Joe's story: reflection on a difficult interaction between a nurse and a patient's wife.

Unfortunately, not all nursing interactions are positive. Incidents can occur that leave nurses with a variety of negative emotions. This paper examines one such interaction that occurred between the author, a dying patient, and the patient's wife. The author reflects on and attempts to explain the feelings aroused by the situation through the use of psychodynamic theories. The characteristic personality and coping styles of the main participants are discussed. Issues relating to staff support are also highlighted.

Nonsteroidal anti-inflammatory drugs and severe psychiatric side effects.

OBJECTIVE: Nonsteroidal anti-inflammatory drugs (NSAIDS) are used extensively in the treatment of pain. This study explored the possibility that psychiatric side effects may be both more frequent and more severe than thought previously. METHOD: Four psychiatric outpatients, three with affective disorders and one with schizophrenia, were treated with NSAIDS for a complaint of pain. The NSAIDs were withdrawn, then restarted for three patients, and then withdrawn again one or more times. The patients were evaluated while on and off NSAIDs. RESULTS: All four patients developed moderate to severe depression and one became severely paranoid while on NSAIDs initially. When the NSAID was withdrawn there was remission of the depressive symptoms and in one case the accompanying paranoia. The depressive symptoms were reproduced when the NSAID was restarted in five instances (involving only 3 of the patients) and remitted when the NSAID was discontinued. One of these three patients also became paranoid in two instances. The paranoia remitted when the NSAID was discontinued. CONCLUSIONS: These findings suggest that NSAIDs can induce or exacerbate reproducible symptoms (depression, paranoia) in patients with either affective disorder or schizophrenia. These adverse effects may be more severe and frequent than thought previously. NSAID-treated patients should be studied for NSAID-induced psychiatric side effects.

[Study of paranoid ideation in college students: an approach using the diathesis-stress model].

A recent study reported that normal people who have social anxiety and attention to social and interpersonal cues show paranoid ideation (Martin & Penn, 2001). The purpose of the present study was to examine whether people with these diatheses show paranoid ideation in the diathesis-stress framework. Questionnaires were administered to 177 college students three times. At Time I, social anxiety and attention to social and interpersonal cues were assessed. These variables were used in Martin and Penn (2001) as the diathesis factor of paranoid ideation. At Time 2, paranoid ideation during the period, and at Time 3, paranoid ideation and stress events between Time 2 and Time 3 were assessed. Setwise hierarchical multiple regression analysis was used to predict the change in the incidence of paranoid ideation from Time 2 to Time 3. Result revealed significant interaction effects between social anxiety and stress. Students with high score on the social anxiety scale showed an increase in paranoid ideation when they experienced more stress events.

A functional psychosis precipitated by quinidine.

A 62-year-old man with a syndrome of gradually progressive cognitive deterioration accompanied by paranoid features is described. He had been taking quinidine since 1974 for a recurring supraventricular tachyarrhythmia. Examination revealed a suspicious man with widespread patchy cognitive deficits but no focal neurological signs. An exhaustive range of investigations gave unremarkable results. Within 24 hours of cessation of quinidine there was a dramatic improvement in his mental state and, after a further four days, he had returned essentially to normal with no demonstrable cognitive deficits. Several months later he suffered a recrudescence with prominent paranoid and depressive features which gradually settled after commencement of pimozide and dothiepin. It is likely that these events reflect a recurring functional psychosis which was precipitated or exacerbated in the first instance by quinidine. This represents a significant complication of quinidine therapy which has been largely unrecognized. Therapy with quinidine should be considered as a potential contributing factor in any patient with dementia or a functional psychosis who is also taking this drug.

[The madness of "The Quixote"].

The aim of this fiction psychiatric article, is to take a psychiatric look to El Quijote and to determine his strange and fascinating madness. His symptoms are analyzed as those of any patient and the conclusion is that he had a pre senile paranoia. This diagnosis, that gratifies clinicians curiosity, says nothing about the charm of his unusual mental profile or the captivating fascination of his nobility and poetry.

[A study of mental health issues in the objects of substance abstinence who got labor education and rehabilitation].

