42 and TAU-44, were characterized with respect to their biological behavior, by comparing them to a previously studied low-malignancy variant, the TAU-39. While the TAU-44 variant formed large s.c. local tumors, the TAU-42 variant formed only small growths or none at all. The TAU-42 lymphoma was found to have the highest malignant potential: it displayed very marked dissemination to spleen, lymph nodes, liver and lungs. The TAU-44 variant had an intermediate degree of metastatic potential but presented a predilection for spread to lymph nodes and spleen and was sometimes found to metastasize to peculiar organs, such as heart and pancreas. Cells derived from the different lymphoma variants varied in their immunophenotype: the highly malignant variant cells expressed more CD4 antigen than the low-malignancy one. The opposite was observed with regard to CD8. The variant cells also differed in their migrating capacity, the more malignant one exhibiting a higher motile activity. Studies on the tumor progression model of AKR lymphoma might contribute to the elucidation of the features determining the aggressiveness of T lymphocytic malignancies.
