Nutrient balance studies in premature infants fed premature formula or fortified preterm human milk.

This report compares fat, nitrogen, calcium, phosphorus, zinc, and copper absorption and retention data from 13 nutritional balance studies performed in 12 appropriate-for-gestational-age premature infants with birth weights less than or equal to 1,600 g fed a proprietary premature formula or their own mother's preterm human milk (PTHM) fortified with a powdered protein-mineral supplement. At the time of each balance study, each infant had a stable condition, was tolerating feedings, and was gaining weight steadily. Stool and urine were collected separately; doses of carmine red given 72 h apart were used to define dietary intake and the stool and urine collections. The balance studies were performed at an average age of 36 postnatal days. Both diets were found to support weight gain and nutrient retention at similar rates. Balance studies (n = 7) with premature formula demonstrated a weight gain of 16.8 +/- 5.2 g/kg/day, with a net fat absorption of 6.36 +/- 0.97 g/kg/day, net nitrogen retention of 427.9 +/- 47.1 mg/kg/day, net calcium retention of 95.0 +/- 14.1 mg/kg/day, net phosphorus retention of 56.7 +/- 5.9 mg/kg/day, net zinc retention of 208 +/- 903 micrograms/kg/day, and net copper retention of 26.4 +/- 30.8 micrograms/kg/day (mean +/- SD). Comparison balance studies (n = 6) with fortified PTHM demonstrated a weight gain of 17.2 +/- 7.1 g/kg/day, net fat absorption of 5.60 +/- 1.55 g/kg/day, net nitrogen retention of 366.0 +/- 84.0 mg/kg/day, net calcium retention of 82.2 +/- 10.8 mg/kg/day, net phosphorus retention of 58.6 +/- 5.9 mg/kg/day, net zinc retention of 685 +/- 363 micrograms/kg/day, and net copper retention of 67.3 +/- 37.2 micrograms/kg/day. These rates of weight gain and of fat, nitrogen, zinc, and copper retention approximate those of the third trimester of intrauterine life; only calcium and phosphorus retention approached the lower range of estimated in utero accretion.

Studies of normal and prematurely ruptured human amniotic membranes: low calcium and magnesium in prematurely ruptured membranes.

Prematurely ruptured amniotic membranes at term, and membranes from normal term pregnancies were studied. The membranes were analyzed for neutral sugars, hexosamines, sialic acid, uronic acid, DNA, RNA, cholesterol, phospholipids, calcium, magnesium, sodium and potassium. The only significant difference found between the normal and the prematurely ruptured membranes was a significantly lower concentration of calcium and magnesium in the latter group. (Calcium 0.32 +/- 0.07, vs. 0.24 +/- 0.07, p less than 0.05; Magnesium 0.30 +/- 0.19 vs. 0.15 +/- 0.04, p less than 0.05; in mg/100 mg protein). The possible metabolic effects of low calcium and magnesium may be of relevance to the pathogenesis of premature rupture of the amniotic membranes.

Extraocular muscle changes after cryotherapy for retinopathy of prematurity and the development of strabismus in premature infants.

To find out whether the cryotherapy for the treatment of the retinopathy of prematurity (ROP) causes structural changes of the extraocular muscle (EOM), and also whether the changes are related with the occurrence of strabismus. To examine the acute stage change, we conducted a transconjunctival cryotherapy around the superior rectus muscle of a rabbit and resected it 0, 3, 7, 14, and 28 days after the cryotherapy. In observing chronic changes, we first categorized patients who had an esotropia surgery into groups, one of which combined prematurity and cryotherapy and one group affected by prematurity but without having had cryotherapy. Then we compared the change of EOM with that of a fullterm infant group. In a rabbit, edema, acute inflammatory cells and a large amount of degenerated muscle fibers were observed immediately after the cryotherapy and on the 3rd day. On the 7th day, regenerated muscle fibers were observed and on the 14th day, the inflammatory cells decreased and the amount of regenerated muscle fiber increased. On the 28th day, abnormal findings were not observed any more and the muscle was found to be normal. When chronic changes of EOM in human on 1 1/2 to 8 1/2 years after cryotherapy, there were no abnormal findings observed in three groups. From the study, we can infer that cryotherapy can cause acute inflammation and necrosis of muscle fiber but such an acute change will improve and does not result in structural change in the long term. Therefore, the occurrence of strabismus in patients with ROP is considered to be attributable to reasons other than injury of EOM.

