[Psychosomatic aspects in orthopedic legal assessment].

AIM: Complaints in orthopaedics that are not sufficiently explainable can be caused (partly) by psychosomatic disorders. A manual psychosomatic disorders for legal assessment in orthopaedics will be supplied. METHOD: Diagnoses essential for orthopaedics are classified by DSM-IV and ICD-10 in consideration of the current literature; evaluation was performed under legal assessment criterions. RESULTS: The most important complaints that are often poorly explainable results in musculoskeletal pain. Since a sensorial perception of pain is combined with emotions, emotions and psychic disorders can be perceived as pain. The most important diagnoses for assessing not sufficiently explainable complaints by somatic cause are "somatoform pain disorder", "somatization disorder", "undifferentiated somatoform disorder", "conversion disorder", "hypochondriasis", "body dysmorphic disorder", "maior depression", "dysthymic disorder", "posttraumatic stress disorder", and "adjustment disorder". The relevance of psychosomatic comorbidity in legal assessment is possible in all affairs of civil and social law. Private accident insurances exclude psychic and psychosomatic diseases from all insurance benefits. CONCLUSION: When composing legal assessments in orthopaedics, especially on pain, the knowledge of psychosomatic (co)morbidity is necessary for meeting the different causes of the complaints. Under awareness of possible psychosomatic disorders, additional psychosomatic legal assessments can be recommended and understood as well.

[Psychosomatic aspects of irritable bladder. A review].

Nervous bladder symptoms without any established organic causes would appear to be a psychosomatic illness that usually affects married women in midlife. The majority of these patients manifest serious neurotic disorders such as anxiety neurosis, hysteria, phobia, and depression or larvate depression. It is often possible to identify psychosexual disorders in these patients. The predominant situations that seem to trigger irritable bladder symptoms are: sexual intercourse that ends unsatisfactorily for physical or psychological reasons, a partnership conflict, or even a separation conflict. Patients with a hysterical personality structure who are dominated by Oedipus or phallic problems and who, by inhibiting the sexual impulse, frequently suffer from sexual disorders may, in a situation experienced in such an atmosphere of conflict, regress to the stage of urethral erotism; at this stage, the symptoms serve as self-punishment as well as reduction of the fear of guilt and punishment; the unconscious vexation and frustration manifest themselves in these symptoms. In the case of depressive patients, unconscious anger and helplessness can be manifested as irritable bladder symptoms. As a result of the close connection between these urinary disorders and sexual disorders, irritable bladder symptoms without any established organic causes would appear not to be a urological but rather a covert functional sexual disorder.

[Psychosomatic aspects of endometriosis].

Endometriosis is a varied and unspecific disorder, which can be detected by way of subtle diagnostics in almost every woman. In one case it is a minor accidental finding, in another case it is a cancer-like, mostly incurable, chronic painful a disease, which often has sterility and aggressive therapies as subsequence. For the origin of endometriosis many causes are discussed. The theory of retrograde menstruation through an hyperperistaltic-dystocic uterus presents on one hand an organic explanation-concept. From the psychosomatic point of view this unphysiological menstruation could also be seen as the result of an unsolved conflict, which might be connected with the gender-role or personal, familiar and social attitudes about menstruation. The exo- and endogenic supply with hormones in a time with less pregnancies and therefore more menstruation is also a contributing factor to the origin and development of this disturbance. Beside the theory of endometriosis as an autoimmune disease also implicates psychosomatic thoughts. Though there is always a repeatedly talk the "cancer of the career-woman", there are however very few psychosomatic research projects regarding endometriosis. Therefore it is a characteristic of this disease, that finding and feeling are very often controversial. This emphasizes the importance of psychosomatic, psychosexual, social and biographical aspects in connection with endometriosis. This point of view makes a relative plausible and comfortable explanation for the CPPS and sterility uncertain. Without question diagnosis and therapy of endometriosis followed on one hand by stigmatization and on the other hand by often restriction of quality of life leads to a lot of subsequent psychosocial problems.
