Coexistence of struma ovarii and Graves' disease.

A 30-year-old woman with hyperthyroidism due to coexisting Graves' disease and struma ovarii is described. Physical examination revealed a diffusely enlarged thyroid and an abdominal mass. CT scan of the abdomen showed a 25-cm predominantly cystic right ovarian mass. 123I uptake and scan showed 75% thyroid uptake at 4 h and a focus of intense uptake in the left upper quadrant of the abdomen. Preoperative treatment for the hyperthyroidism included propylthiouracil, propranolol, and ipodate. Surgery was performed and pathologic examination of the ovarian mass revealed struma ovarii.

Metastatic thyroid carcinoma in the presence of struma ovarii.

A 27-year-old woman presented with metastatic follicular carcinoma of thyroid cell type. Assessment was complicated by recent surgery for "benign" struma ovarii. She underwent total thyroidectomy which failed to show a neoplasm, leaving the struma ovarii as the likely source of the metastasis.

Recombinant human thyrotropin for the diagnosis and treatment of a highly functional metastatic struma ovarii.

The optimal treatment of metastatic thyroid cancer that produces high amounts of thyroid hormone has not been well defined. A 46-yr-old woman presented with a follicular thyroid carcinoma arising from a struma ovarii with hepatic metastases. After the removal of both the struma and the thyroid gland, the liver metastases showed evidence of a high degree of hormonogenesis. Brain, chest, abdomen, and bone imaging was negative for additional metastases. Because iodine uptake by most thyroid carcinomas is quite low in the absence of high levels of ambient TSH, we used recombinant human TSH (rhTSH) (Thyrogen) to achieve a concentration of 131I activity in the tumor high enough for a significant cytotoxic effect. After rhTSH administration (0.9 mg im daily for 2 consecutive days), a 131I diagnostic whole body scan confirmed the existence of 17 discrete hepatic foci of 131I uptake. To calculate the amount of 131I that would deliver an absorbed radiation dose that would be optimally cytotoxic to the metastases (>8000 rad/lesion) and not to the normal liver, we performed lesion dosimetry. Analysis of dosimetric data showed that 15 of 17 lesions would receive an adequate radiation dose following the administration of 65 mCi of 131I. Additionally, we performed whole body dosimetry to assure that this dose would not cause bone marrow toxicity. The patient was reevaluated 6 months after therapy; the liver metastases showed significant, but partial, response. In conclusion, we used the combination of rhTSH with lesional and whole body dosimetry for the treatment of highly functional metastases from follicular thyroid carcinoma arising within a struma ovarii. This strategy can be applied to determine a safe and effective dose of 131I for the treatment of any thyroid cancer metastases that produce enough TH to preclude stimulation of endogenous pituitary TSH secretion.

Struma ovarii: hyperthyroidism in a postmenopausal woman.

A rare case of struma ovarii producing hyperthyroidism in a postmenopausal woman is reported. The ovarian tumor demonstrated uptake of both [99mTc]pertechnetate and 131I, allowing preoperative diagnosis of the condition. In females with unexplained hyperthyroidism and low 131I uptake by the cervical thyroid gland, imaging of the pelvis should be considered.

[A case report of postoperative thyroid crisis accompanied with struma ovarii].

A 39-year-old female without any specific past history was scheduled to receive an operation for myoma uteri and ovarian cyst. She was premedicated with atropine. Anesthesia was induced with thiopental and was maintained with nitrous oxide and enflurane. Tachycardia shortly after premedication with atropine and remarkable sweat during the operation were observed. On the 1st postoperative day an outbreak of thyroid crisis as well tachycardia of 180.min-1 and fever (39.3 degrees C) were observed. Such outbreak of thyroid crisis indicated that the patient had been suffering from Grave disease. Pathological diagnosis of extirpated ovarian cyst was struma ovarii. It is, however, still uncertain whether struma ovarii induced thyroid crisis in this case. It might be concluded that screening of hyperthyroidism at preoperative rounds is essential for prevention of thyroid crisis.

Follicular carcinoma in a functioning struma ovarii.

We describe a case of follicular carcinoma in a functioning struma ovarii, which presented as an ovarian mass in a patient who had undergone a near-total thyroidectomy for a benign lesion. She underwent bilateral salpingo-oophorectomy and received radiotherapy and L-thyroxine treatment with no evidence of metastases in 4 years follow-up.

Metastatic struma ovarii: the burden of truth.