Multicenter trial of cryotherapy for retinopathy of prematurity. One-year outcome--structure and function. Cryotherapy for Retinopathy of Prematurity Cooperative Group.

This study of the safety and efficacy of cryotherapy in treating severe retinopathy of prematurity registered 9751 infants with birth weights less than 1251 g at 23 study centers. Two hundred ninety-one infants developed a defined threshold retinopathy of prematurity, and cryotherapy was performed in approximately half of the eyes through a randomization protocol. Twelve months after randomization, results of masked grading of fundus photographs of the posterior pole were similar to results obtained 3 months after randomization, and indicated an unfavorable outcome in 25.7% of the eyes that received cryotherapy compared with 47.4% of the control eyes (P less than .0001). For the first time, masked Teller Acuity Card assessment of grating acuity was performed in this study group and indicated an unfavorable functional outcome in 35.0% of the treated eyes compared with 56.3% of the control eyes (P less than .0001). These results indicate that cryotherapy reduces the risk of unfavorable retinal and functional outcome from threshold retinopathy of prematurity.

Breathing pattern and efficiency of the respiratory muscles in healthy prematures and in prematures with respiratory distress syndrome.

The relationship between the compliance of the respiratory system (Crs), measured by mouth occlusion technique, and the the breathing pattern, the central inspiratory activity, and the performance of the respiratory muscles respectively, was studied in healthy prematures and in prematures presenting with the respiratory distress syndrome. With decreasing Crs, respiratory rate, instantaneous minute-ventilation, airway pressure at 0.1 s after end-expiratory occlusion (P0.1), effective inspiratory impedance, the average rate of change of the pressure generated during the inspiratory effort against closed airways, and the work of breathing increased; inspiratory and expiratory times decreased overproportionally. We conclude that the changes in lung mechanics predispose prematures with a Crs of less than 1 ml X cmH2O-1 to develop respiratory failure because they cannot perform the respiratory work demanded by intact central regulation mechanisms.

[Premature rupture of the membranes and RDS incidence in prematures (author's transl)].

In the years 1974, 1975 and 1976 495 children were born alive with a birthweight less than 2500 g in the Ist Department of Gynecology and Obstetrics of the University of Vienna. We selected all cases with dystrophy or factors, which were a disposal for RDS, so we got a rest of 188 prematures with a weight from 1000 g to 2500 g. For these children we checked the influence of time of rupture of the membranes (ROM) before delivery to the incidence of RDS. We found a lower incidence of RDS of the newborns when duration of ROM exceeded the time of 12 hours. This fact was statistically significant in the class of premature newborns with a weight of 1500 g to 2000 g. The similar trend was found in 119 dystrophic newborns with a weight between 1000 g to 2500 g, although there was no statistical significance. No influence of duration of labor on the incidence of RDS neither in selected premature nor dystrophic newborns was found.

Spontaneous atrophic patches in extremely premature infants. Anetoderma of prematurity.

BACKGROUND: Anetoderma, characterized clinically by macular depressions or outpouchings of skin, is associated with loss of dermal elastic tissue as noted on histopathologic findings. We report on 9 extremely premature infants who developed patches of anetoderma during their course in the neonatal intensive care unit. OBSERVATIONS: All 9 patients were born between the ages of 24 and 29 weeks of gestation and had numerous complications associated with prematurity. Eight of the 9 infants were noted to have developed anetoderma on the trunk and proximal extremities while in the neonatal intensive care unit. The locations of the lesions on the skin were not explained by previous trauma, although many areas corresponded with placement of monitoring leads or with adhesive for a monitoring device. Reduction or absence of elastic tissue supported the diagnosis of anetoderma in 4 of 5 biopsy specimens. CONCLUSION: We report a previously unrecognized type of anetoderma associated with extreme prematurity. The exact cause is uncertain, although reactions to cutaneous monitoring leads or adhesives is suspected.