OBJECTIVE: To investigate the personality and mental health of the objects of substance (caffeine, cocaine, hallucinogens and so on) abstinence who got labor education and rehabilitation. METHODS: 258 male objects and 250 female objects of substance abstinence selected randomly from institutions of labor education and rehabilitation were assessed with MMPI and SCL-90. RESULTS: MMPI tests indicated the personality abnormality in the objects. SCL-90 tests manifested the mental health abnormality of them compared with Chinese models, especially in the female objects. CONCLUSION: More attention should be paid on the mental health of the objects of substance abstinence.

Borderline personality features in childhood: a short-term longitudinal study.

Borderline personality disorder is a particularly devastating, yet understudied form of psychopathology. One of the most significant gaps in existing knowledge is the lack of systematic, prospective empirical attention to the developmental precursors of borderline personality. The present investigation was an exploratory attempt to address this limitation through (a) development of a psychometrically sound self-report instrument that assesses borderline personality features in childhood, the Borderline Personality Features Scale for Children (BPFS-C); (b) examination of the stability of BPF in childhood; (c) evaluation of gender differences in BPF in childhood; and (d) evaluation of the specificity of the BPFS-C for assessing borderline personality features. These goals were achieved through the prospective study of a normative sample of 400 (54% female) fourth though sixth graders who were assessed during the Fall of Year 1, Spring of Year 1, and Fall of Year 2. The use of linear mixed modeling techniques provided evidence for the construct validity of the BPFS-C. Further, borderline personality features as assessed with the BPFS-C were found to be moderately stable over the course of the study, with girls reporting higher levels of BPF than boys. Results also demonstrated that children's scores on the BPFS-C were uniquely related to indicators of borderline personality pathology above and beyond their scores on the Children's Depression Inventory. The implications of these results for the study of the development and etiology of borderline pathology are discussed.

Relationship of African Americans' sociodemographic characteristics to belief in conspiracies about HIV/AIDS and birth control.

Although prior research shows that substantial proportions of African Americans hold conspiracy beliefs, little is known about the subgroups of African Americans most likely to endorse such beliefs. We examined the relationship of African Americans' sociodemographic characteristics to their conspiracy beliefs about HIV/AIDS and birth control. Anonymous telephone surveys were conducted with a targeted random-digit-dial sample of 500 African Americans (15-44 years) in the contiguous United States. Respondents reported agreement with statements capturing beliefs in HIV/AIDS conspiracies (one scale) and birth control conspiracies (two scales). Sociodemographic variables included gender, age, education, employment, income, number of people income supports, number of living children, marital/cohabitation status, religiosity and black identity. Multivariate analyses indicated that stronger HIV/AIDS conspiracy beliefs were significantly associated with male gender, black identity and lower income. Male gender and lower education were significantly related to black genocide conspiracy beliefs, and male gender and high religiosity were significantly related to contraceptive safety conspiracy beliefs. The set of sociodemographic characteristics explained a moderately small amount of the variance in conspiracy beliefs regarding HIV/AIDS (R2 range=0.07-0.12) and birth control (R2 range=0.05-0.09). Findings suggest that conspiracy beliefs are not isolated to specific segments of the African-American population.

[Accounting failures in public officials or notaries. Medico-legal approach].

Considering 11 cases of psychiatric expert reports in criminal cases, the author's aim is to describe the criminological aspects of offences by notaries, mainly swindle and breach of trust. It appears that can be found in this professional category, personality types already described for "closely acquainted", or business crooks, as well as four obviously pathological cases, coming within the provisions of article 64 of the criminal code. Beyond the fact that there is a specific pre-criminal situation: dealing with contracts, assets, funds etc... the authors notices in several cases a discrepancy between the weak and immature personality of some notaries and the importance of the office with which they are entrusted. Thus the requirement of ability and sufficient professional qualification was not always met. The authors would be interested in further similar observations, to confirm or infirm their conclusions.

White doctors and black patients: influence of race on the doctor-patient relationship.

Effective communication between doctor and patient, a skill not emphasized in medical education programs, is essential for patient satisfaction and optimal patient care. In many teaching hospitals, the doctor is commonly white and middle class and the patient black and indigent. Racial differences, even in the absence of social class differences, may have a negative impact on the quality of the doctor-patient relationship. The impact of racism is reviewed, and recommendations to enhance the relationship between white doctors and black patients, are made.