A 58-year-old woman was found to have metastatic thyroid cancer in her liver. This was identified when she was investigated for upper abdominal pain and underwent biopsy of hepatic lesions. She had no palpable thyroid nodule and had a normal ultrasound of the thyroid. Previously, both ovaries had been removed because of tumors. The pathologic findings in one of the ovaries could not be recovered because the procedure was more than 40 years ago, when the patient was a teenager. By a process of elimination, a diagnosis of metastatic struma ovarii was established. Treatment of metastatic thyroid cancer from struma ovarii, including removal of the normal thyroid and administration of I-131, is presented.

Struma ovarii presenting as acute pseudo-Meigs syndrome with an elevated CA 125 level.

A 62-year-old woman presented with acute hydrothoraces and ascites. The CA 125 level was greatly elevated and pelvic ultrasound revealed an adnexal mass with solid and cystic components. At surgery a benign pure struma ovarii was diagnosed and following removal of the tumour the patient had immediate and complete resolution of her symptoms. This is the first report of struma ovarii causing both pseudo-Meigs syndrome and a marked elevation of CA 125.

[Struma ovarii: rare ovarian neoplasm].

Authors of this research present 3 cases of struma ovarii diagnosed in postoperative histopathological examination in years 1994-1997. The first one was accidentally discovered during examination of a woman in menopausal period, the second was a case of 39-years old woman with hyperthyroidism and struma parenchymatoso-nodosa and the third was detected during examination of a young 21-years old woman with abdominal pain. In first two cases Doppler flowmetry revealed low RI and PI parameters (< 0.5 and < 0.7 respectively) in tumor structures.

Malignant struma ovarii: report of two cases and selected review of the literature.

Malignant struma ovarii is an extremely rare tumor. Two new cases are described and current perspectives provided. Both cases are examples of the mixed form of the disease, and both were treated by surgery alone. One patient underwent total hysterectomy and bilateral salpingooophorectomy; the other had unilateral salpingo-oophorectomy. Both patients remained free of recurrent disease at 20 and 33 months from diagnosis. The controversies relating to therapy and diagnosis are discussed in detail.

Struma ovarii: MRI findings.

We describe the MRI findings in three cases of struma ovarii. In all three cases, MRI showed a multilocular cystic mass with a variable signal intensity within loculi. Some loculi or small cysts within septations showed low signal intensity on T1 weighted images and very low signal intensity on T2 weighted images, corresponding pathologically to gelatinous colloid material in large follicles. In one case, with Gd-DTPA enhanced T1 weighted images, the thick septations and locally thickened wall showed marked enhancement, corresponding microscopically to thyroid tissue.

Ovarian metastasis from thyroid carcinoma 12 years after partial thyroidectomy mimicking struma ovarii: report of a case.

A 29-year-old woman experienced abdominal swelling 12 years after undergoing partial thyroidectomy for follicular carcinoma of the thyroid with vascular invasion and 6 months after a right cerebral tumor that had caused hemiplegia had been excised. Laparotomy disclosed a solid and cystic right ovarian tumor, 17 cm in greatest dimension, and a 12-cm tumor of the right adrenal gland. Microscopic examination showed a malignant tumor of the thyroid follicular type with frequent mitotic figures; other teratomatous elements were absent. Because of the interval since the thyroid cancer, the diagnosis of a malignant struma ovarii was initially considered. The patient rapidly developed widespread metastatic disease and died, supporting the interpretation that the ovarian tumor was a metastasis from the thyroid neoplasm. This case illustrates that when other teratomatous elements are not identified, the diagnosis of malignant struma ovarii should be made only when the possibility of spread from a prior thyroid neoplasm, possibly one treated many years previously, has been excluded.

Malignant struma ovarii: recurrence and response to treatment monitored by thyroglobulin levels.

A patient with malignant struma ovarii who had elevated thyroglobulin levels at the time of her original surgery was diagnosed with recurrence on the basis of progressive thyroglobulin level elevation while on thyroid suppressive therapy. Subsequent treatment with therapeutic doses of iodine-137 resulted in normalization of the elevated thyroglobulin level. The patient remains disease free on thyroid replacement.

[Struma ovarii--case report of a rare ovarian tumor].

It has been reported a case of struma ovarii as an accident evidence during an operative classification of a great pelvic tumor. As it is reported in several publications we found an unilateral ovarian process. Clinical signs of hyperthyreosis or struma colli were missed. In case of benign pathological evidence treatment can be restrict to surgical tumor removal and ambulante observation.