D-Penicillamine for preventing retinopathy of prematurity in preterm infants.

BACKGROUND: This section is under preparation and will be included in the next issue. OBJECTIVES: To answer the question: Among very low birth weight infants, what is the effect of prophylactic administration of d-penicillamine on the incidence of acute ROP or severe ROP, and side effects including death? SEARCH STRATEGY: Searches were made of multiple electronic databases, previous reviews including cross references, abstracts, conference/symposia proceedings, and expert informants. SELECTION CRITERIA: Randomized or quasi-randomized controlled trials that administered d-penicillamine to infants less than 2000g birth weight within the day following birth were considered relevant to this review. Additional case series were examined for potential side effects. DATA COLLECTION AND ANALYSIS: Data on clinical outcomes were excerpted by 3 reviewers independently, and consensus reached. Data analysis was conducted according to the standards of the Neonatal Cochrane Review Group. MAIN RESULTS: Two randomized trials on the effects on ROP were identified. When combined, they showed a significantly lower incidence of acute ROP in the treated infants, relative risk of 0.09, 95% CI [0.01,0.71]. Severe stages of ROP could not be analyzed. There was no effect on death rates, relative risk 0.99 95% CI [0.70,1.39]. No side effects were reported, and follow up at one year revealed no significant differences in spasticity or developmental delay, although there were more rehospitalizations among the controls. In other reports of using d-penicillamine in over 140 infants for hyperbilirubinemia, skin rashes were reported in 2 infants and one had vomiting that may have been related. REVIEWER'S CONCLUSIONS: D-penicillamine is unlikely to affect survival, and may reduce the incidence of acute ROP among survivors. Studies to date justify further investigation of this drug in a broader population; careful attention to possible side effects is needed.

[The condition of premature newborns of patients hospitalized due to premature rupture of fetal membranes (PROM)].

OBJECTIVES OF THE STUDY: The aim of the study was to compare the premature newborns' condition of patients hospitalized due to PROM with the group of premature newborns of patients with amniorrhexis at the time of labor. MATERIAL AND METHODS: The studied group consisted of 74 newborns of 70 gravidas hospitalized in the Department of Pathological Pregnancy and the control group of 77 newborns of 70 patients. On analysing the investigated materials special attention was paid to: the time of PROM, the time and the way of pregnancy, the age of the, obstetrical history, the symptoms of intrauterine infection. In assessment of the newborns' condition the following factors were taken into consideration: characteristics of infection, Apgar scores, birth weight, respiratory disorders and other newborns' diseases. RESULTS: Theoretic risk of sepsis was higher in study than in control group (p = 0.00). The sepsis occurrence was comparable in both groups (7 vs 8). The infection occurred more frequently in study group. CONCLUSIONS: 1. The newborn condition depends on the week of pregnancy termination, with better prognosis for longer pregnancies and with no differences between both groups 2. In spite of statistically important risk of sepsis occurrence in study group, the real occurrence was comparable in both groups 3. There is a statistically important difference between CRP and 5' Apgar score in study group. 4. Percentiles of birthweight and birthlength appropriate for gestational age were comparable between both groups.

Fetal lung maturity. I. Mode of onset of premature labor. Influence of premature rupture of the membranes.

