The burden of conditions not attributable to mental disorders.

OBJECTIVE: The purpose of this study was to extend previous findings on the relationship of V code conditions to use of psychiatric services and treatment outcome. METHOD: The group under study involved 2,542 outpatients from three community mental health centers in Nova Scotia, Canada. From this group, three subgroups of patients were compared: 1) patients with V code conditions, 2) patients with DSM-III-R mental disorders, and 3) patients with both V code conditions and mental disorders. RESULTS: The majority of patients (61.8%) were diagnosed with V code conditions, and a substantial minority (19.6%) had V code conditions as the sole diagnoses. Patients with V code conditions without mental disorders were similar to patients with mental disorders in consumption of treatment resources and treatment outcome. CONCLUSIONS: Strategies for improving the efficiency of mental health services to patients with V code conditions need to be developed.

Effects of video-assisted training on employment-related social skills of adults with severe mental retardation.

Two studies investigated effects of video-assisted training on employment-related social skills of adults with severe mental retardation. In video-assisted training, participants discriminated a model's behavior on videotape and received feedback from the trainer for responses to questions about video scenes. In the first study, 3 adults in an employment program participated in video-assisted training to request their supervisor's assistance when encountering work problems. Results indicated that participants discriminated the target behavior on video but effects did not generalize to the work setting for 2 participants until they rehearsed the behavior. In the second study, 2 participants were taught to fix and report four work problems using video-assisted procedures. Results indicated that after participants rehearsed how to fix and report one or two work problems, they began to fix and report the remaining problems with video-assisted training alone.

Social validation of component behaviors of following instructions, accepting criticism, and negotiating.

This study evaluated whether behaviors often taught as part of social skills training are judged favorably by others. Community judges evaluated the performances of people in various situations requiring one of three social skills: following instructions, accepting criticism, and negotiating to resolve conflicts. These skills were displayed in videotaped scenes by actors with and without mental retardation who acted out roles that had different types of authority relationships, and when different components or clusters of behavior (nonverbal, specific verbal, or general verbal behaviors) were performed well or poorly. The highest ratings by judges were of videotaped scenes that depicted correct use of all behaviors, regardless of which skill was being examined, whether or not the actor had mental retardation, or what the relationship was between the two actors. The lowest ratings were of videotaped scenes that depicted poor performance of all behaviors, and intermediate ratings were obtained when only some of the behaviors were performed poorly. These results, as well as the verbal responses of judges to questions, indicated that the different behaviors commonly used in teaching the skills of following instructions, accepting criticism, and negotiating are relevant to judgment of social performance, and are likely to be reinforced and maintained by social contingencies.

Social cognitive factors in brain injury-associated personality change.

Often social difficulties are the most enduring difficulties after a head injury. The social deficits most frequently reported by relatives 6-12 months post-injury include egocentric styles of social interaction characterized by family members as a change in the head-injured patient's personality style. The described characteristics of head-injured patients in terms of social perspective-taking abilities are similar to those of children reported in the cognitive developmental literature. To demonstrate the nature of the social deficits in head-injured participants, social perspective-taking paradigms in the development literature were used to construct a task to be administered to both head-injured and non-head-injured adults. Results of the comparison between the two groups suggest deficits in social perspective-taking abilities in head-injured adults similar to the level demonstrated by pre-adolescent children.

Space, territory and psychiatry.

Space and territory have been the hidden dimensions of human and other animal life. Beginning with studies reported in animals, and incorporating the few formal studies and several informal observations of man, this paper explains the importance of mastering the unwritten rules of space and territory which differ with individual cultures in relation to the behavioral aberrations possibly resulting from the disrupted territoriality of modern urban life.

[Psychiatry and mental disorders].

The analysis of the relations between social order and mental disorder is first based on the social system considered as the order of the differences and of the significant disparities in a stratified and hierarchial society. As psychiatry belongs to the system, it received the monopoly of access to the symbolic and mental dimension which is included in the disorders. We intend to show by some examples how psychiatry applies in this way a specific social order. Then we analyse the results of the setting of this order, results which are different according to the social position of the patients and their status in the psychiatric system. The efficiency of symbolic domination is not only for patients with disorders but also and more basically for the whole social system and the maintaining of its order.