In a prospective study of 133 spontaneous premature deliveries the relation between premature rupture of the membranes (PRM) and development of respiratory distress syndrome (RDS) in newborn infants is examined. PRM is associated with a significantly decreased incidence of RDS in newborn infants (P less than 0.002). This relation is valid at a gestational age of 28 weeks or more and a birthweight greater than 1000 g. Total respiratory morbidity in newborn infants (transient tachypnea + RDS) is also significantly decreased when labor is associated with PRM (P less than 0.005). Assessment of the influences of sex of the infant, fetal asphyxia, and delivery by cesarian section shows that PRM bears a stronger relation than each of these individual factors to a decreased incidence of RDS. Duration of the latent period has no influence on protection from RDS, and it is suggested that fetal lung maturity occurs before the membranes rupture.

Genetic polymorphisms of platelet glycoprotein Ia and the risk for premature myocardial infarction: effects on the release of sCD40L during the acute phase of premature myocardial infarction.

OBJECTIVES: The aim of this research was to evaluate the effect of genetic polymorphisms C807T and G1648A of platelet glycoprotein Ia (GPIa), on the risk for myocardial infarction (MI) and on the release of soluble CD40 ligand (sCD40L) during the acute phase of MI and one year after the event. BACKGROUND: C807T and G1648A polymorphisms affect the density of GPIa on platelet surface, but their effect on the risk for MI and the release of sCD40L is unknown. METHODS: The study population consisted of 219 patients with premature MI and 389 controls. One year after the event, 67 patients and 232 controls were recalled for the follow-up study. RESULTS: The risk for MI in 807TT was 2.296 (95% confidence interval [CI]: 1.187 to 4.440) p < 0.05 versus CC + CT, 2.269 (95% CI: 1.085 to 4.745) p < 0.05 versus CC, and 2.135 (95% CI: 1.080 to 4.219) p < 0.05 versus CT. During the acute phase of MI, sCD40L was higher in 807CT + TT compared with 807CC (p < 0.01), an effect persisting after one year (p < 0.01). The carriage of 807T allele was an independent predictor for sCD40L during the acute phase of MI (beta = 9.442 [standard error (SE): 2.526], p = 0.001) and in the same patients one year later (beta = 8.282 [SE: 2.044], p = 0.001). In healthy individuals, 807T allele was associated with higher sCD40L levels compared with 807CC (p < 0.05), only among those with von Willebrand factor greater than or equal to median. CONCLUSIONS: Genetic polymorphism C807T increases the risk for premature MI. 807T allele is an independent predictor for sCD40L levels during the acute phase of premature MI as well as one year after the event, while it is associated with elevated sCD40L levels in healthy subjects, only in the presence of high von Willebrand levels.

Prediction models for insulin resistance in girls with premature adrenarche. The premature adrenarche insulin resistance score: PAIR score.

AIM: The purpose of this study was to develop an accurate regression model to predict insulin resistance in girls with premature adrenarche. METHODS: The insulin sensitivity index was calculated from the frequently sampled intravenous glucose tolerance test with tolbutamide. Thirty-five prepubertal girls (23 Caribbean-Hispanic and 12 African-American; mean age 6.8 years) were studied. The insulin sensitivity index was compared to birth weight, body mass index (BMI), the presence of acanthosis nigricans (AN), insulin-like growth factor 1, insulin-like growth factor binding protein 1, sex hormone binding globulin, lipid profile, and adrenocorticotropic hormone stimulated androgens. RESULTS: The best prediction models included birth weight, BMI, and AN (model 1: R(2) = 0.78) and BMI, AN, and serum 17-OH pregnenolone (model 2: R(2) = 0.76). When viewed as screening tests, a cutoff value <5.5 (premature adrenarche insulin resistance score) in both equations showed a sensitivity of 100% and a specificity of 85%. CONCLUSION: Born small for gestational age, premature adrenarche, obesity, AN, and higher serum 17-OH pregnenolone levels may confer negative, but independent, health risks.

Mifepristone is an effective oral alternative for the prevention of premature luteinizing hormone surges and/or premature luteinization in women undergoing controlled ovarian hyperstimulation for in vitro fertilization.

The present clinical study was conducted to investigate the effectiveness of a daily dose of 40 mg mifepristone in preventing premature LH surges in women undergoing controlled ovarian hyperstimulation (COH) for in vitro fertilization and to study the effect of this antiprogestin cotreatment on endometrial receptivity. This was a prospective, open-label, randomized, exploratory study in 15 healthy volunteer oocyte donors who were randomly allocated to the experimental COH group, including mifepristone (group 1), or the control group, using a long protocol with GnRH agonists (group 2), in a ratio of 2:1, i.e. 10 and five subjects, respectively. In group 1, human chorionic gonadotropin (hCG) was randomly administered (group 1A) or was withheld (group 1B) at the end of stimulation, so that two subgroups of five subjects each were formed, differing in the final oocyte maturation trigger. In all patients receiving mifepristone, 50 mg progesterone were administered im at the time of hCG administration to counteract residual antiprogestogenic activity of mifepristone. Serum estradiol, progesterone (P), LH, and FSH levels were monitored in each patient on d 3 and 6 and every 48 h thereafter. Endometrial biopsies were taken 2 and 7 d after hCG or P administration. Endometrial tissue was processed and evaluated in a blinded fashion for endometrial dating and quantitative PCR of at least four genes known to be up-regulated in receptive endometrium. The total FSH dose and duration of treatment in the two arms of the study were similar. The mean LH levels on d 6 of stimulation and the day of hCG/P treatment in the mifepristone group were 0.8 +/- 0.7 and 0.5 +/- 0.6 mIU/ml, and those in control subjects were 2.4 +/- 3.8 and 2.0 +/- 1.7 mIU/ml, respectively. No LH surges were observed in any subject treated with mifepristone. Serum P levels on the day of hCG/P were below the cut-off level (1.2 ng/ml) in all subjects of the mifepristone group (range, <0.5 to 1.05 ng/ml). The mean numbers of cumulus-oocyte complexes retrieved were 11.6 +/- 6.6 and 19.6 +/- 11.8 in the subgroup treated with mifepristone and hCG and in the control group, respectively. The mean percentages of metaphase II, metaphase I, and germinal vesicle stage oocytes were 86.2, 6.9, and 3.4% in the mifepristone group and 68.4, 6.1, and 11.2% in the control group. In the mifepristone group that did not receive hCG and received P only at the end of stimulation, an endogenous LH surge was not observed nor were oocytes obtained. Histological evaluation of endometrial samples in patients treated with mifepristone and hCG (group 1A) confirmed normal development, whereas in patients treated with mifepristone only (group 1B), there was a complete arrest of the endometrial maturation. The expression patterns of glycodelin, IGF-binding protein-7, glutathione peroxidase-3, and solute carrier family 1 member 1 show a striking absence of up-regulation in patients treated with mifepristone (groups 1A and 1B) compared with controls (group 2). The results of this exploratory study provide evidence that mifepristone is effective for the prevention of premature LH surges and/or premature luteinization in women undergoing COH for in vitro fertilization. However, endometrial receptivity status requires additional evaluation after decreasing RU-486 doses before this strategy can be considered as a new alternative to GnRH agonist/antagonist treatment.

Multicenter trial of cryotherapy for retinopathy of prematurity: natural history ROP: ocular outcome at 5(1/2) years in premature infants with birth weights less than 1251 g.

OBJECTIVE: To present ophthalmological outcome data at 5(1/2) years after full term from a natural history cohort of infants who had a birth weight less than 1251 g and were enrolled at 5 centers of the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity (ROP), including eyes without ROP and with a full range of ROP severity. DESIGN: Of the 1199 surviving children in the cohort, 1068 (89.1%) were examined. Study-certified ophthalmologists assessed ROP residua and conducted cycloplegic refractions. Visual acuity was measured by study-trained testers using the Early Treatment for Diabetic Retinopathy Study charts. Eyes that had developed ROP were categorized by the lowest (most posterior) zone and highest (most severe) stage reached during the acute phase of the disease. No eyes that received cryotherapy are included; data analysis included one untreated eye per patient. Fundus outcomes were classified as "unfavorable" if there was macular compromise by retinal folding (more severe than ectopia) or stage 4B or 5 retinal detachment. Visual acuity outcomes of 20/200 or worse were classified as unfavorable. RESULTS: Unfavorable fundus structural outcome occurred in 33 (3.1%) of the 1068 eyes; all 33 eyes had a history of severe ROP. Specifically, unfavorable fundus structure occurred in 62.5% (10/16) of eyes with zone I ROP and in 44.2% (23/52) of eyes with zone II ROP, stage 3+ disease involving more than six 30 degrees -sectors. There were no unfavorable fundus outcomes among eyes that had fewer than 7 clock-hours of stage 3+ ROP in zone II in this cohort. Snellen visual acuity was tested in 1059 eyes, and 5.1% were unfavorable at 20/200 or worse; these unfavorable outcomes were correlated with more severe ROP. In eyes that had zone I ROP, 68.8% (11/16) had unfavorable acuity, and for eyes that had zone II ROP, 7.5% (36/481) had unfavorable acuity results. For eyes with ROP observed only in zone III, 1.8% (2/110) had unfavorable acuity of 20/200 or worse. CONCLUSIONS: Premature infants with birth weights less than 1251 g seldom have poor structural and functional outcomes (3.1% and 5.1%, respectively). All unfavorable fundus structural outcomes and nearly all unfavorable acuity outcomes occurred in eyes with zone I ROP or zone II ROP involving more than 6 sectors of stage 3+ disease.

Revised indications for the treatment of retinopathy of prematurity: results of the early treatment for retinopathy of prematurity randomized trial.

OBJECTIVE: To determine whether earlier treatment using ablation of the avascular retina in high-risk prethreshold retinopathy of prematurity (ROP) results in improved grating visual acuity and retinal structural outcomes compared with conventional treatment. METHODS: Infants with bilateral high-risk prethreshold ROP (n = 317) had one eye randomized to early treatment with the fellow eye managed conventionally (control eye). In asymmetric cases (n = 84), the eye with high-risk prethreshold ROP was randomized to early treatment or conventional management. High risk was determined using a model based on the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity natural history cohort. At a corrected age of 9 months, visual acuity was assessed by masked testers using the Teller acuity card procedure. At corrected ages of 6 and 9 months, eyes were examined for structural outcome. Outcomes for the 2 treatment groups of eyes were compared using chi2 analysis, combining data for bilateral and asymmetric cases. RESULTS: Grating acuity results showed a reduction in unfavorable visual acuity outcomes with earlier treatment, from 19.5% to 14.5% (P =.01). Unfavorable structural outcomes were reduced from 15.6% to 9.1% (P<.001) at 9 months. Further analysis supported retinal ablative therapy for eyes with type 1 ROP, defined as zone I, any stage ROP with plus disease (a degree of dilation and tortuosity of the posterior retinal blood vessels meeting or exceeding that of a standard photograph); zone I, stage 3 ROP without plus disease; or zone II, stage 2 or 3 ROP with plus disease. The analysis supported a wait-and-watch approach to type 2 ROP, defined as zone I, stage 1 or 2 ROP without plus disease or zone II, stage 3 ROP without plus disease. These eyes should be considered for treatment only if they progress to type 1 or threshold ROP. CONCLUSIONS: Early treatment of high-risk prethreshold ROP significantly reduced unfavorable outcomes to a clinically important degree. Additional analyses led to modified recommendations for the use of peripheral retinal ablation in eyes with ROP. Long-term follow-up is being conducted to learn whether the benefits noted in the first year after birth will persist into childhood.

15-year outcomes following threshold retinopathy of prematurity: final results from the multicenter trial of cryotherapy for retinopathy of prematurity.

OBJECTIVE: To report the ocular structure and visual acuity outcomes at age 15 years, and the incidence of retinal detachment between 10 and 15 years of age, for patients in the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity (CRYO-ROP). METHODS: Subjects were 254 survivors from 291 preterm children with birth weights less than 1251 g and severe (threshold) retinopathy of prematurity (ROP) in one or both eyes, who participated in the CRYO-ROP trial. At age 15 years, unfavorable ocular structure was posterior retinal fold or worse judged by study-certified ophthalmologists. Unfavorable distance visual acuity was 20/200 or worse measured by study-certified testers using Early Treatment of Diabetic Retinopathy Study recognition acuity charts. RESULTS: Thirty percent of treated eyes and 51.9% of control eyes (P<.001) had unfavorable structural outcomes. Between 10 and 15 years of age, new retinal folds, detachments, or obscuring of the view of the posterior pole occurred in 4.5% of treated and 7.7% of control eyes. Unfavorable visual acuity outcomes were found in 44.7% of treated and 64.3% of control eyes (P<.001). CONCLUSION: The benefit of cryotherapy for treatment of threshold ROP, for both structure and visual function, was maintained across 15 years of follow-up. New retinal detachments, even in eyes with relatively good structural findings at age 10 years, suggest value in long-term, regular follow-up of eyes that experience threshold ROP.

Visual acuity at 10 years in Cryotherapy for Retinopathy of Prematurity (CRYO-ROP) study eyes: effect of retinal residua of retinopathy of prematurity.

OBJECTIVE: To describe recognition (letter) acuity at age 10 years in eyes with and without retinal residua of retinopathy of prematurity (ROP). DESIGN: Presence and severity of ROP residua were documented by a study ophthalmologist. Masked testers measured monocular recognition visual acuity (Early Treatment of Diabetic Retinopathy Study) when the children were 10 years old. Two hundred forty-seven of 255 surviving Cryotherapy for Retinopathy of Prematurity (CRYO-ROP) randomized trial patients participated. A reference group of 102 of 104 Philadelphia-based CRYO-ROP study participants who did not develop ROP was also tested. RESULTS: More severe retinal residua were associated with worse visual acuity, regardless of whether retinal ablation was performed to treat the severe acute-phase ROP. However, within each ROP residua category, there was a wide range of visual acuity results. CONCLUSIONS: This is the first report of the relation between visual acuity (Early Treatment of Diabetic Retinopathy Study charts) and structural abnormalities related to ROP in a large group of eyes that developed threshold ROP in the perinatal period. Visual deficits are greater in eyes with more severe retinal residua than in eyes with mild or no residua. However, severity of ROP residua does not predict the visual acuity of an individual eye because within a single residua category, acuity may range from near normal to blind.

Macular pigmentary changes as a sequelae of retinal hemorrhages in premature infants with retinopathy of prematurity.

PURPOSE: We assess the incidence of macular changes in ROP patients with retinal hemorrhages. PATIENTS AND METHODS: The premature group consisted of 360 children born 32 weeks gestation and/or with weight below 1,500 g. We used the RetCam-120 Digital Retinal Camera to document retinal changes. RESULTS: Of the 360 premature infants 241 (67%) had no ROP, and 119 (33%) had ROP. Of the preterm infants with ROP retinal hemorrhages were found in 46 (38%) children. Of the newborns with ROP and with retinal hemorrhages, macular pigmentary changes were found in 3 (6%) patients. Of these 3 patients, the first had pre-threshold ROP, the second threshold ROP and had underwent diode laser photocoagulation, and the third patient had stage 2 ROP. In the patients with pre-threshold and threshold ROP retinal hemorrhages appeared 6 weeks after birth and macular pigmentary changes were found 6 months after birth. In the patient with stage 2 ROP hemorrhages appeared 7-8 weeks after birth and macular pigmentary changes were detected 12 months after birth. CONCLUSIONS: Although macular hemorrhages almost always resorb without complications, our study allows the assumption that retinal hemorrhages may cause macular pigmentary changes in the macula, and thus may lead to deprivation amblyopia. Our results may suggest that the presence of the macular pigmentary changes may be related to the hemorrhage and not to the specific therapy or to the disease.
